Achievement of Health Promotion Fund and Projects in Korea (국민건강증진기금사업 10년 성과)

  • There has been 10 years since The Health Promotion Act was legislated. The government began to establish a health promotion fund on the basis of Health Promotion Act in 1995, and to manage and operate the fund from 1998. It is evaluated that health promotion program have had various outcomes in many aspects. First, there has been growing awareness of the impotance of health promotion through the establishment of Health Plan 2010 and the effort to actualize the Plan. Second, the importance of securing health equity and identifying health determinants have been recognized during the planning process of Health Plan 2010. Third, the health promotion program have mainly focused on improving healthy life style of the population. As a result, desirable health behavior change of the population could be expected from the result of 2005 National Health and Nutrition Survey. Fourth, public health centers began to play a crucial role in implementing health promotion programs, and began to build infrastructure for health promotion programs. Fifth, the outcomes of health promotion programs by public health centers and private health organizations have been increasing. Finally, training for health promotion personnel and their participation in the program could be the foundation for the higher level of outcome achievement from the health promotion programs. Important challenges for future health promotion would be identification of the determinants and risk factors of health in Korea, establishment of local health promotion plan, building infrastructure for health promotion, creation of specific action model by public health center, development of diverse health promotion programs and health promotion program for the elderly, conducting research for evidence concerning major factors reducing the need for health care through prevention disease activities, and establishment of evaluation and feed back system for health promotion programs.

Public Health Policy and Health Equity (공중보건정책과 건강 형평성)

  • Equity-focused public health policy has solid theoretical and practical basis, in addition to ethical one. In the Republic of Korea (hereafter Korea), however, equity in health has not had a high priority in policy goals, regardless of policy areas and particular actors or approaches. Equitable health has been only a minor concern in most public health issues and their decision-making. Generic public health policies are needed to reduce inequity in health, but the importance of a firm basis for sound policy-making cannot be overemphasized. Health equity should be 'mainstreamed' in all public health policies. Potential approaches include intersectoral collaboration, health impact assessment, and 'Health in All Policies.' Public policy agendas for equitable health cannot be formulated without measurement and recognition of the problem. Korea is still suffering from the lack of reliable information on the current status of health inequity, resulting in a relatively weak awareness of the problem among both the general public and policy-makers. More information is needed to increase recognition and awareness that will increase intervention and actions. The absence of decision-making and actions should not be justified even by the lack of information on determinants and pathways of health inequities. Generic plausible solutions can often work in the real world according to political and social commitment. I have discussed several aspects of public health policy from the perspective of health equity, focusing on current status and plausible explanation. Policy process, agenda setting in particular, is highlighted and theories and concepts are presented along with analysis and description of current situation.

Determinants of Health Service Utilization of Urban Health Center (도시 보건소 보건의료서비스 이용의 결정요인)

  • This study was carried out to assess the utilization of urban health center and its related factors among the urban residents. The survey was carried out for 2,394 households in Taegu and Kyongju by the structured questionnaire from 28 March to 4 April and from 2 July to 9 July, 1994. Well trained interviewers visited 2,630 households in Taegu and Kyongju, and interviewed with housewives. Of the target households, 91.0%(2,394 households) were responded through three-time visiting. The major results were summarized as follows : The fourth and fifth decad utilized the health center more frequently than any other age groups.. The lower income group showed higher rate of health center utilization than those in higher income group in Taegu City. The mean length of residence among residents of Kyongju City is longer than those of Taegu City, and the longer length of residence, the higher rate of the health center utilization. Those who are living together with neonate and infant or elderly people showed higher rate of health center utilization than those who are living without neonate and infant or elderly people in both Taegu and Kyongju. The most common reason for visiting the health center was 'low cost'. The major reasons for not visiting the health center were 'not regular customer', 'poor health center facility', and 'low quality of care'. Vaccination, communicable disease control, outpatient care, public hygiene, maternal and child health program were well recognized as health center activities. In logistic regression for the utilization of health center, the significant independent variables were length of residence and recognize the site of health center in both Taegu and Kyongju. The improvement of quality of health service, physical environment of health center and public relations on health center's activities shoulod be considered for reactivation and reingorcement of health center functions.

Determinants of Registered Nurse Skill Mix & Staffing Level in Korea (간호인력 구성 및 확보수준 결정 요인)

  • Purpose: This study was done to identify determinants of registered nurse (RN) skill mix and staffing level focused on hospital characteristics. Methods: Data were obtained from health insurance claims data and hospital reporting system in the Health Insurance Review and Assessment Service (HIRA) for the year 2010. Data from 2,998 hospitals were analysed using t-test, ANOVA, Pearson correlation, and regression analysis. Results: The RN skill mix and staffing level were positively related to hospital size and the percentage of inpatients to total patients. RN skill mix and staffing level were statistically different across regions. Including nursing aides (NA), however, there was no difference in staffing levels across regions. Medically vulnerable regions, bed operation rate, and the number of patients per doctor were also related to RN skill mix and staffing level. Conclusion: The statically significant determinants of RN skill mix and staffing level included hospital size, region, bed operation rate, percentage of inpatients, doctor-patient ratio. Further study needs to be done to investigate factors including RN supply and wages.

