Effects of Pioglitazone and Rosiglitazone on Glucose and the Cardiovascular Risk Factors in Patients with Type 2 Diabetes: A Meta-analysis (Pioglitazone과 Rosiglitazone이 제2형 당뇨환자의 혈당조절 및 심혈관계 위험인자에 미치는 효과에 대한 메타분석)

  • A meta-analysis of 63 randomized controlled trials of Pioglitazone and Rosiglitazone in patients with type 2 diabetes was conducted. Pioglitazone significantly lowered fasting plasma glucose and triglyceride (TG) level and increased high-density lipoprotein cholesterol (HDL) level. Rosiglitazone significantly lowered hemoglobin A1C level and fasting plasma glucose, whereas it increased all kinds of lipids (HDL, LDL (low-density lipoprotein cholesterol), TG, total cholesterol). In comparison, glucose lowering effect was higher in Rosiglitazone, and Pioglitazone produced a more favorable lipid profile.

Protective Effects of Fermented Soymilk Extract on High Glucose-Induced Oxidative Stress in Human Umbilical Vein Endothelial Cells

  • We investigated whether the fermented soymilk extract (FSE) has protective effects against high glucose-induced oxidative stress in human umbilical vein endothelial cells (HUVECs). FSE was prepared via fermentation of soymilk with Bacillus subtilis followed by methanol extraction. To determine the protective effect of FSE, oxidative stress was induced by exposing of HUVECs to the high glucose (30 mM) for 48 hr. Exposure of HUVECs to high glucose for 48 hr resulted in a significant (p<0.05) decrease in cell viability, catalase, SOD and GSH-px activity and a significant (p<0.05) increase in intracellular ROS level and thiobarbituric acid reactive substances (TBARS) formation in comparison to the cells treated with 5.5 mM glucose. However, at concentration of 0.1 mg/mL, FSE treatment decreased intracellular ROS level and TBARS formation, and increased cell viability and activities of antioxidant enzymes including catalase, SOD and GSH-px in high glucose pretreated HUVEC. These results suggest that FSE may be able to protect HUVECs from high glucose-induced oxidative stress, partially through the antioxidative defense systems.

Mechanisms of Glucose Uptake in Cancer Tissue (악성종양의 포도당 섭취 기전)

  • Cancer cells are known to show increased rates of glycolysis metabolism. Based on this, PET studies using F-18-fluorodeoxyglucose have been used for the detection of primary and metastatic tumors. To account for this increased glucose uptake, a variety of mechanisms has been proposed. Glucose influx across the cell membrane is mediated by a family of structurally related proteins known as glucose transporters (Gluts). Among 6 isoforms of Gluts, Glut-1 and/or Glut-3 have been reported to show increased expression in various tumors. Increased level of Glut mRNA transcription is supposed to be the basic mechanism of Glut overexpression at the protein level. Some oncogens such as src or ras intensely stimulate Glut-1 by means of increased Glut-1 mRNA levels. Hexokinase activity is another important factor in glucose uptake in cancer cells. Especially hexokinase type II is considered to be involved in glycolysis of cancer cells. Much of the hexokinase of tumor cells is bound to outer membrane of mitochondria by the porin, a hexokinase receptor. Through this interaction, hexokinase may gain preferred access to ATP synthesized via oxidative phosphorylation in the inner mitochondria compartment. Other biologic factors such as tumor blood flow, blood volume, hypoxia, and infiltrating cells in tumor tissue are involved. Relative hypoxia may activate the anaerobic glycotytic pathway. Surrounding macrophages and newly formed granulation tissue in tumor showed greater glucose uptake than did viable cancer cells. To expand the application of FDG PET in oncology, it is important for nuclear medicine physicians to understand the related mechanisms of glucose uptake in cancer tissue.

Preparation of Polyphosphazene Diagnostic Membranes for Blood Glucose Measurements (혈당측정을 위한 폴리포스파젠 진단막의 제조)

  • A new type of diagnostic membranes based on methoxyethoxy and trifluoroethoxy co-substituted polyphosphazene has been prepared to measure blood glucose level of diabetics. Final absorbances at 680 nm through activated polyphosphazene membranes were measured at various concentration of glucose in plasma or blood. The end-point results of varing absorbance values as time (K/S) was found to have a linear relationship toward the blood glucose concentration. The effects of substitution rates with hydrophilic groups and hydrophobic groups on the measurements of glucose concentration were studied. Dose-response slope (DRS) values between glucose concentration and K/S values increased as the hydrophilic substitution rates increased. However, in more than 30% of the substitution rates, it was difficult to measure exact concentration level of glucose because DRS increased rapidly.

