The Comparative Study on the Effect of Pharmacopuncture Treatment, Chuna Treatment, Pharmacopuncture - Chuna Cooperative Treatment for Neck Pain Caused by Traffic Accidents (교통사고 후 경항통 발생환자 중 약침치료군, 추나치료군, 추나약침 병행치료군 비교연구)

  • Objectives : This study was aimed at comparing clinical effectiveness among pharmacopuncture treatment, Chuna treatment, pharmacopuncture-Chuna cooperative treatment for neck pain caused by traffic accidents. Methods : This study was carried out on the 87 patients with neck pain caused by traffic accidents. The patients were divided into 3 groups ; group A was treated with pharmacopuncture. group B was treated with Chuna treatment. group C was treated with pharmacopuncture and Chuna cooperative treatment one at a time. Also all groups were treated with general acupuncture and herb-medicine treatment. We measured visual analog scale(VAS) and neck disability index(NDI) before the treatment and 2, 4 weeks later. Results : In result each group had significant decrease in VAS and NDI after 4 weeks later. group C showed significant difference as compared to group A and B in NDI and VAS scores. In the first period(from pre to 2 weeks treatment) group C showed significant difference as compared to group A and B in VAS scores. Conclusions : According to the results of this study, it was suggested that chuna treatment and pharmacopuncture are effective in reducing neck pain caused by traffic accidents. Pharmacopuncture and Chuna cooperative treatment was more effective in reducing neck pain caused by traffic accidents. Especially in the first period(from pre to 2 weeks treatment) pharmacopuncture-Chuna cooperative treatment was more effective than pharmacopuncture treatment, Chuna treatment for neck pain caused by traffic accidents.

  • Objectives : The following study was undertaken in order to seek the acupuncture operation method of ${\ll}$Sang Han Lun(傷寒論)${\gg}$ Liu-Jing-Bian-Zheng(六經辯證). Methods : Based on the documents quoted in ${\ll}$Sang Hang Za Bing Lun Xu Wen(傷寒雜病論 序文)${\gg}$ of 'Zhang, Zhong-Jing(張仲景)', the relativity of the theory of Jin-Mai(硬脈) and Liu-Jing-Bian-Zheng of convalescence, and from the Liu-Jing-Bing(六經病), the origin and implication that caused so-Yin-Bing(少陰病) to form was studied on the basis of acupuncture medicine publications and the commentary writing of ${\ll}$Sang Han Lun${\gg}$. Results : 1. ${\ll}$Sang Han Lun${\gg}$ Liu-Jing-Bian-Zheng has succeeded and was developed based on Liu-Jing-Fen-Zheng(六經分證) Of ${\ll}$Su Wen Re Lun(素問 熱論)${\gg}$. In addition, the summary of Liu-Jing-Bing became the general principle of Fen-Jing(分經) and Ding-Zheng(定證) that may be applicable to Fenghan(風寒), Wenre(溫熱), Lili(疫?) and Eating(雜病). 2. Most commentators of ${\ll}$Sang Han Lun${\gg}$ in the Song, Ming and Ching Dynasties of ${\ll}$Sang Han Lun${\gg}$ interpreted the so-yin-Bing in physiological and pathological aspects of Rong-Wei(榮衛) as the disease of the Heart and Kidney meridian. 3. From the Liu-Jing-Bing of ${\ll}$Sang Han Lun${\gg}$, the region of acupuncture treatment of So-yin-Bing is treated with the needle from the point of view of Bing-Zheng-Lun-Zhi(辨證論治) with the basis of the important region of acupuncture of the heart of meridian Kindey of meridian.

Review Study of Headache Measurement Tools - Mainly on Reliability and Validity - (두통 평가 척도에 관한 고찰 - 신뢰도와 타당도를 중심으로 -)

  • Objectives : To introduce and compare various headache measurement tools, in order to provide fundamental information for future clinical trials and development of oriental medicine measurement tools. Methods : Eight headache assessment scales, Migraine-Specific Quality of Life Questionnaire(MQoLQ), Migraine-Specific Quality of Life(MSQOL), Migraine-Specific Quality of Life Questionnaire(MSQ), The Headache Impact Questionnaire(HImQ), Headcahe Needs Assessment Survey(HANA), Henry Ford Headache disability Inventory(HDI), Migraine Disablity Assessment Scale(MIDAS) and Headache Impact Test(HIT-6), were searched through Pubmed. General character, the process of development, verification of reliability, internal consistency, responsiveness to patient's change and validity were reviewed. Results : In assessing the quality of life of migraine patients, MQoLQ can be used to measure the change within 24 hours, MSQ and MSQOL can be used to measure the change during a relatively longer period of time. The frequency and bothersomeness of headache can be easily measured by HANA. HImQ can be used to measure the intensity of headache and the influence on normal daily activities. HDI can measure the emotional difficulties of headache patients. HIT-6 and MIDAS makes measurements possible in a short period of time, and the reliability and validity have been verified numerously. HIT-6 and MIDAS are the most frequently used measurement tools because they are simple and easy for both the assessor and the patient to understand and use. Conclusions : In this study, various headache assessment tools have been reviewed. The assessed tools were widely used after being verified for their reliability and validity. We hope this study provides fundamental information in establishing future clinical trial and clinical practice guidelines. We also hope this study to contribute to the development of an oriental medicine assessment tool for headache.