Resource Transfers from Adult Children to Their Elderly Parents (미국 성인자녀의 노부모에 대한 자원이전행동에 관한 연구)

  • The purpose of this study is to examine the influence of parent-to-child financial transfers and economic resources on financial transfers, caregiving, and time donated from middle-aged adult children to their elderly parents. Analyzing data from the Wisconsin Longitudinal Study, which provides long-term observations of financial reciprocity and recent reports about elder care, the current study finds strong positive effects of prior parent-to-child financial transfers in the models of caregiving and time; which indicates the importance of reciprocity. In terms of determinants of resource transfers, the findings of logistic regression analyses suggest that the economic resources of parents and adult children are strong determinants of child-to-parent financial resource transfers. Sociodemographic characteristics of parents and respondents were observed as strong determinants of caregiving or time. In addition, caregiving responds more to the health and income levels of parents whereas donated time is responsive to the net worth of parents and parents' status. For adult children, gender is a strong determinant of both caregiving and time donation. The long-term health problem of adult children is a statistically significant predictor of caregiving, while the employment status of adult children and the number of siblings have statistically a significant association with time donated to care for the parents.

The Determinants of the Long-term Influence of Violence: Focus on Hospital Nurses (폭력의 장기적 영향 결정요인: 병원간호사를 중심으로)

  • Objectives : The purpose of this study was to identify the determinants of the long-term influence of violence on hospital nurses. Methods : Three-hundred hospital nurses were recruited in B city, South Korea. They were asked to complete a questionnaire, and 282 data- sets were included in the multiple regression analysis. Results : Subjects experienced more episodes of verbal violence than that of physical threats or physical violence. Assailants tended to be patients and their caretakers rather than internal customers. Nurses who had religion, worked in a surgical ward, and a 3-6 year career perceived a high level of violent experiences compared to their counterparts. The determinants of the long-term influence of violence were physical violence (t=-2.705, p=.007), emotion-focused coping (t=3.049, p=.003), and emotional response (t=3.611, p<.001). The model was statistically significant explaining 13.0% of the variance (F=14.981, p<.001). Conclusions : Nurse managers should help nurses who are victims of hospital violence by teaching them not to depend on emotion-focused coping and by alleviating their emotional response to violence.

Multidisciplinary Approach to Low Fertility Issue in Korea (저출산 대책에 대한 다학제적 접근)

  • A rapid decrease of total fertility rate to 1.08 in 2005 prompted the Korean government to plan and implement a '5-year plan for ageing society and population policy' starting from 2006. The 1st and 2nd 5-year plans had not shown any discernible impact on the fertility and the 3rd 5-year plan was launched in 2016. However, the fertility rate is going down further. The author reviewed the contents and assessment reports of the fertility promotion plan to suggest ideas for complementing the shortcomings of it. Author defined the major determinants of marriage and child birth as philosophy, politics, sense of value, social norm, culture, healthcare, and education. The plan was examined in view of these determinants. Transformation of Korea from an agricultural society to an industrialized society in a short period of time had brought about changes in most of the determinants of marriage and child birth; in particular philosophy and sense of value. These aspects were not put into consideration in the plan. Author suggested to launch a social education program for the general public to establish a sound philosophy of life, reform the sense of value on family, child birth and education, and cultivate the skill to draw a consensus through discussions on the social issues. A special program to promote marriage of women at the optimum age for child birth was proposed. The government should implement well balanced policy for economic development and labor. Multidisciplinary approach was recommended for these tasks.

An Analysis on the Determinants of Efficiency of the Pharmaceutical Firms using Stochastic Frontier Analysis (Stochastic Frontier Analysis를 이용한 제약회사의 효율성과 그 결정요인분석)

  • Background & Methods: The purpose of this research is to estimate the efficiency of the pharmaceutical firms and the determinants of their efficiency. Stochastic frontier analysis(SFA) and panel study are applied to the data of 60 domestic pharmaceutical firms from 2006 to 2012. Results & Conclusion: First, the result of the stochastic frontier analysis shows that overall efficiency of the pharmaceutical firms is increasing as time goes by. However, if firms are classified by the scale, the larger firms show more efficiency and if classified by the degree of innovativeness, the innovative firms show more efficiency compared to the non-innovative firms. This evidences show that the scale and R&D investment have significant relationships with the efficiency of the pharmaceutical firms. Therefore, it is necessary to increase the national level of investment for the fundamental researches to vitalize R&D of the new drugs. Second, the result of estimation of the determinants of efficiency shows that the firms with larger sales promotion expenses and entertainment expenses have less efficiency compared to the other firms. This can be explained by the structural characteristics of the small generic pharmaceutical firms. Therefore, the government had better make the pharmaceutical firms to reduce sales promotion and entertainment expenses and increase R&D expenses by introducing systems such as global budgeting system on medicine or reference pricing system.

Determinants of Job Satisfaction among Workers at Elderly Care Hospitals (노인전문병원 근무자들의 직무만족도 결정요인)

  • This study purports to investigate the determinant of job satisfaction among workers working at Long-term care hospitals. The independent variables contain three groups of determinants: organizational characteristics variables(job autonomy, job variety, distributive justice, role conflict, supervisor support, job suitability, job significance, job security, organizational support, job growth, promotional opportunity), environmental variables(job opportunity), and psychological variables (met expectation, job efficacy, positive affectivity, and negative affectivity). The sample used in this study consisted of 250 workers from 4 Long-term care hospitals nationwide. Data were collected with self-administered questionnaires and analyzed using multiple regression analysis. The results of the study are as follows: 1) the following variables, listed in order of size, have significant effects on job satisfaction: negative affectivity(-), job significance(+), job growth(+), age(+), positive affectivity(+), organizational support(+), job opportunity(-). 2) the variance of job satisfaction explained by the variables used in the study are 53.8%. When demographic variables added to Model I, job satisfaction explained by variables are 55.4%. 3) the results of this study indicate that three variables of negative affectivity, job significance, job growth are especially important for improving the level of job satisfaction among workers at Long-term care hospitals.
