Microbial isolates and antibiotic sensitivity in patients hospitalized with odontogenic infections at a tertiary center over 10 years

  • Objectives: This study investigated causative strains and their antibiotic sensitivity in patients who were hospitalized for maxillofacial odontogenic infections at a tertiary center in South Korea over the past 10 years with the aim of providing guidelines for the selection of appropriate empirical antibiotics. Materials and Methods: Patients with head and neck fascial space abscesses due to odontogenic infections who underwent incision and drainage surgery with pus culture tests between 2013 and 2022 at the Department of Oral and Maxillofacial Surgery, Dankook University Hospital were included. The bacterial isolates and antibiotic sensitivity of each strain were analyzed for 2013-2022, 2013-2017, and 2018-2022. The affected fascial spaces were classified into primary, secondary, and deep neck spaces. Results: In the 192 patients included in this study, 302 strains were detected. Viridans streptococcus had the highest frequency (51.7%), followed by Prevotella spp. (16.9%), Staphylococcus spp. (5.6%), and Klebsiella pneumoniae (4.6%). The identification rate of viridans streptococcus significantly increased from 41.8% in 2013-2017 to 60.9% in 2018-2022. Viridans streptococcus showed an antibiotic sensitivity of 80.5% to ampicillin; the sensitivity to penicillin antibiotics decreased over the study period. Antibiotic susceptibility was approximately 94% for third-generation cephalosporins. K. pneumoniae, which was identified at a high percentage in patients with deep neck space infection, showed increasing antibiotic resistance to most antibiotics over the study period. Conclusion: Viridans streptococcus was identified in head and neck fascial space abscesses with the highest frequency. Empirical antibiotics should be effective against this strain; penicillin antibiotics are considered inappropriate. For effective treatment of deep neck space abscesses, bacterial culture and antibiotic sensitivity tests performed as soon as possible are essential.

  • This thesis was conducted a numerical analysis on the radiant heat performance according to factors of design of heat sink for cooling of the automotive LED head lamp. The heat sinks were designed with 5 different types to fit the limited internal space by formula based on an existing product (Type 1). Designed heat sinks of five types were analyzed by ANSYS CFD V12.1, and the analysis results were compared with the existing type. The results of simulation were analyzed temperature distribution and average temperature, air flow characteristic, heat flux etc. This thesis was researched on the correlation of the cooling performance according to the heat sink structure and the fin shape. Through numerical analysis, could be confirmed heat sink Type 2 as the best results.

  • In order to improve the fuel economy and dynamic behavior of automobiles, the weight reduction tendency of automobile parts is obvious. Also, in order to maximize assembly and maintenance convenience, various parts are integrated and modularized. Multi-piece methods require many manufacturing processes and become a factor of lowering the strength of parts. It is advantageous to overcome the disadvantages by integrally manufacturing to reduce the processing steps and ensure the strength of the parts. However, when it is necessary to process in a narrow space inside the part, it is impossible to process with the existing spindle. The angle head spindle is only a component of a machine tool, but it is a core part that requires high technology and is highly utilizable in products requiring high precision machining. Therefore, various and continuous studies needs for angle head spindles in areas such as vibration absorption, operational safety, excellent dimensional stability, and strength. In this paper, we propose an optimal design for angle head spindle by performing structural analysis and shape optimization for angle head spindle gear and case.

A Study on the Relationship between Anterior Tooth Contact and Temporomandibular Joint Space (전치부 접촉과 측두하악 관절강폭과의 관계에 관한 연구)

  • The aim of this study was to investigate the relationship between the presence or absence of anterior tooth contact and the changes in temporomandibular joint space. The study sample consisted of 32symptom-free dental students and 79 craniomandibular disorders patients with unilateral joint dysfunction. The two groups were categorized into control group or experimental group, respectively. Recordings of the number and distribution of occlusal contacts were made by T-Scan system. Transcranial radiographs were taken with using of accurad-100 head positioner. Measured items in transcranial radiographs were anterior, superior, posterior joint space and relative condylar position to deepest position of glenoid folla. According to the presence or absence of anterior tooth contact, each group was subdivided and compared with each other with respect to TM joint space. Data were processed and tested with SPSS/PC + package. The results of the study showed that the joint space in control group were wider than those of experimental group and the difference of the width of joint space was more remarkable in subjects with anterior contact between control group and experimental group. However, in same group whether the presence of anterior tooth contact could hardly affect the difference of the width of joint space. And anterior tooth contact in grouip are more frequent than in experimental group.

