Effects of a Complex Exercise Program on the Distance between Knees and Balance in Individuals in their 20s with Genu Varum

  • Background: Thera-Band, Narrow squats, Kinesiology taping helps in the reduction of loading on the knee joints. Despite the fact that the varus knee negatively affects the alignment of the lower extremities, most of the studies have analyzed each independently. Objectives: To investigate the effects of a complex exercise program consisting of elastic band exercises and squat exercises on the distance between the inner knees and balance in young adults with genu varum. Design: A cluster randomized controlled trial. Methods: The complex exercise group performed resistance exercises using an elastic band. The taping group used kinesiology tape on the vastus lateralis and biceps femoris. To select those to be included in the study, we measured the distance between the knees using digital Vernier calipers and to measure the balance ability, we used a balance training system. The data were analyzed with the independent t-test and paired t-test. Results: The study indicated a significant difference in the distance between the knees between the two groups, but no significant differences in the dynamic balance between the groups. Also, the static balance comparison between the groups according to the intervention method included the trace length, C90 area, C90 angle and velocity. There were no significant differences in the static balance between the groups. In addition, the complex exercise program was more effective than taping. Conclusion: The results of this study demonstrate that the complex exercise program and taping decrease the between both the knee and increase the balance.

  • The aim of this study is to identify the effects of the correction exercise program on femoral intercondylar distance, Q-angle and plantar pressure in undergraduate with genu varum. A experimental study with a randomized controlled trial design was used. Thirty subjects with genu varum were allocated in two groups: the experimental group (n=15) or the control group (n=15). The experimental group conducted correction exercise program for 30 minutes a day, 5 times a week, for 6 weeks, while the control group did not perform any exercise. The correction exercise program group showed a significant decreased femoral intercondylar distance, plantar pressure of M 4, M 5 (Metatarsal 4, 5). Also the correction exercise program group significantly increased Q-angle, plantar pressure of Toe 2-5 and M 1, M 2, M 3 (Metatarsal 1, 2, 3) (Rt, Lt) (p<.05). Thus, we suggested that the correction exercise program may be suitable intervention to improve normal alignment in undergraduate with genu varum.

Comparison of Q-angle and Muscle activity when Elastic band and Lunge are Applied to People with Genu Varum (내반슬을 가진 사람에게 탄력밴드와 런지 적용 시 Q각 및 근 활성도 비교)

  • The purpose of this study was to compare the effects of elastic band exercises and lunge on the knee Electromyography and Q-Angle in people with genu varum. The subjects of this study were healthy general people who had genu varum of 18 subjects. The subjects were randomly divided into two groups as a elastic band group (n=9, 6 males and 3 females, age: $22.55{\pm}2.05years$, height: $169.8{\pm}6.07cm$, weight: $66.44{\pm}9.83kg$) and a lunge group (n=9, 7 males and 2 females, age: $23.88{\pm}1.37years$, height: $173.66{\pm}6.72cm$, weight: $67.22{\pm}13.30kg$).The elastic band exercise and the lunge were performed every week, for four weeks. These results suggest that the Q-Angle of both knees showed differences before and after the intervention in elastic band group and lunge group. Also, The MVIC was no changes in both groups before and after the intervention except for the left foot in elastic band group and lunge group. As a result, the space distance of knees in both groups was significantly reduced. These result suggested that the elastic band exercise and lunge were effective ways in alleviating genu varum.

Effect of Theratainment Low Extremity Complex Exercise using Unstable Surface on Knee and Plantar Pressure in Patient with Genu Varum (불안정 지지면을 적용한 테라테이먼트 하지복합운동이 안굽이 무릎 하지 변형자의 무릎과 족저압에 미치는 영향)

  • The purpose of this study was to investigate the effect of theratainment low extremity complex exercise using unstable surface on knee and static plantar pressure in patient with genu varum. 25 adult subjects were recruited and randomized into stable surface exercise group(SEG) and unstable surface exercise group(UEG). Subjects carried out complex exercise(elastic band exercise & squat exercise) for 40 minutes, 4 times per week for 5 weeks. The knee joint distance and Q-angle of all subjects measured, the foot pressure measured distribution on the front and rear of both foots during pre and post. The most outcome at post was significantly improved than the pre outcome in the both group(p<.05). The result of this study show that the low extremity complex exercise using unstable surface may be appropriate for improving structure of knee joint and static plantar pressure in patient with genu varum. This will enable the application of low extremity complex exercise using unstable surface in rehabilitation therapy of genu varum and it will help guide the selection of the therapist as one therapeutic basis.

