Increasing the use of nursing staff in hospitals instead of relying on family members' assistance (보호자없는 병원과 간호인력 활용방안)

  • The number of nurses per bed at acute-stage hospitals is quite low in Korea compared with other OECD countries. In order to prevent the degradation of the quality of inpatient nursing services due to insufficient nurse staffs, the national health insurance introduced the differentiated nursing care fee system. This did not work as a motive for inducing the employment of nursing staff due to insufficient cost compensation. Because of insufficient nursing staff, family members have to stay with the patient or patients have to hire a personal care attendant. This increases the burden and cost to families. For the activation of hospitals without guardians, there should be policies for raising additional nursing staff such as standardizing jobs among nursing staff, particularly between nurses and nursing assistants, setting adequate standards of staffing in nursing according to medical service, substantiating the cost of nursing under the differentiated nursing care fee system, improving the medical fee system of hospitals without guardians including health insurance payment, supplying nursing staff stably through improving their working conditions such as providing child rearing services and salary increase, clarifying the qualification of personal care attendants working at acute.stage hospitals, developing indexes for assessing the quality of nursing care services, and monitoring for the management of uniform quality.

Possible Effects of Senior Hospital Selection Factors and Satisfaction on Revisit Intention (노인병원 선택요인과 만족도가 재이용의사에 미치는 영향 - 환자보호자 관점에서 -)

  • In order to analyze possible effects of senior hospital selection factors and service satisfaction on senior hospital patients' revisit of senior hospital, this study surveyed guardians of total 204 patients hospitalized in major 4 senior hospitals in Busan. As a result, this study came to the following conclusions: First, it was found that guardians considered appropriateness of medical treatment cost as the biggest factor of choice, which was followed by kindness of doctor and hospital employees, locational accessibility (traffic convenience), and doctor's medical capacity. Second, the factor analysis about patients' satisfaction showed that the influential factors consisted of satisfaction at medical staffs, satisfaction at nursing and care, and satisfaction at facilities. Third, there was not significant difference in the overall impression of hospital and the intention of revisit depending upon sociological variables. Fourth, satisfaction at nursing and care and satisfaction at facilities had significant effects on better satisfaction at overall impression of hospital, and it was noted that the shorter admission period and the less complaint led to the higher intention of revisit. Therefore, it is necessary to improve satisfaction at medical staffs, nursing and care, and facilities rather than patients' own sociological characteristics.

Analysis on the Causes and Characteristics of Child Loss through Surveys

  • Since lost child occurs frequently around us and there is a possibility of leading to an accident or a crime subsequently, it is necessary to study and have a plan for preventing child loss in advance. However, the preceding studies do not systemize the causes of child loss by places or situations, and the policy focuses only on the countermeasures afterward instead of prevention. In such perspective, this study derived the causes of child loss through the analysis of 202 cases by the bottom-up method. In addition, the causes were analyzed by dividing them into as negligence of guardian, breakaway of child, and environmental characteristics. As a result, it is found that children get lost by complex reasons usually with two or three causes combined together. And children got lost when guardians were not able to pay attention to their children, or when children moved away from their guardians. Furthermore, the environmental characteristics act as the catalyst by arousing child loss or making it more difficult for guardian to find the lost child. As a fundamental research, this study may be helpful in developing a environmental design certification system for preventing child loss in advance.

Food Safety Behavior of Low-Income Parents and Guardians of Infants in the U.S.

  • The U.S. government have concerned about food safety over the last two decades. The concept of the continuum, “from farm to table” was created to explore ways to prevent foodborne illnesses in all stages of food systems. On the continuum, consumers were recognized as the last line of defense to prevent foodborne illnesses, and much efforts were made to educate them safe food handling. This research was conducted to investigate infant formula handling and hand-washing behaviors of low-income families, especially parents and guardians of infants. The subject was selected from participants of the Special Supplemental Nutrition Program for Women, Infants, and Children(WIC), a federal program for low-income families in the U.S. Stratified 200 local WIC offices were randomly selected based on the number of WIC participants in each state, and 20 randomly selected WIC participants from each selected office were asked to complete questionnaires. SPSS for Windows was used for statistical analyses including frequency, cross- tabulation, and chi-square analyses. A total of 87 WIC offices returned completed question-naires (N = 1,598), and 492 were parents/guardians of infants. Most respondents were white (51.3%), high school graduates (41.5%) , and participated in WIC>1 yew. Most respondents (80.9%) teamed about food safety from WIC, and only limited number of respondents (10.2%) used the Internet for food safety information. Most respondents stored prepared formula safely (94.6%) and discarded formula left in the bottle after feeding (84.5%) , but fewer used brushes to wash formula bottles (71.3%) and boiled water(15.2%) Chi-square analyses showed respondents in different race/ethnicity had different food handling behaviors. Respondents showed generally good hand-washing behaviors as 94.2% always washing hands after using restroom, 93.2% after touching meat items, and 87.l% before preparing foods. Fewer respondents, however, washed hands after changing baby diapers (77.0%) and touching pets (67.2%). Researchers concluded that WIC education on food safety was effective, as limited food safety education covered during WIC education were followed well (e.g., storing prepared formula and discarding leftover). However, results also indicated that there were many behaviors needed to be reinforced especially to overcome family tradition and culture on food handling behaviors. The WIC may serve as good food safety resources and education agents utilizing mandatory education sessions because the vast amount of food safety information on the Internet was not readily accessible for this low-Income Population.

