Vasodilatory Activities of the Water Extracts of Seventeen Medicinal Remedy (활혈효능(活血效能)을 가진 한약재 17종의 내피세포 의존성 혈관이완작용에 대한 연구)

  • The pharmacological effects of medicinal remedies traditionally used in Asian countries for improving the blood circulation were examined on isolated rat thoracic aorta strips in organ baths. Each experimental medicine was consecutively extracted under reflux with water. Of 17 plants, Curcuma longa (CL) having the strongest acute relaxant activity in endothelium-intact arteries, Mucunae caulis (MC), Cirsium pendulum (CP), Rumex longiflius (RL), Paeonia suffruticosa (PS), Curcuma zedoaria (CZ), Scirpus maritimus (SM), Siphonostegia chinensis (SC), Leonurs sibiricus (LS) and Typha orientalis (TO) were showing dose-dependent relaxant activity. Long-term relaxant effects were showed in Curcuma aromatia (CA), MC, CP, RL, PS, Potulacae grandiflorae (PG), CZ, Panax notoginseng (PN), Achyranthes japonica (AJ), CL, SC, Lycoppus lucidus (LL) and Corydalis turtschaninovii (CT). In endothelium-injury test using carbachol, CL, SC, MC, RL and PS which are having the acute vasorelaxing activity and CA and CT which are not showing vasorelaxaing activity were damaged to endothelium. As a result of this study, the possibility that a part of medicinal remedy may contribute to the beneficial effects in blood circulation was proposed, but inter-individual variation has been observed. Also, further studies on the vasorelaxant effects of these remedies are still required.

A Case Report of Post Hysterectomy Syndrome Treated with a Combination of Korean Medical Treatment (자궁절제술 후 증후군 환자의 복합 한방 증례 보고)

  • Objectives The purpose of this study was to report a case of a woman diagnosed with post hysterectomy syndrome treated with combined Korean medical treatment. Methods A combination of treatment including chuna manual therapy, pharmacopuncture at the lumbar facet joints, acupuncture, cupping, and herbal medicine was provided for 1 month. Progress was assessed using the Numeric Rating Scale, Oswestry Disability Index, EuroQol-5Dimension, and Hot Flush Score before and after treatment. Results After treatment, the symptoms were reduced and the quality of life was enhanced, although there was no improvement in the abnormal sensation in the anterior part of the thigh. No adverse events were observed. Conclusions The findings indicate that a combination of Korean medical treatment can be a safe and effective alternative treatment in reducing LBP and hot flushes after hysterectomy.

Prevalence of Balanced Chromosomal Translocations in Couples with Abnormal Reproductive Outcomes and Prenatal Cytogenetic Diagnosis in the Carriers (비정상 산과력을 가진 부부에서의 균형전좌형 염색체 보인자의 빈도 및 그 보인자들에서의 산전 세포유전학적 진단)

  • Cytogenetic analysis was performed in 1321 couples and 141 women with history of abnormal reproductive outcome during 1988-1996. The use of high resolution banding technique and fluorescence in situ hybridization (FISH) in the chromosome analysis has made the precise evaluation of chromosome aberrations. The prevalence of balanced chromosomal translocation carriers were 3.74% (104/2783 patients). 70 cases (2.52%) were reciprocal translocation carriers and 34 (1.22%) had Robertsonian translocations. Chromosome aberrations were more frequent in women (73 cases) than in men (31 cases). No phenotypical abnormalities were found in all carriers, but they experienced abnormal reproductive outcomes such as recurrent spontaneous abortions, anomalous offsprings or infertility problem. Prenatal diagnosis was carried out on 36 subsequent pregnancies in balanced translocation carriers. The fetal karyotypes showed that 12 cases (33%) were normal, 22 (61%) were balanced translocations, and two (6%) were unbalanced translocations. It is concluded that the prevalence of balanced chromosomal translocations in patients with abnormal reproductive outcome is higher than that of the normal population. Most of the fetal samples showed normal karyotypes or balanced translocations. Although the incidence of chromosomal imbalance in the fetuses was relatively low in prenatal diagnosis, individuals with balanced translocations are predisposed to abnormal offspring with partial trisomy or monosomy. Therefore we recommend that genetic counselling and cytogenetic prenatal diagnosis for translocation carriers have to be offered to prevent recurrent chromosomal abnormal babies.

