Factors Affecting the Healthcare Utilization of Spinal and Joint Surgery in Elderly Patients (65세 이상 노인의 척추·관절 수술별 의료이용에 미치는 영향요인 분석)

  • Background: The purpose of this study is to analyze the current status and factors of elderly patients' hospitalization for hip replacement, knee replacement, and general spine surgery. Methods: National health insurance data in 2018 was provided by the National Health Insurance Service. We used multiple regression to analyze factors associated with the medical utilization of hip replacement, knee replacement, and general spine surgery in elderly patients over 65 years old. The dependent variables are the length of stay and total health expenditure. The independent variables are the demographic-social factors (sex, age, region, insurance type, income level) and surgery-related factors (institution type, location of the hospital, surgery classification). Results: The most common factor affecting surgery was the location of medical institutions. Compared with the medical institutions located in metropolitan, the length of stay in rural medical institutions was higher and total health expenditure was lower. The lower quartile of income, the higher the length of stay and total health expenditure. In addition, the variables of age, type of health insurance, and type of medical institution were statistically significant. Conclusion: In this study, we confirmed the effect of sociodemographic factors and medical institution factors on the Healthcare Utilization of spinal and joint surgery.

Comparison of Demographic Characteristics, Health and Quality of Life between General Adults and Adults Living with Dementia Patients : The 2016 Community Health Survey (치매 환자와 함께 거주하는 성인과 일반 성인 사이의 인구사회학적 특성, 건강 및 삶의 질 비교 : 2016년 지역사회건강조사 원시자료를 이용하여)

  • Objective : The purpose of this study was to compare the demographic characteristics, health and quality of life between general adults and adults living with dementia. Method : The data were collected using raw data of the 2016 community health survey and compared between 2,592 adults living with dementia patients and 225,840 general adults. health were assessed for sleep time, stress level, depression, and subjective health status, and quality of life was measured by EQ-5D. Result : In comparison of demographic characteristics, age and family number of adults living with dementia patients were significantly higher than general adults (p<.001), income and eduation levels were low (p<.001), and marital status was higher rate of living with spouse (p<.05). In comparison of health status, adults living with dementia patients were significantly longer in sleep time than the general adults (p<.001), and stress level was higher (p<.001), the percentage of experience of depression was higher (p<.001), and the subjective health status was worse (p<.001). Adults living with dementia patients were significantly lower in quality of life total score and all sub-domain than general adults (p<.001). Conclusion : Based on the results of this study, it is necessary to seek ways to improve the health and quality of life of dementia patients' families.

Perceived Health Knowledge, Health Education Needs, and Health Behavior Different by Grade in Some Elementary School Students (보건지식, 보건교육요구도, 보건행동 자각수준의 학년별 차이: 서울 일부지역 초등학교 고학년 학생들을 대상으로)

  • Objectives: This study was conducted to describe child perceived health knowledge, health education needs, and health behaviors by grade in a representative general characteristic and examined their associations for students' better health behaviors. Methods: The survey participants were 410 fourth to sixth grade students in two elementary schools in Seoul, Korea. A total of 12 classes in two elementary schools were randomly selected and all students of the selected classes participated in the self-administered survey. The questionnaire contained the items of perceived health knowledge, health education needs(health topics which they want to know more), health behavior, and general characteristics. Results: Perceived health knowledge, health education needs, and health behaviors were, generally, better in the $4^{th}$ grade students than $5^{th}$ or $6^{th}$ grade students. That is, higher grade students had lower perceived health knowledge, lower health education needs, and lower health behavior status. Specifically, there was significant grade differences in 'significance of publichealth,' 'nutritionandhealthyeating,' 'desirablehealthhabits,' 'humansex,' 'physical development in childhood,' 'stress management & drugcontrol,' and 'injury prevention.' Correlations between perceived health knowledge and health behavior were low or medium in all grades. However, correlations between perceived health education needs and health were significant in the $4^{th}$ grade students but not significant in the $5^{th}$ and $6^{th}$ grade students. Conclusions: There was significant grade difference of the relationship between health knowledge, health education needs, and health behaviors among children. In general, the lower the grade the better the health knowledge, educational needs, and health behavior. Health education needs were more significant by grade than perceived health knowledge and health behaviors.