A Survey of Guardian's Attitudes toward Behavior Guidance Techniques (행동유도법에 대한 보호자의 인식도 조사)

  • The purpose of this study was to examine guardian's attitudes toward behavior guidance techniques used in pediatric dentistry. In this study, 117 guardians participated and the several behavior guidance techniques being used during actual pediatric dental treatment were explained to the guardians prior to writing a questionnaire. The behavior guidance techniques explained were: tell-show-do, voice control, protective stabilization by device, protective stabilization by staff, nitrous oxide sedation, sedation, and general anesthesia. For analysis, 106 completed survey forms were selected. Tell-show-do was rated as the most acceptable technique, followed (in order of decreasing acceptance) by: voice control; protective stabilization by staff; nitrous oxide sedation; protective stabilization by device; sedation; general anesthesia. Acceptance of each behavior guidance technique was not related to guardian age, gender, patient age, patient gender, patient dental experience, type of visit, and position of doctor. Within the limit of this study, the communicative guidance techniques (tell-show-do, voice control) were more acceptable than advanced behavior guidance techniques (protective stabilization, sedation, general anesthesia). The acceptability of general anesthesia was the lowest. The results of this survey may contribute to maintain optimal dentist - guardians communication in pediatric dentistry.

Oral Health Relevance to Disabled According to the Relationship Types between them and their Guardian (장애인과 보호자와의 관계 유형에 따른 장애인의 구강건강상태 관련성)

  • This study was conducted to examine the relations of oral health relevance to disabilities according to the relationship types between them and their guardian. The guardians were surveyed using self-writing questionnaires regarding the cohesion and communication process, and the oral health of disabled was analyzed by evaluation table. Total 78 copies were analyzed. SPSS win. 24.0 statistical program was used and the significance level was verified at .05. There was significance in the communication process(p<.01) according to the type of relationship with the disabled and their guardians. In oral health of the disabled according to the type of relationship with their guardians, there was significance in DMFT index(p<.01), first molar soundness(p<.01) and Pocket depth(p<.01).Therefore, the mother had a positive effect for the disabled on most items such as cohesion, communication process, and oral health. However, it is expected that policy support and research to increase the interests in oral health care management are needed for personal assistants, who showed a low impact on the cohesion, and communication process with disabled persons, and spouses, who showed a low influence on oral health of persons with disabilities.

The relationships between the realities of child's dental caries and guardian's acknowledgments and managements (장애아동 보호자의 구강건강에 관한 인식도와 장애아동 치아우식경험과의 관련성)

  • This study was tried to suppose the necessity for the participations of the guardians or parents into dental health educations and for developing an educational program on the control of denial health in order to improve the dental health of handicapped children. For this research, it was investigated by a survey on the actual condition of dental health with an object of handicapped children, and simultaneously by analyzing the relationships between the realities of child's dental caries and guardian's acknowledgments and managements with regard to the dental health of the children. The 209 children and their guardians being in special school for the handicapped were surveyed about the actual conditions of dental health from 15 May to 30 June 2000. 1. The average of DT, MT, FT and DMFT index were 2.46, 0.13, 1.18 and 3.78, respectively. 2. As the parents' educational level (p<0.05) and economical incomes were high, the proportions the DMFT index were low (p<0.05). As the father of the handicapped children had a duty, the DMFT index were lower than the opposite case that the father had no duty. However. in the case of their mother, the presence of the duty has not showed any relationships with the DMFT index. And it was revealed that there was a tendency that the lower age of the parents showed the lower proportions (p<0.05) and index (p<0.001). 3. There was no meaningful differences for the DMFT index in the comparison between the children who can brush themselves or not, even though the children who can brush themselves showed relatively and slightly lower DMFT index than the children who can't do it themselves. 4. More frequent observations on the children's dental conditions by the parent resulted in the lower DMFT index(p<0.01). 5. As the parent had relatively more knowledges about the dental disease. the DMFT index were low. Therefore, as it was considered form these results, the parent should be required more positive participations for the improvement of their children's dental health. For promoting this participations, it must be required the development of educational programs and political services for the parent or guardians in order to improve the attendance into the educations.

