건강도시사업 추진과 정책 과제 (Review and Prospective of the Heathy Cities in Korea)

  • Since 1996, the Health Promotion Programme spearheaded by the Korean Central Government has been actively developing and recently, the Healthy City Project led by the local autonomous entities have also been actively promoted. Healthy City is one in which the health and well-being of the citizens are given the utmost importance in the decision-making of the city. While the Health Promotion Programme focuses on changing the "health behavior" of the people, the Healthy City Project, a policy to improve the existing inequality of public health services, deals with more essential health factors and requires political support as well as a new organization. The Healthy City paradigm based on the New Public Health started in England and ever since the Healthy City Model Project spearheaded by the EURO WHO began in 1986, the Alliance for Healthy Cities centered in the West Pacific region supported by the WHO in Oct 2003 was inaugurated. 19 Korean cities are full members of the Alliance for Healthy Cities and 2 laboratories are associate members. The Ministry of Health and Welfare has held the Healthy City Forum consisting of related officials, experts and representatives of civic bodies on 6 occasions since Dec 2005. The need for adequate administrative and financial support from the Central Government to the local autonomous entities governing the Healthy Cities was raised. It is hoped that this Healthy City Project will bring about the improved health conditions of the people as well as promote the equality of the public health services.

지역사회 중심 노인 건강증진 사업의 효과 (Effects of a Community-Based Health Promotion Project for Elderly People)

  • Purpose: The purpose of this study was to identify the effect of a community-based health promotion program for elders. Method: The research design was a nonequivalent control group experimental design. The participants were 415 elders who were above 65 years of age (mean age: 80.7, experimental group: 126, control group: 289). For 4 months, the health promotion project which consisted of health education and activity related to smoking, drinking, nutrition and exercise was given to the experimental group. Data were analyzed using descriptive statistics, ${\chi}^2$-test, and t-test with the SAS V8e program. Result: After the intervention, the experimental group showed a higher level of health promotion behavior compared to the control group in health education experience, exercise, smoking, and drinking. There were no differences between the experimental and control groups for nutrition, or high risk drinking and smoking. Conclusion: The results of this study show that the health promoting program for elders provides useful information in developing effective community-based programs and can be recommended as effective interventions to improve the health promoting behavior of elders.

강북구 148 마을 건강주민운동사업의 성과와 한계 (Performances and Limitations of the Health Community Organization Project in 148 Village, Gangbuk-gu)

  • 이 연구는 서울시 강북구 건강마을 사업에 대한 문헌고찰과 운영주체를 대상으로 한 심층면접을 통해 주민운동의 관점에서 사업의 성과와 한계를 살펴보고, 기존 건강증진 사업과의 차별성을 찾아 향후 건강증진 사업의 새로운 방향성을 모색하는 것을 목적으로 하였다. 연구 결과 빈곤층의 비율이 높고, 물리적 환경이 열악한 지역사회 주민들의 건강불평등을 완화하기 위한 건강친화 마을 만들기 사업은 주민조직화의 원리를 적용한 것으로 기존 건강증진사업과 다음과 같은 방법론적 차별성과 성과가 있었다. 첫째, 주민 스스로 지역보건의료 계획을 수립하고 주민의 성장을 통한 건강증진사업이 이루어졌다. 둘째, 기존 건강증진사업에서는 볼 수 없었던 주민활동가가 팀을 이루어 보건소 및 전문가, 주민 간 긴밀한 협력을 위해 매개자로 활동하였다. 셋째, 병의원이나 보건소가 아닌 대안적 주민참여 건강증진 공간인 건강카페를 통한 건강증진 구심점이 마련되어 다양한 프로그램이 활성화되었다. 넷째, 건강의 사회적 결정요인을 고려한 접근을 통한 건강증진 프로그램이 활성화 되었다. 따라서 향후 건강친화 마을 만들기와 같은 지역사회 건강불평등 완화를 위한 사업을 실시할 때 이 연구에서와 같은 방법론적 접근을 고려해볼 필요가 있다. 하지만 다양한 사업주체들이 모인만큼 이해관계와 역할분담과 관련된 어려움이 발생한 점과 시범사업의 성과와 한계를 사전에 명확히 예상하기 못해 갈등이 유발되었던 점 등의 한계가 있었던 만큼 향후 유사한 사업의 계획 시 이 연구에서 나타난 한계점 등을 고려한 내용이 반영되어야 할 것이다. 주민건강운동의 관점에서 건강마을사업 확산 된다면 사회 전체에 건강증진 효과를 만들 것이다.

