노인요양시설 요양보호사의 구강관리 및 노인구강위생관리에 따른 구강보건지식 수준 (Oral health knowledge level on oral care and elder oral hygiene care of the nursing home staff in long-term care facilities)

  • Objectives : This research was conducted to investigate geriatric care workers' oral care, and the oral care conditions of residents of elder care facilities in relation to workers' knowledge of oral care. Methods : A survey was conducted from November 2011 to February 2012 with geriatric-care workers in elder care facilities located in Busan and other areas of Gyeongsangnam-do. Results : With regard to their oral self-care practices, 52.5% of the geriatric workers "brushed their teeth from top to bottom" and 58.1% brushed their teeth 3 times a day. Further, 52.0% did not use any oral care products, while 36.5% used Mouthrinese. The workers who brushed their teeth in their own ways or from top to bottom had significantly higher oral care knowledge scores than those who brushed from left to right (p<.05). Concerning oral hygiene care for residents, 72.6% of the geriatric workers recommended that elders visit dentists for oral care, 96.6% helped elders oral wash their teeth every day, and 54.7% used oral care products for the elders. The workers who did not recommend visits to dental clinics scored slightly higher than those who did (p<.05). Additionally, those who did not help elders oral wash their teeth scored significantly higher than those who did (p<.05). Except for oral wash to recuperated old man and recommendation of visiting the dentist, relation between oral care and knowledge about oral health and oral hygiene care is plus mutual relation. Conclusions : To manage the accuracy and quality of oral care work performed by geriatric care workers in elder care facilities, enhancing standard training and education on oral care is necessary. Providing systematic, regular education on such knowledge and practices would also be essential.

장기요양시설 요양보호사의 구강건강관리에 대한 지식, 태도, 행동 (Knowledge, Attitude, and Behavior Status on Oral Health Care of Geriatric Care Workers in Long-Term Care Facilities)

  • 본 연구는 요양보호사의 구강관리교육 프로그램에 필요한 근거를 마련하기 위해 전국 장기요양시설에 근무하는 요양보호사를 대상으로 구강관리교육 실태와 관련 지식, 태도 및 행동을 설문조사하여 다음과 같은 결과를 얻었다. 연구대상자의 27.9%가 구강관리에 관한 교육을 수혜한 경험이 있다고 하였고, 배운 내용으로는 틀니관리와 이 닦기 방법이 90%로 가장 많았다. 그리고 연구대상자들의 구강관리에 대한 지식수준을 확인한 결과, 노인이 복용하는 약물의 부작용, 오연성 폐렴과 치태의 연관성, 잇몸질환, 틀니세척 방법, 와상노인의 구강위생관리에 대한 지식이 부족함을 알 수 있었다. 입소노인의 구강관리의 책임자로서 연구대상자들의 태도수준은 총 10개의 조사 문항 중 8개 문항에서 3점 이상(5점 만점)의 높은 동의수준을 보였으며, 노인의 구강관리를 제공하기 어려운 이유로 구강관리 방법에 대한 어려움(46.4%), 지식의 부족(27.1%) 순으로 응답하였고, 치과의사와 치과위생사가 업무수행에 도움자로서 매우 필요하다(57.3%, 52.9%)고 생각하였다. 이러한 결과를 통해 연구대상자들은 요양시설 입소노인에게 구강관리를 제공하기 위한 태도는 높았지만, 이와 관련한 교육이 부족하며 구강관리업무를 수행하는 데 있어 전문가의 도움이 필요하다는 것을 확인하였다.

The Geriatric Care Workers' Role Care for Elderly of Sanatorium in Korea

  • This paper is to research the difference of care workers' role in Sanatorium between urban and rural areas. Interviews have been conducted with 100 care managers with structured questionnaires in community care settings. The findings of the study are as follows. In the analysis of working with carers a care manager acting as a counsellor in rural was required rather more than any other role in working with clients' carers. In working with formal and informal networks, an administrative specialist role was also important in both areas. With resource management, there were some regional variances between rural and urban. In the urban area, a care managers as a coordinator was more required than as a broker. In the rural area, a care manager as a broker, selecting service resources for elderly clients was the most suitable role. In conclusion, in general rural care managers' roles were similar to those of many core managers in urban area. Among the many possible roles of care managers that effective continuity of care is to be provided for elderly clients in community care, two have been specified as essential roles. The first is the role of care managers that provides coordination and integration of services at the clients' levels as a care manager as an implementer, a linkman, counsellor. The second is at the system level which is possible role for coordination and linkage of programs as a characteristics of care managers, task with formal & informal network, community resources, available residential & NHS resourcesw.

마을건강원 역할 및 업무내용에 관한 고찰 (A Review on the Role and Functions of the Village Health Worker)

  • The objective of this study is to review the role and function of the village health worker in the wake of the recent social changes in Korea. The stud}' also aimed at' elucidating' the characteristics of the village health worker organizations developed through community participation in this country. The study methods employed were: a review of literature and other existing information related to VHW's activities ; the use of Questionnaire and interview with VHW's on their activities in one selected county in 1992. The major findings obtained are as follows: 1. So far the community health care activities have been carried out by medical colleges, foreign aid agencies and religious organizations with a view to improving health care for the population in rural area. 2. The VHWs employed by the local community health services received remuneratiorin cash or in kind. However, presently no remuneration is paid to VHWs employed by the government village health projects. The training for VHWs is organized by County Health Center once a year for 1-2days regardless of pre-employment or refresher. 3. The needs of care for maternal and child health and family planning has been decline due to the decrease of the target population, but on the other hand, the needs of care by VHW for prevention of infectious diseases, health of the aging, management of geriatric diseases, prevention of poisoning of pesticides and environmental sanitation has beer increased. 4. It was felt strongly that the training for VHWs should be strengthened by means of developing a problem-solving oriented educational program in order to improve practical skills of VHWs.

