Ectopic Intrapulmonary Thyroid: A Case Report

  • An ectopic thyroid is caused by abnormalities in migration of the thyroid during development and rarely occurs in the thoracic cavity or the abdominal cavity. We report the case of a 64-year-old female who had abnormal findings from a thyroid hormone test during follow-up after thyroid cancer surgery. Based on the radioisotope diagnostic test, an ectopic thyroid inside the thoracic cavity was suspected. Through surgical treatment, the patient was diagnosed with ectopic intrapulmonary thyroid. Ectopic intrapulmonary thyroid is reported to be very rare and the case is described along with a literature review.

  • A congenital diaphragmatic hernia, which mainly occurs in the left thorax, requires an emergency operative procedure during the neonatal periods. A right-sided congenital diaphragmatic hernia is rare, and often detected after the neonatal period due to the mild symptoms. Traditionally, the treatment repairs the diaphragmatic defect via a thoracotomy. However, good results of thoracoscopic repairs have been reported. Herein, the case of a 5-month-old girl, who received a thoracoscopic repair of a right-sided congenital diaphragmatic hernia, is reported.

  • It was previously reported from our laboratory that the rate of deterioration of contractile force was slower in the heart of the ginseng extract treated rats. It was also found that ginseng may have an ability to sustain the normal function of the heart by sustaining Ca accumulation by sarcoplasmic reticulum. $Ca^{++}-dependent$ ATPase plays the central role in movement of $Ca^{++}$ ion from sarcoplasm into sarcoplasmic reticulum. In this investigation, the fragment of sarcoplasmic reticulum was prepared from rat heart treated with ginseng water extract orally 100mg/kg/day for 7 to 10 days and from normal rat heart. $Ca^{++}-dependent$ APTase activity was estimated by a modified method of Fiske and Subbarow's procedure. Experimental groups were divided into 6 groups, depending on the preincubation time, 5, 30 and 60min. at ${25}^{\circ}C$ and ${37}^{\circ}C$ respectively. In both of the groups of ${25}^{\circ}C$ and ${37}^{\circ}C$, $Ca^{++}-dependent$ ATPase activities of the ginseng treated rat hearts were higher than that of normal hearts. Therefore, it can be concluded that $Ca^{++}-dependent$ ATPase activities in sarcoplasmic reticulum of rat hearts were increased by the treatment with ginseng extract.

Guillain-Barré Syndrome after Lung Transplantation in the Immediate Postoperative Period: Case Report

  • A 58-year-old man, incapable of maintaining oxygen saturation with mechanical ventilation, was admitted to our hospital for veno-venous extracorporeal membrane oxygenation (ECMO) treatment. He was diagnosed with acute respiratory distress syndrome (ARDS) due to influenza A pneumonia. His condition stabilized with antibiotics and steroid administration, but weaning from ECMO failed due to post-infectious pulmonary sequelae. On day 84 after admission, he underwent bilateral lung transplantation. In the postoperative phase, he did not regain consciousness even after discontinuation of sedatives for 3 days. However, spontaneous pupillary reflex and eye movements were preserved, while communication and upper and lower limb movements were affected. The nerve conduction study was diagnostic of Guillain-Barré syndrome. He was managed with intravenous immunoglobulins and plasmapheresis. Mild recovery of the facial muscles was seen, but he died 24 days post-surgery due to progressive ARDS and sepsis.

  • Heart failure (HF) stands as a prevalent chronic ailment, imposing a substantial burden on global healthcare systems due to recurrent hospitalizations, intricate management, persistent symptoms, and polypharmacy challenges. The augmentation of patient safety and treatment efficacy across various care stages, facilitated by a multidisciplinary HF team inclusive of a clinical pharmacist, emerges as paramount. Evidence underscores that the collaborative engagement of a physician and a clinical pharmacist engenders proficient and secure management, forestalling avoidable adversities stemming from drug reactions and prescription inaccuracies. This synergistic approach tailors treatments optimally to individual patients. Post-discharge, the vulnerability of HF patients to re-hospitalization looms large, historically holding sway as the foremost cause of 30-day readmissions. Diverse strategies have been instituted to fortify patient well-being, leading to the formulation of specialized transitional care programs that shepherd patients effectively from hospital to outpatient settings. These initiatives have demonstrably curtailed readmission rates. This review outlines a spectrum of roles assumed by clinical pharmacists within the healthcare cohort, spanning inpatient care, transitional phases, and outpatient services. Moreover, it traverses a compendium of studies spotlighting the affirmative impact instigated by integrating clinical pharmacists into these fields.

