Health Education Curriculum Constructs and Dimensional Properties for Korean Middle School Students in Multidimensional Scaling Analysis (다차원척도법을 이용한 중학교 보건교육 교과영역 구축 및 속성 분석)

  • Background: School is a primary health education setting for adolescents and the continuous support should be provided to renew school health education curriculum correspondent to cultural changes in Korean society. Objectives: This study was conducted to identify the principals and teachers' health education needs for their students and to analyze their conceptual map for health education curriculum at school. Methods: The sample size of the preliminary study was 321 of the teachers in elementary, middle, and high school, and that of the main study was 355 middle school principals and teachers over the country. The self-administered mailing survey was conducted to collect the available health education topics in the preliminary study, to identify the factor structure of the health education topics and to analyze the conceptual properties on health education with exploratory factor analysis and multidimensional scaling analysis in SPSS 12.0. Results: A total of 21 health education topics were collected from the preliminary survey and 31 topics were, comprehensively, generated for the main survey. In exploratory factor analysis, seven factors were generated in 1.0 or greater Eigen value standard. The seven factors were 'life health promotion,' 'disease prevention and drug control,' 'bulling and aggression prevention,' 'injury and sexual harassment prevention,' human-efficacy and regulation,' 'health protection for adolescence,' and 'alcohol and tobacco control.' The educational need scores were the highest in 'human-efficacy and regulation' and 'injury and sexual harassment prevention.' The two-dimensional cooperates were generated for the 31 health education topics and the two dimensional properties which divided the conceptual space were 'health-safety' for one and 'public/environmental-individual/personal' for the other. That is, middle school principals and teachers primarily, understand the health education curriculum in the sense of 'health vs. safety' and 'public/environmental vs individual/personal.' Conclusions: Health education curriculum and textbook should be developed based on teachers' needs and conditions for health education in school fields. The field-based health education programs or textbook would make more possible problem-solving health education for youth in real school fields.

A Study of relationship between Housewives' Health KAP level and Family Health in Buan and Chunan Area. (가정주부의 건강에 대한 지식, 태도 및 실천수준과 가족건강간의 관련성 연구 - 천안 및 부안지역을 중심으로 -)

  • This study aims to explore the relationship between housewives' health KAP level and the physical health of families. The data used in this study are obtained from the Last Evaluation Program of the National Note for Health surveyed in July, 1989. The respondents for this study are 770 housewives residing in Chunan and Buan. The reason why this study focus on KAP level of housewives is to find out whether a housewife as a emotional supporter contributes to the physical health of her families. A housewife gives her families emotional satisfaction insteade of economic satisfaction. So she has the most interest in family health among the members of her family. Therefore, housewife's KAP level will influence her family health. The independent variables chosen for the analysis are the general characteristic variables and KAP level. And the dependent variable is the physical health of families which excluded psychosocial one. This level of family health includes weight for family health and seriouseness of disease. The result of this study was summarized as follows. (1) KAP level was significantly correlated with variables which have mainly the socioeconomic characteristics. The variables were: area of residence, education level occupation, self assessment on wealth, and exposure to mass communication. (2) In the analysis of relationship between the general characteristic variables and family health, family health was significantly correlated with almost all variables. The variables were: Presence of the aged, Health status, Experience in disease, Self assessment on health, No. of families, Occupation, Education level, Self assessment on wealth, Concern on health, and Exposure to mass communication. (3) In the analysis of relationship between family health and KAP level, family health was not significantly correlated with KAP level but. (4) Also in the stepwise regression analysis, the general variables account for about 32.1 percent of the variance in the dependent variable, family health. The variable with the greatest explanatory power was presence of the aged. On the contrary, KAP level explain about 0.4 percent of the variance in the dependent variable. In sum, the study shows that housewives' health KAP level has relatively weak relationship with the physical health of families

Analysis of Factors Affecting the Quality of Work Life of Dental Hygienists Based on the Culture-Work-Health Model

  • This study investigated the relationship between the organizational culture, organizational support, organizational health, personal health, and quality of work life of dental hygienists and analyzed the factors affecting the quality of work life in order to identify ways to improve their quality of work life. A total of 320 dental hygienists completed a self-administered survey; after excluding data from 21 respondents, 299 responses were included in the analysis. Frequency analyses, t-tests, one-way analysis of variation (ANOVA), and correlation analyses were conducted. A path analysis was also conducted to confirm the causal relationships. The findings are as follows. First, there was a significant difference in several general characteristics of the organizational culture including years in the current job and the number of dental hygienists; organizational support including age and the number of dental hygienists; organizational health including years in the current job and annual salary; and personal health including annual salary. Second, the quality of work life showed a positive correlation with organizational culture, organizational support, personal health, and organizational health in that order. Third, the results of path analysis revealed that organizational culture had a positive effect on organizational support; organizational support and personal health on organizational health; organizational support on personal health; and organizational support and organizational health on quality of work life. In addition, organizational support and organizational health had a direct effect on the quality of work life, while organizational culture, organizational support, and personal health had an indirect effect. These results indicated existence of a relationship among organizational culture, organizational support, organizational health, personal health, and quality of work life. It is necessary to identify ways to improve the quality of work life of dental hygienists.

