데이터베이스 프로그램에 기반한 심장판막 치환수술 환자의 레지스트리 확립 및 위험인자 분석 (Establishment of Valve Replacement Registry and Risk Factor Analysis Based on Database Application Program)

  • 배경: 심장 판막증은 국내에서 아직까지 가장 많은 유병률을 보이는 중요 심장질환이다. 1958년 이후 1999년까지 한국에서는 총 94,586례의 개심술이 시행되었으며 그중 36,247례가 성인심장질환이었고 그중 20,704례는 판막질환이었다. 그러나 제대로 된 데이터베이스가 구축되어 있지 않아 많은 자료들을 효과적으로 활용하지 못하고 있어 저자들은 판막레지스트리를 확립하고 활용할 수 있는 시스템을 구축하고자 하였다. 대상 및 방법: 마이크로소프트 악세스를 이용하여 데이터베이스 프로그램을 개발하였다. 이는 관계형 구조를 갖는 비교적 작고 간결한 프로그램으로 그 사용자환경이 쉽고 효율적으로 구성되어 있으며 각종 질의와 보고서 기능 등을 이용하여 사용자가 원하는 자료를 쉽고 빠르게 추출해 낼 수 있는 장점이 있다. 결과: 서울대학교병원 흉부외과에서는 1968년부터 1999년까지 약 3000여명의 환자에 대하여 판막치환술을 시행하였으며 여기에 사용된 총 판막의 수는 약 3700 개였다. 삽입된 판막을 부위별로 살펴보면 승모판막 1600명, 대동맥판막 584명, 삼첨판막 76명 등이었으며, 700여명에서 2개이상의 판막을 치환하였다 전체 판막환자의 약 46%인 1280여명이 조직판막을 치환 받았으며, 54%인 약 1500여명이 금속판막을 치환 받았다. 약 16%인 460여명이 판막재치환술을 시행받았으며 해마다 약 40여명 전후에서 시행되고 있다. 결론: 저자들은 판막레지스트리를 확립하기 위한 데이터베이스 프로그램을 성공적으로 개발하였으며 이는 향후 연구활동과 보건의료분야의 발전에 효과적으로 활용될 수 있다.

  • Left ventricular (LV) wall thickening, or LV hypertrophy (LVH), is common and occurs in diverse conditions including hypertrophic cardiomyopathy (HCM), hypertensive heart disease, aortic valve stenosis, lysosomal storage disorders, cardiac amyloidosis, mitochondrial cardiomyopathy, sarcoidosis and athlete's heart. Cardiac magnetic resonance (CMR) imaging provides various tissue contrasts and characteristics that reflect histological changes in the myocardium, such as cellular hypertrophy, cardiomyocyte disarray, interstitial fibrosis, extracellular accumulation of insoluble proteins, intracellular accumulation of fat, and intracellular vacuolar changes. Therefore, CMR imaging may be beneficial in establishing a differential diagnosis of LVH. Although various diseases share LV wall thickening as a common feature, the histologic changes that underscore each disease are distinct. This review focuses on CMR multiparametric myocardial analysis, which may provide clues for the differentiation of thickened myocardium based on the histologic features of HCM and its phenocopies.

심장판막수술 환자의 비계획적 재입원 영향요인: 전자의무기록분석 (Factor affecting Unplanned Readmissions after Cardiac Valve Surgery: Analysis of Electric Medical Record)

