A Study on the Analysis of Self-Assessment Form based on Certification of Barrier Free in School Facilities (학교시설의 장애물 없는 생활환경(BF) 인증기준 자체평가서 분석에 관한 연구)

  • Based on basic studies on the current status index of BF certification application for school facilities according to the BF certification implementation, this study sought to improve the direction of the different BF certification evaluation indexes for school facilities by investigating and analyzing the status of BF Reservation Certification for elementary schools. A comparative analysis of the status and evaluation indexes of 30 elementary schools in the Gyeongsang region that included indicators of external and internal facilities, sanitary facilities, guidance facilities, and other facilities indicated that school facilities are insufficient to discriminate based on criteria and thus need to be updated for accurate grading. For school facilities that are open for anyone to use, quality improvement should be induced by improving standards and adjusting points for items that earn middle and lower grades.

Comparative Efficacy and Safety of Long-acting Injectable and Oral Second-generation Antipsychotics for the Treatment of Schizophrenia: A Systematic Review and Meta-analysis

  • We aimed to compare the efficacy and safety of long-acting injectable (LAI) and oral second-generation antipsychotics (SGAs) in treating schizophrenia by performing a systematic review and meta-analysis. MEDLINE, EMBASE, PsycINFO, CINAHL, and the Cochrane Library, as well as five Korean databases, were systemically searched to identify studies published from 2000 to 16 April 2015, which compared the efficacy and safety of LAI and oral SGAs. Using data from randomized controlled trials (RCTs), meta-analyses were conducted. In addition, the GRADE (the Grading of Recommendations, Assessment, Development and Evaluation) approach was applied to explicitly assess the quality of the evidence. A total of 30 studies including 17 RCTs and 13 observational studies were selected. The group treated with LAI SGAs was characterized by significantly lower relapse rates, longer times to relapse and fewer hospital days, but also by a higher occurrence of extrapyramidal syndrome and prolactin-related symptoms than that in the group treated with oral SGAs. Our findings demonstrate that there is moderate to high level of evidence suggesting that in the treatment of schizophrenia, LAI SGAs have higher efficacy and are associated with higher rates of extrapyramidal syndrome and prolactin-related symptoms. Additionally, the use of LAI SGAs should be combined with appropriate measures to reduce dopamine $D_2$ antagonism-related symptoms.

Updates of Nursing Practice Guideline for Pressure Injury (욕창간호 실무지침 개정)

  • Purpose: This study was conducted to update the existing evidence-based nursing clinical practice guideline in management of pressure injury in South Korea. Methods: The update process underwent the 22 steps according to the update method based on the international standards. Results: The updated nursing practice guideline for pressure injury consists of 4 domains and 436 recommendations. The numbers of recommendations in each domain were 25 for hospital policies, 51 for assessment, 350 for prevention and management, and 10 for education. There were 2.5% of A, 13.3% of B, 84.2% of C in terms of grading of recommendations. Among these, the major revision was done in 32 recommendations (7.4%). A total of 299 recommendations (68.6%) were added newly. Minor revisions, such as change or addition of some words, were also made in 25 recommendations(5.7%). No change was made in 80 recommendations (18.3%) compared to the previous ones. Conclusion: The nursing practice guideline for pressure injury has been updated. This updated guideline can be used as educational materials for both healthcare workers and patients with pressure injury.

Dental hygiene curriculum proposals to improve the ability of dental hygienists as medical professionals (의료인 치과위생사의 전문능력 향상을 위한 치위생학과 교육과정 제안)

  • Objectives: The purpose of this study was to propose contents of a curriculum and training program for dental hygienists as medical professionals by surveying the opinions of clinical dental hygienists and dental hygiene professors. Methods: The subjects were 192 clinical dental hygienists and 193 dental hygiene professors. They answered questionnaires that consisted of grading each task based on its importance: a) for dental hygiene students to learn, b) to perform autonomously in clinical practice, and c) the expectancy of the task to change when dental hygienists become medical professionals. Data analysis was performed using an independent sample T test to capture differences between clinical dental hygienists and dental hygiene professors. The terms in the answers of open-ended questionnaires were extracted. We used R 3.5.0, R Recommender, and Wordcloud software packages. Results: Calculus removal had the highest scores for dental hygiene students to learn and expectancy to change when dental hygienists become medical professionals. Physiotherapy of temporomandibular disorders (TMD), planning, performing, and assessment of community oral health programs had the lowest scores in autonomy in clinical practice. The dental hygiene professors gave higher scores in most of the tasks for dental hygiene students to learn, autonomy in clinical practice, and expectancy to change, than did clinical dental hygienists. Cardiopulmonary resuscitation (CPR), job ethics, and communication were the most frequently mentioned terms in the training as medical professionals program contents. Conclusions: In the future, it will be necessary to study the curriculum to improve the proficiency of dental hygienists as medical professionals.