Analysis on Factors Relating to External Medical Service Use of Health Insurance Patients Using Spatial Regression Analysis (공간효과분석을 이용한 건강보험 환자 관외 의료이용도와 관련된 요소분석)

  • Background: The purpose of this study was to analyze the association between areas of Korea Train Express (KTX) region and external medical service use in Korean society using spatial statistical model. Methods: The data which was used in this study was extracted from 2011 regional health care utilization statistics and health insurance key statistics from National Health Insurance Corporation. A total spatial units of 229 districts (si-gun-gu) were included in this study and spatial area was all parts of the country excepted Jeju, Ulleungdo island. We conducted Kruskal-Wallis test, correlation, Moran's I and hot-spot analysis. And after, ordinary linear regression, spatial lag, spatial error analysis was performed in order to find factors which were associated with external medical service use. The data was processed by SAS ver. 9.1 and Geoda095i (windows). Results: Moran's I of health insurance patients' external medical service use was 0.644. Also, population density, Seoul region, doctor factors positively associated with health insurance patients' external medical service. In contrast, average age, health care organization per 100 thousand were negatively associated with health insurance patients' external medical service use. Conclusion: The finding of this study suggested that health insurance patient's external medical service use correlated for seoul region in korea. The study results imply the need for more attention medical needs in the region (si-gun-gu unit) for health insurance patients of seoul region. It is important to adapt strategy to activation of primary health care as well as enhancing public health institution for prevent leakage of patients to other areas.

The Safety Assessment of Aflatoxins and Deoxynivalenol in Cereals and Their Products (곡류 및 그 가공품 중 아플라톡신과 데옥시니발레놀의 안전성 평가)

  • This study was carried out to assess the safety of cereals and their products (20 species, 205 samples) distributed in Gyeonggi province by analyzing contamination levels such as exposure frequency of Aflatoxins and Deoxynivalenol (DON). Aflatoxins were detected in 16 (8%) samples in total with a range of 0.01~27.88 ${\mu}g/kg$, including 3 of 9 bake flour (33%), 2 of 6 dried corn (33%), 3 of 12 frying flour (25%) and 2 of 11 sorghum (18%). DON was found in 56 (27%) samples including 11 of 13 adlay (85%), 6 of 8 wheat (75%), 6 of 10 processed corn for popcorn (60%), 6 of 11 sorghum (55%) and 7 of 16 barley (44%) with a range of 2.2~754.4 ${\mu}g/kg$. In particular, both Aflatoxins and DON were detected in 8 samples (2 millet, 2 wheat, 1 sorghum, 1 adlay, 1 dried corn, 1 bake flour) simultaneously. As a result of this study, we found that an adult is exposed to Aflatoxins of $0.80({\times})10^{-3}{\mu}g/kg$ b.w./day and DON of 0.18 ${\mu}g/kg$ b.w./day. The quantity of exposure to DON amounted to 18.5 percentage level compared with PMTDI 1 ${\mu}g/kg$ b.w./day suggested on JECFA, therefore we can assess that the possibility of health risks by intake cereals distributed in Gyeonggi province is low. However the concentration of Aflatoxins in one dried corn was 27.88 ${\mu}g/kg$ which was over the maximum residue limits (MRL) suggested on internal and external level, the monitoring about mycotoxin should be conducted on continuously.

Analysis of Research Trends on School Health (최근 학교보건의 연구동향 분석)

  • Purpose: This study was to analyze the trends of research on school health by examining papers related to school health. Method: This studt was analyzed all related papers published in journals concerning school health from 2000 to 2006. The data were processed statistically by frequency percentage and content analysis. Results: Among 463 papers in 6 journals related to school health. 'The Journal of the Korean Society of School Health' had the largest number of articles. Descriptive research design was the most frequently used research design, and students were the most frequent research target. Among research writers, the professor group was largest, and schools were the most frequently used place for research. The most common sampling method was convenient sampling. The most common research instrument was questionnaire survey. Parametric statistics were the main methods of data analysis. Among the themes of school health, the adoption of nursing process frames and assessment domains occupied the largest portion. Health was the most common key concept, and the mainly used theory in the journals was factor-isolating. Conclusion: Through the analysis of research trends, we expect a higher level of school health research.

