Ultrasonographic Appearance of the Gestational Structures throughout Pregnancy in Pet Dogs II. Estimation of Gestational Age and Prediction of Parturition Day by Measurement of the Gestational Structu (애완견에서 임신일령에 따른 임신구조물의 초음파상 II. 임신구조물의 측정에 의한 임신일령의 판정과 분만일의 예시)

  • Serial ultrasonographic examinations were daily performed on 8 bitches (5 Maltese and 3 Yorkshire terrier) from day 15 until parturition to determine the size of gestational structures. Gestational age was timed from the day of ovulation (day 0), which was estimated to occur when plasma progesterone concentration was first increased above 4.0 ng/ml. When the size of fetal and extra-fetal structures according to gestational age was measured, there was a difference between Maltese and Yorkshire terrier in extra-fetal structures (P<0.001), and no difference in fetal structrures (P<0.001). The correlations between the gestational age and the growth in fetal and extra-fetal structures were examined by regression analysis. The extra-fetal structure which showed high correlation from day 20 to 35 was inner chorionic cavity diameter in two breeds. The fetal structures which showed high correlation from day 36 until parturition was fetal head diameter in two breeds. In conclusions inner chorionic cavity diameter was the most accurate for estimation of gestational age and prediction of whelping day from day 20 to 35, and fetal head diameter from day 36 to parturitions respectively. Ultrasonography is very useful tool to evaluate the gestational age and to predict the whelping day in pet dogs.

  • Background: The change in size and weight of the female reproductive organs during gestation and birth might be affect the perineal muscles and this condition in dairy cow not been reported. This study aimed to assess the ultrasonographic image of coccygeus muscle in 11 inseminated dairy cows with different gestational ages and postpartum. Methods: Gestational age was calculated based on the record of artificial insemination and confirmed by using transrectal brightness mode ultrasonography. Perineal hair between the sacrum and ischium bones was shaved along 3-5 cm before being ultrasound. The images of perineal area were obtained by transcutaneous ultrasound using a 5.0 MHz transducer. The thickness and intensity of the coccygeus muscle were measured and analyzed by gestational status and postpartum to show the differences. Results: The results showed that the thickness of coccygeus muscle increased with the increase in gestational age. Muscle intensity only increased at young gestational age. However, it decreased with the increase in gestational age (P < 0.05). Conclusions: The ultrasound image of coccygeus muscle was affected by gestational status, thus this method may be used as one of the new methods of indirect gestational detection on dairy cows.

  • Fetal cerebellum is grow depending on the gestational age, measurement of transverse cerebellar diameter(TCD) is being used import indicator of fetal growth prediction in clinical. In this study, the subjects were normal pregnant women 20~37 week of gestation, and the volume scan was conducted on the 340 subjects. The research reports was indicated by regression curve the growth of fetal TCD in accordance with the gestational age. It got to the value of the results from a linear regression equation. Measurement fetal TCD using 3D US was statistically significant(p<0.001) and useful in the prediction of gestational age. TCD increases with gestational age can also distinguish between the normal fetal and prediction of accurate gestational age of fetal growth retardation. If the basic data of the present study, ongoing research is performed, the TCD using by 3D US are expected to be usefully applied in the correct prediction gestational age.

  • Purpose:It is critical that the exact gestational age of a newborn baby, especially premature baby, be determined to evaluate the status of a disease and its management and to estimate the prognosis of a patient. This study aimed to investigate an easy and accurate method to estimate gestational age on cranial ultrasound, requiring minimal additional time and equipment. Methods:A high-resolution coronal sonographic image was obtained via the anterior fontanel with a 5-12 Mhz linear probe after the usual cranial sonographic examination. We measured the depth of cerebral hemisphere, thickness of corpus callosum, and depth of cingulate sulcus and obtained the correlations between these factors and gestational age. Results:Depth of cingulate sulcus had the highest correlation coefficiency with gestational age (r=0.878, P=0.000). All the cases, except for 2 cases, had a gestational age of more than 37 weeks, when the depths of cingulate sulcus were more than 0.55 cm, and had a gestational age less than 34 weeks, when the depths of cingulate sulcus were less than 0.35 cm. Conclusion:Measurement of the depth of cingulate sulcus was a simple and accurate method to estimate the gestational age on cranial ultrasound. The gestational age is more than 37 weeks, when the depth of cingulate sulcus is more than 0.55 cm, and is less than 34 weeks, when the depth of cingulate sulcus is less than 0.35 cm.