Facial reanimation with masseter nerve-innervated free gracilis muscle transfer in established facial palsy patients

  • Background The masseter nerve is a useful donor nerve for reconstruction in patients with established facial palsy, with numerous advantages including low morbidity, a strong motor impulse, high reliability, and fast reinnervation. In this study, we assessed the results of masseter nerve-innervated free gracilis muscle transfer in established facial palsy patients. Methods Ten patients with facial palsy who received treatment from January 2015 to January 2017 were enrolled in this study. Three patients received masseter nerve-only free gracilis transfer, and seven received double-innervated free gracilis transfer (masseter nerve and a cross-face nerve graft). Patients were evaluated using the Facial Assessment by Computer Evaluation software (FACEgram) to quantify oral commissure excursion and symmetry at rest and when smiling after muscle transfer. Results The mean time between surgery and initial movement was roughly 167.7 days. A statistically significant increase in excursion at rest and when smiling was seen after muscle transfer. There was a significant increase in the distance of oral commissure excursion at rest and when smiling. A statistically significant increase was observed in symmetry when smiling. Terzis' functional and aesthetic grading scores showed significant improvements postoperatively. Conclusions Masseter nerve innervation is a good option with many uses in in established facial palsy patients. For some conditions, it is the first-line treatment. Free gracilis muscle transfer using the masseter nerve has excellent results with good symmetry and an effective degree of recovery.

The Establishment of Ecological Landscape Conservation and Restoration Sections for Urban Stream - Case studies of Jeonjucheon and Samcheon in Jeonju-si - (도시하천의 생태경관 보전 및 복원구역 설정에 관한 연구 - 전주시 전주천과 삼천을 중심으로 -)

  • The purpose of this study is to establish the conservation and restoration areas for sustainable stream management by reflecting the ecological health, cultural characteristics, and the citizens' needs for stream uses. Therefore, we extended spatial ranges of stream evaluation to riparian areas in addition to stream sections. The evaluation indexes are stream naturalness, availability, biota assessment, and riparian characteristics such as land uses and legally protected areas. The grading system was unified with five classes. The spatial evaluation units for stream section are classified as right and left for laterally and 500m for longitudinally. For riparian areas, 30m and 100m of distance from the edge of stream are applied as spatial evaluation units. The six types of stream sections for management are conservation sections(priority/general) and restoration sections(priority/general) for ecological landscape and core and general sections for cultural landscape. The established system for evaluation and designating areas for stream management was applied to Jeonjucheon and Samcheon in Jeonju-si, and the conservation and restoration areas were designated reflecting the characteristics of urban and non-urban areas and left and right of stream sections. The results of this study will provide detailed guidelines for designating stream sections and practical management strategies for sustainable urban stream management.

A Study on the Perceptions of Professors and Students of Engineering Colleges on Online Classes for Spring Semester 2020 (2020년 1학기 공과대학 교수와 학생의 온라인 수업에 관한 인식 연구)

  • In 2020, the COVID-19 pandemic has brought dramatic changes in the field of engineering education. Contrary to the traditional engineering education emphasis on content-oriented, design-based, hands-on, experimental, and field experience, most of engineering classes in 2020 had to be undertaken remotely online. However, it has not been explored how professors and students perceive about such a shift in engineering education. The aim of the current study was to investigate the perceptions of professors and students on online classes in engineering colleges during spring Semester 2020. Questionnaire data were collected from 100 professors and 4,152 students in the college of engineering. The results of this study were as following: Students were less satisfied with the online classes than professors. The online lecture method that students were most satisfied with was the recorded lecture. This is likely due to the fact that the recorded lectures can be repeated multiple times anytime, anywhere. Moreover, the experimental classes, which conventionally has more of an emphasis on the hands-on experience, also had to be conducted remotely, showing even lower satisfaction among students. Most of professors reported that the average hours they spent on preparing for online lecture increased compared to face-to-face class. Both professors and students preferred in-person exam as a desirable method of end-of-semester assessment for grading. The results of the current study have important implications for the improvement of online course environments. It is important for professors to design a structured class suitable for online education and understand the challenges students encounter during online classes. Also, professors should communicate more openly about their expectations and rubrics for class goals and assignments. Schools also needs to make effort to provide the support for the internet environment of students.

