Differences of Illness Behavior in Depressive Patients According to the Presence of Somatization (우울증 환자에서 신체화 증상에 따른 질병행동의 차이)

  • Objectives : Illness behavior is defined as the persistence of an adaptive/maladaptive mode of perceiving, evaluating and responding to health status and symptoms according to the status. In a cognitive aspect, somatizing symptoms are regarded as being originated from distortions, including magnification and/or amplification, of perceiving, evaluating and responding to symptoms such as bodily sensations. That is somatization may be explained by maladaptive illness behavior. In this study, we tried to investigate differences of illness behavior in depressive patient according to the presence of somatization. Methods : We divided 45 patients who were diagnosed as depression with ICD-10 diagnostic criteria into two groups(somatizing and non-somatizing group) according to the somatization subscale of Korean Depression Scale and compared two groups in the differences of illness behavior using the Illness Behavior Questionnaire. Results : Somatizing group showed significantly higher scores than non-somatizing group on the disease affirmation subscale($6.79{\pm}2.08$ vs. 4.76, p=0.003) and the denial subscale($3.25{\pm}1.22$ vs. $2.10{\pm}1.41$, p=0.006). There were no significant differences between two groups on the general hypochondriasis subscale and the affective unstability subscale. In a logistic regression analysis, somatizing group also showed higher odds ratio (OR) scores on the disease affirmation subscale(OR=1.418, p=0.089) and the denial subscale(OR=1.880, p=0.083). Conclusion : The disease affirmation and denial may be a discriminative mechanism of somatization in depressive patients. These subscales of Illness Behavior Questionnare could be useful markers, and psychiatric illnesses with somatizing and depressive symptoms may be differentially diagnosed and be predicted through these subscales.

Relations between the Emotional Labor and Depression Symptoms of Female Workers in Cosmetics Sales - With a focus on spiritual resilience - (화장품 판매직 여성근로자의 감정노동과 우울증상과의 관련성 -정신적 회복탄력성을 중심으로-)

  • The purposes of this study were to examine the depression level of sales workers at a cosmetics company in South Korea and investigate the effects of emotional labor, job stress, and resilience on their depression symptoms. A structured questionnaire was distributed to 189 workers at the cosmetics company to assess their general characteristics, diseases diagnosed by the doctor, depression, emotional labor, job stress, and resilience. Univariate analysis and multiple logistic regression analysis was conducted to examine connections among depression symptoms, emotional labor, job stress, and resilience. The findings show that the depression symptom rate of the cosmetics sales workers was 32.3%. The multiple logistic regression analysis results of the high risk group in depression symptoms show that depression had significant relations with resilience(OR : 3.84, CI : 1.68-8.77), diseases diagnosed by the doctor(OR : 3.82, CI : 1.73-8.47), emotional disharmony(OR : 2.51, CI : 1.06-5.96), and job demand(OR : 2.12, CI : 0.93-4.85). The low risk group of emotional labor was corrected in age, doctor's diagnosis, and job stress with the reference value to analyze connections with depression symptoms according to the degree of resilience. The analysis results show that depression symptoms increased by 2.35 times(CI : 0.73-7.53) when they had high emotional labor and good resilience, by 3.74 times(CI : 1.17-11.97) when they had low emotional labor and bad resilience, and by 10.39 times(CI : 3.34-32.28) when they had high emotional labor and bad resilience. These findings raise a need to run a program to increase resilience, reduce emotional disharmony through emotional labor management, and take health management measures to control physical illness and lower job stress so that cosmetics sales workers can manage their depression symptoms.