Effect of ZNimesulide on the Differentiation and Survival of Endothelial Progenitor Cells

  • Nonsteroidal anti-inflammatory drugs (NSAIDs), particularly the highly selective cyclooxygenase (COX)-2 inhibitors have been shown to decrease the growth of tumor, in part, by inhibition of neovascularization. Recently, besides mature endothelial cells, endothelial progenitor cells (EPCs) have been shown to contribute neovascularization in angiogenic tissues. In this study, we addressed a question whether nimesulide, a selective COX-2 inhibitor, could affect differentiation of EPCs into adhesive endothelial cells in vitro. Total mononuclear cells were isolated from cord blood by Ficoll density gradient centrifugation, and then the cells were incubated with nimesulide or vehicle control for 7 days. The number of adherent and spindle-shaped cells decreased by nimesulide treatment in a concentration-dependent fashion at a concentration range of 5 - 200 ${\mu}M$. Moreover, the adherent cells double positive for DiI-ac-LDL uptake and lectin binding significantly decreased upon nimesulide treatment. There was no change of expression of CD31 between treatment and control groups, whereas slight reduction was detected upon treatment in expression of VE-cadherin, ICAM-1, vWF, ${\alpha}v$, and ${\alpha}5$. Nimesulide also reduced cell viability during first 3 days' culture and induced apoptosis in adherent EPCs, resulting in increased annexin-V-positive and propidium iodide-negative cells. Taken together, these results suggest that nimesulide could be applied for the inhibition of new vessel formation, in part, by inhibiting differentiation and survival of EPCs.

Research of Relationship Between Cold Hypersensitivity and Sasang Constitution (냉증을 호소하는 여성 환자의 사상체질과 다빈도질환에 대한 조사연구)

  • Objectives: The purpose of this research is to investigate the relationship between Sasang constitution and cold hypersensitivity. Methods: We investigated 391 outpatients who visited Dong-Eui Oriental Hospital OB & GY from June 12, 2013 to April 18, 2014. Among 134 patients who complained feeling of cold, we analyzed 107 patients whose Sasang constitution is confirmed. Results: 1. There were 21 persons 19.8% of under age 27, 28 persons 26.4% of age 28-34, 23 persons 21.7% of age 35-41, 18 persons 17% of age 42-48, 16 persons 15.1% of age over 49. 2. Among 107 patients, there were 52 persons 48.6% of Soeumin, 29 persons 27.1% of Taeeumin, 25 persons 23.4% of Soyangin, 1 person 0.9% of Taeyangin. And Taeyangin interior disease was 1 case 100%, Soyangin exterior disease was 22 cases 88% and interior disease was 3 cases 12%, Taeeumin exterior disease was 11 cases 37.9% and interior disease was 18 cases 62.1%, Soeumin exterior disease was 19 cases 36.5% and interior disease was 33 cases 3.5%. 3. In the distribution of cold-hypersensitive part, hands and feet with chills were 58 cases 4.7% by largest number, and the following was hands and feet in 24 cases 22.6%. 4. In the distribution of chief complaint, there were 25 cases 23.58% of menstrual pain, 19 cases 17.92% of postpartum symptoms, 17 cases 16.04% of oligomenorrhea and hypomenorrhea, 14 cases 13.21% of cold hypersensitivity. 5. In the distribution of chief complaint depending on age, menstrual pain of under age 27, postpartum symptoms of age 35-41 and cold hypersensitivity of over age 49 were at a high rate. Conclusions: Results suggest that patients with symptom of cold hypersensitivity have some tendencies in age, Sasang constitution, chief complaint, cold-hypertensive part.

