Our purpose is to present a novel technique for delivering craniospinal irradiation in the supine position using a perfect match, field-in-field (FIF) intrafractional feathering, and simple forward-optimization technique. To achieve this purpose, computed tomography simulation was performed with patients in the supine position. Half-beam, blocked, opposed, lateral, cranial fields with a collimator rotation were matched to the divergence of the superior border of an upper-spinal field. Fixed field parameters were used, and the isocenter of the upper-spinal field was placed at the same source-to-axis distance (SAD), 20 cm inferior to the cranial isocenter. For a lower-spinal field, the isocenter was placed 40 cm inferior to the cranial isocenter at a constant SAD. Both gantry and couch rotations for the lower-spinal field were used to achieve perfect divergence match with the inferior border of the upper-spinal field. A FIF technique was used to feather the craniospinal and spinal-spinal junction daily by varying the match line over 2 cm. The dose throughout the target volume was modulated using the FIF simple forward optimization technique to obtain homogenous coverage. Daily, image-guided therapy was used to assure and verify the setup. This supine-position, perfect match craniospinal irradiation technique with FIF intrafractional feathering and dose modulation provides a simple and safe way to deliver treatment while minimizing dose inhomogeneity.
New exercise technology such as the virtual reality (VR)-based exercise system is required to meet soaring demand for target participants in exercises and to alleviate the difficulties in personnel mobilization through an alternative approach to the exercise system. In a previous study, event tree methodologies were introduced in setting up an exercise scenario of a VR-based radiological exercise system. In the scenario, the locations at which major events occur are rephrased as nodes, routes as paths, and public response actions as protective actions or contents of an exercise at individual locations. In the study, a model for estimating effective doses to the participants is proposed to evaluate the exercise system, using the effective dose rates at particular times and locations derived from a computer program. The effective dose received by a student when she/he follows a successful route is about a half of the dose received when she/he does not follow the exercise guide directions. In addition, elapsed time to finish an exercise when following a successful route is less than one-third of the time spent to finish an exercise when following the guide's directions.
Tannic acid (TA) is a water-soluble polyphenol compound found in various herbal plants. We investigated the chemopreventive effects of TA on FaDu hypopharyngeal squamous carcinoma cells. In an 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay, TA showed dose-dependent cytotoxicity with a half maximal inhibitory concentration (IC50) of 50 ?M. Cell cycle analysis and immunofluorescence imaging demonstrated that under low-dose ($25{\mu}M$) treatment, FaDu cells were arrested in G2/M phase, and as the dose of TA was increased, apoptosis was induced with the increase of cell population at sub-G1 phase. The expressions of various cyclins, including cyclin D1 and cyclin-dependent kinases (CDK-1 and CDK-2), were down-regulated at low doses of TA, whereas apoptotic effectors such as cleaved caspase 3, cleaved caspase 7, and poly (ADP-ribose) polymerase (PARP) were expressed in a dose-dependent manner in Western blotting. In addition, TA-induced apoptosis of FaDu cells might be mediated by the extracellular signal-regulated kinase (ERK)/mitogen-activated protein kinase pathway, with the upregulation of p-AKT/p-PKB (phosphorylated protein kinase B) and p-ERK. Overall, our data support the hypothesis that TA is a potential candidate agent for the treatment of hypopharyngeal cancer.
Background: The radionuclides released by the Fukushima Daiichi Nuclear Power Plant (FDNPP) accident 9 years ago are still being monitored by various research teams and the Japanese government. Comparison of different surveys' results could help evaluate the exposure doses and the mechanism of radiocesium behavior in the urban environment in the area. In this study, we clarified the relationship between land use and temporal changes in the ambient dose rates (air dose rates) using big data. Materials and Methods: We set a series of 1 × 1 km2 meshes within the 80 km zone of the FDNPP to compare the different survey results. We then prepared an analysis dataset from all survey meshes to analyze the temporal change in the air dose rate. The selected meshes included data from all survey types (airborne, fixed point, backpack, and carborne) obtained through the all-time survey campaigns. Results and Discussion: The characteristics of each survey's results were then evaluated using this dataset, as they depended on the measurement object. The dataset analysis revealed that, for example, the results of the carborne survey were smaller than those of the other surveys because the field of view of the carborne survey was limited to paved roads. The location factor of different land uses was also evaluated considering the characteristics of the four survey methods. Nine years after the FDNPP accident, the location factor ranged from 0.26 to 0.49, while the half-life of the air dose rate ranged from 1.2 to 1.6. Conclusion: We found that the decreasing trend in the air dose rate of the FDNPP accident was similar to the results obtained after the Chernobyl accident. These parameters will be useful for the prediction of the future exposure dose at the post-accident.
