The Comparison on the General Characteristics of Acute Stroke Patient between Heat pattern group and Cold pattern group (한열변증에 따른 중풍 환자의 제반특성 비교)

  • Object : This study was conducted as part of the national project to standardize stroke diagnosis in Korean medicine. In this study, we aimed to evaluate the characteristics of acute stroke between heat pattern group and cold pattern group. Methods : We recruited stroke patients from 5 universities(Kyung-Hee University oriental medical center, Kyung-Hee University East-West Neo medical center, Kyungwon university lncheon oriental medical center, Kyungwon university Songpa oriental medical center and DongGuk university llsan oriental medical center) from April, 2007 until February, 2010. We diagnosed them and selected 463 heat pattern patients and 182 cold pattern patients. Results : We find that the risk factor of smoking, alcohol, diet(prefer to meat) are more associated with the heat pattern group. On the other hand, inflammation history in recent 3 months and diet(prefer to sea food) are more associated with the cold pattern group. RBC, Hg, Hct, TG, CK and Cl are more related to the heat pattern group. CPT, total cholesterol and HDL are more related to the cold pattern group. Tae-eum type takes high distribution of the heat pattern group, and So-eum type takes high distribution of the cold pattern group. Conclusions : This study provides evidence that the heat pattern group have more risk factor than the cold pattern group.

Smart Helmet for Vital Sign-Based Heatstroke Detection Using Support Vector Machine (SVM 이용한 다중 생체신호기반 온열질환 감지 스마트 안전모 개발)

  • Recently, owing to global warming, average summer temperatures are increasing and the number of hot days is increasing is increasing, which leads to an increase in heat stroke. In particular, outdoor workers directly exposed to the heat are at higher risk of heat stroke; therefore, preventing heat-related illnesses and managing safety have become important. Although various wearable devices have been developed to prevent heat stroke for outdoor workers, applying various sensors to the safety helmets that workers must wear is an excellent alternative. In this study, we developed a smart helmet that measures various vital signs of the wearer such as body temperature, heart rate, and sweat rate; external environmental signals such as temperature and humidity; and movement signals of the wearer such as roll and pitch angles. The smart helmet can acquire the various data by connecting with a smartphone application. Environmental data can check the status of heat wave advisory, and the individual vital signs can monitor the health of workers. In addition, we developed an algorithm that classifies the risk of heat-related illness as normal and abnormal by inputting a set of vital signs of the wearer using a support vector machine technique, which is a machine learning technique that allows for rapid binary classification with high reliability. Furthermore, the classified results suggest that the safety manager can supervise the prevention of heat stroke by receiving feedback from the control system.

  • Many children have died (Heat stroke deaths) in the U.S. after being left alone in cars during hot weather, especially summer season. According to related report, more than 800 children have died of heat stroke from being trapped in a hot car since 1998. The regulation party, government has started to make not only technical regulation to prevent tragedy but also legislate to punish. However the 75% of accident has occurred unintended by their parents. So punishment is not the best solution for this case. So in this study, we analyze the trend of regulation and technology to save occupant who remained back seat. And finally we propose a countermeasure to prevent heat stroke deaths.

The Study of Correlation between Pattern Identification of Stroke Patients and Meteorological Elements (중풍 환자 변증과 기후 요소와의 상관성에 관한 연구)

  • There are many reports about correlations between meteorological elements and stroke. In Oriental medicine, it is recognized that the weather affects the human body and diseases, but there are few studies about the correlation between meteorological elements and pattern identification of stroke. 105 stroke patients classified into fire-heat pattern or dampress-phlegm pattern were registered during the study period. We took the measurement of each meteorological element (atmospheric pressure, temperature, humidity, wind speed) according to pattern identification and analyzed pattern identification into two groups according to mean of each meteorological element during the study period. Mean temperature was higher with the heat-fire pattern than with the dampness-phlegm pattern. Heat-fire pattern also had higher frequency when temperature was higher than mean temperature. There was no correlation between atmospheric pressure, relative humidity, or wind speed and pattern identification.

Different Level of Plasma Free Hemoglobin between Qi-deficiency and Fire Heat among Korean Stoke Subjects (한국인 중풍환자의 기허군 화열군의 plasma free hemoglobin의 비교)

  • The purpose of this study was to fine proteins, which have significantly different level in plasma between Qi-deficiency and Fire-heat group of Korean Oriental Stroke pattern identification (PI) among Korean stroke patients. Eighteen stroke patients with Qi-deficiency and forty nine patients with Fire-heat, which had critical syndrome of each PI, were participated in this study. Plasma protein pattern were analyzed by SELDI-TOF MS using Q10 strong anion exchange chip and Mass spectral data (m/z) statistically determined. The expression level of proteins, which were different between Qi-deficiency and Fire-heat in the results by SELDI-TOF MS, were confirmed by western blot. As a result of analyzing plasma protein by SELDI-TOF MS, six protein peaks were significantly higher in Fire-heat group than Qi-deficiency group. Two peaks among of them, M15003 and M15745, were respectively identified as hemoglobin alpha and beta in previous study. Expression level of plasma free hemoglobin of Fire-heat group was also confirmed higher in Fire-heat group than in Qi-deficiency group. These findings suggest that plasma free hemoglobin is a candidate for discriminating Qi-deficiency and Fire-heat group according to pattern identification (PI) of stroke.