Fasting Serum Glucose Level and Gastric Cancer Risk in a Nested Case-control Study (공복 혈당과 위암 발생 위험에 관한 코호트 내 환자-대조군 연구)

  • Objectives : Diabetes has been reported as a risk factor for several cancers. However, the association between diabetes and gastric cancer has been inconsistent. The aim of this study was to evaluate the association between the fasting serum glucose level and gastric cancer risk in Korea. Methods : Among the members of the Korean Multi-Center Cancer Cohort (KMCC) from 1993 to 2004, a total of 100 incident gastric cancer cases were ascertained until December 31, 2002 and 400 controls were matched according to age, sex, and year and area of enrollment. Of the eligible subjects, those without fasting serum glucose level information were excluded, with a total of 64 cases and 236 controls finally selected. On enrollment, all subjects completed a baseline demographic and lifestyle characteristics questionnaire, and had their fasting serum glucose level measured. The Helicobacter pylori infection status was determined by an immunoblot assay using long-term stored serum. The odds ratios (ORs) were estimated using conditional and unconditional logistic regression models adjusted for the H. pylori infection status, smoking, drinking, education, follow-up period and matching variables. Results : The ORs for risk of gastric cancer according to the serum glucose level were 1.33 [95% CI=0.50-3.53] and 1.66 [95% CI=0.55-5.02] for the categories of 100-125 and 126 mg/dL or greater, respectively, compared to the category of less than 100 mg/dL. No increased risk of gastric cancer according to the serum glucose level was found (p-trend=0.337). Conclusions : This study provides no evidence for an association of the serum glucose level with gastric cancer.

Effect of Saryuktangkamibang on Streptozotocin-induced Diabetic Rat (사육탕가미방(四六湯加味方)이 streptozotocin으로 유발(誘發)된 백서(白鼠)의 실험적(實驗的) 탕뇨(糖尿)에 미치는 영향)

  • Objectives : The objective of this study is to observe the effect of Saryuktangkamibang, Saryuktang with Liriopis Tuber, Lycii Fructus, Schizandrae Fructus, and Bombyx Batryticatus, on the experimental diabetes. Methods : In order to induce diabetes experimentally, injected streptozotocin to the vein in the tail of rats and then treated oral administration of Saryuktangkamibang water extracts. In the measurement of the variation levels of glucose, ALP, creatinine, and BUN concentration for each concentration levels for serum (0.2g/100g, 0.4g/l00g, 0.6g/100g), concentration level of glucose significantly decreased in 0.4g/l00g concentration level of Saryuktangkamibang. With this 0.4g/l00g concentration level of Saryuktangkamibang, the following conclusion was derived from the measurement of the serum levels of glucose. ALP, AST, ALT, creatinine, and BUN concentration for the injection of each 4th, 11th, and 18th day. Results : 1. In the measurement for each concentrations, the glucose concentration level in the serum significantly decreased on the 9th day in the Saryuktangkamibang-dosed group. 2. The creatinine and BUN concentration level in the serum significantly decreased on the 9th day in the Saryuktangkamibang-dosed group to which 0.6g/100g injected. 3. In the measurement for each dosage dates, the glucose concentration level in the serum significantly decreased on the 11th day in the Saryuktangkamibang-dosed group to which 0.6g/100g injected. 4. The AST concentration level in the serum significantly decreased on the 11th day in the Saryuktangkamibang-dosed group. Conclusion : Saryuktangkamibang, Saruktang with Liriopis Tuber, Lycii Fructus, Schizandrae Fructus, and Bombyx Batryticatus, has an effect of hypoglycemia, with no hepato-renal dysfunction. For the experimental results above, in that nephrotoxicity significantly decreased on the 0.6g/100g concentration, another research such as study for effect in other ranges and study for effect of dosage term except the concentration range of this study is necessary. Also dosage of appropriate concentration of Saryuktangkamibang is thought to provide valuable aid to cure on diabetes.

A Study on the Effect of Sangbaegpitang & Supungsungiwhan on the Glucose Metabolism of db/db Mice (상백피탕(桑白皮湯)과 수풍순기환(搜風順氣丸)이 db/db Mice의 당대사(糖代謝)에 미치는 영향(影響))