Immediate Effects of the Pronation Squat on the Genu Varum and the Muscles Around the Knee (엎침 스쿼트 운동이 안굽이와 무릎 주위 근육에 미치는 즉각적 효과)

  • Purpose : The purpose of this study was to compare the ankle pronation squat with the general squat and investigate the effect on the geun varum and the muscles around the knee. Methods : Subjects were chosen as the target for squat exercise with the distance between the knees more than 5 cm. The selected 30 students were randomly divided into 15 pronation squat group and 15 general squat group, and performed five sets movements 20 times. Global postural system (GPS) and digital goniometer were used to check the distance between the knees and the Q angle, and muscle activity was measured with EMG during squat exercise. Results: The result is as follows. First, as a result of analyzing the change in the distance between the knees, the distance between the knees decreased and the Q angle increased in the pronation squat group after exercise. Second, as a result of analyzing the change in muscle activity of the peroneus longus, the muscle activity increased in the pronation squat group after exercise, and it was more effective than the general squat group. Third, as a result of analyzing the change in the VMO (vastus medialis oblique) and VL (vastus lateralis) muscle activity ratio, the activity ratio of the pronation squat group increased after exercise, and the imbalance in the VMO/VL muscle activity was decreased. Conclusion: The pronation squat exercise applied to the ankle will greatly affect what is made into a balanced leg as decreased of the distance between knees, increased of Q angle, increased of the muscle activity of the peroneus longus and the ratio of VMO/VL.

Effect of Self-gravity Acupressure on Varus Knee Correction: Retrospective Observational Study (자가 중력 지압에 의한 내반슬 교정 효과: 후향적 관찰 연구)

  • Background: Despite the positive effects of the self-gravity acupressure program on genu varum correction, no systematic observation has yet to be made. Purpose: In this study, the effect of the self-gravity acupressure program on varus knee correction was confirmed retrospectively by comparing the medial angle of the knee before and after the self-gravity acupressure program. Methods: Fifty people with varus knees were subjected to a 75-minute acupressure program while lying on an acupressure tool, and then the medial knee angles were compared before and after the program. Results: The average angle of the inside of the knee of the study subjects increased by 7.29±3.85° (p<.000) on the left side and 7.08±3.86° (p<.000) on the right side after the acupressure program was implemented, indicating that the study subject's varus knee significantly improved even after applying for a short period time. It was confirmed. Conclusion: This study confirmed that the self-gravity acupressure is a valuable program for correcting the genus varus. However, to use the results of this study clinically in the future, further studies are required to optimize the mechanism and clinical effects of the self-gravity acupressure program on the improvement of the genu varus knee.

The Effect of Change in Transversus Abdominis Thickness Using Ultrasound Image during a Hip Adductor Contraction (고관절 내전근 수축이 복횡근의 두께변화에 미치는 영향)

  • Purpose :The present study invesigated the effect of changes in transversus abdominis thickness using ultrasonography during a hip adductor contraction. Methods : This study was carred out in a volunteer sample of adults (N=30) without a history of low back pain or injury. In standing position, muscle thickness measurements of transversus abdominis(TrA) were measured using ultrasonography at rest and during a hip adducor contraction. Results : TrA thickness were influenced a hip adductor during a voluntary contraction in people without LBP. TrA showed significantly greater thickness changes on a hip adductor contraction.(p=0.000) Conclusion : The results from this study showed that the hip adductor contraction improves the ability to increase change in TrA thickness. These results can be a good evidence to prevent low back pain due to hip adductor weakness or genu varum deformity of knee osteoarthritis.