Network Analysis about Study on the Oral Health of People with Disabilities

  • Background: This study aimed to analyze trends in oral health research among people with disabilities. Methods: Data were obtained from 70 oral studies on individuals with disabilities from 2000 to 2024. Keywords were analyzed. Frequency, betweenness centrality, and cluster analyses were performed using NetMiner. Results: The main keywords for oral health research on disabled people were oral health, dental caries, DMFT (decayed-missing-filled-teeth), dental treatment, oral health centers, and disabled children. As a result of the centrality analysis, DMFT had the highest connectivity, followed by disabled children, special care dentistry, oral health behavior, periodontitis, and health insurance. Cluster analysis results of research on disabled people: Group 1, oral diseases and functions of disabled people; Group 2, oral care for disabled children; Group 3, dental treatment for disabled people; Group 4, oral health policy; Group 5, oral care by dental hygienists; and Group 6, conservative dentistry. Conclusion: Considering the increase in the number of disabled people in a super-aging society, research on ways to promote oral health for disabled people, oral health policies, and training of oral health experts for disabled people is required.

Analysis of Unmet Medical Needs according to Mental Health (정신건강유형에 따른 미충족 의료 현황 분석)

  • Objectives : This study investigated the prevalence of unmet health care needs among Korean adults and related factors. Methods : The study participants were adults over the age of 20 mental health experience from the Korea Health Panel in 2012(n=4,730). Statistical analysis methods used in this study were the ${\chi}^2$-test, Logistic Regression Analysis and other basic statistics such frequency-and percentage using SPSS version 22.0. Results : (1)Significant variables of stress: Factors were age, economic activity, subjective health status, and activity limitation. (2)Significant variables of depression: Factors were age, income class(low) and activity limitation. (3)Significant variables of suicidal impulse: Factors were age, chronic diseases, income class, and activity limitation. Conclusions : Stress, depression, and suicidal impulse can be unmet medical factors; therefore improvement measures and mental health counseling programs in response to suicide impulses, should be developed. Thus there is a need for a health sciences approach.

Determinants of Quality of Life, Depending on the Presence or Absence of Asthma in Adults, Based on the 6th Korea National Health and Nutrition Examination Survey (제6기 국민건강영양조사 자료에 기초한 성인의 천식 유무에 따른 삶의 질 영향요인)

  • Objectives: This study examined the determinants of quality of life, depending on the presence or absence of asthma in adults, based on secondary wave data. Methods: Among the 21,724 people participating in the 6th Korea National Health and Nutrition Examination Survey as it was conducted from the first to third period, 495 participants who were aged 19 or older and responded to the question of the presence or absence of asthma were included in the final analysis. Demographic characteristics were examined using the SPSS/WIN 23.0 software tool for analysis of complex sample survey data. Health-related characteristics were presented using descriptive and multivariate analysis of data. Rao_Scott ${\chi}^2$ was used for the analysis of differences in quality of life, and multiple regression analysis of complex sample survey data was used to analyze factors affecting quality of life. Results: The variable factors negatively influencing quality of life were aging, cognition of their ill health, and limited activities. Conclusions: Based on the analysis, the study suggests that practical and ongoing nursing intervention proposals to improve the quality of life of asthmatic patients should be implemented not only for physical limitations and aging but also for psychological factors that reflect subjective health statuses.

A Study on Factors Affecting the Use of Ambulatory Physician Services (의사방문수 결정요인 분석)

  • In order to study factors affecting the use of the ambulatory physician services. Andersen's model for health utilization was modified by adding the health behavior component and examined with three different approaches. Three different approaches were the multiople regression model, logistic regression model, and LISREL model. For multiple regression, dependent variable was reported illness-related visits to a physician during past one year and independent variables are variaous variables measuring predisposing factor, enabling factor, need factor and health behavior. For the logistic regression, dependent variable was visit or no-visit to a physician during past one year and independent variables were same as the multiple regression analysis. For the LISREL, five endogenous variables of health utiliztion, predisposing factor, enabling factor, need factor, and health behavior and 20 exogeneous variables which measures five endogenous variables were used. According to the multiple regression analysis, chronic illness, health status, perceived health status of the need factor; residence, sex, age, marital status, education of the predisposing factor ; health insurance, usual source for medical care of enabling factor were the siginificant exploratory variables for the health utilization. Out of the logistic regression analysis, health status, chronic illness, residence, marital status, education, drinking, use of health aid were found to be significant exploratory variables. From LISREL, need factor affect utilization most following by predisposing factor, enabling factor and health behavior. For LISREL model, age, education, and residence for predisposing factor; health status, chronic illess, and perceived health status for need factor; medical insurance for enabling factor; and doing any kind of health behavior for the health behavior were found as the significant observed variables for each theoretical variables.