  • Objectives: The objective of this study was to report the general characteristics, satisfaction, and changes before and after treatment to foreign athletes and officials who were received Korean Medicine at the 2019 Chungju World Martial Arts Masterships Korean Medical Clinic. Methods: We investigated age, gender, nationality, type of sports, and, onset through medical records. Besides, we developed the patient satisfaction survey to determine how satisfied the patients were with the treatment they received. The survey included type of injury, location of the injury, cause of injury, Numeric Rating Scale (NRS) before and after treatment, and treatment satisfaction. Result: This study showed that knee and lower leg injuries (S80-S89) were the most frequent injuries in the Korean Medicine(KM) clinic of the 2019 Chungju World Martial Arts Masterships. We provided KM treatments including acupuncture, cupping therapy, Chuna manual therapy, herbal medicine, dressing, and taping. Acupuncture was the most frequently performed and the most satisfactory treatment. The average NRS after treatment significantly decreased, suggesting that KM is effective in controlling pain in sports injuries. The overall satisfaction was also high with KM treatments.

  • This study investigates the effect of a combination of homeopathic medicine and electro- acupuncture in two patients with breast cancer and severe liver disease who could not receive standard anaesthesia therapy due to liver problems. Specifically, measurable and quantifiable parameters were used to evaluate whether an integrated approach-consisting of electro- acupuncture and a homeopathic medicine diluted above Avogadro's limit (that is, above a potency of 12CH) during the pre-surgical, surgical and post-surgical phases -can improve general well-being of a patient undergoing breast cancer surgery. In breast cancer surgery, we employed an integrated approach consisting of induction with hypnotics and muscle relaxants, followed by maintenance with anaesthetic gas, combined with a homeopathic treatment (Arnica montana 15CH and Apis mellifica 15CH) before and after surgery and an electro- acupuncture treatment performed in the pre- and post-surgical phases without any analgesic/pain relieving medications. Both of the patients treated with the integrated approach improved their overall condition without need for other common pain relieving medicines. Additionally, thanks to their rapid awakening, the patients were not relocated to a protected area and the hospitalization was shorter. A multidisciplinary approach incorporating homeopathic medicine and electro-acupuncture can be a solution for patients who need or ask about a different and/or safer alternative to the standard treatment. This approach can offer a safe, much less expensive, non-invasive and viable alternative for such cases. Moreover it can be useful for an opioids free anesthesia.

The Clinical Study on 80 Cases of Low Back Pain Patients (요통환자 80례에 대한 임상적 고찰)

  • Objectives : This study was performed to evaluate the general distribution and the efficacy of oriental medical treatment for low back pain. Methods: We have selected of the moxa bucket moxibustion. We make a comparative study of the thermodynamic characteristics of moxa bucket moxibustion. We examined combustion times, temperatures, temperature gradients in each period during a combustion of moxa bucket moxibustion made by oak wood. Results : The 80 cases of low back pain patients was analyzed according to the distribution of sex, age, the period of disease, contributing factor, the patient-condition on admission, the symptom on admission, the duration of admission, the reading of the X-ray and the treatment efficacy was evaluated respectively. 1. Female was more than male in the ratio of 1:1.2, and thirties and seventies 25% the most, the acutest phase 46.3% the most, reason unknown 33.8% the most, Grade III 50% the most, L.B.P. only 51.3% the most, the day of 6-10 30% the most, compression Fx 30% the most. 2. In the total treatment result, the good was 51.3%, the excellent 22.5%, the fair 17.5%, the poor 8.8% in order. About 'the effective rate'(the percentage of positive effective treatment cases) of each distribution, the fifties 100% the most, the acutest phase 97.3% the most, the slip down, weight lifting, overuse 100% the most respectively, Grade III 97.5% the most, L.B.P. only 95.1% the most, the day of 11-15 100% the most, HN.P. 93.3% the most. Conclusions : We have known the efficacy of oriental medical treatment for L.B.P. was good and early treatment was better than late treatment and main cause of L.B.P. was degenerative change.

