Blood Pressure and the Risk of Death From Non-cardiovascular Diseases: A Population-based Cohort Study of Korean Adults

  • Objectives: The objective of this study was to assess the relationship between systolic and diastolic blood pressure (SBP, DBP) and the risk of death from specific causes other than cardiovascular diseases. Methods: We calculated the risk of specific death by SBP and DBP categories for 506 508 health examinees in 2002-2003 using hazard ratios (HRs) and 95% confidence intervals (CIs) in a Cox proportional hazards model. Results: Compared to normal levels (SBP <120 or DBP <90 mmHg), stage I systolic and diastolic hypertension (SBP 140-159, DBP 85-89 mmHg, respectively) were associated with an increased risk of death from diabetes mellitus, alcoholic liver disease, and renal failure (HR, 1.83; 95% CI, 1.51 to 2.22; HR, 1.24; 95% CI, 1.06 to 1.46; HR, 2.30; 95% CI, 1.64 to 3.21; HR, 1.67; 95% CI, 1.27 to 2.20; HR, 1.99; 95% CI, 1.41 to 2.81; HR, 1.31; 95% CI, 0.99 to 1.73, respectively), but a decreased risk of death from intestinal pneumonia (HR, 0.64; 95% CI, 0.42 to 0.98; HR, 0.59; 95% CI, 0.39 to 0.91). Only stage II systolic hypertension (SBP ${\geq}160mmHg$) was associated with an increased risk of death from pneumonia, liver cirrhosis, and intestinal ischemia (HR, 1.54; 95% CI, 1.19 to 1.98; HR, 1.46; 95% CI, 1.00 to 2.15; HR, 3.77; 95% CI, 1.24 to 11.40, respectively), and stage I and II diastolic hypertension (SBP 140-159 and ${\geq}160mmHg$) were associated with an increased risk of death from intestinal ischemia (HR, 3.07; 95% CI, 1.27 to 7.38; HR, 4.39; 95% CI, 1.62 to 11.88, respectively). Conclusions: An increase in blood pressure levels may alter the risk of death from certain causes other than cardiovascular diseases, a well-known outcome of hypertension, although the mechanism of these associations is not well documented.

Risk Assessment for Disaster Reduction in Small-Scale Construction Sites (소규모 건축현장 재해감소를 위한 위험성평가 방안)

  • Purpose: Small-scale construction sites have insufficient systematic safety management activities, and due to the characteristics of the construction site, the production structure is complex due to external environmental factors, and the risk of construction equipment is very high. We would like to propose a checklist method among practical risk assessment techniques that can derive risk factors for disaster prevention at small construction sites and reduce disasters. Method: Risk factors were derived by analyzing literature research and disaster cases, and detailed work for a checklist of risk assessment suitable for small-scale construction sites was classified based on risk factor items. Result: Hazard factors were divided into 6 major categories, and 29 detailed types of work were classified based on actual work types, and 80 detailed works were classified accordingly. Conclusion: By arranging risk factors suitable for small-scale construction sites according to the classification system, the lack of expertise in the construction site can be supplemented, and risk factors can be derived more easily and disaster reduction can be expected through establishment of safety measures.

Telecommunication Service Usage as Predictor of the Timing of Handset Buyers' Replacement Purchases (통신서비스 이용행태 분석을 통한 휴대폰 교체기간 예측)

  • With the explosive growth of mobile products industry, tons of newer versions of products are putting on the market. From the marketer's perspective, understanding consumers' replacement purchases, especially the replacement timing, is essential to product planning and selling. This study presents an approach to finding out factors influencing the timing of buyers' replacement purchases of cell phones, using duration analysis; a hazard function specification is applied to describe consumers' replacement timing decision. Based on the data collected from a mobile telecommunication company, five categories of factors have been inspected. These are consumer's innovative service usage, data service usage, voice service usage, participation in loyalty programs, and the demographic characteristics. The results of the study are as follows. Firstly, the positive coefficient of 'the number of related services used' suggests that the consumers who have more usage knowledge tend to replace faster. Secondly, customers participating in the membership service are positively associated with early replacement purchases. Lastly, younger customers(vs. older) and male(vs. female) customers turned out to replace cell phones earlier.

Analyzing the Status of Industrial Accidents and Investigation of Improve Methods of Hazard Factors in Franchise Industry (프랜차이즈 산업의 산업재해 현황 및 유해위험 요인에 대한 개선방안 조사)

  • Article 79 of the Occupational Safety and Health Act mandates franchise headquarters with over 200 franchises to implement industrial accident prevention measures. Notably, the franchise business information provision system designates restaurants and wholesale/retail as the primary categories for information disclosure, with convenience stores classified as secondary. This study aims to identify law-regulated franchise headquarters and franchises and analyze industrial accidents within these sectors. Furthermore, it seeks to investigate adverse risk factors for individual processes across major industries. Pertinent franchise headquarters and franchises were discerned using the Fair Trade Commission's business information system. Data regarding the status of industrial accidents, as published by the Ministry of Employment and Labor, was leveraged in the analysis of industrial accidents. Additionally, a survey was conducted to derive detrimental factors for each process in major industries. The inquiry revealed a total of 7 wholesale and retail franchise headquarters subject to the law, along with 126 within the restaurant industry. Overall, there were 50,488 franchises in the wholesale and retail sector, and 71,283 in the restaurant business. Risk factors and improvement measures for industrial processes were determined for five industries: convenience stores, Korean food restaurants, coffee places, pizza eateries, and chicken vendors. Locating the currently developed safety and health program establishment guide proves to be challenging. Moreover, utilizing it can be difficult due to the industry-specific content, further complicated by the prevalence of franchise stores featuring numerous small businesses. Therefore, this study highlights the imperative need to develop guidelines that incorporate preventive measures tailored to each industry.