Differences in Static Lower Extremity Alignment according to the History of Lateral Ankle Sprain: Efficacy and Limitation of Static Lower Limb Alignment Measurement as a Predictor of Lateral Ankle Sprain (외측 발목 염좌 병력에 따른 정적 하지 정렬 차이: 외측 발목 염좌의 예측인자로서 정적 하지 정렬 검사의 효용성과 한계점)

  • Objective: The aim of this study was to investigate 1) the difference in static lower extremity alignment (SLEA) according to a history of lateral ankle sprain (LAS), 2) to identify SLEA factors affecting LAS, and 3) to present the cut-off value and 4) the usefulness and limitations of the SLEA measurement. Method: This case-control study recruited 88 men (age: 27.78±4.69 yrs) and 39 women (age: 24.62±4.20 yrs) subjects with and without LAS. SLEA measurement protocol included Q angle, tibiofemoral angle, genu recurvatum, rear foot (RF) angle, tibal varum and torsion, navicular drop, ankle dorsiflexion range of motion (DF ROM). Independent t-test, logistic regression and receiver operating characteristic (ROC) curve were used for statistical analysis. Results: Men with a history of LAS had significantly smaller Q angles both in standing and in supine position, while women with a history of LAS had significantly greater DF ROM in non-weight bearing (NWB; p < 0.05). Logistic regression model suggests tibial varum (OR = 0.779, p = 0.021) and WB DF ROM (OR = 1.067, p = 0.045) were associated with LAS in men. In case of women, there were no significant SLEA factors for LAS, however, ROC curve analysis revealed standing RF angle (AUC = 0.647, p = 0.028) and NWB DF ROM (AUC = 0.648, p = 0.026) could be affecting factors for LAS. Conclusion: There are differences in SLEA according to the history of LAS, furthermore, the identified items were different by sex. In case of men, tibial varum and WB DF ROM affect LAS occurrence. Standing RF angle and NWB DF ROM of women could be a predictor for LAS. However, since the sensitivity and specificity in most of the SLEA measurements are low, kinematic in dynamic tasks should be considered together for a more accurate evaluation of LAS risk.

  • Achondroplasia and hypochondroplasia are the two most common forms of short-limb dwarfism. They are autosomal dominant diseases that are characterized by a rhizomelic shortening of the limbs, large head with frontal bossing, hypoplasia of the mid-face, genu varum and trident hands. Mutations in the fibroblast growth factor receptor-3 (FGFR3) gene, which is located on chromosome 4p16.3, have been reported to cause achondroplasia and hypochondroplasia. More than 98% of achondroplasia cases are caused by the G380R mutation (c.1138G>A or c.1138G>C). In contrast, the N540K mutation (c.1620C>A) is detected in 60-65% of hypochondroplasia cases. Tests for common mutations are often unable to detect the mutation in patients with a clinical diagnosis of hypochondroplasia. In this study, we presented a case of familial hypochondroplasia with a rare mutation in FGFR3 identified by next generation sequencing.

Hindfoot Alignment Change after High Tibial Valgization Osteotomy in a Patient with an Ipsilateral Fused Ankle: A Case Report (족관절이 유합되어 있는 상태에서 동측 슬관절 고위경골외반절골술을 시행한 이후 후족부 정렬의 변화: 증례 보고)

  • Ankle arthrodesis was performed on a 55-year-old male patient with an active lifestyle who developed severe arthritis in the left ankle. Over the follow-up period, high tibial valgization osteotomy was conducted for painful medial knee joint arthritis with genu varum deformity to correct overall lower limb alignment from varus to valgus with respect to the fused ankle. This study was conducted to investigate how hindfoot alignment would change when the overall alignment of the lower limb shifted from varus to valgus with the ipsilateral ankle in a fused state. Conclusively, while no intrinsic changes in the hindfoot alignment were observed following the alteration of lower limb alignment, the hindfoot naturally adjusted to valgus deviation in response to limb valgus realignment. Moreover, symptoms changed in line with this adjustment. Given the absence of similar case studies or reports, a review of relevant literature is included to contribute to knowledge of this subject.
