근막동통 증후군의 요통환자에 대한 고찰 (Observation on the Low Back Pain with Myofascial Pain Syndrome)

  • Myofascial pain syndrome is a common cause of physical disability and reduces the activity of the patient. The purpose of this study was review and analysis efficiency of myofascial pain syndrome with low back pain on 50 case who were treated at the Tae Jon Nam, Myung Ho rehabilitation clinic, from January 1, 1994 to May 31, 1994. The results of this study are as follows : 1. of the 50 cases, 24(48.0%) were male and 26(52.0 %) were female. The most common age group was 40 to 30 years old. 2. The most common duration of the treatment and onset were more than 25 months with 22(44.0 %) and less : 3. As for a major causative disease of low back pain with myofascial pain sndrome, without known cause(30.0 %), lifting object(16.0 %), post exercise(14.0 %) and heavy work(12.0 %). 4. Involved muscle with low back pain reviewed I. C. L(31.0 %), Q. L, Gluteus and others muscle(23.0 %). 5. Disease history and treatment duration were proportioned. 6. Involved muscle distribution for causative disease was 26.0 % post exercise to I. C. L, 33.5 % lifting object to Q. L, 40.0 % without known to gluteus. 7. Low back pain with syndrome was buttock and thigh Pain with 44.0 %, only low back pain with 36.0 %. 8. The improvement by physical therapy on the low back pain was good 24.0 %, fair 56.0 % etc respectively. * I.C.L : Iliocostalis lumborum * Q.L : Quadratus lumborum.

윌리엄 & 멕켄지 운동을 이용한 스위스 볼 운동이 요부유연성, 근력과 균형감각에 미치는 영향 (The Effect of Improve the Waist Flexibility, the Waist Muscular Strength and the Waist Balance which Grafted in William & McKenzie Exercise with Swiss Ball)

  • PURPOSE: The purpose of this research was comparing and analyzing the exercise effect for the public who was not suffering back pain exercise in improving the waist flexibility, the waist muscular strength and the waist sense of equilibrium after grafted in William & McKenzie exercise with swiss ball. METHODS: 16 people who were not in progress of the chronic low-back pain were recruited. They were grouped into 8 for each experiment(GSBE=Group which play swiss ball exercise, GWME=Group which William & McKenzie exercise). RESULTS: The following result was obtained through measurement of three items in both pre and post examination. CONCLUSION: Comparing the average result of flexibility improvements of each group after 12-week exercise program, the result showed that GSBE was more effective than GWME with increase of muscular strength. Comparing the average result of muscular strength of each group after 12-week exercise program, the result showed that GSBE was more effective than GWME with increase of flexibility improvements. Comparing the average result of sense equilibrium of each group after 12-week exercise program, the result showed that GSBE was more effective than GWME with increase of sense equilibrium.

Comparison of three different exercise trainings in patients with chronic neck pain: a randomized controlled study

  • Background: Neck pain is a common problem in the general population and second only to low back pain in musculoskeletal problems. The aim of this study is to compare three different types of exercise training in patients with chronic neck pain. Methods: This study was conducted on 45 patients with neck pain. Patients were divided into 3 groups: Group 1 (conventional treatment), Group 2 (conventional treatment plus deep cervical flexor training), and Group 3 (conventional treatment plus stabilization of the neck and core region). The exercise programs were applied for four weeks, three days a week. The demographic data, pain intensity (verbal numeric pain scale), posture (Reedco's posture scale), cervical range of motion ([ROM] goniometer), and disability (Neck Disability Index [NDI]) were evaluated. Results: In all groups, a significant improvement was found in terms of pain, posture, ROM, and NDI values in all groups (P < 0.001). Between the groups, analyses showed that the pain and posture improved more in Group 3, while the ROM and NDI improved more in Group 2. Conclusions: In addition to conventional treatment, applying core stabilization exercises or deep cervical flexor muscle training to patients with neck pain may be more effective in reducing pain and disability and increasing ROM than conventional treatment alone.

골반안정화 운동프로그램이 엉치엉덩관절 통증을 동반한 만성요통환자의 통증과 엉덩관절에 미치는 영향 (The Effect of Pelvic Stability Exercise Program on Pain and Hip Joint of Patients with Chronic Low Back Pain involving Sacroiliac Joint Pain)