A qualitative content analysis based on an extended parallel process model study of daycare center teacher behaviors concerning the eye health of preschool children (어린이집 교사 대상 학령전기 아동의 눈건강에 대한 확장된 병행과정 모델 기반 질적 내용분석 연구)

  • Purpose: This study is to explore the antecedent factors of daycare teacher behaviors concerning the eye health of preschool children by applying an extended parallel process model. Methods: Focus group interviews were conducted with ten daycare center teachers on September 4 and 14, 2023. A data analysis was performed according to the content analysis method by clustering the data into the four categories: the two threat factors of severity and susceptibility and the two efficacy factors of self-efficacy and response-efficacy. Results: Daycare center teachers' perception of the severity of eye health problems in preschool children was high in relation to eye trauma, but it was recognized that viewing the electronic devices were of a less severe because symptoms were not noticed in a short period of time. They also showed low susceptibility because they were not sufficiently interested in the eye health hazard behaviors of preschool children. The self-efficacy of daycare center teachers was low because this was a lack of knowledge about symptoms of eye problems. However, they recognized that eye health activities performed in the preschool age could prevent negative eye health outcomes, thus showing a high response efficacy. Conclusion: In the future, it is necessary to increase the sensitivity and engagement of daycare center teachers concerning with the eye health of preschool children and to increase their self-efficacy. It will also be necessary to develop various interventions to improve eye health for preschool children that can be implemented by daycare center teachers.

Effects of Private Insurance on Medical Expenditure (민간의료보험 가입이 의료이용에 미치는 영향)

  • Nearly all Koreans are insured through National Health Insurance(NHI). While NHI coverage is nearly universal, it is not complete. Coverage is largely limited to minimal level of hospital and physician expenses, and copayments are required in each case. As a result, Korea's public insurance system covers roughly 50% of overall individual health expenditures, and the remaining 50% consists of copayments for basic services, spending on services that are either not covered or poorly covered by the public system. In response to these gaps in the public system, 64% of the Korean population has supplemental private health insurance. Expansion of private health insurance raises negative externality issue. Like public financing schemes in other countries, the Korean system imposes cost-sharing on patients as a strategy for controlling utilization. Because most insurance policies reimburse patients for their out-of-pocket payments, supplemental insurance is likely to negate the impact of the policy, raising both total and public sector health spending. So far, most empirical analysis of supplemental health insurance to date has focused on the US Medigap programme. It is found that those with supplements apparently consume more health care. Two reasons for higher health care consumption by those with supplements suggest themselves. One is the moral hazard effect: by eliminating copayments and deductibles, supplements reduce the marginal price of care and induce additional consumption. The other explanation is that supplements are purchased by those who anticipate high health expenditures - adverse effect. The main issue addressed has been the separation of the moral hazard effect from the adverse selection one. The general conclusion is that the evidence on adverse selection based on observable variables is mixed. This article investigates the extent to which private supplementary insurance affect use of health care services by public health insurance enrollees, using Korean administrative data and private supplements related data collected through all relevant private insurance companies. I applied a multivariate two-part model to analyze the effects of various types of supplements on the likelihood and level of public health insurance spending and estimated marginal effects of supplements. Separate models were estimated for inpatients and outpatients in public insurance spending. The first part of the model estimated the likelihood of positive spending using probit regression, and the second part estimated the log of spending for those with positive spending. Use of a detailed information of individuals' public health insurance from administration data and of private insurance status from insurance companies made it possible to control for health status, the types of supplemental insurance owned by theses individuals, and other factors that explain spending variations across supplemental insurance categories in isolating the effects of supplemental insurance. Data from 2004 to 2006 were used, and this study found that private insurance increased the probability of a physician visit by less than 1 percent and a hospital admission by about 1 percent. However, supplemental insurance was not found to be associated with a bigger health care service utilization. Two-part models of health care utilization and expenditures showed that those without supplemental insurance had higher inpatient and outpatient expenditures than those with supplements, even after controlling for observable differences.