  • 본 연구의 목적은 심장판막 수술 받은 환자를 대상으로 전자의무기록 검토를 통해 퇴원 후 30일 내 비계획적 재입원군의 특성과, 재입원 영향요인을 규명하고자 하는 후향적 조사연구이다. 대상자는 S시에 위치한 상급종합병원에서 2018년 1월부터 2019년 8월까지 심장판막수술을 받고 포함기준에 맞는 퇴원환자 423명이었다. 비계획적 재입원 대상자는 총 48명(11.3%)이었고, 그 원인은 심방세동 13건 (27.1%)과 수술부위 통증이 13건(27.1%)으로 가장 많았고, 와파린의 치료농도가 안 맞는 경우 10건 (20.8%), 전신 위약감 7건 (14.6%). 저혈압 5건 (10.4%). 심낭삼출 4건 (8.3%), 수술상처 감염 3건 (6.3%), 출혈 3건 (6.3%), 고열 3건 (6.3%), 뇌경색 1건(2.1%)이었다. 재입원에 영향을 미치는 변수는 암의 병력 (OR=2.60, 95% CI 1.13-6.03, p=.025), 퇴원 후 거주지는 병원이 아닌 집으로 간 경우 (OR=2.91, 95% CI 1.33-6.36, p=.008), 판막수술종류로서 승모판막성형술이 대동맥판막 치환술보다 재입원률이 높았다(OR=1.21, 95% CI 1.21-4.98, p=.012). 비계획적 재입원 감소를 위해 환자와 돌봄 제공자에게 퇴원 전 만성질환병력관리와 재입원의 위험 요인을 미리 사정하고 병원 방문할 수 있도록 하는 개별화된 교육프로그램이 필요하다.

소아 후천성심장판막질환의 판막치환수술 (Cardiac valve replacement in children with acquired lesions)

  • Between 1974 and 1982, 31 patients from 7 to 15 years of age have undergone valve replacement for their acuqired cardiac valvular diseases at Seoul National University Hospital. Furteen patients (45.2%) had a definite history of rheumatic fever and only 4 patients (12.9%) had atrial fibrillation on their preoperative electrocardiograms. Characteristically, the valvular lesions were ones of insufficiency with or without associated stenosis in all patients except only one whose mitral valve was tightly stenotic. Thrity-seven valves were replaced in 31 patietns including a case of successful replacement of his failed xenogragt mitral valve : 4 mechanical valves were used in 3 patients and 33 xenograft valves were used in the remaining 28 patients. The size of the valves were not the major problem at the time of opertion because most of the patients had a dilated heart from disease. There were 3 diaths within 30 days of surgery (9.7% operative mortality rate) and 3 late deaths (9.7% late mortality rate) with an overall mortality rate of 19.4%. Twenty-eight early survivors were followed up for a total of 488 patient-months. Thromboembolic complications occurred in 5 patients with 2 deaths: cmbolic rate of 17.9% or the actuarial embolic incidence of 12.29%/patients-year. four xenograft tissue valves in 4 patients had failed during the period from 19 to 41 months of surgery with an overall valve failure rate of xenograft of 14.3% or the actuarial incidence of 9.84% failure/patient-year. One of these 4 patients had required replacement of his failed mitral xenograft valve which had severe calcification and tissue disruption with primary tissue failure rate of 3.6% or the actuarial incidence of 3.13% failure/patient-year. The actuarial survial including the operative morality was 50.0% at 5 years of surgery. /the actuarial incidence free from thromboembolism in bioprosthetic group was 85.4% at 42 months, while it was 33.4% in mechanicial group at 60 months after operation. The actuarial incidence free from overall valve failur of 100.0% until 18 months after surgery was followed by a rapid decrease during the next 2-year period, and it was only 17.8% at the follow-up end of 42 months after surgery. It was suggested that the major advantage of low thrombogenecity with xenograft valve should be balanced against the high incidence of accelerated valve failure when it is used in children whose age is younger than 15 years old. The possible role of recurrent rheumatic attacks to the early failure of xenograft tissue valve was also discussed.

