Background: The aim of this study is to compare the numbers of axillary lymph nodes (ALN) taken out by dissection between patients with breast cancer operated on after having neoadjuvant chemotherapy (NAC) treatment and otherswithout having neoadjuvant chemotherapy, and to investigate factors affecting lymph node positivity. Materials and Methods: A total of 49 patients operated due to advanced breast cancer after neoadjuvant chemotherapy and 144 patients with a similar stage of the cancer having primary surgical treatment without chemotherapy at the general surgery clinic of Ondokuz Mayis University Medicine Faculty between the dates 01.01.2006 and 31.10.2012 were included in the study. The total number of lymph nodes taken out by axillary dissection (ALND) was categorized as the number of positive lymph nodes and divided into <10 and ${\geq}10$. The variables to be compared were analysed using the program SPSS 15.0 with P<0.05 accepted as significant. Results: Median number of dissected lymph nodes from the patient group having neoadjuvant chemotherapy was 16 (16-33) while it was 20 (5-55) without chemotherapy. The respective median numbers of positive lymph nodes were 5 (0-19) and 10 (0-51). In 8 out of 49 neoadjuvant chemotherapy patients (16.3%), the number of dissected lymph nodes was below 10, and it was below 10 in 17 out of 144 primary surgery patients. Differences in numbers of dissected total and positive lymph nodes between two groups were significant, but this was not the case for numbers of <10 lymph nodes. Conclusions: The number of dissected lymph nodes from the patients with breast cancer having neoadjuvant chemotherapy may be less than without chemotherapy. This may not always be attributed to an inadequate axillary dissection. More research to evaluate the numbers of positive lymph nodes are required in order to increase the reliability of staging in the patients with breast cancer undergoing neoadjuvant chemotherapy.

In this paper, diversity-multiplexing tradeoff (DMT) of three-user wireless multiple-antenna cooperative systems is investigated in general fading channels when half-duplex and decode-and-forward relay is employed. Three protocols, i.e., adaptive protocol, receive diversity protocol, and dual-hop relaying protocol, are considered. The general fading channels may include transmit and/or receive correlation and nonzero channel means, and are extensions of independent and identically distributed Rayleigh or Rician fading channels. Firstly, simple DMT expressions are derived for general fading channels with zero channel means and no correlation when users employ arbitrary number of antennas. Explicit DMT expressions are also obtained when all users employ the same number of antennas and the channels between any two users are of the same fading statistics. Finally, the impact of nonzero channel means and/or correlation on DMT is evaluated. It is revealed theoretically that the DMTs depend on the number of antennas at each user, channel means (except for Rayleigh and Rician fading statistics), transmit and/or receive correlation, and the polynomial behavior near zero of the channel gain probability density function. Examples are also provided to illustrate the analysis and results.
This study aims to forecast of the number of smart phone addicted youths and to evaluate the effect of rehabilitation programs. A system dynamics model is developed to describe the processes of addiction and transits between phases as well as the diffusion of smart phones. The youths are grouped into non users, general users, potential risky users, and high risky users. The model utilizes the population distributions over ages for the next 30 years forecasted by Korean Government. The number of youths decreases for the next decade or so, and the number of youths who owns smart phone will reach maximum at 2017. As for the rehabilitation programs, the model includes the preventive education for general users, counseling for potential risky users, and professional therapy for high risky users. The preventive education restricts the transit from general users to potential risky users. Counseling increases the transit from potential risky users to general users while it decreases the transit to high risky users. Professional therapy improves transit to potential risky users and to general users. Although the model cannot be validated the accuracy owing to the lack of data, it describes these transit within the reasonable ranges and can be used to study the allocation of the limited resources to maximize the outcome.
Objective: To compare the IVF outcomes of mild ovarian stimulation with conventional ovarian stimulation in poor responders. Methods: From 2004 to 2009, 389 IVF cycles in 285 women showed poor responses (defined as either a basal FSH level ${\geq}$12 mIU/mL, or the number of retrieved oocytes ${\leq}$3, or serum $E_2$ level on hCG day <500 pg/mL) were analyzed, retrospectively. In total, 119 cycles with mild ovarian stimulation (m-IVF) and 270 cycles with conventional ovarian stimulation (c-IVF) were included. Both groups were divided based on their age, into groups over and under 37 years old. Results: The m-IVF group was lower than the c-IVF group in the duration of stimulation, total doses of gonadotropins used, serum $E_2$ level on hCG day, the number of retrieved oocytes, and the number of mature oocytes. However, there was no significant difference in the number of good embryos, the number of transferred embryos, the cancellation rate, or the clinical pregnancy rate. In the m-IVF group over 37 years old, the clinical pregnancy rate and live birth rate were higher when compared with the c-IVF group, but this result was not statistically significant. Conclusion: In poor responder groups, mild ovarian stimulation is more cost effective and patient friendly than conventional IVF. Therefore, we suggest that mild ovarian stimulation could be considered for poor responders over 37 years old.