Application of Spatial Analysis Modeling to Evaluating Functional Suitability of Forest Lands against Land Slide Hazards (공간분석(空間分析)모델링에 의한 산지(山地)의 토사붕괴방재기능(土砂崩壞防災機能) 적합도(適合度) 평가(評價))

  • The objective of this study is to develop a spatial analysis modeling technique to evaluate the functional suitability of forest lands for land slide prevention. The functional suitability is classified into 3 categories of high, medium and low according to the potential of land slide on forest lands. The potential of land slide hazards is estimated using the measurements of 7 major site factors : slope, bed rock, soil depth, shape of slope, forest type and D.B.H. class of trees. The analytic hierarchical process is applied to determining the relative weight of site factors in estimating the potential of land slides. The spatial analysis modeling starts building base layers for the 7 major site factors by $25m{\times}25m$ grid analysis or TIN analysis, reclassifies them and produces new layers containing standardized attribute values, needed in estimating land slide potential. To these attributes, applied is the weight for the corresponding site factor to build the suitability classification map by map algebra analysis. Then, finally, cell-grouping operations convert the suitability classification map to the land unit function map. The whole procedures of the spatial analysis modeling are presented in this paper.

Percentile Approach of Drought Severity Classification in Evaporative Stress Index for South Korea (Evaporative Stress Index (ESI)의 국내 가뭄 심도 분류 기준 제시)

  • Drought is considered as a devastating hazard that causes serious agricultural, ecological and socio-economic impacts worldwide. Fundamentally, the drought can be defined as temporarily different levels of inadequate precipitation, soil moisture, and water supply relative to the long-term average conditions. From no unified definition of droughts, droughts have been divided into different severity level, i.e., moderate drought, severe drought, extreme drought and exceptional drought. The drought severity classification defined the ranges for each indicator for each dryness level. Because the ranges of the various indicators often don't coincide, the final drought category tends to be based on what the majority of the indicators show and on local observations. Evaporative Stress Index (ESI), a satellite-based drought index using the ratio of potential and actual evaporation, is being used as a index of the droughts occurring rapidly in a short period of time from studies showing a more sensitive and fast response to drought compared to Standardized Precipitation Index (SPI), and Palmer Drought Severity Index (PDSI). However, ESI is difficult to provide an objective drought assessment because it does not have clear drought severity classification criteria. In this study, U.S. Drought Monitor (USDM), the standard for drought determination used in the United States, was applied to ESI, and the Percentile method was used to classify drought categories by severity. Regarding the actual 2017 drought event in South Korea, we compare the spatial distribution of drought area and understand the USDM-based ESI by comparing the results of Standardized Groundwater level Index (SGI) and drought impact information. These results demonstrated that the USDM-based ESI could be an effective tool to provide objective drought conditions to inform management decisions for drought policy.

Preliminary Estimation of Earthquake Losses Based on HAZUS in a Coastal Facility Area with Blocks Applying Site Classification (블록별 부지분류 적용 해안시설 영역에서의 HAZUS 기반 지진피해 추정)

  • HAZUS-MH is a GIS-based computer program that estimates potential losses from multi-hazard phenomena: earthquakes, floods and hurricanes. With respect to seismic disaster, characteristics of a hypothetical or actual earthquake are entered into HAZUS. Then HAZUS estimates the intensity of ground shaking and calculates the correspondent losses. In this study, HAZUS was used as a part of the preparations of the future seismic events at a coastal plant facility area. To reliably characterize the target facility area, many geotechnical characteristics data were synthesized from the existing site investigation reports. And the buildings and facilities were sorted by analyzing their material and structural characteristics. In particular, the study area was divided into 17 blocks taking into account the situation of both land development and facility distribution. The ground conditions of blocks were categorized according to the site classification scheme for earthquake-resistant design. Moreover, seismic fragility curves of a main facilities were derived based on the numerical modeling and were incorporated into the database in HAZUS. The results estimated in the study area using HAZUS showed various seismic damage and loss potentials depending on site conditions and structural categories. This case study verified the usefulness of the HAZUS for estimating earthquake losses in coastal facility areas.

Persistence of Employment Types (취업형태의 지속성에 관한 연구)

  • This paper uses the Korean Labor Panel data to investigate changes in the employment types of male workers following their job changes with the classification of workers into three categories: regular wage workers, non-regular wage workers, and self-employed workers. It also estimates a competing-risks hazard model to analyze the determinants of employment types of workers. The results show that the type of employment of a worker at an immediate previous job has a critical importance in determining his employment type at a new job and that the types of employment at jobs other than the immediate previous job also play some role in determining the type of employment at a new job, although their impact declines as the number of intervening jobs increases. A job loser, who worked as a non-regular worker at his immediate previous job, for example, is considerably less likely to find a regular job, but more likely to get reemployed at another non-regular job than one who worked as a regular worker at his immediate previous job. Similarly, a worker who quit self-employment is much less likely to find a regular job but more likely to restart his own business than one who worked as a regular worker at his immediate previous job. These findings suggest that it is not easy at all for a worker who worked as either a non-regular worker or self-employed worker to become a regular worker, although it might be premature to assert that non-regular jobs or self-employed jobs are dead-end jobs. Another interesting finding of this analysis is that a high unemployment rate lowers a probability of reemployment at either regular jobs or self-employed jobs, but raises a non-regular job reemployment probability, which strongly implies that as labor market conditions become adverse to workers the proportion of non-regular employment can rise rapidly.