전북 일부 지역 노인구강보건사업 실태와 노인구강건강평가지수 및 삶의 만족도에 관한 연구 (A study on Oral Health Assessment Index (GOHAI) and life satisfaction of the state of oral health project of the elderly in Jeonbuk Korea)

  • Objectives: The purpose of the study is to investigate the oral health assessment and satisfaction according to recognition and beneficiary oral health projects for oral health promotion of the elderly. Methods: The subjects were elderly over 65 years old in Jeonbuk province. The study instrument was structured questionnaire including general characteristics, oral care condition, recognition of project, satisfaction of project, geriatric oral health assessment index, life satisfaction. For data analysis, the study used independent t-test, one way ANOVA and Pearson correlation analysis. Results: The subjects of this study were well aware of the perceived health status of oral health projects. Satisfaction scaling project was the highest in the satisfaction of the received projects. People who recognized the oral health project had a high oral health assessment index. People who benefit from oral health project showed higher satisfaction in life. Conclusions: As for the elderly's perception on oral health care, they showed higher oral health assessment index and life satisfaction.

계층 분석법을 적용한 부산시 건강도시 사업의 우선순위 설정 (Priority Setting for the Healthy City Program in Busan Using the Analytic Hierarchy Process)

  • Objectives: Busan had the highest mortality and the shortest life expectancy at birth among 16 provinces in Korea in 2008 and there were considerable health inequalities within the region. This study was performed to build up a priority setting framework in Healthy City Busan project. Methods: Analytic hierarchy process was used to determine the relative priority weight for different strategic and program dimensions along with the consistency of response. An on-site workshop-based meeting (calculating importance) and online survey (calculating risk) were conducted to obtain data from 8 experts. Results: The results showed that in strategic criteria "active health promotion & diseases prevention" and "building infrastructure for the Health City project" were two most important factors. In program criteria, considering both importance and risk scores, "making a healthy community" and "building community health centers" in disadvantaged areas were a top priority group. In addition, "enacting an ordinance for the Healthy City", "building the infrastructure for health impact assessment" and "making health care safety net for vulnerable population" were also higher priorities group. Conclusions: Our findings suggest that the Healthy City project in Busan should be focused on strengthening health equity and building infrastructure for sustainability of the project.

SPIRIT 체크리스트를 활용한 건강도시평가: 원주시 사례 (Evaluation of Healthy City Project Using SPIRIT Checklist: Wonju City Case)

  • 본 연구의 목적은 원주 건강도시사업을 평가하여 문제점을 확인하고 건강도시사업의 원칙과 전략에 근거하여 사업을 발전시키고자 하는데 있다. 연구를 위하여 건강도시연맹에서 개발한 과정평가 도구인 SPIRIT Checklist를 사용하였다. 평가팀은 39개의 관련문서를 분석, 평가하고 건강도시사업 담당자, 관련부서 담당자, 자문위원과의 회의를 통해 평가 결과에 대한 의견수렴을 실시하였고 최종적으로는 AFHC의 SPIRIT평가 전문가와의 연석회의를 통하여 분석 결과를 검증하였다. 원주시의 건강도시사업을 평가한 결과 강력한 정치적 지원에 근거하여 지속적인 건강도시사업을 가능하게 하는 자원, 중기사업계획, 인프라, 협력적 조직, 건강도시네트워크 등이 갖추어져 있는 것을 확인하였고, 건강증진 전략을 적용한 사업의 보완 및 개선이 필요한 것으로 나타났다. 건강도시사업의 과정평가를 위해 개발된 SPIRIT 체크리스트는 질적 평가도구로서, 향후 건강도시간의 비교를 위하여 질적 평가방법에 기초한 양적 평가 지표를 추가할 필요가 있다.

