Pathological Studies on the Esopha-gastric Ulcers in Swine (돼지 위궤양 발생에 관한 병리학적 조사)

  • Based on the gross lesion, a survey of ulcer prevalence was carried out on pigs (3,161 heads) at a slaughterhouse in seoul from April to December in 1989. The pigs stomachs were observed anatomically and classified, according to the presence or absence of foreign bodies. Ulcer prevalence is usually classified according to size of pig farm, ages, and supplied month. The results in the studies were as follows, 1. Fundus followed the highest rate with 28.2%, esophagus with 24.5% each of cardiac and pyloric part with 7.8% and 4.1% in cases of gastric mucosa pathological studies examed total 3,161. In the acute ulcer prevalence, fundus follwed with 5.4%, esophagus with 2.6%, each of cardiac and pyloric part with 0.8% and 0.3%. When prevalent frequencies of fundus are compared with cardiac and pyloric part, they have meaning.(P <0.05). 2. In the survey pig of total 3,161, pathological changes of epithelium tissue foliowed 44.0%, erosion(11.5%), chronic ulcer(14.8%), acute ulcer(1.6%), scar formation (1.7%) and perforation (0.9%). 3. In general monthly prevalence rates of gastric lesion are higher in spring and fall than in summer. 4. In the size of pig farms, ulcer showed higher rate at professional farms than raising on the side. 5. When there are more fluid condition in stomach and they are old, the ulcer appeared for more. 6. Foreign bodies in stomach were sand, hairballs, and intestinal worms were detected (1%).

Tracheogastric Fistula : A Rare Complication of Total Laryngopharyngoesophagectomy with Gastric Pull-up Procedure (기관-위 샛길 : 전인두후두식도 절제술후 위를 이용한 재건 후 발생한 드문 합병증 1예)

  • Tracheo-gastric fistula is a very rare late complication of total laryngopharyngoesophagectomy with gastric pull-up procedure. It usually occurs after transhiatal esophagectomy, but it has only rarely been reported after total laryngopharyngoesophagectomy with gastric pull-up procedure. Chronic irritation and gastric ulcer may be the cause of tracheogastric fistula. To prevent fistula formation, active management of gastric ulcer and avoidance of mucosal irritation are necessary. We report a case of a tracheogastric fistula which occurred two years after total laryngopharyngoesophagectomy with gastric pull-up procedure that maybe occurred by chronic irritation and gastric ulcer.

Antigastritic and Antiulcerative Activities of Water Extracts Derived from Scutellaria baicalensis

  • Gastritis and gastric ulcer were known to be induced by gastic acid, stress, ethanol, Helicobacter pylori and free radical, etc. This study was performed for the development of a new drug or nutraceutical from medicinal plants or natural products with anti-gastritis, anti-ulcerative and gastroprotective activities. The water extract of Scutellaria baicalensis was exhibited potent inhibition in HCl ethanol-induced gastric lesion, acetic acid-induced and Shay ulcers, indicating the effects on gastric lesion and ulcer in rats. The water extract of Scutellaria baicalensis significantly inhibited HCl ethanol-induced gastric lesions at the oral dose of 300, 500 mg/kg. In pylorus ligated rats, the treatments of the water extract from Scutellaria baicalensis showed decrease in the volume of gastric secretion and acid output and increase pH at oral dose of 300, 500 mg/kg. And significantly reduced acetic acid-induced ulcer at the oral dose of 500 mg/kg for 12 days. In this study, we have found that the water extract from Scutellaria baicalensis had significant improvement in acute gastritis and ulcer at the dose of 300, 500 mg/kg and in chronic gastritis and ulcer at the dose of 500 mg/kg. Also we evaluated the antibacterial activity against Helicobacter pylori treated with Scutellaria baicalensis. Scutellaria baicalensis had a equivalent antibacterial activity with ampicilin against H. pylori at the dose of $100\;{\mu}/ml$. In histological examination, the water extract of Scutellaria baicalensis drastically restored gastric damages induced by HCl ethanol solution, pylorus- ligature and acetic acid. Therefore, we may use the water extract from Scutellaria baicalensis as antigastritic and antiulcerative agent for the purpose of the improvement or treatment of gastritis and gastric ulcer.