Quality of Sexual Life in Married Women: Its Factor Structure and Determinants (기혼 여성에 있어서 성생활의 질: 그 요인 구조와 결정 변인)

  • Purpose: This study was examined the quality of the sexual life among married women in terms of the factor structure of Sexual Quality of Life(SQOL) and evaluated the predictive value of 11 variables for SQOL. Method: Two hundred and ninety-three married women completed the Korean version SQOL scale with information on age, education, length of marriage, family income, health, sexual knowledge, coital frequency, harmony with family members, levels of affection, and general and sexual communication. Data were analysed with principal components and regression analyses. Result: The SQOL scale had two clearly defined factors, Negative Quality of Sexual Life and Positive Quality of Sexual Life, on which all scale items were significantly loaded, accounting for 54.3% of the total variance. Independent regression analysis showed all variables except education to be significant predictors of SQOL with interpersonal variables (general and sexual communication, affection and harmony with family members) being the most powerful predictors. However when the 11 variables were combined together in stepwise analysis, only 6 variables (general communication, age, affection, coital frequency, sexual knowledge and health) were significant, accounting for 39% of the total variance. Conclusion: The Korean version SQOL scale is a good measure to tackle SQOL in married women. To enhance SQOL, strengthening interpersonal interaction focusing on general/sexual communication, affection and harmony with family members seems to be more important than sexual activities and personal characteristics.

Prevalence of Musculoskeletal Symptoms Related With Activities of Daily Living and Contributing Factors in Korean Adults

  • Objectives: This study aims to investigate the prevalence of musculoskeletal symptoms and factors related to daily activities in a representative Korean population. Methods: This study was based on the questions about musculoskeletal symptoms in the Korean General Social Survey 2010. The questionnaire about musculoskeletal symptoms was adopted from Korean Occupational Safety and Health Agency guide, and it includes general characteristics, characteristics of pain, work type, work intensity and a 12-item Short Form Health Survey (SF-12). We utilized the criteria of the National Institute for Occupational Safety and Health to define the prevalence of musculoskeletal symptoms. Demographic, behavioral and socioeconomic factors were analyzed using logistic regression. Results: The prevalence of musculoskeletal symptoms was 38.3%. The prevalence was higher in females, the elderly, those without health insurance, and those with a low income, low education, and occupations with a heavy workload. The prevalence by body part was highest in the back, shoulder, and knee, in that order. The physical component summary and mental component summary of the SF-12 decreased with increasing musculoskeletal symptoms. Conclusions: Musculoskeletal symptoms are very common in the general population, and related to various socio-demographic factors. These results suggest that active prevention and management of musculoskeletal symptoms is needed at a national level.

Estimation of Utility Weights for Prostate-related Health States in Korea

  • Objectives: Very limited previous research has investigated the utility weights of prostate-related diseases in the general population in Korea. The purpose of this study was to calculate the utility of prostate-related health states in the Korean general public using the standard gamble (SG) method. Methods: Seven health states for hypothetical prostate cancers, 1 for benign prostate hyperplasia, and 1 for erectile dysfunction were developed based on patient education material and previous publications. In total, 460 responses from the Korean general population were used to analyze the utility of prostate-related health states. Computer-assisted personal interviews were conducted, and utility values were measured using a visual analogue scale (VAS) and SG. Mean utility values were calculated for each prostate-related health state. Results: The mean utility values of prostate cancer derived from SG ranged from 0.281 (metastatic castration-refractory prostate cancer) to 0.779 (localized prostate cancer requiring prostatectomy). The utility value of benign prostate hyperplasia was 0.871, and that of erectile dysfunction was 0.812. The utility values obtained using the SG method in all conditions were higher than the values obtained by VAS. There were no significant demographic variables affecting utility values in multivariate analysis. Conclusions: Our findings might be useful for economic evaluation and utility calculation of screening and interventions for prostate-related conditions in the general population.

A Study on the Knowledge, Attitude, and Practice of Public Health Management In Industry in Kyeongnam Province (경남지역 일부 근로자들의 산업장에서의 건강관리에 대한 지식, 태도, 실천)