Effects of Dietary Fatty Acids on fatty Acid Pattern in Development Rat Brain Phospholipids - Effects on P/M/S and $\omega$3/$\omega$6 Fatty Acid Ratios -

  • Docosahexaenoic acid(DHA), a $\omega$3 series fatty acid and arachidonic acid(AA). a $\omega$6 series fatty acid were found in relatively high concentrations in the phospholipids(PLs) of cell membranes of nerve tissues, and they can be affected by various factors. The present study examined the effects of dietary $\omega$6 and $\omega$3 fatty acid composition on P/M/S and on $\omega$3/$\omega$6 fatty acid ratios in brain PLs of 2nd generation rats. The expeimental diets consisted of 10% fat(by wt), which were computer- searched mixed oil('M') with P/M/S ratio, 1 : 1.4 : 1 and $\omega$6/$\omega$3 ratio, 6 : 1 and safflower oil('S') poor in $\omega$3 fatty acids. The experimental diets were started 3-4 wks prior to conception. During the lactation period, the feeding mothers were switched 1 wk after birth and provided the pups for 2 wks with milk which had compositions different from that of their natural mother. The same diet as their mothers was provided from weaning to 9 wks of age. The 'M'and 'S' rats were again subdivided into MM, MS, SS, SM rats according to diet which their lactating mothers were fed from the begining of the experiment. The relative percentage of P/M/S fatty acids in brain PLs in all experimental groups converged to a very similar value at 9 wks of age, indicating the existence of a control mechanism for the degree of fatty acids, unsaturation. The $\omega$3/$\omega$6 fatty acid ratios of brain PLs converged to about 1.0 in MM & SM groups and to 0.7 in SS & MS groups, suggesting also the existence of some balance between $\omega$3 and $\omega$6 fatty acids in developing rat brain. The concentrations of $\omega$3 fatty acids, especially DHA, in the SM group were increased and became similar to those in MM group at 9 wks of age. The increase in DHA of brain PLs was counterbalanced b)r a decrease in 22 5$\omega$6. Therefore, the ratios of 22 : 6$\omega$6/22 : 5$\omega$6 were higher in both MM & SM groups than those of SS & MS groups at 9 wak of age. Although dietary $\omega$3 and $\omega$6 fatty acids affected 22 : 6$\omega$S and 22 : 5$\omega$6 contained in rat brain PLs reciprocally, the relative percentage of AA did not appear to be significantly influenced by the diet in all groups at 9 wks of age, suggesting that a mechanism for the maintenance of a certain level of AA in brain PLs exists. In conclusion, the $\omega$3/$\omega$6 fatty acid and 22 : 6$\omega$3/22 : 5$\omega$6 ratios, but not P/M/S ratio, of rat brain PLs were affected by the postnatal dietary changes. Futher studies are required to clarify the mechanism(S) of ensuring a certain level of DHA and of maintaining a similar level of AA in rat brain PLs after. weaning(9 wk) regardless of prenatal and postnatal dietary changes. Use of herbal medicine in epileptic children (소아 간질 환자에서 한약 복용 실태)

  • Purpose : Herbal medicine is thought to be widely used by children with epilepsy, but there have been few studies. This study aimed to investigate the perceptions and the actual conditions of usage of herbal medicine by children with epilepsy. Methods : From July to August 2007, three hundred seventy eight questionnaires were completed by parents of epileptic children who visited pediatric neurology clinic of Korea University hospital and Kwangmyung-Sungae hospital. Demographic data and patterns of usage of herbal medicine were investigated. Results : Among 378 patients, sixty five (17.2%) reported using herbal medicine. Major reasons for using herbal medicine were 'to enhance general health' and 'belief that it has fewer side effects'. Majority of respondents did not notify their physician about using herbal medicine because 'they did not need to share this with doctor' and 'they thought their doctor would disapprove'. After taking herbal medicine, 33.3% of patients reported that it did not benefit their seizure but they felt healthier. Usage of herbal medicine was significantly related to number of AEDs (antiepileptic drugs), duration of AED treatment, association with psychosomatic disorders and motivation by other people or by mass media. Conclusion : Considerable numbers of epileptic children were using herbal medicine during treatment with AED and did not inform their physician about usage of it. Physicians should be aware of benefits and harms of herbal medicine and actively intervene in the usage of herbal medicine by epileptic patients.