Preventive Effect of Polysaccharide of Larimichthys crocea Swim Bladder on Reserpine Induced Gastric Ulcer in ICR Mice

  • This project's aim was to determine the reserpine-induced gastric ulcer preventive effect of polysaccharide of Larimichthys crocea swim bladder (PLCSB) in ICR mice. The anti-gastric ulcer effects of polysaccharide of Larimichthys crocea swim bladder was evaluated in mice model using morphological test, serum levels assay, cytokine levels assay, tissue contents analysis, reverse transcription-polymerase chain reaction (RT-PCR) analysis and western bolt assay. High concentration (50 mg/kg dose) of PLCSB reduced IFN-${\gamma}$ as compared to low concentration (25 mg/kg dose) and control mice. SS and VIP serum levels of PLCSB treated mice were higher than those of control mice, and MOT and SP serum levels were lower than control mice. Gastric ulcer inhibitory index of PLCSB treatment groups mice were much lower than control mice, and the high concentration treated mice were similar to the ranitidine treated mice. The SOD and GSH-Px activities of PLCSB treated mice were higher than control mice, close to normal mice and ranitidine treated mice. PLCSB treated mice also showed the similar contents of NO and MDA to normal group. By RT-PCR and western blot assay, PLCSB significantly induced inflammation in tissues of mice by downregulating NF-${\kappa}B$, iNOS, and COX-2, and upregulating $I{\kappa}B-{\alpha}$. These results suggest that PLCSB showed a good gastric ulcer preventive effect as the gastric ulcer drug of ranitidine. Polysaccharide of Larimichthys crocea swim bladder may be used as a drug material from marine products.

Pharmacological Studies on Root Bark Extract of Aralia elata - Antigastritic and Antiulcerative Effects in Rats - (두릅나무 근피 추출물의 약물학적 연구 -흰쥐의 위염 및 웨궤양에 대한 효과-)

  • In a preliminary screening of the plant extracts for the antigastritic action in rats, the extract of Aralia elata(Araliaceae) showed positive activity in HCI plus ethanol induced gastric lesion. Systematic fractions with hexane, chloroform, ethyl acetate and butanol resulted in the most patent activity with the butanol fraction: This butanol fraction at the oral dose of 200 mg/kg exhibited significant inhibition of absolute alcohol induced gastric lesion which was more potent than 100 mg/kg of cimetidine and had significant stimulation of mucus secretion. The butanol fraction showed significant decreases in the ulcer indices of Shay ulcers and inhibition of gastric juice secretion with acid output in pylorus-ligated stomachs of rats. It also suppressed the acetic acid induced gastric ulcer. These results might suggest that the butanol fraction had inhibitory action in gastric lesion and ulceration through inhibition of gastric acid secretion and stimulation of mucin secretion in the stomachs of rats.

The Protective Effects of $Hwangyeon-tang$ on Acute Gastric Ulcer induced by HCl/EtOH solution in Rats (흰쥐의 급성 위점막 손상에서 황연탕(黃蓮湯)이 apoptosis 관련단백질 및 성장인자 발현에 미치는 영향)

  • The apoptotic process of gastric mucosa triggered by induction of proapoptotic gene expression, such as Bax. Stress-inducing factors may affect Bcl-2/Bax ratio and thus the rate of apoptosis through modulation of the expression of both proteins depending upon the experimental model. TGF-${\beta}$ is believed to be essential in wound healing for regulation of cell growth and differentiation and is known to be involved in tissue repair and remodeling. The polypeptide growth factors, such as vascular endothelial growth factor(VEGF), regulate essential cell functions involved in tissue healing including cell proliferation, migration, and differentiation. The purpose of this study was to investigate whether the oral administration of $Hwangyeon-tang$ (HYT) would have protect effects on gastric ulcer in rat. Sprague-Dawley rats (n=40) were randomly divided into 4 groups ; Normal, Saline, Cimetidine and HYT group. The saline, cimetidine and HYT extract were orally administrated to each group and gastric ulcer was induced with HCl/EtOH solution. After 1 hour, the stomachs were collected for histological observation and immunohistochemistry. In Results, the wound healing of gastric ulcer was promoted by HYT and the significant alterations of BAX/Bcl-2, TGF-${\beta}1$ and VEGF proteins in gastric mucosa were observed. These results suggest that Fritillaria ussuriensis extract promotes wound healing and has protective effects on gastric ulcer in rats.