A Study on the Operation Regional Emergency Medical Center and the Using Behavior by Visiting Patients (한 지역 권역 응급의료센터 내원환자의 이용 행태 분석)

  • Currently, we have seen sudden increase of demand for emergency medical services by reason for high-speed economic development increase of traffics, etc. in this society. Consequently the government enforced & operated emergency medical system in 1991 as a link of more positive countermeasure against it, but many problem; still remain in reality. In particular, the regional emergency medical center designated by the Ministry of Health and Welfare falls short of our expectations for its essential role by the reason of such as insufficiency in professional manpower and institutions concerned in small-scale hospital, matters with transportation system, preference of large hospital, etc. Therefore, this study was conducted grasp for the actual conditions of emergency medical system based upon literature research & the preceding studies and interview research the motive of coming hospital, satisfaction & understand to the subject of 150 persons of patients and their guardians who used regional emergency center of Pusan National University Hospital, thereby examining & analysing the cause of emergency room overcrowding by non-emergency cases, one of the problem; enumerated from preceding studies. The main result of this study is as follows. First the actual condition of non-emergency patients coming hospital for examining overcrowding of emergency medical center showed that, of the patients who used the emergency medical center, non-emergency patients accounted for 49.3%, which acted as the main cause of delaying medical care for emergency cases, cases of which medical person or first-aid man decided to come hospital accounted for 36.1 %, thereby suggesting essential need for re-education & wide public information to even the professional manpower besides patients & guardians for their using emergency medical system. Second, as the result of researching patient acknowledgement with reference to their using emergency medical center, the rate of their giving right answer is no more than 60%, which means that non-emergency cases' using as such is due to the shortage of their knowledge of the said emergency medical center, which suggests us that wider P.R for emergency medical system to common people who may be one of the patients of it at any time is still in need. Third, the result of researching for finding out a future remedy of emergency medical system showed that the users who know well of the way of using emergency medical center had relatively high satisfaction of it, ones who have lesser knowledge of it lower satisfaction and users who feel in need of emergency specialized manpower feel the necessity of public information of emergency medical information center(1339) at the same time. The finding of examining the subject of study, in conclusion, showed that the degree of the patients & their guardians' understanding of emergency medical system is lower and the medical persons concerned also had no distinct difference in their understanding of it from the common people's, which suggests us for extensive enforcement of systematic education and public information in aspect of the government via various media for the purpose of effective operation of emergency medical center.

Preparing for the Future of Children with Disabilities - Focusing on parents' plans of residence and guardians for children with disabilities after the death of a parent - (부모 사후 장애자녀의 미래계획은 있는가?: 장애자녀를 가진 부모의 미래 주거지 및 보호자 계획을 중심으로)

  • This study aimed to explore the parents'intentions of choosing a residential institution as the future alternative for their children with disabilities after the death of a parent, regarding the recent disability service and policy focusing on the deinstitutionalization of services for people with disabilities. For this purpose, the effects of factors relating to the children with disabilities, families and society on the parents' future plans of residential place and guardians are examined. First, parents of children with disabilities are less likely to choose a residential institution as the future living place for their children when their children have a better communication competence, their families are more positive, and their caring burden is lower, compared to others. Second, parents are less likely to choose a residential institution as the future guarding body for their children when their children could communicate well and their families are positive. This result indicated the communication competence of children with disabilities and the positiveness of families are important factors associating with the future plan decision and the additional support for the prevention of institution arrangement after the death of a parent. Findings are discussed in relation to the importance of future plans of residence and guardians for the life of children with disabilities after the death of parents and the implication for social welfare.

Adult Guardian's Misconduct in Japan and Its Implication for Korea (일본 성년후견인의 부정행위와 한국에서의 함의)

  • The adult guardianship system takes effect from July 2013. At this point, just a month prior to the enforcement of the system, Korea should be prepared for specific countermeasures to implement the optimized system. Japan has high similarities in social and culture matters with Korea and is evaluated that the introduction background of adult guardianship system and overall operation system is similar to Korea's. As misconducts by adult guardians among the various problems occurred in Japan bring decreasing trust and delay the system settlement, proactive and posterior measures should be prepared for these problems. Therefore, in this study, misconducts by adult guardians occurred in the enforcement of adult guardianship system in Japan is to be examined and through these findings, several strategies were suggested to prevent misconducts in Korea which include the expansion of organization and manpower in family Court, dual supervision system and emergent appointment system etc.