요양보호사의 직무스트레스와 정신건강의 관계에서 정서조절곤란의 매개효과 (The Mediating Effects of Emotional Dysregulation on the Relationship between Care Worker's Job Stress and Mental Health)

  • 본 연구는 요양보호사의 직무스트레스와 정신건강과의 관계에서 정서조절곤란의 매개효과를 검증하였다. 노인요양기관에서 근무하는 요양보호사 154명을 대상으로 요양보호사 직무스트레스척도, 한국판 정서조절곤란 질문지, 간이정신진단검사를 시행하였다. 위계적 회귀분석을 통해 직무스트레스, 정서조절곤란, 정신건강 간의 직접 효과를 검증하고, 부트스트랩 검증을 통해 직무스트레스와 정신건강에 대한 정서조절곤란의 매개효과 유의성을 검증하였다. 분석결과, 직무스트레스와 정신건강과의 관계에서 정서조절곤란의 하위요인 중 전략적 접근 제한과 명료성의 부족은 완전매개역할을 하는 것으로 나타났다. 이는 요양보호사의 정신건강을 위해서는 직무스트레스를 경감시키는 것만으로는 부족하고, 요양보호사들이 자신의 부정적인 정서를 다룰 수 있는 조절능력을 기를 수 있는 방안이 필요함을 시사한다.

요양보호사의 직무만족도에 관한 연구 (The Study on Job Satisfaction of Care Helpers)

  • Purpose: The purpose of this study was to evaluate Job Satisfaction of Care Helpers. Method: Subjects included 306 Care Helpers. working at geriatric care facilities; these data were collected from May 4 to May 15, 2012. The SPSS WIN 12.0 program was used for statistical analysis of collected data, including actual number, percentage, average, standard deviation, t-test, one way ANOVA, and Cronbach alpha coefficient. Results: 1. The average Job satisfaction was 3.76 and the average of each job satisfaction was the task (3.83), co-worker (2.23), and job turn over (2.25). 2. The satisfaction of the job itself, depending on back ground, had a significantly effect on age, protective person for a day, duty pattern, contract pattern, and motive. 3. The job satisfaction of human relationships, depending on their back ground, had a significant effect on degree, care-giving experience, duty place, protective person for a day, duty pattern, contract pattern, and duty motive. 4. The Job satisfaction of job turn over, depending on back ground had an. effect on job satisfaction statistically: age, degree, duty place, protective person for a day, duty pattern, and motive. Conclusion: These results found showed to contribute to job satisfaction of care helpers.

교육훈련대상자의 학습전이와 그 영향요인에 관한 연구: 치매전문교육을 받은 요양보호사 중심 (A Study on Learning Transfer and Its Influencing Factors of Job Training Program Participants: Focusing on the Geriatric Care Worker who Received the Dementia Job Training)

  • 본 연구의 목적은 치매교육훈련을 받은 요양보호사들의 학습전이 수준과, 학습전이에 미치는 영향요인간의 관계 및 설명력을 살펴보는데 있다. 서울시와 경기도에 근무하는 요양보호사 중 치매교육훈련을 수료한 후 3개월 이상 12개월 미만인 자를 연구대상으로 하였고, 총 279명에 받은 설문지를 바탕으로 다중회귀분석을 실시하였다. 분석결과, 치매교육훈련 학습자의 자기효능감이 높을수록, 교육훈련설계 중 교육방법과 강사역량이 높을수록, 업무환경 중 전이기회가 높을수록 학습전이가 높게 나타났다. 이러한 결과는 학습전이를 향상시키기 위하여 요양보호사의 자기효능감 증진, 업무현장에서 적용할 수 있는 실무 중심의 교육훈련, 장기요양업무관련 실무경험이 많은 강사선정의 중요성, 업무현장에서 요양보호사에게 더 많은 전이기회가 제공되어야 함을 시사한다.

노인요양병원 사회복지사의 대처전략과 직무효능감이 직무소진에 미치는 영향 (Influences of Coping Strategies and Vocational Self-Efficacy on Burnout among Social Workers in Long-Term Care Hospitals)

  • 본 연구의 목적은 노인요양병원에 종사하는 사회복지사의 직무소진 정도를 살펴보고, 대처전략과 직무효능감이 직무소진에 미치는 영향을 확인하는 것이다. 이를 위해 노인요양병원협회 회원으로 등록된 전국 528개(2015년 9월 기준) 노인요양병원 중 병상의 규모가 100병상 이상인 병원의 사회복지사 138명을 대상으로 설문조사를 실시하였다. 분석 결과 근무시간, 외재화 전략, 자기 효능감, 집단효능감이 직무소진과 유의한 영향을 보이고 있었다. 즉, 근무시간이 길수록 소진의 정도가 심각해지는 반면, 외재화 전략, 자기효능감, 집단효능감이 높을수록 직무소진이 경감되었다. 본 연구결과를 토대로 노인요양병원 사회복지사의 직무소진을 경감시키기 위해 슈퍼비전 체계의 구축. 소진 경감 프로그램의 개발, 그리고 보수 교육 제공 등과 같은 개입 방안들을 제안하였다.