The Study on the Effect of Acupuncture on UPDRS and Heart Rate Variability in the Patients with Idiopathic Parkinson's Disease (태충(太衝) 양릉천(陽陵泉)의 침(針)자극이 특발성 파킨슨 환자의 UPDRS 및 HRV Parameter에 미치는 영향)

  • Introduction : This study was designed to evaluate the effect of acupuncture on Unified Parkinson's Disease Rating Scale (UPDRS) and Heart Rate Variability (HRV) of patients with Parkinson's disease Methods: Subjects were voluntarily recruited through newspaper and internet advertisement. All the subjects were confirmed as idiopathic parkinson's disease by a neurologist. Acupuncture was applied 2 times a week for four weeks by oriental medical doctor at Kyung-hee University hospital. Acupuncture points used were GB34 and LR3. The patient's symptoms were assessed before and after 4 weeks of treatment by UPDRS and HRV. HRV was measured for 5 minutes before and after 4 weeks of treatment. Results : The results were as follows; 1. In both groups, UPDRS scores were significantly improved after 4 weeks compared to the pre-treatment 2. After 4 weeks of treatment UPDRS score differences between the two groups were insignificant 3. After 4 weeks of treatment HRV parameter scores showed statistically insignificant differences between the two groups. SDNN parameters of the Experimental group were significantly increased. Conclusion : This study suggests that acupuncture treatments can be applicable to improve symptoms in the patients with idiopathic Parkinson's disease. Further study on the acupuncture and HRV in the patients with Parkinson's disease is recommended.

  • This is a literature Study on ophthalmologic disease remedies of the four famous physicians in Jin Yuan period. The results were as follows: 1. Yu Wan So regarded the cause of ophthalmologic disease as the wind and heat(風熱), the blood deficiency(血少), or the deficiency of vital function and essence ,of the kidney(腎氣虛). For each treatment he used the method to remove wind and heat(除風散熱), the tranquillizing and tonifying method to nourish the heart and to relieve mental strain due to dificiency of vital essence(養血安神). the method to nourish Yin in the kidney(補腎水). The important nature of the these medicines is bitter and cold. 2. Jang Jong Jeung regarded the cause of ophthalmologic disease as the blood excess(血太過). or the blood deficiency(血不及). or the fire and heat(火熱). For each treatment he used the method to extract blood(出血療法). the method to nourish the liver and the kidney(補肝腎療法). or the method of vomiting and diarrhea(吐不法). The medicines are usuallly classified into external treatment medicines(外用藥). medicines to disintegrate mass(破積聚藥). purgating medicines(攻下藥). 3. Lee Dong Won regarded the cause of ophthalmologic disease as the spleen and the stomach weakness(脾胃虛弱), excessive fire in the heart(心火太盛). For each treatment he used the method to tranquillize the spleen and the stomach (調理脾胃). the tranquillizing and tonifying method to nourish the heart and to relieve mental strain due to deficiency of vital essence(養血安神). 4. Ju Dan Gye regarded the cause of ophthalmologic disease as the wind and heat(風熱). the blood deficiency(血少). the mental exhaustion(神勞). the kidney weakness(腎弱). For each treatment he used the method to remove wind and heat(除風散熱), the method to nourish blood and yin water(養血補水), the method to relieve mental strain and to remove spiritual heat(安腎瀉神火). the method to nourish yin water(養陰水). These drugs are usually classified into the medicines to remove endogenous heat(淸熱藥). the medicines to nourish yin(補陰藥).

Triptolide improves myocardial fibrosis in rats through inhibition of nuclear factor kappa B and NLR family pyrin domain containing 3 inflammasome pathway

  • Myocardial fibrosis (MF) is the result of persistent and repeated aggravation of myocardial ischemia and hypoxia, leading to the gradual development of heart failure of chronic ischemic heart disease. Triptolide (TPL) is identified to be involved in the treatment for MF. This study aims to explore the mechanism of TPL in the treatment of MF. The MF rat model was established, subcutaneously injected with isoproterenol and treated by subcutaneous injection of TPL. The cardiac function of each group was evaluated, including LVEF, LVFS, LVES, and LVED. The expressions of ANP, BNP, inflammatory related factors (IL-1β, IL-18, TNF-α, MCP-1, VCAM1), NLRP3 inflammasome factors (NLRP3, ASC) and fibrosis related factors (TGF-β1, COL1, and COL3) in rats were dete cted. H&E staining and Masson staining were used to observe myocardial cell inflammation and fibrosis of rats. Western blot was used to detect the p-P65 and t-P65 levels in nucleoprotein of rat myocardial tissues. LVED and LVES of MF group were significantly upregulated, LVEF and LVFS were significantly downregulated, while TPL treatment reversed these trends; TPL treatment downregulated the tissue injury and improved the pathological damage of MF rats. TPL treatment downregulated the levels of inflammatory factors and fibrosis factors, and inhibited the activation of NLRP3 inflammasome. Activation of NLRP3 inflammasome or NF-κB pathway reversed the effect of TPL on MF. Collectively, TPL inhibited the activation of NLRP3 inflammasome by inhibiting NF-κB pathway, and improved MF in MF rats.