The Clinical Effects of Heating-conduction Acupuncture Therapy with General Acupuncture Therapy for Lateral Ligament Injury Induced by Ankle Sprain Grade II (전열침(傳熱鍼)과 일반 침의 병행치료가 발목 염좌로 인한 외측인대 2도 손상에 미치는 임상적 효과)

  • Objectives: The purpose of this study is to investigate the clinical effects of heating-conduction therapy on lateral ligament injury induced by ankle sprain grade II. Methods: The 67 outpatients who were diagnosed as ankle sprain grade II were performed heating-conduction acupuncture therapy on lateral ligament injury. To evaluate the efficiency of heating-conduction acupuncture therapy, pain threshold with pressure algometer and visual analogue scale(VAS) were applied pretreatment and after 1st, 2nd treatment. Results: 1. The pain threshold and VAS score after 1st and 2nd treatment with an isolated injury of the anterior talofibular ligament showed statistically significant improvement compared with pretreatment. 2. The VAS score after 1st and 2nd treatment with an combined injury to the anterior talofibular and the calcaneofibular ligament showed statistically significant improvement compared with pretreatment. 3. The VAS score after 1st and 2nd treatment with an combined injury to the anterior talofibular, calcaneofibular and posterior talofibular ligament showed statistically significant improvement compared with pretreatment. 4. After 1st and 2nd treatment on the posterior talofibular ligament and after 2nd treatment on the anterior taofibular and calcaneofibular ligament with an combined injury to the anterior talofibular, calcaneofibular and posterior talofibular ligament the pain threshold showed statistically significant improvement compared with pretreatment. 5. Between isolated injury and combined injury of lateral ligament, the difference in pain threshold and VAS score was not statistically significant. Conclusions: Heating-conduction acupuncture therapy has clinical effects of pain reduction on patient with an isolated injury and combined injury of lateral ligament induced by ankle sprain grade II.

Safety and Effectiveness of Fluoroscopy-Guided Acupotomy for Carpal Tunnel Syndrome: Protocol for a Pilot Randomized, Patient-Assessor Blind, Parallel Clinical Trial

  • Background: In Korean medicine, carpal tunnel syndrome is treated by stimulating the acupoints around the wrist. Although a deep understanding of anatomy and guidance is needed to stimulate these acupoints to avoid undesirable side-effects, currently there are no published guidelines for acupotomy treatment. The aim of this study is to evaluate the effectiveness and safety of fluoroscopy-guided acupotomy compared with conventional acupotomy treatment. Methods: This is a randomized, patient-assessor, patient blind, parallel clinical trial. A total of 30 patients will be enrolled at Wonkwang University Gwangju Hospital, and will be allocated to either an experimental group or a control group. The experimental group will be treated using fluoroscopy-guided acupotomy and the control group will be treated using the conventional acupotomy method. Results: The primary outcome measure will be identification of a cross-section area of the median nerve measured by ultrasonography, and the secondary outcome measure will be the alleviation of pain measured by the Visual Analogue Scale, improvement in the Nerve Conduction Study, Tinel test, Phalen's test, EuroQol 5-dimension scale, and Boston Carpal Tunnel Questionnaire score. Safety components will be measured by monitoring vital signs, electrocardiographs, blood tests, general chemical tests, urine tests and pregnancy tests. In addition, observations for adverse effects will be performed during the trial. Conclusion: This study will provide a more effective, and less harmful way of treating carpal tunnel syndrome compared with conventional acupotomy. Fluoroscopy-guided acupotomy will help practitioners to be accurate in direction and depth of the needle for treating carpal tunnel syndrome.

Study on Perception About Traditional Korean Medicine (TKM) Hospital Treatment among the General Population and Patients Visiting a TKM Hospital (일반인 및 내원환자의 한방병원 진료에 대한 인식도 연구)

  • Objectives : This study was carried out to measure perceptions about Traditional Korean Medicine hospitals and provide data for improvement of TKM hospital management. Methods : We surveyed the general population from 1st to 17th December, 2008, and patients visiting a TKM hospital from 1st June to 9th December, 2008. Results : Result as follows. 1. The response ratio for the generally preferred medical institution was in the order of western medical clinic, pharmacy, general hospital, TKM clinic, TKM hospital. 2. The response ratio for the preferred medical institution for herbal medicine treatment was in the order of TKM clinic, herbal medicine store, TKM hospital, pharmacy. The preferred medical institution for acupuncture treatment was in the order of TKM clinic, TKM hospital, acupuncture clinic, public health center. 3. The most common response to reason for receiving traditional Korean medical care was "traditional Korean medical care is more effective than western medical care." 4. The proportion of first treatment and second treatment of patients visiting at TKM hospital was about 1:1. 5. 72.3% of patients had been to another medical institution. 6. The response ratio for the first visited medical institution for the same disease was in the order of general hospital, western medical hospital, western medical clinic, TKM clinic. 7. The response ratio for the concern about TKM treatment was in the order of expensive charge for TKM treatment, uncertainty of TKM treatment effect. Conclusions : Through this research, we can understand the perception about TKM hospital treatment of the general population and patients visiting TKM hospitals.