Effects of Rhodiola Rosea on Anti-Fatigue and Hypothalamic IEGs Expressions of Forced Swimming Rats (홍경천(紅景天)이 강제유영 흰쥐의 항피로 및 시상하부 IEGs 발현에 미치는 영향)

  • Objectives: In this study the authors investigated effects of the ethanolic extract of Rhodjola Rosea(HKC) on fatigue and hypothalamic IEG expression in rat forced swimming(FS) model. Methods: Sprague-Dawley rats were administered HKC extract(25 mg/100g, p.o.) for 3 days prior to FS, some rats underwent 10 min FS and others exhaustive forced swimming(EFS). In addition, other rats were administered extract at different times after EFS over 3 consecutive days. Results: When HKC administered before 10 mins of FS, serum actate dehydrogenase(LDH) and creatine phosphokinase(CPK) activities were significantly lower than control group. When HKC administered prior to EFS, blood lactate was significantly lower versus control group. When HKC was administered after EFS, blood lactate(at 6 and 24 hours after EFS) were significantly lower and serum LDH, CPK activities(at 24 hours after EFS) were significantly lower versus control group. When HKC was administered after EFS, c-Fos positive neurons in hypothalamic periventricular area(PVA), medial part(mPVN) and anterior hypothalamic nucleus caudal part(AHC) were significantly lower at 24 hours after EFS than in control group. HSP-72 positive neuron numbers in hypothalamus were significantly lower at 24 hours after EFS than in control group. Finally, when HKC was administered prior to 10 mins FS, HIF-$1{\alpha}$ expression in the gastrocnemius muscle was significantly increased. Conclusions: These results suggest that HKC extract has an anti-fatigue effect, and it reduces neuronal cell stress responses induced by physical stress by having a beneficial effect on lactate metabolism.

A Study of School Doctor Program for Dysmenorrhea in Korean Medicine (월경통 한의 교의 사업에 관한 연구)

  • Objectives This study aims to investigate the effect of the Korean medicine in treating dysmenorrhea as a part of school health program (SHP) and satisfaction rate of the participated students in this program. With this study, we tried to contribute to the development of health promotion program of Korean medicine. Methods As part of the 'Korean Medicine Health Promotion Project', we conducted a dysmenorrhea program at one of many high schools in Eumseong-gun. From April to June 2018, we conducted the promotion program a total of 7 times and analyzed the questionnaire regarding satisfaction and the monthly experience of menstruation. Results The study participants' grade distribution was $2^{nd}$ grade 32, $3^{rd}$ grade 23 with a total 55 students. Mean dysmenorrhea VAS was $6.07{\pm}2.10$, and the mean number of visits to the dysmenorrhea program was $3.61{\pm}1.47$. There were no significant differences in the average number of visits between grades. Overall satisfaction was $4.06{\pm}0.97$, teacher satisfaction was $4.29{\pm}1.10$, and lecture satisfaction was $4.18{\pm}0.95$. Dysmenorrhea decreased but was not statistically significant. Conclusions Korean medicine is appropriate for the health promotion program. With cooperation and support from local schools, it will be very possible to carry out a successful business. It is also necessary to organize the program according to the viewpoint of youth and to interact with the students.

A Study on Abdominal Temperature of Dysmenorrhea Patients

  • Dysmenorrhea is one of the common gynecologic disorders of menstruation women. Primary dysmenorrhea is menstrual pain without pelvic pathology. whereas secondary one is painful mestruation with underlying pathology. The cause of primary dysmenorrhea is increased endometrial prostaglandin. The mechanisims underlying secondary dysmenorrhea are not eluciated. There are many blood vessels under the skin and they play a very important role in the thermal control of peripheral part. The control of blood circulation is mainly controlled by autonomic nervous system and it is known that D.I.T.I. (digital infra-red thermographic imaging) is an objective method showing the body temperature. I observed the abdominal themerature of 49 patients complaning of dysmenorrhea who visited the gynecologic department of Pundang Oriental Medical Hospital during 1997. In order to rule out thermal abnormality due to obesity. the causes whose obesity index were above I. were excluded in this study. The mean temperature of lower abdomen of the dysmenorrhea group was $35.22{\pm}1.33^{\circ}C$ and control group was $36.01{\pm}0.74^{\circ}C$.