  • 본 연구는 엉치엉덩관절 통증을 동반한 만성 요통환자 52명을 대상으로 6주 간 골반운동프로그램 및 전통적 물리치료를 병행한 실험군 27명과 전통적 물리치료만을 중재한 대조군 25명을 무작위 임상 표본 추출 하였다. 두 군 모두 통증의 정도를 알아보는 오스웨스트리-요통장애지수검사와 보행 시 엉덩관절의 각도분석을 한 후, 집단별 실험 전과 실험 후의 오스웨스트리-요통장애지수검사와 보행 시 엉덩관절의 각도분석을 비교하여 골반안정화 운동프로그램이 엉치엉덩관절 통증을 동반한 만성요통환자의 통증과 엉덩관절에 미치는 영향을 알아보고자 하였다. 요통의 변화는 집단 내와 집단 간에서 모두 유의하게 감소되었음을 알 수 있었고, 엉덩관절 각도비교에서는 실험군내 좌측 엉덩관절에서 중간디딤기, 말기디딤기, 유각전기, 초기흔들기와 우측 엉덩관절의 중간디딤기, 말기디딤기, 유각전기, 초기흔들기에서 차이가 있었다. 대조군 내 좌측 엉덩관절에서는 중간디딤기, 말기디딤기, 초기흔들기와 우측 엉덩관절의 체중전달기, 중간디딤기, 말기흔들기, 유각전기에서 유의한 차이가 있었다. 따라서 골반안정화 운동프로그램이 통증을 포함한 요통기능장애수준에 효과적이고 보행 시 입각기와 유각기 동안 엉덩관절의 굽힘과 폄이 리듬 있게 이동하여 보행이 대칭적으로 자연스럽게 이동되는데 도움이 되는 것으로 나타났다.

건식수치료가 요통환자의 통증과 요부장애에 미치는 영향 (The Effects of Dry-Hydrotherapy on The Low Back Disability and Pain for Patients with Low Back Pain)

  • Purpose: The purpose of this study was to evaluate the effects of dry-hydrotherapy to release pain in patient with low back pain. Methods: The subjects of the study were fifty patient with low back pain in orthopedic clinic. Fifty patients who had low back pain were randomly divided into 2 groups, 1) dry-hydrotherapy group 2) control group. Visual analog scale(VAS) and Oswestry low back pain disability questionnaire were measured before and after treatment. The treatment plan Was as follows; 15 min. exercise per day and 5 days per week for 2 weeks. Results: 1. Before dry-hydrotherapy, there was no significant difference in VAS score and Oswestry low back pain disability questionnaire score between two groups. 2. The VAS score was significantly decreased in both groups after 2-week dry-hydrotherapy and the decrease in the AS score in dry-hydrotherapy group was significantly lower than in control group. 3. The Oswestry low back pain disability questionnaire score was significantly decreased in both groups after 2-week dry-hydrotherapy and the decrease in Oswestry low back pain disability questionnaire score dry-hydrotherapy group was significantly lower than in control group. Conclusion: In conclusion, the dry-hydrotherapy was effective to alleviate pain in patients with low back pain.

급성 요통환자와 만성 요통환자에서 척추주변근육의 정량적 분석 (A Qualitative Analysis on Paraspinal Muscles in Patients with Acute Low Back Pain and Chronic Low Back Pain)

  • 본 연구는 MRI를 사용하여 급성 및 만성 요통환자들의 척추주변근육에 나타나는 변화를 정량적으로 측정 및 분석하고 이를 토대로 요통에 대한 진단 및 치료에 임상적 기초자료를 제공하고자 2012년 8월부터 2013년 1월까지 내원한 환자로 요통이 발생한지 12주 이내의 급성요통환자 20명과 요통이 발생한지 12주 이상 경과된 만성 요통 환자 20명을 대상자로 선정하여 MRI를 촬영한 후 측정하여 비교하였다. 왼쪽과 오른쪽의 지방변성율에서는 척추세움근과 뭇갈래근에서 유의한 차이가 있었고(p<.001), 전체 집단 간 지방변성율 비교에서도 급성요통군과 만성요통군간의 큰허리근, 척추세움근, 뭇갈래근에서 유의한 차이가 있었다(p<.001). 사후검정에서는 급성요통군과 만성요통군에서 각각 다열근과 척추세움근의 지방변성율이 가장 높은 것으로 나타났다. 만성 요통군에서 다열근과 척추기립근의 심한 지방율의 변성은 척추의 안정성을 유지하는 근육의 근력 약화로 나타나는데 본 연구결과를 토대로 급성 만성요통환자들의 재활운동과정에 있어서 뭇갈래근, 급성 요통군은 뭇갈래근과 큰허리근에 더 주의를 기울이고 재활운동 선정 과정에서 중요한 자료와 근거를 제시 할 수 있으라고 사료된다.