Background: Nerve blocks, including epidural analgesia, can be risky for terminal cancer pain patients in generally poor conditions. We performed this study to evaluate the efficacy of intravenous patient-controlled analgesia(PCA) to treat severe pain of terminally ill cancer patients during the last days of life. Methods: We explained the patient's poor general condition to relatives and received a written consent to administer PCA. The starting dose of opioid for PCA in cancer pain management was based on previous 24-hour dose. Previous 24-hour opioid dose was converted to intramuscular morphine equivalent. The concentration of opioid mixed into Basal Bolus $Infusor^{(R)}$ was controlled to allow for one half of the previous 24-hour equianalgesic dose to infuse continuously. Patients controlled their pain by pushing the PCA module themselves. Patients were observed by pain service team. Some discharged patients were treated at home until death. Results: Forty eight patients received PCA for last two years. The most common reason receiving a PCA was the patient's poor general condition(52.0%). The mean starting dose of PCA was $20.6{\pm}16.2$ mg of morphine. Over eighty percents of the patients were in good or tolerable state of analgesia. Half of the patients expired within one week. The mean duration of PCA was $8.7{\pm}7.0$ days. The problems during PCA were: difficulty in maintaining intravenous routes, early loss of mentality after starting PCA, hypotension and nausea. Conclusion: We concluded that PCA, if correctly, is an effective, relatively safe and readily controllable method of pain management in terminally ill cancer patients during the last days of life. For future considerations, terminal patients may expire at the comfort of their own homes after the resolution of legal problems regarding using opioid in home care.

Three benzimidazole pesticides commonly used in korean lettuce were subjected to a field residue trial to ensure safety of terminal residue in the harvest. The residual patterns of three benzimidazole pesticides, which were carbendazim, benomyl and thiophanate-methyl were examined after applying with the recommended dose in two types of korean lettuce (Chima and Chuckmeon) and their DT50 were calculated. In Chima lettuce, biological half-lives of carbendazim, benomyl and thiophanate-methyl were 2.56, 1.37 and 2.54 days, respectively and their required time under MRL(5.0 mg/kg as carbendazim) were 4.5, 2.2 and 1.0days. In Chuckmeon lettuce, biological half-lives of them were 3.41, 1.70 and 4.20 days, respectively and their required time under MRL were 5.4, 1.9 and 0.5days.

Effect of radial position of half-wave resonator is investigated experimentally for acoustic damping in a combustion chamber by adopting acoustic cold test. Acoustic damping is quantified by damping factor. When resonator with optimal tuning length is installed, damping is enhanced as its radial location is away from the center of the faceplate. And, spatial profile of damping factor is similar to that of the amplitude of the acoustic mode to be damped. As the location is close to the center, acoustic damping is mitigated and independent of the resonator length. On the other hand, the resonator with non-optimal length dose not show any effects of its radial position. Acoustic-damping capacity can be evaluated as functions of resonator length and position.
Fixed-dose combinations development requires pharmacokinetic drugdrug interaction (DDI) studies between active ingredients. For some drugs, pharmacokinetic properties such as long half-life or delayed distribution, make it difficult to conduct such clinical trials and to estimate the exact magnitude of DDI. In this study, the conventional (non-compartmental analysis and bioequivalence [BE]) and model-based analyses were compared for their performance to evaluate DDI using amlodipine as an example. Raw data without DDI or simulated data using pharmacokinetic models were compared to the data obtained after concomitant administration. Regardless of the methodology, all the results fell within the classical BE limit. It was shown that the model-based approach may be valid as the conventional approach and reduce the possibility of DDI overestimation. Several advantages (i.e., quantitative changes in parameters and precision of confidence interval) of the model-based approach were demonstrated, and possible application methods were proposed. Therefore, it is expected that the model-based analysis is appropriately utilized according to the situation and purpose.
The pharmacokinetics of lithium carbonate were investigated in rabbits with folate-induced renal failure. The blood level, the area under the blood concentration curve (AUC) and the biological half·life were increased significantly, and the urinary excretion was decreased significantly compared with those of normal rabbits. Correlation of serum creatinine concentration and AUC, biological half-life, and correlation of creatinine clearance and renal clearance of lithium carbonate have linear relationship respectively. In short, dosage regimen of lithium carbonate is considered to be adjusted in the dose size and the dosing interval by degree of experimental renal failure.
Ethosuximide is an oral anticonvulsantic agent used in the first choice anti-absence seizure drug. The purpose of this study was to assess the pharmacokinetic profile of the ethosuximide in healthy Korean volunteers and to develop the efficient assay method of ethosuximide in human plasma. The pharmacokinetics of ethosuximide administered orally was evaluated after a dose of 500 mg. Ethosuximide was assayed from plasma by a specific HPLC method reading absorbance at 195 nm. AUC was 1222$\pm$160 $\mu\textrm{g}$/$m\ell$$.$hr, $C_{max}$ 14.2l$\pm$1.74 $\mu\textrm{g}$/$m\ell$, $T_{max}$ 1.06$\pm$0.62 hr and half-life 77.83$\pm$12.46 hr.