보건간호사의 건강증진사업 수행 현황과 역할 확대 방안 모색을 위한 연구 (A Study on Developing Strategies for Expanding the Roles of Public Health Nurses)

  • Purpose: This study was to identify activities and related problems on health promotion of public health nurses, and to investigate basic data proposed to revise strategies of Health Promotion Nurse Specialists (HPNS). Method: The subjects of this study were 787 public health nurses sampled by clustering of the entire nation of Korea. Data were collected by focus group studies and cross-sectional survey during April to October of 1999. Results: 13.3% of public health nurses took responsibility for health promotion projects and 45.9% were involved in health promotion work with in home care or other routine activities. Also, the factors public health nurses perceived as barriers to implement activities for health promotion were time limitation, lack of ability for planning projects, insufficient time for specialties, inadequate understanding about health promotion project. Conclusion: According to the study, they need to have special education and further technical support. These results have important implications for the establishment for the role of Health Promotion Nurse Specialist. Also, in developing systems and curriculums for Health Promotion Nurse Specialist, the major factors described above need to be considered carefully.

한의약건강증진사업 발전을 위한 공중보건한의사 역할 탐색 (Exploring the roles of Korean public health doctors in the development of Korean medicine health promotion project)

  • Objectives : In an effort to suggest a developmental plan of Traditional Korean Medicine health promotion project(TKM-HPP), this study was conducted to identify the problems experienced by public health Traditional Korean Medicine(PH-TKM) doctors in duty and to analyze successful cases of the health promotion project. Methods : Two PH-TKM doctors in duty at the Public Health Centre branch and Public Health Centre respectively, and one TKM doctor in charge of Public Health Centre participated in the in-depth interview. The research procedure was as follows; 1) inform study objectives and interview questions to participants, 2) in-depth interview with semi-structured questions, and 3) post-confirmation and verification. The interview was recorded and transcripted. In-depth interview was composed of 6 subsections: 1) Introducing participants and share purpose of interview, 2) Confirmation of the job status and difficulty of public health centre, 3) Reflecting and sharing experiences of public health clinic job performance cases, 4) Current status and improvement needs of TKM-HPP, 5) Discussions on improvement of TKM-HPP in public health center, 6) Sharing and reconfirmation of interview results. Thematic analysis was conducted from the narratives. Results : The study showed that; 1) the support from the local government was critical for the successful TKM-HPP and participation of the PH-TKMs; 2) the major problems experienced by PH-TKMs were lack of training for TKM-HPP in the field and lack of knowledge at local government level; 3) the suggestions to provide PH-TKMs with manual to educate residents effectively and redefine roles of PH-TKMs in the project. It also suggests that TKM-HPP model should be developed by the district in a way of reflecting regional characteristics. Conclusions : Firstly, more efforts should be made on the projects rather than medical treatment in public health centers, except for areas where medical institutions are scarce. Secondly, in order to facilitate participation of PH-TKM doctors, each local office should reduce medical care and increase efforts related with projects. Thirdly, the institution has to develop feasible TKM-HPP plans that can be carried out by PH-TKMs by the manual. Lastly, pre-training should be provided to PH-TKMs to understand contents of TKM-HPP related to public health centre work before the placement.

한의약건강증진사업의 예산편성여부에 미치는 영향 연구 (A Study on the Effect of Korean Medicine Health Promotion Project on Budgeting)

  • 본 연구는 전국 226개의 기초자치단체를 대상으로 2020년 기준 지역사회 통합건강증진사업 내 한의약건강증진사업 예산편성여부의 관련 요인을 파악하는데 목적이 있다. 2020년도 지역별 예산서를 활용하여 지역사회 통합건강증진사업 내 한의약건강증진사업 예산편성여부를 파악한 후 예산편성여부의 관련 요인을 분석하고자 기술통계 및 차이검정과 이분형 로지스틱 회귀분석을 실시하였다. 분석 결과 한의약난임치료지원 예산편성여부, 한의사수, 65세이상 인구수, 고령인구비율의 변수 차이는 다르게 나타났고, 한의약난임치료지원 예산편성여부, 고령인구비율, 건강도시가입여부에서 영향을 미치는 것으로 나타나 향후 한의학관련 정책 및 사업예산을 편성하는데 참고자료로 활용될 것으로 생각된다.
