Epidemiological Study of Laryngeal Carcinoma in Western Nepal

  • Background: Laryngeal malignancy is a common malignancy of the head and neck region. Affected patients usually present with features that are characteristic of certain subsites. The larynx is oncologically divided into three: supraglottis, glottis and subglottis. Studies from Western countries have shown that the glottis is the commonest subsite to harbour laryngeal malignancy. However, the supraglottis has been reported to be the commonest subsite in developing countries, including examples in the Indian subcontinent. To our knowledge, no study has been carried out in western Nepal about the epidemiology of laryngeal cancer to date. The purpose of this study was to analyse the epidemiology of laryngeal cancer in relation to its risk factors, age distribution,and preferred subsites and to see if there is any recent change in the subsite wise distribution of laryngeal malignancy in western Nepal. Materials and Methods: Patients of all ages and both sexes with suspected laryngeal malignancy were enrolled in the initial study. Detailed history taking and clinical examination was performed to find out the involved subsite in relation to the clinical features. Direct laryngoscopy was performed to further confirm the subsite and to take biopsy from the growth under general anesthesia. After confirmation of malignancy from the biopsy report, patients were finally included in the study. Data were analysed and observations were made to find out the distribution of laryngeal malignancy in different subsites. Results: The supraglottic larynx was the commonest subsite to harbor laryngeal malignancy. Smoking and alcohol were found to be the common risk factors. The mean age of the patients was in their sixties. Conclusions: Laryngeal malignancy is common in elderly individuals. Supraglottic laryngeal malignancy is the commonest laryngeal malignancy in people who smoke and drink alcohol in Nepal. Avoidance of alcohol use and smoking will be a milestone to reduce the incidence of laryngeal cancers and associated mortality.

A Basic Study on the Accident Prevention Measures of Maritime Pilots during Embarkation and Disembarkation (도선사 승·하선 사고예방 방안에 관한 기초연구)

  • An accident during pilotage may cause several problems such as crippling the harbour function, environmental disruption, loss of life. Since the pilot plays an important role in the safe operation of the port, securing their safety is directly connected to the vessel's safety coming into port in Korea. Pilotage service is offered on request without distinction of day and night. And traveling by helicopter and boarding/disembarking using a pilot ladder from and to a pilot boat have a high risk of falling into the sea. So, pilots can fall into the sea while boarding/disembarking from ship which can lead to serious injury by cardiac arrest. A number of pilots have died or severely injured due to fall from the pilot ladder during embarkation and disembarkation up to now. Current safety measures at embarkation and disembarkation required by pilotage act and IMO resolutions could not prevent all the pilot related accidents. Therefore, to prevent pilot accident during boarding/disembarking from ship, it is necessary to include safety training requirement for new pilots under the requirements for licensing of pilotage act. It is also necessary to compel pilots to take necessary personal protective equipment when they use a pilot ladder. In addition, the placement of emergency kit on pilot boats and first-aid training are required in order to deal with emergency situation promptly.

  • A historical tragic disaster happened by capsizing the passenger ship Sewol at South Western Sea of Korea in 16, April 2014. The ship which left Incheon harbour to bound for Jeju port passed Maengol strait and reached to approach of Byung Pung island, and then capsized and sank with a sudden inclination to the portside in the mean time of starboard the helm. In this time, the ship which has very poor stability without sufficient ballast waters and with over loading cargo listed port side caused by the centrifugal force acting to the outside of turning. A lot of cargoes not fastened moved to the port side consequently, and the ship came to beam end to capsize and sank in the end. No crews including especially captain would offer their own duties in a such extremely urgent time, as a result, enormous number of victims broke out including a lot of student. In this report, author carried out some calculation on the factors which influenced on the stability of the ship, i.e. the ship's speed, the rudder angle, the weight of cargoes and distance of movement, the surface effect of liquid in the tank. We found out that the most causes of capsize were the poor stability with heavy cargoes and insufficient amount of ballast water against the rule, and the cargoes unfastened moved one side to add the inclination as well. Above all, the owner be blamable because of the illegally operating the ship without keeping the rule.

Wave control fuction and friction damping of a pile-supported floating body (말뚝계류식 부유체의 파랑제어 기능과 마찰감에 관한 연구)

  • The floating body discussed in this study is a 2-D rectangular floating unit supported by four vertical piles at its corners. Structures of this type are frequently seen as floating piers for the crafts in a small harbour. The movement in some modes of motion of such a flating body is fully or partially restrincted by the piles. The authors(Kim et al. 1994) carried out a series of model tests on its wave control function, its motion and the loads on piles. The experimental results showed that a certain degree of intial constriction force which clamps the floating unit in the horizontal direction can effectively reduce the body motion and wave energy without increasing mooring forces. This may be due to the friction forces occuring between the piles and the rollers installed in the mooring equipments on the floating unit. In this paper, we develop a numerical model for the prediction of wave transformation and floating body motions, where the friction force is idealized as the Coulomb friction and linearized into a damping force using the equivalent damping cofficient. This linearization is verified by comparing the results of motions between the linear and nonlinear analysis of the ezuations of motion. We further compare the caculation results by the linear model with the experimental results and discuss the effect of the friction force or the constriction force on body motions and wave energy dissipation.