A Development of Program Outcome(PO) Evaluation System of Non-face-to-face Capstone Design (비대면 설계교과목의 학습성과(PO) 평가체계 개발)

  • The objective of this research is to devise a BARS evaluation system as a performance evaluation plan for non-face-to-face capstone design and to verify the validity through the expert FGI as the remote education is highlighted as a new normal standard in the post corona epoch. The conclusion of this research is as follows. First, the non-face-to-face capstone design is a competency centered subject that allows you to develop the engineering and majoring knowledge and its function and attitude, and the achievement of program outcome is the objective competency, and the researcher proposes the BARS method evaluation, one of competency evaluation method, as a new performance evaluation plan. Second, for the evaluation of PO achievement of non-face-to-face capstone design, the researcher deduced 20 behavior identification standard(anchor) of BARS evaluation system, and developed the achievement standard per 4 levels. Third, as the evaluation tool of non-face-to-face capstone design, the presentation data(PPT), presentation video, product such as trial product(model), non-face-to-face class participation video, discussion participating video, team activity report, and result report for the evidential data of BARS evaluation were appeared as proper. Finally, the BARS evaluation plan of non-face-to-face capstone design would be efficiently made through the establishment of evaluation plan, the establishment of grading standard of BARS evaluation scale, the determination of evaluation subject and online BARS evaluation site.

A meta-analysis on advantages of peripheral nerve block post-total knee arthroplasty

  • Background: Postoperative pain management is crucial for patients undergoing total knee arthroplasty (TKA). There have been many recent clinical trials on post-TKA peripheral nerve block; however, they have reported inconsistent findings. In this meta-analysis, we aimed to comprehensively analyze studies on post-TKA analgesia to provide evidence-based clinical suggestions. Methods: We performed a computer-based query of PubMed, Embase, the Cochrane Library, and the Web of Science to retrieve related articles using neurothe following search terms: nerve block, nerve blockade, chemodenervation, chemical neurolysis, peridural block, epidural anesthesia, extradural anesthesia, total knee arthroplasty, total knee replacement, partial knee replacement, and others. After quality evaluation and data extraction, we analyzed the complications, visual analogue scale (VAS) score, patient satisfaction, perioperative opioid dosage, and rehabilitation indices. Evidence was rated using the Grading of Recommendations Assessment, Development, and Evaluation approach. Results: We included 16 randomized controlled trials involving 981 patients (511 receiving peripheral nerve block and 470 receiving epidural block) in the final analysis. Compared with an epidural block, a peripheral nerve block significantly reduced complications. There were no significant between-group differences in the postoperative VAS score, patient satisfaction, perioperative opioid dosage, and rehabilitation indices. Conclusions: Our findings demonstrate that the peripheral nerve block is superior to the epidural block in reducing complications without compromising the analgesic effect and patient satisfaction. Therefore, a peripheral nerve block is a safe and effective postoperative analgesic method with encouraging clinical prospects.

Acupuncture for Symptomatic Rotator Cuff Disease: A Systematic Review and Meta-Analysis

  • The objective was to evaluate the effectiveness and safety of acupuncture for patients with rotator cuff diseases. There were 12 electronic databases and 3 trial registries searched up to November 30th, 2019. All randomized trials were eligible, regardless of language, date of publication, or settings. The primary outcomes were pain, shoulder function, and proportion of improved participants assessed within 12 weeks of randomization of the trial. The Cochrane risk of bias for the studies was assessed. Effects sizes were presented as a risk ratio, mean difference, or standardized mean difference with a 95% confidence intervals. Grading of Recommendations Assessment, Development and Evaluation approach was adopted to rate certainty of evidence. Of the 3,686 records screened, 28 randomized trials (2,216 participants) were included in this review. The types of acupuncture included manual acupuncture, dry needling, electroacupuncture, acupotomy, warm needle acupuncture, and fire needle acupuncture. All of the studies had an unclear or high risk of bias related to more than 1 domain. Significant benefits of acupuncture in terms of pain and shoulder function were observed in all comparisons, however, the proportion of improved participants was not described in 2 comparisons. There was substantial heterogeneity among meta-analyzed trials. No serious harm was observed. For primary outcomes, the overall certainty of evidence was very low. There was very low certainty of evidence for the benefits of acupuncture for patients with rotator cuff diseases. The safety of acupuncture remains unclear due to the incompleteness of reporting. Future well-designed randomized trials with transparent reporting are required.