  • Purpose: The outcomes of small for gestational age (SGA) infants especially in extremely low birth weight infants (ELBWIs) are controversial. This study evaluated the mortality and morbidity of ELBWIs, focusing on whether or not they were also SGA. Methods: The medical records of 415 ELBWIs (birth weight<1,000 g), who were inborn and admitted to the Samsung Medical Center neonatal intensive care unit from January 2000 to December 2008, were reviewed retrospectively. Mortality and morbidities were compared by body size groups: very SGA (VSGA), birth weight ${\leq}$3rd percentile; SGA, 3rd to 10th percentile; and appropriate for gestational age (AGA) infants, >10th percentile for gestational age. For gestational subgroup analysis, groups were divided into infants with gestational age ${\leq}24^{+6}$ weeks (subgroup I), $25^{+0}$ to $26^{+6}$ weeks (subgroup II), and ${\geq}27^{+0}$ weeks (subgroup III) Results: Gestational age was $29^{+2}{\pm}2^{+6}$ weeks in the VSGA infants (n=49), $27^{+5}{\pm}2^{+2}$weeks in the SGA infants (n=45), and $25^{+4}{\pm}1^{+4}$ weeks in AGA infants (n=321). Birth weight was $692{\pm}186.6$ g, $768{\pm}132.9$ g, and $780{\pm}142.5$ g in the VSGA, SGA, and AGA groups, respectively. Cesarean section rate and maternal pregnancy-induced hypertension were more common in the VSGA and SGA than in AGA pregnancies. However, chorioamnionitis was more common in the AGA group. The mortalities of the lowest gestational group (subgroup I), and also of the lower gestational group (subgroup I+II) were significantly higher in the VSGA group than the SGA or AGA groups (P=0.020 and P=0.012, respectively). VSGA and SGA infants showed lower incidence in respiratory distress syndrome, ductal ligation, bronchopulmonary dysplasia, intraventricular hemorrhage than AGA group did. However, by multiple logistic regression analysis of each gestational subgroup, the differences were not significant. Conclusion: Of ELBWIs, extremely SGA in the lower gestational subgroups, had an impact on mortality, which may provide information useful for prenatal counseling.

Estimation of Gestational Age by Measurement of Fetal and Extra-fetal Structures in Miniature Schnauzer Bitches

  • Serial ultrasonographic examinations were performed on 9 Miniature Schnauzer bitches from day 15 of gestation until parturition to determine the ultrasonographic measurement of gestational structures. Ovulation was designated the day that plasma progesterone concentration exceeded 4.0 ng/ml (day 0). Extra-fetal structures were measured from day 17 or 18 to 60. Outer uterine diameter (OUD) increased from $6.50{\pm}1.06mm$ ($Mean{\pm}SD$) to $50.89{\pm}5.62mm$, inner chorionic cavity diameter (ICCD) increased from $2.10{\pm}0.15mm$ to $37.15{\pm}4.36mm$, and length of placenta (PL) increased from $7.50 {\pm}1.41mm$ to $40.62{\pm}3.27mm$. OUD and ICCD were significantly and linearly relative to gestational age especially through day 37, whereas PL was not significantly relative to gestational age. Of the extra-fetal structures, ICCD was the most accurate for estimation of gestational age before day 38 of gestation. Fetal structures increased at a linear rate to parturition. Fetal crown-rump length (CRL) increased from $2.55{\pm}0.07mm$ on day 22 to $85.25{\pm}9.89mm$ on day 48. Fetal head diameter (HD) increased from $3.43{\pm}0.64mm$ on day 27 to $25.06{\pm}0.41mm$ on day 63. Fetal body diameter (BD) increased from $ 5.96{\pm}0.84mm$ on day 30 to $43.76{\pm}3.36mm$ on day 63. Of the fetal structures, HD was the most accurate for estimation of gestational age after day 38 to parturition.

Estimation of Gestational Age by Measurement of Deep Portion of Telencephalic Vesicle in Pekingese Fetus

  • This study was performed to establish a new parameter for estimating gestational age and predicting parturition day by ultrasonographic measurement of deep portion of telencephalic vesicle (DPTV) diameter in small dogs. Fetal head diameter (HD) and DPTV diameter were measured in 15 pregnant Pekingese bitches, from Day 15 to the parturition day, and evaluated the correlation between gestational age. HD was measured from day 29 of pregnancy to parturition day and increased from $4.9{\pm}2mm$ to $25.5{\pm}0.7mm$. Especially, from day 38 of pregnancy to parturition day, HD uniformly increased about 0.6 mm per day and was significantly and linearly relative to gestational age during this period ($r^2$ >0.99). DPTV diameter was measured from day 35 to day 60 of pregnancy and increased from $3.2{\pm}0.9mm$ to $11.5{\pm}0.7mm$. Especially from day 38 to day 60 of pregnancy, DPTV diameter uniformly increased about 1 mm per 3 days and was significantly and linearly relative to gestational age during this period ($r^2$ >0.99). In conclusion, DPTV diameter could to be a useful parameter for the estimation of gestational age and the prediction of parturition day when used alone or in combination with HD during the second half of pregnancy.