체외순환후의 혈청효소치 변동에 관한 연구 (Serum Enzyme Values after Extracorporeal Circulation)

  • The effect of cardiopulmonary bypass procedures on the blood components were studied through the measurements of several serum enzymes, SGOT, SGPT, CPK and LDH with its isozymes in the patients who had surgery for their heart diseases. They wre 85 patients: 40 Males and 45 Females. Their ages ranged widely between one and fifty-six. They were divided into 5 groups: Group I.patent ductus arteriosus without extracorporeal circulation, Group II-atrial septal defect, Group III-ventricular septal defect, Group IV-tetralogy of Fallot, and Group V-valve replacement. Generally serum enzymes revealed the increased values after surgery and the tendency returning toward preoperative levels. With the high total serum LDH levels seen uniformly after operation which persisted long in patients with replaced valves, the changes of LDH-isozymes were further analyzed. The isozymes, especially LDI and LDs increased their activities postoperatively, and the other fractions varied little. And $LD_1$/$LD_2$ ratios also showed similar rises and falls while the elevated values lasted longer in the patients with cardiac valve replacement. The significant values of LDH-isozymes in predicting the hemolysis occuring after extracorporeal circulation and in following the patients whose valves were replaced have been further discussed with the special stress placed on the $LD_1$/$LD_2$ratios.

심장성 원인의 호흡곤란 및 두근거림을 방기복령탕가미방(Fangji Fuling Decoction)을 포함한 복합 한의치료로 호전시킨 증례 1례 - 증례보고 (Improvement of Dyspnea and Palpitations of Cardiac Origin Using Comprehensive Traditional Korean Medicine Treatment Including Bangibokryeong-tang (Fangji Fuling Decoction) : A Case Report)

  • This case report demonstrates the effect of modified Bangibokryeong-tang (Fangji Fuling Decoction), a traditional herbal prescription in treating dyspnea and palpitations, symptoms that often lead to anxiety and reduced quality of life of cardiac patients. A female patient in her late 70s with congenital valve malformation and atrial fibrillation presented with dyspnea and palpitations, each rated at a severity of 8 on a numeric rating scale (NRS). After voluntarily discontinuing Western medication, she received modified BGBRT for 25 days, leading to significant symptom relief and NRS improvement to 0-2. The patient reported maintaining an improved condition and showed a significant increase in vitality. This improvement was sustained for 7 months, but the symptoms recurred; thus, modified BGBRT was restarted. Modified BGBRT showed substantial effects on persistent cardiac-origin dyspnea and palpitations. Thus, this TKM has the potential to manage symptoms and enhance the quality of life of cardiac patients.

선천성 심장기형에 대한 2차수술로서의 개심술 (Reoperation for congenital heart disease)

  • Between March 1978 and August 1985, 29 cases at various congenital heart diseases were reoperated because of remnant shunt of residual anomalies at Seoul National University Hospital. They were consisted of 10 cases of Tetralogy, 4 simple VSD, 6 complicated VSD. 3 partial ECD, and 5 other rare congenital anomalies. The interval between the initial and the second procedure ranged from 1 day to 122 months [mean; 26.9 months]. In 4 cases of them, the second procedure was done during initial hospitalization within 3 weeks post-operatively. The primary operation intended to be corrective surgery except four whose primary operation was palliative or exploratory one even though it was done with extracorporeal circulation. The indication for second operation was mainly residual shunt or valvular obstruction due to patch detachment or inadequate relief of stenotic lesion. Others were paravalvular leak, valvuloplasty failure, prosthetic valve failure, and inadequate primary diagnosis. Four patients were dead [14.3%]; three complicated VSD`s and one Tetralogy. There were 7 cases of nonfatal complication with subsequent improvement except one [diffuse cerebral dysfunction].

