A comparison of different gingival depigmentation techniques: ablation by erbium:yttrium-aluminum-garnet laser and abrasion by rotary instruments

  • Purpose: The aim of this study is to compare two different gingival depigmentation techniques using an erbium:yttrium-aluminum-garnet (Er:YAG) laser and rotary instruments. Methods: Two patients with melanin pigmentation of gingiva were treated with different gingival depigmentation techniques. Ablation of the gingiva by Er:YAG laser was performed on the right side, and abrasion with a rotary round bur on the opposite side. Results: The patients were satisfied with the esthetically significant improvement with each method. However, some pigment still remained on the marginal gingival and papilla. The visual analog scale did not yield much difference between the two methods, with slightly more pain on the Er:YAG laser treated site. Conclusions: The results of these cases suggest that ablation of the gingiva by an Er:YAG laser and abrasion with a rotary round bur is good enough to achieve esthetic satisfaction and fair wound healing without infection or severe pain. Prudent care about the gingival condition, such as the gingival thickness and degree of pigmentation along with appropriate assessment is needed in ablation by the Er:YAG laser procedure.

  • The attractiveness of the gingiva is determined by its color, shape, and the shape and location of the boundary between the teeth and the gingival tissue. The standards beauty, balance, and health of the gingiva are all different, but the general public would agree that a coral pink gingiva is more beautiful than black or brown gingiva. Hence, one would be able to smile more confidently in public if he or she receives a gingival pigmentation removal surgery that changes the color of black or brown gums to a beautiful pink color with relative simplicity. The color of one's gingiva varies from pale pink to deep bluish purple, depending on many health components. The most prominent among these include the vascular supply, epithelial thickness, the degree of keratinization, and the presence of pigment in the epithelium. Melanin, carotene, reduced hemoglobulin, and oxyhemoglobulin are the main pigments contributing to the normal color of the oral mucosa. The health of one's gingival tissue are essential for an attractive smile. Excessive melanin deposits in the basal and early basal layers of the epithelium stored in the form of melanosomes frequently cause pigmentation.   • Purpose: Excessive gingival display and short clinical crowns due to altered passive eruptions are major concerns for a considerable number of patients visiting dental clinics. Altered passive eruptions could be corrected through various types of periodontal surgery conformable to a classification. 3 cases are reported here on the esthetic correction of altered passive eruption to evaluate results of crown lengthening procedure. Methods: Three patients whose major complaints were excessive gingival display and short teeth were picked out for this case study. Before treatment, clinical and radiological exam was performed to choose type of surgery. Thickness and width of keratinized gingiva was measured in all three patients then they were treated by surgical methods including flap operation and depigmentation under subsequent diagnosis. Results: Uneventful healing and stable gingival margin were observed in all three patients except recurrence of gingival pigmentation of one patient. Conclusions: The treatment of altered passive eruption requires precise diagnostic procedure and could achieve better esthetic outcomes when it is accompanied by other orthodontic and orthognathic treatment.

Natural Blue Pigment from Gardenia jasminoides Ellis (Rubiaceae) as a Dental Plaque Disclosant

  • Background: Dental caries and periodontal disease are bacterial infectious disease, mainly caused by plaque, a bacterial colony deposited on the tooth surface and gum tissue. Dental plaque disclosants easily stain the dental plaque, making them effective for scaling and tooth brushing education. As the erythrosine typically contained in dental plaque disclosants is highly cytotoxic, a low toxicity additive is needed. In this study, we aimed to examine the natural pigments with negligible cytotoxicity but can effectively stain the dental plaques for use in dental plaque disclosants. Methods: The pigmentation of eight types of natural pigments was tested on bovine tongue and teeth, as well as on head and neck tissue sections of experimental ICR mice. The cytotoxicity of gingival epithelial cells was measured via MTT assay. Pigmentation was performed on the bovine tongue and tooth surface. Pigmentation in the oral environment was observed in four mandibular incisors. A 2 Tone was used as a control. Results: Of the eight types of natural pigments, purple and blue pigments were effective in coloring dental plaques on the enamel surface as well as in the head and neck tissue sections. Additionally, purple and blue pigments were visible on the surface of the bovine tongue. Red, pink, orange, green, purple, and yellow pigments showed strong cytotoxicity, whereas brown and blue pigments had relatively low cytotoxicity. Blue pigment was effective in staining the dental plaque of four mandibular incisors. Conclusion: We suggest that the blue pigment derived from Gardenia jasminoides Ellis (Rubiaceae), which is effective for coloring dental plaques and has low cytotoxicity, is useful as a naturally derived dental disclosant.

Isolation of Melanogenesis Inhibitors from Cnidii Rhizoma (천궁으로부터 멜라닌 생성억제 물질 분리)

  • Gingival hyperpigmentation may cause esthetic problems and embarrassment. Especially in patients with a gummy smile. Melanin pigmentation is related to estiologic factor such as hormon, systemic factor, drug, smoking and gingival inflamation. During our search for new inhibitory components on melanogenesis from natural resources, MeOH extracts of more than 100 higher plants were tested for the inhibitory effect on melanogenesis in cultured B-16 mouse melanoma cell lines, and methylene chloride soluble part extract of Cnidii Rhizoma MeoH extraction was found to have potent activity. Cnidii Rhizoma, the root of Cnidium officinale Makino (Umbelliferae), is used for the treatment of abdominal pain, arthralgia, headache, hypertension, intestinal colic and for menstrual disorders and uterine cramps for its anti-blood stagnation effect. Two compounds were isolated and their chemical structures were determined as linoleic acid methyl ester(1), 1,3-dilinoleoyl-2-stearoyl glycerol(2), on the basis of physical and spectral data.

  • Sturge - Weber Syndrome is a congenital disorder and characterized by facial hemangioma following one or more divisions of the trigeminal nerve, epilepsy, mental retardation, contralateral hemiplegia, occlular involvement, gingival involvement. A 34 year old Korean man, a 25 year old Korean woman and a 48 year old Korean woman were found to have red - purple colored pigmentation on the hemifacial area and upper oral mucosal area.