This study purported to acquire information necessary to improve the management of general hospitals. It tried to determine major indices which represent managerial performance of general hospitals and to identify the managerial characteristics of general hospitals which affect the major financial indices. Eighty-eight hospitals were chosen from 188 hospitals which were subject to standardization audit by the Korean Hospital Association. The results of a discriminant analysis are summarized as followings. First, when a single index was used to measure managerial performance of the sample hospitals, the ration of net profit to total capital was the best index and its discriminant power was 58.14%. The ratio of the number of boardmen((M. D.) and average daily medical cost were highly related to this index. Second, when two indices were used, income growth rte and the ration of net profit to total capital had the highest discriminant distinction ability. Their discriminant power was 61.9%. In this case, the ratio of the number of boardmen(M. D.) was significantly and highly related to the indices. Third, when all three indices-income growth rate, the ration of net profit to total capital and quick ratio - were used together, a discriminant function was statistically insignificant. Therefore, using all three indices was not useful in measuring managerial performance of the sample hospitals. In conclusion, using two indices-income growth rate and the ration of net profit to total capital-was better in measuring manegerial performance of general hospitals than using a single index. The independent variable which affected these indices was the ration of the number of boardmen. Purpose : The purpose of this study was to analyze the trend of the number of nursing staffs and skill mix and to assess the effectiveness of hospital nurse expansion policies in Korea. Methods : The trend of the number of nursing staffs and skill mix were analyzed using time series data, which composed of yearly series data from 1975 to 2009. The impact of hospital nurse expansion policies was estimated by autoregressive integrated moving average(ARIMA) intervention model. Results : The number of general hospital and hospital nurses per 100 beds was decreased in late 1980s and late 1990s due to rapid growth of beds. As a result of the number of nurse aids per 100 beds decreased, skill mix became high in general hospital but nurse ratio among hospital nursing staffs was about 50%. Expansion of new nurse and revised differentiated inpatient fee were only effective in expansion of hospital nursing staffs. But they had no effect in general hospitals. Conclusion : In Korea, a few policies related to expansion of hospital nurses have an effect on increasing the number of hospital nurse. Nevertheless, level of hospital nursing staffs is inferior to that of general hospital.
The most difficult and time-intensive issue in the successful implementation of genetic algorithms is to find good parameter setting, one of the most popular subjects of current research in genetic algorithms. In this study, we present a new efficient experimental design method for parameter optimization in a genetic algorithm for general job shop scheduling problem using the Taguchi method. Four genetic parameters including the population size, the crossover rate, the mutation rate, and the stopping condition are treated as design factors. For the performance characteristic, makespan is adopted. The number of jobs, the number of operations required to be processed in each job, and the number of machines are considered as noise factors in generating various job shop environments. A robust design experiment with inner and outer orthogonal arrays is conducted by computer simulation, and the optimal parameter setting is presented which consists of a combination of the level of each design factor. The validity of the optimal parameter setting is investigated by comparing its SN ratios with those obtained by an experiment with full factorial designs.

Objectives : This study was done to suggest appropriate number and time of dispensing herbal medicine for each Korean pharmacist. Methods : For this study, we visited one herbal dispensary institution which was considered as one of the biggest herbal dispensary preparing the largest number of prescription. We observed the work in the dispensary and recorded time for preparing prescriptions of 5 Korean Pharmacists. And we also recorded time for preparing prescriptions with toxic medicinal herbs of each Korean Pharmacist. Investigation has been done for 3 weeks, from 11, Jan. to 4, Feb. in 2016. Results : The whole dispensed prescription number of 5 Korean Pharmacists during 3 weeks were 899. These prescriptions include toxic medicinal herbs and general prescriptions. Ratio of prescriptions including toxic medicinal herbs was 38.4% while general prescriptions was 61.6%. Average direct dispensing time for herbal prescription of Korean pharmacists were 6.2 minutes. Applied personal allowances, appropriate number for dispensing in each day was 22.7. With allowances rate in general workplace, appropriate number for preparing was 20.5. According to ILO allowances regulation, appropriate number for preparing was 18.8. Conclusions : Suggested number of dispensing herbal medicine per day for each Korean pharmacist was 20.5. Applying this would be beneficial for development of Korean medicine circumstances.