Considerations for Invasive Dental Treatment in Disabled Patients

  • Dental treatment for the disabled should be a customized that considers the characteristics and degree of cooperation of each disabled patient. There are additional considerations during implant treatment and tooth extraction in disabled patients. Since some brain lesion disorder or cardiac disease patients may be taking antiplatelet or anticoagulant medications, it is necessary to evaluate whether these medications should be discontinued before an invasive procedure. Precautions should be taken for patients with heart valve disease considering the risk of infective endocarditis, especially during invasive dental procedures. Moreover, disabled patients may have difficulty in following instructions and cautions. There are specific considerations for each stage of implant treatment in disabled patients. In the case of patients who are in the pre- or post-transplant state, it is necessary to assess their general condition and oral disease due to the risk of infection. Since disabled patients with various systemic diseases may visit the dental clinic, it is important to understand their characteristics and treatment process in order to flexibly adjust the dental treatment plan accordingly.

Alagille syndrome and a JAG1 mutation: 41 cases of experience at a single center

  • Purpose: Alagille syndrome is a complex hereditary disorder that is associated with cardiac, hepatic, skeletal, ocular, and facial abnormalities. Mutations in the Notch signaling pathway, such as in JAG1 and NOTCH2, play a key role in embryonic development. A cardiac or hepatic presentation is a critical factor for determining the prognosis. Methods: We conducted a retrospective study of 41 patients with Alagille syndrome or a JAG1 mutation between 1983 and 2013. Results: The first presentations were jaundice, murmur, cyanosis, and small bowel obstruction at a median age of 1.0 months (range, 0-24 months). The JAG1 mutation was found in 27 of the 28 genetically-tested patients. Cardiovascular anomalies were identified in 36 patients, chronic cholestasis was identified in 34, and liver transplantation was performed in 9. There was no significant correlation between the severity of the liver and cardiac diseases. The most common cardiovascular anomaly was peripheral pulmonary stenosis (83.3%), with 13 patients having significant hemodynamic derangement and 12 undergoing surgical repair. A total bilirubin level of >15 mg/dL with a complex surgical procedure increased the surgical mortality (P=0.022). Eight patients died after a median period of 2.67 years (range, 0.33-15 years). The groups with fetal presentation and with combined severe liver and heart disease had the poorest survival (P<0.001). Conclusion: The group with combined severe liver and heart disease had the poorest survival, and a multidisciplinary approach is necessary to improve the outcome.

대동맥 박리증의 수술요법 -27례의 수술환자를 대상으로 한 5 년간의 성적- (Operative treatment of aortic dissections - Experience with 27 patients over a 5-year period -)

  • Current therapy of aortic dissections remains unstandardized because of the relative rarity of these catastrophic events and conflicting reported results of various therapeutic strategies. Hence, we reviewed our current results and planned to purify our method of interpretation of results and so, to standardize therapeutic managements. This study comprised unselected, consecutive 27 patients with aortic dissections who were operated at Seoul National University Hospital from Jan 1983 to March 1988. The results from analysis of their preoperative, operative and postoperative finding were as follows: 1] 7 patients had acute type A, 14 had chronic type A, 4 had acute type B, and 2 had chronic type B. 2] The causes of dissections were unclear, but 8 patients had Marfan`s syndromes, 2 had previous operative histories on cardiovascular systems and 2 had congenital heart diseases. 3] Multiple preoperative variables were found to correlate significantly with operative mortality and complications. The prevalences of such preoperative major complicating factors were significantly more frequent in acute than chronic [P < 0.05] and type A than type B [P < 0.01]. 4] Operations were performed according to the type of the dissections and whether it was acute or chronic. Usually dacron tube graft replacements were performed[25/26]. Intraluminal sutureless graft replacement was performed in 11 patients. Of the 14 patients with combined aortic regurgitation, concomitant aortic valve resuspension in 4, seperative aortic valve replacement in 1, and aortic valve replacement with coronary reimplantation were performed in 9 patients. 2 patients had concomitant arch vessel managements. 5] Over-all operative mortality rate was 33% and 54% for acute type A, 25% for acute type B, 29% for chronic type A, 0% for chronic type B respectively. The main causes of operative mortality were cardiovascular complications [mainly CPB-weaning failure] in acute cases and hemorrhagic complications in chronic cases.