Incidence of congenital hypothyroidism by gestational age: a retrospective observational study

  • Background: Congenital hypothyroidism (CH) is the leading cause of preventable physical and intellectual disabilities. This study aimed to assess the incidence and clinical characteristics of CH in newborns. Methods: We retrospectively reviewed the medical records of all newborns delivered at the Pusan National University Hospital between January 2011 and March 2021. The incidence of CH was compared according to gestational age, birth weight, and small for gestational age (SGA). The patients aged ≥3 years who could not maintain normal thyroid function and required levothyroxine treatment were diagnosed with permanent CH. Logistic regression analysis was performed to compare CH risks. Results: Of 3,722 newborns, 40 were diagnosed with CH (1.07%). Gestational age and birth weight were significantly associated with CH incidence. The odds ratios (ORs) of CH in infants delivered at 32-37, 28-31, and <28 weeks were 2.568 (95% confidence interval [CI], 1.141-5.778), 5.917 (95% CI, 2.264-15.464), and 7.441 (95% CI, 2.617-21.159) times higher, respectively, than those delivered at term. The ORs of CH in infants weighing 1,500-2,499 g, 1,000-1,499 g, and <1,000 g were 4.664 (95% CI, 1.928-11.279), 11.076 (95% CI, 4.089-29.999), and 12.544 (95% CI, 4.350-36.176) times greater, respectively, than those in infants weighing ≥2,500 g. The OR of CH was 6.795 (95% CI, 3.553-13.692) times greater in SGA than in non-SGA infants. Conclusion: The CH incidence in South Korea has increased significantly compared with that in the past. Gestational age, birth weight, and SGA were significantly associated with CH incidence.

Birth weight for gestational age patterns by sex, plurality, and parity in Korean population (한국의 성별, 태아수별, 출산수별 임신주수에 따른 출생체중)

  • Purpose : A universal standard of the birth weight for gestational age cannot be made since birth weight distribution varies with races, nations and eras. This report aims to establish the birth weight for gestational age patterns by sex, plurality, and parity, specific for Korean live births. Methods : The national birth certificate data of all live births in Korea from January 2000 to December 2004 were used: for live births with gestational age 24 weeks to 42 weeks (n=2,585,5160), mean birth weight, standard deviation and 10th, 25th, 75th and 90th percentile values were obtained for each gestational age group by one week increment. To establish final standard values of Korean birth weight distribution by gestational age, the finite mixture model to eliminated erroneous birth weights was used for respective gestational age. Same as above method the birth weight for gestational age standard by sex, plurality, and parity were completed. Results : The male newborns are more heavier than female during the entire gestational age. The singletons are more heavier than twin during the entire gestational age. The para 2 are more heavier than the para 1 during the entire gestational age. Korean standard was more heavier in 10th and 50th percentile than Lubchenco's standard. Alexander's standard was more heavier in 50th and 90th percentile than Korean standard. Conclusion : These birth weight for gestational age patterns by sex, plurality, and parity are similar to the other standards. I hope that for Korean infants, this curve will help clinicians in defining and managing the large for gestational age infants and also for infants with intrauterine growth retardation.

Risk Factors Associated with Germinal Matrix-Intraventricular Hemorrhage in Preterm Neonates

  • Objective : The purpose of this study is to identify the risk factors associated with the development of germinal matrix-intraventricular hemorrhage (GM-IVH) and the relationship of the severity of disease and prematurity. Methods : A total of 168 premature neonates whose birth weight ${\leq}1500g$ or gestational age ${\leq}34$ weeks were examined by cranial ultrasound (CUS) for detection of GM-IVH among the babies admitted between January 2011 and December 2012 in our medical center neonatal intensive care unit. The babies were divided into two groups : GM-IVH and non-IVH. Clinical presentations, precipitating factors of the patients and maternal factors were analyzed. Results : In univariate analysis, gestational age, birth weight, delivery method, presence of premature rupture of membrane (PROM) and level of sodium and glucose were statistically meaningful factors (p<0.05). But only two factors, gestational age and presence of patent ductus arteriosus (PDA) were statistically meaningful in multivariate logistic regression (p<0.05). Delivery method [normal vaginal delivery (NVD) to Caeserean section] was borderline significant (p<0.10). Conclusion : Presence of PDA and gestational age were the important risk factors associated with development of GM-IVH.