  • Phlegm-retention disease, which is called dameum(痰飮), is a kind of a pathologic secretion products, accumulated excessive water in a special part of a body. Urticaria is a common dermatopathy, coming under “eun-jin(隱疹)” in Korean herbal medicine. Headache and vertigo is a very common diseases in life. There are so many causes which induce uticaria, headache and vertigo. We experienced 31-year old woman case suffering from chronic urticaria, headache, vertigo with digestive disorder, nausea. We diagnosed those symptoms as dameum(痰飮). So we administered Jeongjeongamiyijintang-gamibang(正傳加味二陳湯加味方) to the urticaria, and Banhabaekchulcheonma-tang (半夏白朮天麻湯) to the headache, vertigo. They are both famous herbal medicines for treating dameum-related symptoms, because both of them contains Yijin-tang(二陳湯) which is the main Herbal medicine for dameum(痰飮). We suggest that dameun-related urticaria, headache, vertigo can be treated by preceding medicines.

Effect of Korean Red Ginseng on Psychological Functions Patients with Severe Climacteric Syndromes : A Comprehensive Study from the Viewpoint of Traditional KAMPO-medicine and Western Medicine

  • Objective; Antistress effect of Korean red ginseng (RG) on postmenopausal women with severe climacteric syndrome (CS) were evaluated from the viewpoint of traditional KAMPO-medicine and Western medicine. Methods; All patients with CS were treated with daily oral administration of 6g RG for 30 days. Nine patients with CS were evaluated with the use of diagnostic scores for KI-deficiency (deficiency of vital energy) and OKETSU (blood stagnation) syndrome from the viewpoint of KAMPO-medicine. In the same patients with CS, peripheral blood levels of ${\beta}$-endorphin and total plasminogen activator inhibitor-1 (t-PAI-1) were measured before and after treatment with RG. In another group, 12 patients with CS, psychological test using CMI, STAI and SDS were performed from the viewpoint of Western medicine. Stress related hormones, such as ACTH, cortisol and DHEA-S in those 12 patients with CS were also measured before and after treatment with RG. Results; KI-deficiency score and OKETSU score in patients with CS were significantly (p<0.001) higher than those in patients without CS. After treatment with RG, both scores were markedly (p<0.001) decreased compared to before treatment with RG. ${\beta}$-endorphin levels in patients with CS were significantly (p<0.05) higher than those in patients without CS. Total PAI-levels in patients with CS were increased before treatment with RG. No significant difference, however, were observed between patients with and without CS. After treatment with RG, both levels of ${\beta}$-endorphin and total PAI-1 in patients with CS were significantly (p<0.001 and p<0.05, respectively) decreased compared to before treatment with RG. CMI and STAI scores in patients with CS were significantly (p<0.05) higher than those in patients without CS. SDS scores in patients with CS were also markedly (p<0.00l) higher than in those without CS. After treatment with RG, all scores decreased within normal range. DHEA-S levels in patients with CS were about a half of those without CS. Consequently, cortisol/DHEA-S (C/D) ratio was significantly(p<0.001) higher in patients with CS than in those without CS. Although the decreased DHEA-S levels were not restored to the levels in patients without CS, the C/D ratio decreased significantly (p<0.05) after treatment with RG. Conclusion; Reinforcement of vital energy and improvement of stagnant blood circulations by oral administration of RG were elucidated from the viewpoint of traditional KAMPO-medicine. From the viewpoint of Western medicine, effect of RG on postmenopsusal women with CS seemed to be brought about in part by not only an improvement of psychoneuroendocrine dysfunctions but also an amelioration of blood coagulation systems.