The Factors Related to Musculoskeletal Symptoms of Family Care-Givers who Have a Patient with Brain Damage (뇌손상 가족 간병인의 근골격계 자각증상과 관련요인)

  • This study was conducted to identify factors related to the musculoskeletal symptoms of 340 family caregivers who have a patient with brain damage based on self-administered questionnaires. The questionnaires included general characteristics, characteristics of care activities and caregivers' musculoskeletal symptoms. Data were collected from many long-term care hospitals and rehabilitation centers during March 17 to March 21, 2014. The results indicated that complaint rates of work-related musculoskeletal systems of the body sites differed. Factor analysis revealed that neck complaints were related to education (under mid), while shoulder complaints were related to sex (female), age (50-59), education (mid) and duration of care (< 2 years). Arm/elbow complaints were related to age (40-49), education (high) and duration of care (12-24). Complaints regarding the hand/wrist/finger were related to age (50-59) and duration of care (12-24), while those associated with the legs/feet were related to age (50-59) and duration of care (< 6[ED highlight - years? Please specify.]). Back problems were related to education (under mid, mid stage, high) and gait. The complaint rate regarding musculoskeletal symptoms during daily life was not statistically significant. Logistic regression analysis of significant factors related to subjective musculoskeletal symptoms identified ambulation and gait as having the greatest influence and complaint factor among family caregivers. The complaint rate of family caregivers differed among body parts. Being a caregiver for less than one year was found to have a significant impact on pain. Overall, long term family caregivers could be faced with risk factors for musculoskeletal problems, but there are many different factors that affect musculoskeletal symptoms with regards to their activities. Accordingly, comprehensive and systematic prevention plans for family caregivers who have patients with brain damage should be developed.

A Survey on the Satisfaction of Middle School Students with School Meals and Their Food Preferences (일부 중학생의 학교 급식 만족도 및 기호도 조사 연구)

  • A survey of the satisfaction of school meals and food preferences was conducted with an objective to enhance the quality of food service. The study conducted a survey for 292 middle school students (152 boys and 140 girls), who were either in $1^{st}$ or $2^{nd}$ year. According to the general information, the number of $1^{st}$ and $2^{nd}$ year students was 134 and 158, respectively. Usually, the mother (92.8%) prepared meal the most, followed in order by the grandmother (5.8%) and father (1.4%). The top priorities of choosing food were taste (82.9%), good for health (6.5%), nutritional value (4.5%), and flavor (3.1%). The satisfaction was investigated using a 5 point scale. The satisfaction of taste was rice ($3.99{\pm}0.74$), side dishes ($3.99{\pm}0.65$), soup ($3.61{\pm}0.86$), kimchi ($3.54{\pm}1.12$). The satisfaction of quantity was rice ($3.88{\pm}0.83$), soup ($3.87{\pm}0.79$), kimchi ($3.80{\pm}0.89$), and side dishes ($3.71{\pm}0.93$). The satisfaction of diversity was side dishes ($4.09{\pm}0.67$), rice ($4.01{\pm}0.77$), soup ($4.01{\pm}0.64$), kimchi ($3.84{\pm}0.86$). For food preferences, the noodle was ranked highest while vegetables showed the lowest. Overall, support will be needed to develop diverse menus for students. This study suggests the need for active nutrition education for establishing proper eating habits.