  • In this study, body weight levels of glucose, insulin and triglyceride in blood and glucosidase activity of the small intestine were investigated to determine the effect of Sangbaegpitang and Supungsungiwhan on the glucose metabolism of db/db mice. The GLUT4 mRNA of muscle tissue and the Acetyl CoA Carboxylase and the activation rate of GLUT2 mRNA of liver tissue were measured by the reverse transcription-polymerase chain reaction method and by the vitro transcription. The results were obtained as follows: 1. In the Sangbaegpitang administration group, (1) The level of triglyceride was decreased significantly and the glucosidase activity of the small intestine was inhibited remarkably, (2) The amounts of the GLUT4 mRNA in muscle tissue and Acetyl CoA Carboxylase mRNA in liver tissue were increased significantly. (3) Though glucose level in both fasting and non-fasting, were decreased and the insulin level in blood was increased, the results showed no statistical significance. 2. In the Supungsungiwhan administration group, (1) The levels of glucose and triglyceride were decreased significantly in the blood of non-fasting animals. (2) The glucosidase activity of small intestine was inhibited markedly and the amounts of GLUT4 mRNA of muscle tissue and GLUT2 mRNA of liver tissue were increased significantly. (3) The glucose levels in the fasting group were reduced, while insulin level was increased but showed no statistical significance, Based on the above results, our conclusions are as follows: Sangbaegpitang & Supungsungiwhan are thought to be capable of inhibiting the activity glucosidase, the enzyme which influences carbohydrate metabolism in the small intestine of db/db mice(the experimental diabetic model) and delaying the absorption of carbohydrate, thus proving effective on inhibiting the increase of non-fasting glucose level effectively. Futhermore Sangbaegpitang and Supungsungiwhan are though: to be capable of preventing the composition of free fatty acids by restoring the production of GLUT4 mRNA of muscle tissues and GLUT2 mRNA of liver tissues. Those results suggests that above prescriptions can be applied to non-insulin dependent diabetes mellitus in order to improve insulin resistance.

The Effects of Tryptophan and Tyrosine-Enriched Diet on the Serum Cortisol, Glucose and Free Fatty Acid Levels of Stressed Rats (Tryptophan 및 Tyrosine 보강 식이가 Stress로 인한 혈청 Cortisol, Glucose 및 Free Fatty Acid의 농도 변화에 미치는 영향)

  • We made S.D male rats eat 3.6% tryptophan(trp) or tyrosine(thr)-enriched diet for 4 days and measured the trp or tyr use in serum of stressed rats as well as cortisol, glucose and free fatty acid(ffa) changes in serum. When control group had received stress treatment, their tyr level has dropped significantly but no changes in trp level and their cortisol, glucose, ffa levels in serum were increased significantly all together. Trp-enrichment alone can't change serum cortisol and ffa levels but trp pools in serum and brain enlarged by dietary enrichment. Trp-enriched group's serum glucose concentration was significantly lower than control-dieted group. When trp-enriched group received stress treatment, their responses to stress were different from control-dieted group. Serum trp concentration of trp-enriched-with-stress group has dropped significantly and cortisol level was increased significantly but not as much as control-dieted-with-stress group. Glucose and ffa levels of trp-enriched-with-stress group did not increase at all. Tyr-enriched group has also larger serum pool of tyr and lowest basal cortisol level in all three diet group. tyr-enriched group's and ffa levels were in normal range and those responses to stress were same pattern with control diet group. Most importantly in tyr-enriched-with-stress group, only slight but not significant increase of cortisol level was shown.

Clinical Implications of the Glucose Test Strip Method for Early Detection of Pulmonary Aspiration in Nasogastric Tube- Fed Patients (비위관영양환자의 폐흡인 조기확인을 위한 포도당 검사지[glucose test strips] 방법의 임상적 유용성)

  • Purpose: This study was performed to test the clinical usefulness of the glucose test strip method for early detection of pulmonary aspiration in tube fed patients. Method: The subjects for the study were 36 patients who were receiving enteral feedings and 39 patients who were not given enteral feedings. For the analysis, the tube fed patients were divided into two groups (clinically significant aspiration and no aspiration) according to criteria. Result: The mean glucose concentration of tracheal secretions from non enteral fed patients was 26.35mg/dl and were lower than those concentrations found in tube fed patients (32.75mg/dl). The mean glucose concentration of the aspiration group was 45.60mg/dl and the glucose concentration of the non aspiration group was 19.93mg/dl. The difference was statistically significant (t=2.163, p=.038). More subjects in the no aspiration group ($73\%$) than the aspiration group ($56\%$) had glucose concentrations below 20mg/dl. After deleting the cases that had samples containing blood, glucose concentrations of tracheal aspirates were lower in both groups. Conclusion: The glucose level of the aspiration group was significantly lower than the no aspiration group and more subjects in the aspiration group had a glucose level higher than 101mg/dl. Therefore, the glucose test of tracheal secretions in tube fed patients could be a desirable test for screening for tracheal aspiration. Especially the patient who is showing repeatedly high glucose levels should not be given feedings until reassessment is completed.

  • In this study, a mechanism by which glucose level modulates glucose transport in Jurkat cells was investigated. Glucose uptake was more efficient in the cells cultivated in low glucose (2.5 mM) medium than that grown in high glucose (20 $\mu$M) medium. Vmax (0.74 n㏖/10$^6$ cells$\cdot$min) of glucose uptake measured with the cells grown in the low glucose medium was higher than the one (1.06 n㏖/10$^6$ cells$\cdot$min) in the high glucose medium while Km was almost consistent through the change of glucose levels, indicating the increase of glucose transporter number.
