Hemorrhagic Complications of Intracranial Arachnoid Cyst Following Minor Head Injury : Report of 5 Cases

  • Intracranial arachnoid cyst is presumed to be a developmental anomaly and its natural history is not well defined. Often it is detected incidentally in a asymptomatic patient and hemorrhagic events of arachnoid cyst following head injury are rarely reported. We report hemorrhagic complications including two intracystic hemorrhages, two subdural hematomas, and an epidural hematoma in 5 patients with intracranial arachnoid cyst after minor head injury and review pertinent literatures.

  • The Neuropsychiatric assessment and management of postconcussion syndrome(PCS), the most prevalent and controversial neuropsychiatric sequelae of traumatic brain injury, were reviewed. First, the definition and general concept of postconcussion syndrome Were summerized. This summary was followed by an overview of the clinical manifestation including cognitive, somatic, and behavioral components of PCS. Next, neuropsychological findings related to PCS were presented. Finally, the treatment issues including psychotherapy and pharamacotherapy were briefly summerized.

  • In this study, for a safety assessment of wheelchair occupant in side impact, we used a dynamic sled impact test results. The test was carried out total 6 times and impact speed was 13g$\pm$0.43/28km/h$\pm$0.95, By using EURO SID-1 dummy, head performance criteria(HPC), abdominal peak force, etc. were measured. We evaluated wheelchair occupant safety by motion criteria(MC) which was measured by head, trunk and side deformation change of wheelchair and Head & Neck injury criteria(HNI) measured by using head and neck deformation angle and time relation. When we assumed that the maximum injury value in side impact was 100%, the results of motion criteria(MC) of wheelchair occupant were max 80.3, mim 32.3 and average 60.3%, Head & Neck injury criteria(HNI) value were max 118.4, min 14.5 and average 59.7%.

  • The driving position of head restraints and the relative risk of neck injury were studied in the computer simulation. MADYMO human model with the detail neck model was used to define the magnitude and direction of internal forces acting on the cervical spine during rear-end impact and to determine the effect of the initial position of the occupant's head with respect to the head restraints. Maximum reaction forces were generated during the head contact to the restraint and relatively large forces were generated at each spinal components in lower cervical spine in proportion to backset and height distance increasement.

Predictors of Outcome in Management of Paediatric Head Trauma in a Tertiary Healthcare Institution in North-Central Nigeria

  • Objective : Trauma is a leading causes of death and disability in all ages. The aim of this study was to describe the demography and characteristics of paediatric head trauma in our institution and examine the predictors of outcome and incidence of injury related mortality. Methods : We examined our institutional Trauma Registry over a 2 year period. Results : A total of 1100 trauma patients were seen over the study period. Of the 579 patients who had head injury 99 were in the paediatric age group. Of the paediatric head trauma patients 79 had documented Glasgow coma score (GCS), 38 (48.1%), 17 (21.5%) and 24 (30.4%) had mild, moderate and severe head injury respectively. The percentage mortality of head injury in the paediatric age group was 6.06% (6/99). There is an association between mortality and GCS (p=0.008), necessity for intensive care unit (ICU) admission (p=0.0001), associated burns (p=0.0001) and complications such as aspiration pneumonia (p=0.0001). The significant predictors of outcome are aspiration (p=0.004), the need for ICU admission (p=0.0001) and associated burns (p=0.005) using logistic binary regression. During the study period 46 children underwent surgical intervention with extradural haematoma 16 (34.8%), depressed skull fracture 14 (30.4%) and chronic subdural haematoma five (10.9%) being the commonest indication for surgeries. Conclusion : Paediatric head injury accounted for 9.0% (99/1100) of all trauma admissions. Majority of patients had mild or moderate injuries. Burns, aspiration pneumonitis and the need for ICU admission were important predictors of outcome in children with traumatic brain injury.

  • 1. Objectives The purpose of this case is to report that a Soeumin patient with cognitive disorder from traumatic head injury needed to be managed with the consideration of the difference of Nature and Emotion(性情). 2. Methods To evaluate the patient's cognition disorder and mental state, we performed the MMSE(Mini-Mental State Examination) and the MMPI(Minnesota Multiphasic Personality Inventory). and he was diagnosed as Soeumin and treated by Kwakhyangchungki-san(藿香正氣散) mainly. 3. Results and Conclusions (1) In this case, we observed specifically-biased Nature and Emotion(性情) of Soeumin like ‘恒欲處而不欲出’, ‘恒欲爲雌而不欲爲雄’, ‘喜別人之助己也’. ‘喜好不定’, ‘不安定之心’ (2) The management of Soeumin with cognitive disorder from traumatic head injury is that he or she should live together his or her family, and have an active manner with a generous and broad-minded mental attitude. (3) A therapist must give a patient mental support and inspire him or her with courage. (4) The deviation of Nature and Emotion(性情) should be taken into consideration when we manage the patient with cognitive disorder from traumatic head injury.