Changes in Protease and Formonitrogen of Salted Fish and Shellfish (1) (젓갈 성숙과정(成熟過程)에 있어 Protease 및 Formonitrogen의 변화(變化)에 관(關)한 연구(硏究)(1))

  • Salted fish and shellfish have been widely used in Korea from olden times as side-dishes, although the processes and methods of pickling have varied depending on localities. The common raw materials for these subsidiary food articles include anchovy, shrimp, yellow corvina, oysters, octopus, top-shell, shellfish, pollack roe and pollack intestines. It must be pointed out here, however, that the salted stuffs now marketed locally are highly unscientific and unsanitary in the way they are processed and sold, and this has prompted this writer to undertake a study on these native food articles. The following findings have been obtained from this study on the changes in Formonitrogen and Protease Activity, effected by the density of salt and the degree of storing temperature, of the pickles of cedfish gills, codfish intestines, pollack intestines, shellfish, oysters, cuttle fish and octopus. 1) Codfish Gills The Protease Activity of the pickled codfish gills was greater in the groups of lesser doses of salt and higher degrees of storing temperature. The same was true in the case of Formonifrogen, too. The Formonitrogen of the pickled codfish gills was larger in the groups of lesser salt and higher teimperature. 2) Codfish Intestines The amount of Formonitrogen of the pickled codfiah intestines became greater, as time went by, in the groups of lesser salt than those of larger doses of salt, with the speed of its formation getting faster as the storing temperature rose from $5^{\circ}C$ to $15^{\circ}C$ and $30^{\circ}C$. The Protease Activity was also greater in the groups of lesser salt and higher temperature. The group, stored at 10% salt and $30^{\circ}C$, rotted in five days. 3) Pollack Intestines The amount of Formonitrogen of the pickled pollack intestines was greater in the groups of lesser amount of salt and higher degrees of storing temperature. The Protease Activity of the pickled pollack intestines began decreasing from the 11th day after the pickling in the groups stored at colmparatively high degrees of temperature$(15^{\circ}C-30^{\circ}C)$, while that of the group stored at $5^{\circ}C$ kept rising. The effects of the amount of salt were little. The group stored at 15% salt and $30^{\circ}C$ started rotting on the 13th day while that stored at $30^{\circ}C$ decayed on the 7th day. The group stored at 20% salt and $30^{\circ}C$ rotted on the 9th day. 4) Oysters The amount of Formonitrogen of the pickled oysters became greater as the storing temperature rose and the doses of salt were lowered. The Protease Activity was not affected at any measurable degree by the density of salt in the group stored at $5^{\circ}C$, but became greater as the storing temperature rose to $15^{\circ}C$ and $30^{\circ}C$. The group stored at 10% salt and $30^{\circ}C$ rotted on the 5th day while that stored at 20% salt and $30^{\circ}C$ on the 13th day. 5) Shellfish The amount of Formonitrogen of the pickled shellfish became greater, as time went by, it the groups of lower consistency of salt than the groups of higher density of salt, although the decay of the former groups was faster than the latter groups. The density of salt best fitted for the pickling appeared to be about 20% with the storing temperature to be $15^{\circ}C$, at which the pickled stuff became most tasty on the 7th day. The oysters stored in three groups at $5^{\circ}C$, $15^{\circ}C$ and $30^{\circ}C$ respectively showed the greatest Protease Activity alike at 0% of salt, but the activity declined as the density of salt increased. The Protease Activity of each group rose for the first 11 days after the pickling, but began declining from the 13th day onward, with the groups of higher temperature retaining higher Protease Activity than the groups of lower temperature. 6) Cuttlefish Both the amount of Formonitrogen and the degree of Protease Activity of the pickled cuttlefish were greater in the groups of lower density of salt and higher degree of storing temperature. The oysters pickled at 10% salt and $15^{\circ}C$ degenerated on the 13th day while that of 10% salt and $30^{\circ}C$ deteriorated on the 7th day. 7) Octopus Both the Formonitrogen and the Protease Activity of the pickled octopus were greater in the groups of lower density of salt, but as time went by, the Protease Activity in all groups dwindled after a climbing. In general, the Formonitrogen and the Protease Activity of the pickled oysters became greater as the storing temperature got higher. One group stored at 10% salt and $15^{\circ}C$ rotted in 13days while another group stored at $30^{\circ}C$ decayed in 7 days.