  • The purpose of the study is to provide basic data on the public health management in the industrial work places, and some implications on the public health education related to workers self care capacity of their own health. To achieve this purpose a questionnaire was provided to 332 workers during February 11-28, 1993. This was do in Changwon-shi Geojeoi-kun, Chungmoo-shi, Jinjoo-shi, of Kyeongnam province. Through the analysis of the survey results, a relationship was deduced between worker's general characteristics and basic elements of health management in the work place. The main results of this study can be summarized as follows : 1. In the knowledge area special health screening received the highest score 3.18. While the educational program scored the lowest .85. In the Attitude area the highest score was achieved in the affirmative attitude to the time consumed by health checks (3.28). The lowest by the management of health checks(1.53). In the Practice area, participation in health checking is the most active (3.44) , and public health education is the least active (0.95). 2. The differences of knowledge by workers' general characteristics is statistically significant in the work sector (P<.05), age (P<.05), sex (p<.001), marital status (P<.001), and prior career(P<.01). But in Attitude general characteristics are not significant factors. In the Practice area, did have a statistical significance, work sector (p<.001), age (p<.001), sex (P<.05), marital status (p<.001) and work career (P<.001). 3. The total difference by workers' general characteristics has a statistical significance only in the case of marital status (P>05). The note of married workers is higher than that of single workers. 4. The correlation between any two of Knowledge, Attitude, and Practice do have a statistical significance (p<.001).

Effects of a Volunteer-Run Peer Support Program on Health and Satisfaction with Social Support of Older Adults Living Alone (지역사회 노인 자원봉사자를 활용한 독거노인 건강지킴이 프로그램이 독거노인의 건강상태와 사회적 지지 만족도에 미치는 효과)

  • Purpose: The purpose of the study was to evaluate effectiveness of a peer support program conducted by older community volunteers for older adults living alone. Methods: Thirty volunteers trained as peer supporters were matched with low-income, older adults living alone in the community on gender. Visits occurred on a weekly basis over the 12 month study period, and the volunteers provided peer support for health management to solitary older adults. Data were collected, before the start of the program and again 6 and 12 months after its initialization, from intervention and control groups regarding physical health, general health, mental health, depression, social functioning, and satisfaction with social support. Repeated measures ANOVA was used to analyze data. Results: By the end of the program, socially isolated older adults in the intervention group had significantly higher scores in physical health and general health than elders in the control group. Significant interaction effects between time and group were found for depression, social functioning, and satisfaction with social support. Conclusion: The peer support program undertaken by older community volunteers was effective in improving physical health, general health, depression, social functioning, and satisfaction with social support in socially isolated, low-income, older adults.

Use of a Machine Learning Algorithm to Predict Individuals with Suicide Ideation in the General Population

  • Objective In this study, we aimed to develop a model predicting individuals with suicide ideation within a general population using a machine learning algorithm. Methods Among 35,116 individuals aged over 19 years from the Korea National Health & Nutrition Examination Survey, we selected 11,628 individuals via random down-sampling. This included 5,814 suicide ideators and the same number of non-suicide ideators. We randomly assigned the subjects to a training set (n=10,466) and a test set (n=1,162). In the training set, a random forest model was trained with 15 features selected with recursive feature elimination via 10-fold cross validation. Subsequently, the fitted model was used to predict suicide ideators in the test set and among the total of 35,116 subjects. All analyses were conducted in R. Results The prediction model achieved a good performance [area under receiver operating characteristic curve (AUC)=0.85] in the test set and predicted suicide ideators among the total samples with an accuracy of 0.821, sensitivity of 0.836, and specificity of 0.807. Conclusion This study shows the possibility that a machine learning approach can enable screening for suicide risk in the general population. Further work is warranted to increase the accuracy of prediction.

Influence of Fatigue, Health Perception, and Happiness on Health Promoting Behaviors among General Hospital Nurses (종합병원 간호사의 피로, 건강지각 및 행복이 건강증진행위에 미치는 영향)

  • This study was conducted to examine the correlations between fatigue, health perception, happiness, and health promoting behaviors, and to identify the influencing factors on health promoting behaviors of clinical nurses in two general hospitals. Data were collected from 153 clinical nurses from February 5 to March 30, 2018. Data were collected through personal interviews using a questionnaire. Data were analyzed using t-test, one-way ANOVA, Pearson's correlation coefficients, and stepwise multiple regression. Fatigue according to the general characteristics showed significant differences in drinking and job stress. Health perception according to the general characteristics showed significant differences in disease. Happiness according to the general characteristics showed significant differences in marital status, type of work, clinical career, nursing unit satisfaction, economic level, and job stress. Health promoting behavior according to the general characteristics showed significant differences in clinical career and economic level. Health promoting behaviors showed significant positive correlations with health perception and happiness, but a significantly negative correlation with fatigue. Happiness and economic level, which accounted for 24% of the variance, were significant predictors influencing health promoting behavior in clinical nurses. To improve health promoting behavior in clinical nurses, it is necessary to develop a program that can raise the happiness considering the economic level.