The Comparison of Perceived Stress and Coping Strategy between Patients with Gastric Ulcer and Those with Chronic Gastritis (위궤양환자들과 만성위염환자들간의 스트레스지각 및 대응전략의 비교)

  • Objectives : The object of this study was to make a comparison regarding perceived stressor, perceived stress responses, and coping strategies between patients with gastric ulcer and patients with chronic gastritis. Subject and Methods : Subjects consisted of 40 patients with gastric ulcer and 100 patients with chronic superficial gastritis. Global assessment of recent stress(GARS) scale and perceived stress response inventory(PSRI) were used to measure perception for stressors and stress responses. Coping scale was used to measure coping strategies. Results : Scores of perceived stress related to work or job, changes in relationship on GARS scale were significantly higher in patients with gastric ulcer than those with chronic gastritis. Scores of perceived stress responses related to general somatic symptom, specific somatic symptom, passive-responsive and careless behavior on PSRI were significantly higher in patients with chronic gastritis than those with gastric ulcer. Scores of seeking social support, escape-avoidance on coping scale were significantly higher in patients with gastric ulcer than those with chronic gastritis. Conclusion : The above results suggest that perception for stressors were likely to be higher in patients with gastric ulcer than those with gastritis, whereas perception for stress responses were likely to be higher in the latter than the former. It is also suggested that patients with gastric ulcer were likely to use more dependent and passive coping strategies than patients with gastritis.

Protective Effect of Liriodendrin Isolated from Kalopanax pictus against Gastric Injury

  • In this study, we investigated the inhibitory activities on gastritis and gastric ulcer using liriodendrin which is a constituent isolated from Kalopanax pictus. To elucidate its abilities to prevent gastric injury, we measured the quantity of prostaglandin $E_2$ ($PGE_2$) as the protective factor, and we assessed inhibition of activities related to excessive gastric acid be notorious for aggressive factor and inhibition of Helicobacter pylori (H. pylori) colonization known as a cause of chronic gastritis, gastric ulcer, and gastric cancer. Liriodendrin exhibited higher $PGE_2$ level than rebamipide used as a positive control group at the dose of $500{\mu}M$. It was also exhibited acid-neutralizing capacity (10.3%) and $H^+/K^+$-ATPase inhibition of 42.6% ($500{\mu}M$). In pylorus-ligated rats, liriodendrin showed lower volume of gastric juice ($4.38{\pm}2.14ml$), slightly higher pH ($1.53{\pm}0.41$), and smaller total acid output ($0.47{\pm}0.3mEq/4hrs$) than the control group. Furthermore liriodendrin inhibited colonization of H. pylori effectively. In vivo test, liriodendrin significantly inhibited both of HCl/EtOH-induced gastritis (46.9 %) and indomethacin-induced gastric ulcer (46.1%). From these results, we suggest that liriodendrin could be utilized for the treatment and/or protection of gastritis and gastric ulcer.

Effect of Dioscorea Rhizoma on Gastritis by Acute Gastric Ulcer in Rats (산약이 흰쥐의 급성 위궤양에 의한 위장 장애에 미치는 효과)

  • In order to verify the effects of Dioscorea Rhizoma on gastrointestinal damages, we investigated the protective role of the crude extracts on induced gastric ulceration in rats. Rats administered of Dioscorea Rhizoma extracts showed gradually decreased congestion and hemorrhage, but control group did not show any symptom in gastric tissue. Moreover, Dioscorea Rhizoma extract had a role in lowering gastrin and histamine levels in gastric ulcer rats, thereby inhibiting the gastric tissue damages. Excessive production of malondialdehyde shown in gastric ulcer rats was declined in all rats administered with Dioscorea Rhizoma extract as well as the levels of SOD and GPX surged by acute gastric ulcer. Also, the increased activity of CAT showed an effect in activation of antioxidant enzyme to normal state. All data suggest that Dioscorea Rhizoma extract was verified to be highly effective resource in improving the gastrointestinal function of rats by preventing from gastric tissue damage in acute gastric ulcer and restoring the activities of plasma substances and antioxidant enzymes.

The Effects of Shogunjungtang-ga-yonggol.morea on Gastric Ulcer (소건중탕가룡골(小建中湯加龍骨).모려(牡蠣)가 흰쥐의 실험적 위궤양에 미치는 영향)

  • Object : This study was carried out to examine the effects of Shogunjungtang-ga-younggol morea on gastric ulcers. Methods : In order to study the effects of Shogunjungtang-ga-younggol morea on gastric ulcers. Gastric ulcers were induced in HCI-aspirin in rats. The experiments were done by oral administration and measured by anatohistological features of ulcer lesions, and the changes of the number of parietal cells, chief cells, gastrin and somatostatin- immunoreactive cells. Results : In the Shogunjungtang-ga-younggol morea administrated groups, no gross lesions of ulcer and anatohistologically, just minor injury of gastric mucosa were detected. The number of parietal cells were significantly decreased, and the number of chief cells were significantly increased, in administrated groups. The number of gastrin-immunoreactive cells and somatostatin-immunoreactive cells was significantly increased in administrated groups. Conclusion : According to the results, it is considered that the administration of Shogunjungtang-ga-younggol morea seems to be applicable to the treatment of gastric ulcers.