Pain Relieving Effect of Intraoral Sucrose Replacement in Neonates (신생아의 통증완화를 위한 자당(sucrose) 경구투여 효과)

  • The ability of neonates to perceive and react to pain, has been acknowledged recently. Recent researches have been shown that even short term pain can have lasting negative effects. We know that most of the anatomical pathway and neurotrans-mitter function necessary for pain perception are fully or nearly fully developed in the neonatal period. Many people are still reluctant to believe that pain felt by neonates may be as severe as that felt by older children or adults yet. The objective of the study is to assess and compare the analgesic effects of orally administered sucrose and pacifiers. And to determine the synergistic analgesic effects of sucrose and pacifiers. The tools for this study is Lawrence's NIPS (neonatal infant pain scale) with behavioural pain responses and index for pain physiological reponses as heart rate, respiratory rate and degree of SaO₂. The participants are 96 healthy infants of neonatal age 1-7days and randomised to receive heel prick before 2minutes to blood sampling and physiological test in Nursery of K Medical University Hospital on May-July, 2000. The experimental group assigned to one of three treatment groups : no treatment; a pacifier; placebo(2ml 25% sucrose). Collected data were analyzed with the SAS program using X²-test, ANOVA and Duncan's multiple range test as post hoc. The results were as follows. 1) Pain behaviour responses: The pain score of placebo group(2ml 25% sucrose) is lower than no treatment group and pacifier group significantly(P=.000). Placebo group is different from no treatment and pacifier group with Duncan's multiple range test. 2) Pain physiologic responses (1) heart rate: The heart rate of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group significantly(P=.000). The heart rate change of placebo group is less than the other groups. (2) respiratory rate: The respiratory rate of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group insignificantly (P=.2340). But, the respiratory rate of placebo group is lower than the other groups. (3) SaO₂: The SaO₂ score of placebo group(2ml 25% sucrose) is different from that of no treatment group and pacifier group insignificantly (P=.3265). But, the change of SaO₂ score of placebo group is less than the other groups. In conclusion, the sucrose placebo showed pain relief effect in behavior responses and less physiological responses. Accordingly, the sucrose placebo should be applied nursing intervention for simple pain management as heel prick.

Effect of Banhabakchulchunma-tang on the Hepatic, Splenic and Cardiac Toxicity induced by Doxorubicin (반하백출천마탕(半夏白朮天麻湯)이 Doxorubicin에 의해 유발(誘發)된 독성(毒性)에 미치는 영향(影響))

  • Object : The effect of Banhabakchulchunma-tang extracts on the hepatic, splenic and cardiac toxicity induced by Doxorubicin administration(Three injection protocol) were monitored using male ICR mice. Methods : The changes of body weight, clinical signs, necropsy findings and organ weights of liver, spleen and heart were observed with blood GOT and GPT levels. Results : 1. Decrease of body weight and The degrees of anorexia, ataxia and dehydration after Doxorubicin treatment were dose-dependently inhibited by Banhabakchulchunma-tang extracts. 2. Increase of absolute and relative liver and heart weight observed in Doxorubicin treatment group were dose-dependently inhibited by Banhabakchulchunma-tang extracts. In addition, the degrees of liver congestion necrotic spot and the degrees of heart congestion enlargement were dose-dependently decreased after Banhabakchulchunma-tang extracts dosing groups compared to that of doxorubicin treatment group. It is also demonstrated that elevated serum GOT and GPT levels in doxorubicin treatment group were significantly decreased in Banhabakchulchunma-tang extracts dosing groups. 3. Decrease of absolute and relative spleen weight observed in doxorubicin treatment group were dose-dependently inhibited by Banhabakchulchunma-tang extracts. In addition, the degrees of splenic atrophy were significantly and dose-dependently decreased after Banhabakchulchunma-tang extracts dosing groups compared to that of doxorubicin treatment group. Conclusion : the toxicity of doxorubicin treatment(decrease of body weights, clinical signs such as anorexia, ataxia and dehydration, changes of organ weights of liver, spleen and heart, elevation of serum GOT and GPT levels) was inhibited and/or prevented by Banhabakchulchunma-tang extracts. According to these results, it is considered that Banhabakchulchunma-tang has some preventive effect against to doxorubicin induced toxicity.