Effective Analysis Framework for Construction Post Evaluation (건설공사 사후평가 수행결과의 효과적인 분석방법)

  • A construction post evaluation is introduced to provide information that can be used for similar construction projects in the future through the evaluation of the efficiency of a public construction project and the verification of the appropriateness of the project results. However, it is not widely used due to a lack of awareness of the method. For this reason, the aim of this study is to provide a basic frame that can be utilized as a reference for a similar project in the future based on the construction post evaluation. More specifically, a more effective analysis method for the construction post evaluation is proposed and then an exemplary analysis is performed based on the method. The paper analyzes 293 cases stored in construction CALS. Construction cost growth rates of railway projects are tend to be greater than other project types like highway, expressway, and harbour. Then, construction schedule grwoth rates of expressway are greater than others. This is expected to provide useful information for a future project, and serve as a basic frame. Moreover, it is also meaningful that a project can be analyzed from different perspectives through the construction post evaluation.

  • The aim of this study is to forcast the economic impact of Yosu-Kwangyang harbour on the regional employment. Industries in relation to this harbor can be divided into three parts; related industries to harbor(RI), direct dependent industries on harbor(DDI), indirect dependent industries(IDI). Research regions are 4 cities (Yosu, Yochun, Soonchun, Dongkwangyang) and 3 counties (Yochun, Seungju, Kwangyang).In the aspect of employment, the Yosu-Kwangyang harbor produced a good effect in these regions. In 1995, the rates of the dependence on harbor were showed up RI 1.63%, DBI 3.33%, IDI 15.99%, TOTAL 19.95%. In the Suture, the rates may be showed up various results as; in TOTAL rates, 33.59% in 2000, 44.95% in 2005, 48.03% in 2010, 49.82% in 2015.

A Study on the Drift Phenomenon of a Ship on the Waterway near the Breakwater of Busan and Gamcheon Port (부산항과 감천항 방파제 인접 항로에서 선체에 작용하는 횡압류 현상에 관한 연구)

  • In order to secure the traffic safety especially in the entrance waterway of harbour, it is important that the breakwater and the port facilities should be designed properly considering ship-handling difficulty and traffic flow. In this study, the lateral force acting on ship hull under the external force(wind, current, wave) is investigated quantitatively for the container ship approaching to the Busan and Gamcheon breakwater. The relation of ship-handling difficulty to the breakwater and the arrangement of ship's routine are examined based on the lateral force under the external force. Some of reviews to secure traffic safety on the design of breakwater are discussed.

ASF Measurements on Maritime by the Signal of the Pohang Loran-C (9930M) (포항 로란-C (9930M) 신호를 이용한 ASF 해상측정)

  • A significant factor limiting the ranging accuracy of Loran (Long Range Navigation) signal is the additional secondary factor (ASF) in the time of arrival (TOA) measurements. Precise ASF values are essential if Loran deliver the high absolute accuracies demanded for aircraft approach, maritime harbour entrance. We measured the absolute propagation delay between Pohang Loran signal and Loran receiver output signal by comparing with Cesium atomic clock. In this study we measured ASFs between Pohang 9930M station and the 12 measurement points in the Yeongil Bay by using the measurement technique of absolute time delay. The measurement points were spaced at interval of 3 km by 3 km. An E-field antenna and an H-field antenna were used to improve the accuracy of ASF measurements and a DGPS (Differential GPS) receiver was used for accurate positions. We have gotten the result that the measured ASFs were compared with the predicted ASFs through this measurement technique.

  • To prevent accidents and take immediate actions to minimize the damages caused by such disasters, there have been several information system related to marine transportation in or near harbour areas and coastal waters. However, those systems were operated independently and not able to exchange each others important data necessary for the cooperation of the relevant authorities in case of accidents. Standards of data formats and their exchanges needed to make information systems play efficiently their roles are proposed in this paper, and the proposed method is expected to help manage the marine transportation in the waters of jurisdiction and prevent marine accidents which cause huge amounts of casualties in the shipping industries and the natural environments.

An Epidemilolgical Survey of Echinostoma hortense Infection in Chongsong-gun, Kyongbuk Province (경북 청송군 일부 기역의 호르틴스극구흡충 유행양상)

  • An epidemiological study was performed to know the infection status of 5. hortense among the human residents and fish intermediate hosts in Chongsong·gun, Kyongsangbuk·do. Out of 263 people examined, 59(22.4%) were positive for the eggs of E. hortense. Total 152 fresh water fishes (9 species) were ellamined, and Odentobutis obscura interrupts, Misgurnus an- guiIZicaudatus and Squalidus coreanus were found to harbour the metacercariae of E. hortense. Juvenile or adult E. hertense were expelled from 35 out of 54 cases treated with pragiquantel. The number of worms Per individual ranged from 1 to 649, with the average burden of 51. The eosinophil count of infected persons ranged 1∼24% (6% in average). The number of eggs per day Per worm (EPDPW) in human host was estimated about 1,478. The results showed that Chongsong-gun is an endemic area of E. hortense.