  • The purpose of this study was to investigate the effects of apo E polymorphisms and dietary counseling on the levels of plasma lipids in hyperlipidemic patients. The changes of serum lipids were assessed f3r 34 hyperlipidemic out-patients who changed their basal diet containing 20.1% fat(236.0mg cholesterol/day), 15.7% protein, and 64.2% carbohydrate to a diet containing 18.3% fat(109.8mg cholesterol/day), 15.7% protein, and 66.0% carbohydrate for 12 weeks. Management Improvement of Big and Old Trees in the Byeol-seo Scenic Sites (별서 명승지역 내 노거수목의 관리개선방안)

  • Big and old trees in the scenic spots with the attributes of remote villas are vulnerable to man-made damages and very sensitive to the external environment such as soil conditions, so the corresponding management plans are required. Thus this study has been conducted to survey the big and old trees in the scenic remote villas and suggest the ideal management plans. The results can be summarized as follows. First, regarding the tree heath above the ground, transformation of tree, death of branches, and death of barks are closely related to tree vigor. Particularly, the areas receiving many visitors require prompt countermeasures against the dried and dead tress above the ground and the areas in which dried and dead tress occurred and also the safety measures for the visitors and facilities. Second, regarding the soil environment, visitor traffic is closely related to the tree vigor. In 15 remote villa gardens, 64% of trees are exposed to heavy traffic and the tree vigor has declined due to an increase of visitor. Thus, there is a need to give positive consideration the installation of the complementary facilities and the plantation of herbal plants in the congested areas to form the ground surface that can tolerate the heavy visitor traffic. Third, remote gardens are in general located adjacent to ponds and mountain streams and thus the trees in the waterfront areas require the prompt countermeasures against the decline of growth due to the excess-moisture in the soil. Further the blockage of the sewage system due to the heavy rains dampens the surrounding soil, which results in lethal damages to the trees. Thus, there is a need of the maintenance of the waterfront areas and sewage system before and after the rainy season. In addition, there is a need to establish medium-long term management polices through the recognition of the importance of the main trees of remote villa gardens in scenic spots and prepare the tree management manual depending on the attributes of the corresponding areas. I strongly suggest making manuals for the systematic management as well as the extensive PR activities and education for the preservation of tress on a long-term basis; and furthermore securing the budget and manpower for the research and development of a systematic management system.

Current status of dietary education in elementary, middle and high school in Gyeonggi province: Comparison according to school level and placement of nutrition teacher (경기도 초·중·고등학교의 식생활 교육 현황 : 학교급 및 영양교사 배치여부에 따른 비교)

  • Purpose: The purpose of this study was to investigate the current status of dietary education carried out by dieticians or nutrition teachers at elementary, middle, and high schools in Gyeonggi province. We compared dietary education status not only among school level but also between dieticians and nutrition teachers. Methods: The survey using self-administered questionnaires was conducted by dieticians or nutrition teachers at 91 schools (43 elementary schools, 33 middle schools, and 15 high schools) in Gyeonggi province in December, 2015. The questionnaire consisted of questions regarding general characteristics of respondents, current status of dietary education, opinion about dietary education methods, and cooperation level of persons concerned. Results: The placement rate of nutrition teachers was 50.5% overall. Only 36 out of 91 schools conducted regular dietary education, and the percentage of schools conducting regular dietary education was significantly different according to school level with the lowest percentage in high schools, equivalent to 20% (p = 0.003). The average annual dietary education time was 12.4 hours, and it was very low in high schools (5.1 hours) and in schools without a nutrition teacher (1.6 hours). The levels of cooperation and support from principals, teachers, parents, and students were significantly lower in high schools than elementary and middle schools (all p < 0.001) and in schools without a nutrition teacher than schools with a nutrition teacher (all p < 0.05). Conclusion: The results show that the level of dietary education is not enough and needs to be improved, especially at middle and high schools and at schools without a nutrition teacher. Government support polices need to be implemented to encourage dietary educational activities.