Therapeutic Singing on Speech Production Parameters in Head and Neck Cancer Patients: Case Studies (치료적 노래부르기를 통한 두경부암 환자의 말산출 기능 향상 사례)

  • This case study investigated the changes in speech intelligibility of patients with head and neck cancers who participated in a therapeutic singing-based intervention. Three patients received a total of twelve 30-minute individual sessions. The intervention consisted of three steps: movements for relaxing breathing muscles, vocalization for increasing the range of articulatory movements, and therapeutic singing. In order to examine the changes in speech intelligibility, the voice quality parameters, diadochokinesis (DDK) and the quadrangle vowel space area (VSA) were measured at pre- and posttest. The recording of what each patient read a written paragraph, which were transcribed by blinded assessors, were also analyzed. The results demonstrated that all of the patients showed positive changes in the voice quality, the rate of repetitive syllable production measured by DDK, and the articulatory working space measured by VSA. Along with these measured changes, increases in positive mood and rehabilitation motivation reported by the patients support that the therapeutic singing-based intervention could induce meaningful changes in terms of speech intelligibility from patients with head and neck cancers. Given that this study was conducted with a small sample size, suggestions for further investigation on the effects of the intervention were also presented.

Verification of the Star Tracker Sun Exclusion Angle of GEO-KOMPSAT-2A Through In-Orbit Operation (천리안 2A호 별추적기 태양 차폐각 궤도상 운영 검증)

  • The star tracker detects microscopic star light in space and compares it with a stored list of stars to calculate the satellite's position in the inertial coordinate system. If other light, such as the sun or the earth, enters the optical head, the star cannot be recognized and the star tracker cannot be operated. In particular, strong light such as the sun affects not only operation but also the performance of the star tracker. The sun exclusion angle of the star tracker is one of the important factors determining the performance of the star tracker. This paper performs the verification of the star tracker's sun exclusion angle. In order to verify the sun exclusion angle, we predict the sun exclusion time of the star tracker and compare it to the actual sun exclusion time of the GEO-KOMPSAT-2A star tracker. In addition, the performance of the star tracker is analyzed for normal operations against the sun exclusion in the optical head. It shows that the actual sun exclusion is maintained under the range of 26 degrees, the performance requirement of the star tracker, and the star tracker operates normally in spite of the sun exclusion.

  • Clustering techniques in wireless sensor networks is developed to minimize the energy consumption of node, show the effect that increases the network lifetime. Existing clustering techniques proposed the method that increases the network lifetime equalizing each node's the energy consumption by rotating the role of CH(Cluster Head), but these algorithm did not present the resolution that minimizes the energy consumption of neighboring nodes with sink. In this paper, we propose the clustering algorithm that prolongs the network lifetime by not including a part of nodes in POS(Personal Operating Space) of the sink in a cluster and communicating with sink directly to reduce the energy consumption of CH closed to sink.

  • Mixed Reality (MR) shows a composite image of a virtual object in the real world. It has been applied to various fields by the introduction of head mounted display (HMD) such as Microsoft's Hololens [1-3]. The virtual studio in broadcasting combines the contents created by computer graphics with the actual set to reproduce the 3D image screen. This requires physical space such as a set of chroma keys. It also requires professional knowledge and manpower and costly equipment to post-process the graphics and information for long periods of time. Therefore, in this paper, we aim to study the implementation of virtual studio based on Mixed Reality using Microsoft Hololens. Through the implementation of 'Holo-studio' application, realistic and virtual objects of broadcasting camera viewpoint were acquired at the same time. Using Microsoft's spectator view library, the frame rate is degraded in objects with high polygons (100,000 polygons). The proposed method maintains 60 fps image transmission in high polygon objects. The results of this paper show the possibility of using virtual studio at low cost which does not need separate physical space.