동승석 에어백 핵심 성능 인자 및 상해위험도 예측 기법 개발에 대한 연구 (A Study on the Key Performance Factors of Passenger Airbag and Injury Risk Prediction Technique Development)

  • Until now, passenger airbag design is based on the referred car design and many repetitive crash tests have been done to meet the crash performance. In this paper, it was suggested a new design process of passenger airbag. First, key performance factors were determined by analyzing the injury risk effectiveness of each performance factor. And it was made a relationship between injury risk and performance factor by using the response surface model. By using this one, it can be predicted the injury risk of head and neck. Predicted injury risk of optimal design was obtained through this injury risk prediction model and it was verified by FE analysis result within 18% error of head and 9% error of neck. It was shown that a target crash performance can be met by controlling the key performance factors only.

  • Purpose: Diffuse axonal injury (DAI) is clinically defined as a coma of over six hours in a head trauma victim without a focal mass lesion. The emergency physician usually resuscitates and stabilizes a comatose head trauma victim in the emergency Department. After assessment and treatment, the prognosis is very important to both the victim and the physician. The prognosis for DAI is based on Glasgow Coma Scale (GCS) and other imaging data. We investigated the prognostic value of computed tomography (CT) and gradient-echo magnetic resonance imaging (GRI) for head trauma victims with DAI. Methods: Fifty-three(53) head trauma victims of DAI were enrolled in this study from 2007 to 2012. During the study period of six years, data on trauma victims were collected retrospectively. We analyzed the differences in the Glasgow Outcome Scale (GOS) result between the CT and the GRI modalities. Results: We classified the study group by using GOS. Between the good outcome subgroup (GOS scores of 4 and 5) and the poor outcome subgroup (GOS score of 1-3), there were no statistical difference in sex, age, initial vital signs and initial GCS score. The good outcome subgroup had non-hemorrhage on CT(52%), which was correlated with good outcome and a shorter awakening time, while a larger number and a deeper location of hemorrhagic lesions on in GRI were correlated with poor outcome in DAI. Conclusion: We conclude that the existence of hemorrhagic lesions on CT, and the number and location of those lesions on GRI had good prognostic value for head trauma victims with DAI.

  • According to the IIHS (Insurance Institute fur Highway Safety), side impacts are made up 30% of all accidents (reported 1998). In the case of auto accidents, head and neck injuries were most common as 58%, injuries to the body's trunk equaled 32%, and injuries to the abdomen were 21%. Therefore in this study, injury of wheelchair occupant in frontal and side impact of wheelchair loaded vehicle was analyzed using computer simulation method. The occupant was restrained at the rear of wheelchair by the lap belt. The detailed fixation and restrain conditions of the wheelchair occupant are referred to SAR J2249's recommendation. We estimated HIC(Head Injury Criteria) and HNIC (Head and Neck Injury Criteria) based on measured data.

경도 두부외상 환자에서 뇌좌상주위 부종영역에서의 자기공명분광법을 이용한 대사변화 (Metabolic Changes in Pericontusional Edematous Areas in Mild Head Injury Evaluated by Proton MRS)

  • Objectives : In order to evaluate the metabolic changes associated with pericontusional edematous area in mild head injury, proton magnetic resonance spectroscopy(1H-MRS) was performed in mild head injury patients (initial GCS score 13-15) with focal brain contusion. Methods : Seven head injury patients with initial GCS 13-15(3 males and 4 females : age range 15-65 years, mean age 33 years) have underwent 1H-MRS evaluations. The patients were examined within 7 days after injury(n=7) and 2 months after injury(n=5). The region of interest(ROI) was selected in the edematous area adjacent to traumatic brain contusion upon T2-weighted MR images and a corresponding region of the contralateral hemisphere (ROC, region of contralateral corresponding hemisphere) was examined as well. The metabolic ratios of NAA/Cr and lactate/Cr were compared between ROIs, ROCs and control values. Results : In initial NAA/Cr ratios, the values of ROIs were significantly lower than those of the controls(p=0.009), but there was no difference either between ROIs and ROCs(p=0.410) or between ROCs of patients and the control (p=0.199). In lactate/Cr ratios, the ROIs in all seven patients and the ROCs in two showed increased lactate signals. The lactate/Cr ratios of the ROIs were significantly elevated as compared to those of the ROCs(p=0.02) and the control(p=0.015). In two months follow-up, lactate signals were absent or significantly reduced(p=0.015). In no patients, clinical or radiological deterioration has been observed. Conclusion : Our 1H-MRS results demonstrate that there are significant ischemic changes in pericontusional edematous areas as indicated by elevated lactate signals in the patients with mild head injury. But there were no consistent neural loss or dysfunction in these area. There findings suggest that pericontusional edematous areas can be vulnerable to secondary brain insults even in the patients with mild head injury.