요추부 불안정성을 가진 요통환자에게 요부안정화운동과 신경가동술의 치료적 효과 (The Effect of Lumbar Stabilization Exercise and The Neurodynamic Technique on Patients with Low Back Pain and Lumbar Instability)

  • PURPOSE: This study aimed to compare the effects of lumbar stabilization exercise with those of the neurodynamic technique on low back pain in patients with lumbar instability. METHODS: The subjects included those with more than three tests with positive results for lumbar instability and those with a positive sign during the neurodynamic test, all of whom had low back pain. For the lumbar stabilization exercise group (n=15), lumbar stabilization exercise was performed using a sling system, while the neurodynamic technique group (n=15) performed the neurodynamic technique. The intervention was performed 5 days a week for 4 weeks. All measurement of each subject were measure at pre-intervention and post-intervention (after 4 weeks). SPSS/PC ver. 18.0 program was used to compile results. RESULTS: There was no significant difference in general characteristics of subjects between both groups (p>.05). Assessed items included the visual analog scale score (VAS), Korean version of the Oswestry Disability Index (KODI), lumbar instability test positive response counter (LIC) and Fear-Avoidance Beliefs Questionnaire score (FABQ), and a significant reduction was observed post-intervention compared to pre-intervention values in both groups (p<.01). Other assessed items such as trunk flexibility and lumbar extensor endurance and lumbar flexor endurance were significantly increased post-intervention in both groups compared to pre-intervention values (p<.01). There was no significant difference in all parameters between both groups (p>.05). CONCLUSION: Our results indicate that the neurodynamic technique may be useful for improving low back pain in patients with lumbar instability.

크로커다일호흡 운동이 요통 환자들의 척주세움근 근활성도에 미치는 영향 (The Effects of the Crocodile Breathing Exercise on the Muscle Activity of the Erector Spinae Muscle in Patients with Low Back Pain)

  • Purpose: This study aimed to investigate the effects of the crocodile breathing exercise on the muscle activity of the erector spinae muscle in patients with low back pain. Methods: The study subjects included 36 patients with low back pain. The patients were divided equally into the experimental group (EG) and the control group (CG). The EG performed the crocodile breathing exercise, and the CG performed a chest expansion breathing exercise. The intervention was conducted for 10 minutes each day for a total of eight weeks. Measurements of muscle activity were conducted using an MP150 system. An electrode was attached 2 cm to the side of the spinous process at the L4-L5 level. The muscle activity value used was %MVIC, and the statistical significance was 0.05. The paired t-test was the statistical method used to determine the pre- and post-average value of each breathing exercise, while the independent t-test was used to assess the delta value of muscle activity in the pre-post test. Results: Inspiration muscle activity showed a significant increase in both the EG and the CG, while expiration muscle activity decreased significantly in both groups. The delta value of muscle activity showed a significant difference in inspiration (p<0.05), but for expiration, there was no significant difference in muscle activity (p>0.05). Conclusion: This study suggests that crocodile breathing is a good method for improving muscle activity in patients with low back pain.

허리엉치 안정화운동이 만성허리통증환자의 허리통증장애지수와 보행속도에 미치는 영향 (The Effect of Lumbosacral Stabilization Exercise on Oswestry Disability Index and Gait Velocity of Patients with Chronic Low Back Pain)

  • 본 연구는 허리엉치 안정화 운동프로그램이 만성 허리통증환자의 허리통증장애와 보행속도에 미치는 영향을 알아보고자 만성 허리통증환자 34명을 대상으로 실험군 18명과 대조군 16명을 무작위 추출 하였다. 실험 전 오스웨스트리-허리통증장애지수(Oswestry disability index) 검사와 보행속도 분석을 한 후, 실험군은 허리엉치 안정화 운동과 함께 물리치료의 전통적 중재 기법을 시행하고 대조군은 전통적 중재 기법만을 주 4회, 하루 30분, 4주간 시행하여 사후검사를 한 후, 집단별 실험 전과 실험 후의 허리통증장애지수와 보행속도를 비교하였다. 허리통증장애지수는 실험군과 대조군의 집단 내에서 유의한 차이가 있었고(p>.01)(p>.05), 보행속도는 실험군 집단 내와 집단간에서 유의한 차이가 있었다(p>.001)(p>.05).

제조업 여성 근로자의 요통 발생에 영향을 미치는 요인 (Factors Affecting the Occurrence of Low Back Pain of Women Workers in the Manufacturing Industry)

  • For the purpose of grasping the actual condition of low back pain in women workers, 166 women in two neighboring factories were studied using questionnaire, 60% of the workers complained of low back pain. and 63% of them responded that their low back pain come from their job. About the severity, 77% marked the moderate, and 15% said absences owing to the pain. But 60% of them have not sought medical help. The low back pain occurred mainly in the workers of sitting posture and in the workers of treating heavy objects. And the workers who have worked over three years are dominant(44%) in the pain group. Comparing the pain group with the non-pain group, two variables - whether the worker treats heavy materials or not, and how heavy the materials are - showed statistically significant differences. So. there must be concern over the low back pain of women workers. In operation allocation. treating heavy objects and lifting up should be carefully considered. And for the prevention of low back pain. the workers need education and pain exercise.