  • ■ Objectives The purpose of this study was to investigate the correlation of The Korean Standard Pattern Identification for Stroke and Intima-Media Thickness of common carotid artery(CCA-IMT). ■ Methods 100 subjects with acute ischemic stroke were recruited from the patients admitted to DongEui Medical Center from the June 2011 to January 2014. We assessed 100 patient's Common Carotid Intima Media Thickness data by B Mode ultrasonography. We investigated pattern identification of all subjects based on Korean Standard Pattern Identification for Stroke-III. then, We analyzed their characteristics, risk factor, blood test result, life style. ■ Results We analyzed indicators of Korean Standard Pattern Identification for Stroke-III by Common Carotid Intima Media Thickness difference. As a result, pale complexion, look powerless, drowsiness like to lie, heat vexation and aversion to heat were significantly higher in the CCAIMT ≧ 1.0mm group then in the CCA-IMT < 1.0mm group. ■ Conclusion According to the analysis, the Relationship between Carotid Intima-Media Thickness and The Korean Standard Pattern Identification for Stroke in Ischemic Stroke Patients were founded. To acquire more concrete conclusion on this theme, we need more research.

A Literatual Study on the Treatment of Stroke in The 《Dongyi Suse Bowon》 (동의수세보원(東醫壽世保元)의 문헌적(文獻的) 자료(資料)에 근거(根據)한 중풍(中風) 치료(治療)에 대한 고찰(考察))

  • The purpose of this study was to find the way of treatment and prevention the stroke by Sasang constitutional medical therapy. I came to get some conclusions after considering literatures of ${\ll}$Dongyi Suse Bowon${\gg}$ and the conclusion as follows. 1. In ${\ll}$Dongyi Suse Bowon${\gg}$ the stroke of Soumin is caused by 'Interior cold disease'. When 'Interior Yin' can not descend, the disease arise. In this case Lee Je-ma used 'Sohabhyang-won(蘇合香元)', 'Chulaek-su(鐵液水)'. 2. The stroke of Soyangin is caused by 'Interior heat disease'. When the hot Qi of stomach and spleen is blocked, the disease arise. In this case Lee Je-ma used 'Dokhwal-jihwang-tang(獨活地黃湯)', 'Jihwang-baekho-tang(地黃白虎湯)'. 3. The stroke of Taeumin is caused by 'Dry and heat disease'. When the heat of liver and the dryness of lung is excessive, the disease arise. In this case Lee Je-ma treated with 'Woohwang-chungsim-hwan(牛黃淸心丸)', 'Wonji-sukchangpo-tang(遠志石菖蒲湯)', and 'Kwache-san(瓜帶散)' etc. Above results indicate that not only taking medicine but also filling up the Healthy energy(保命之主) is important to treat stroke.

  • Object : The purpose of this study is to observe the changes of symptoms and signs diagnosised by differentiation of the pattern identification in stroke patients. Methods : Seventy subjects were recruited from patients with stroke within ten days of onset. We chose twenty-nine subjects diagnosised as same differentiation of the pattern identification, and who had at least on follow up session. We had investigated change of symptoms and signs diagnosised by differentiation of the pattern identification. Results : There were five symptoms and signs (thick fur, dry fur, difficult defecation, heat vexation and aversion to heat, normal pulse) and ten (heavy-headedness, frequency of defecation, hard defecation, feel heavy, slippery pulsem, dry mouth, bitter taste in the mouth, feel lazy, look lazy) symptoms and signs that were statistically significant improved in fire-heat patterns and dampness-phlegm pattern, respectively. Conclusions : This study provides evidence that collaborative treatment is effective in improving some symptoms and signs in acute ischemic stroke patients diagnosised by fire-heat pattern and dampness-phlegm pattern. Further studies with larger scale, longer observation period would be required.

  • To develop the diagnostic tool for Fire-heat pattern, we analyzed sensitivity and specificity of symptom signs to diagnose the Fire-heat pattern in stroke patients. Korean medicine doctor surveyed Fire-Heat of the symptoms for the Stroke(KSDS) case report form in stroke patients within 1 month of onset. The sensitivity of "more 1/5 in major sings and 2/11 in helpful sings", "more 2/5 in major sings and 2/11 in helpful sings", "more 3/5 in major sings and 2/11 in helpful sings", "more 1/5 in major sings and 3/11 in helpful sings" "more 2/5 in major sings and 3/11 in helpful sings" "more 3/5 in major sings and 3/11 in helpful sings" are respectively 93%, 59%, 33%, 80%, 53%, 32%. The specificity are respectively 93%, 59%, 33%, 80%, 53%, 32%. The sensitivity(59%) and specificity(80%) of "more 2/5 in major sings and 2/11 in helpful sings" that to be implanted.

Correlation between Post-Stroke Depression and Cold, Heat, Deficiency and Excess Patterns (뇌졸중 후 우울증과 한열허실 변증의 상관관계)

  • Objectives : The aim of this study was to analyze relationship between post-stroke depression (PSD) and cold, heat, deficiency and excess patterns. Methods : Twenty-eight PSD patients were recruited from STROKE center and measured with questionnaires for cold, heat, deficiency and excess patternvalues and saliva for cortisol awakening response (CAR). Saliva samples were collected immediately, 15, 30 and 45 min after awakening. In addition, Beck Depression Inventory (BDI) and Hamilton Depression Rating Scale (HDRS) were conducted for PSD severity. We conducted correlation analysis to find the relationship between cold, heat, deficiency and excess patterns and CAR or BDI and HDRS. Results : Deficiency and excess patterns werepositively correlated with area under the curve with respect to the increase (AUCi), but not with area under the curve with respect to the global (AUCg), in CAR. Furthermore, it was negatively correlated with BDI and HDRS, while cold and heat patterns were not correlated with CAR, BDI and HDRS. Conclusions : In terms of deficiency and excess patterns, the higher the PSD severity, the higher the deficiency and the lower the PSD severity, the higher the excess. However, there was not a significant relationship between PSD and cold and heat patterns.
