Effective consultation strategy for patients of aesthetic case (심미 증례 환자의 효과적인 상담 전략)

  • In the modern society, the awareness of images is affecting the body-related consumer behavior through mass media such as TV and internet, and SNS that has grown explosively with the wide availability of smartphones. As a consequence, the demands for aesthetic treatments by patients who wish to improve their inferiority complex are increasing. Patients who want aesthetic treatments display more active inclinations than those receiving general treatments, and have the expectation for the improvement in images along with the anxiety over potential functional problems and satisfaction about the outcomes. In the consultation of the patient of aesthetic case, the basic starting point is that the surgeon and the patient recognize improvement points that the patient expects through the aesthetic treatment. The consultation starting from the viewpoint of the patient proceeds in a way that the consensus is reached through the medical approach, eventually finding the balance between ideals and naturalness. The consultation process from the initial meeting is established at each stage of treatment, at which the consultation is correctly recorded and shared by the surgeon and the care staff. In addition, the consistent policy is implemented from the initial consultation through to the maintenance care. Together, these will result in the consultation based on the true communications leading to the outcome that satisfies the patient, the surgeon and the care staff, rather than the one that gives the patient an unfounded hope and leads to the unsympathetic outcome. This article introduces the consultation strategy for patients of aesthetic case based on the sympathy with them through cases of aesthetic treatment by type.

A research on the health characteristics and the elements of Han-Ok : Contents analysis of articles of the four daily newspapers since 1960 (한옥의 건강특성과 요소에 대한 언론보도 연구 : 1960년 이후 4대 일간지 기사 내용분석)

  • The purpose of this study was to investigate the trend changes of the health characteristics and the elements of Han-Ok appeared in the articles of the daily newspaper, and to provide basic information for applying to the healthy housing for the public living in modern society and furthermore the various facilities for healing. For the study, the relevant articles of 4 daily newspapers published from 1st of January in 1960 to 28th of February in 2010 were collected and analyzed. The results of the study were as follows: First, the articles about the health characteristics and the elements of Han-Ok have rapidly grown since the year of 2000. Especially, the residents' interviews, case reports and actual living condition of Han-Ok recently tend to increase. Second, the health characteristics of Han-Ok in the recent articles were focused more on the mental and social health than on the physical health. The physical health had the characteristics of the block of hazard environments, the offer of good quality environments, and the enhancements of residents' health. The mental health consisted of the psychological comfort, the sensory satisfaction and the emotional growth. There have been improvements of family relationships and the interactions with neighbors in the characteristics of social health since 2000. Third, the health elements of Han-Ok in terms of architecture and interior design were the structure, space, material, doors & windows, furniture & props, nature, and villages. The material elements were more stressed before 2000, but the spaces and the nature elements were highly focused after 2000 and the structure elements were steadily high regardless of years. Fourth, of the health elements, the structure and the material had the high effects on the physical health characteristics, on the other hand the spaces and the nature influenced more on the mental health. On-dol, Ma-dang, Dae-cheong, Ma-ru, soil, wood, Changhoji, natural environments, and doors & windows were the detail health elements which had high effects on the health characteristics of Han-Ok. For the future study, the experience based in-depth studies of the residents living in Han-Ok are suggested to grasp the actual effects of Han-Ok on the health. The professional consultations are also needed for the extracts of the health characteristics of Han-Ok and the functional elements which can be applied to the modern buildings.

Clinical Consideration of Visual Fatigue on 3D Images (3D 영상 시청에 대한 시각적 피로도의 임상적 고찰)

  • Previous studies based on the theoretical evidence and based on the empirical data have been constantly revealing related factors affecting visual fatigue in viewing 3D images. however, the measurement data coupled with the factors that affect the viewers to feel fatigue is insufficient. this study research that the viewers feel the visual functional data is based on subjective fatigue such as IPD, AC/A ratio, and the degree of phoria and the effects on variance in these data, is to examine the correlation between these data. based on the average score of subjective fatigue which is $6.00{\pm}3.88$, total fatigue points from 0 up to 6 is classified as a group not appealing for the fatigue and from 7 up to 16, they were classified as the fatigue appealing group. the results showed no difference in satisfaction of the images by each subjective fatigue, IPD and far phoria. AC/A ratio was higher from the fatigue not appealing group, near phoria was higher from the fatigue appealing group. after watching 3D images, the fatigue not appealing group showed a tendency of higher increase of exophoria. correlations appeared in this experiment showed that AC/A ratio and near phoria are closely related to level of subjective fatigue which feel when watching 3D images. when watching 3D images based on two data appear to be able to predict the fatigue, even during content production fatigue reduce the data dimensions to be used as the main basis is expected.

Implant stability installed with CAD-CAM assisted flapless surgery : A pilot study (CAD-CAM assisted flapless 수술법으로 식립된 임플란트의 안정성 : 기초연구)

  • It was proposed that technologies derived from CAD-CAM and computed tomography may be useful for flapless implant treatment procedures. The aims of this study were to validate the reliability of this concept in a prospective 12-month clinical study. Twelve patients with fully edentulous areas in their mandibles were included in this study. A total of 71 implants were inserted in interforaminal regions by use of a CAD/CAM drill template($NobelGuide^{TM}$), specially designed for flapless implant surgery. To assess the degree of pain and discomfort, the patients were examined at 2 days and 1 week after surgery. Patient satisfaction and implant functionality were further evaluated at follow-up intervals of 1, 3, 6, and 12 months postoperatively. One implant failed early in 1 patient. All of the other implants were in a good functional state throughout the study. The mean marginal bone loss after 1 year of follow-up was 0.3 mm (SD, 0.1) at center, 0.5 mm (SD, 0.1) at canine and 0.7 mm (SD, 0.2) at distal fixtures, respectively. Statistically, there was not significant differences among each sites(P>.05)The mean ISQ change after 1 year of follow-up was -1.05 (SD, 2.76) at center, -0.85 (SD, 2.59) at canine and -1.27 (SD, 2.18) at distal fixtures, respectively. This prospective pilot study showed that the use of CAD/CAM technology and flapless implant surgery may be considered reliable for fully edentulous mandible of patients.

Factors Affecting Selection of Delivery Facilities Pregnant Women (산모의 분만기관 선택관련 요인)

  • This study was designed to investigate the mar factors affecting selection of delivery facilities by pregnant women. Five hundred women hospitalized at 23 Seoul-area delivery facilities, such as university hospitals, general hospitals, hospitals, and clinics were selected and given questionnaires from April 24 to May 7, 1990. A total of 350 questionnaires were collected and analysed for the study. The results are as follows ; 1. In general, variables which significantly affected the choice of delivery facilities included the age of women, their educational level, the educational level of their husbands, monthly average incomes and residential areas. 2. In analyzing the obstetrical characteristics of the women, those variables significantly affecting the choice of delivery facilities were the gestational period, the facilities for prenatal care, the frequency of prenatal care, the type of delivery, the frequency of miscarriage, previous delivery experiences and the awareness on prenatal care. 3. In comparing the motivation factors for selecting the delivery facilities, all the factors except convenience and need for hospitalization differed significantly among delivery facilities. 4. The factor analysis was assessed for twenty possible factors motivating the choice of delivery facilities. Six factors including personal service, scale of the facility, reputation, urgency, convenience, and experience were noted explaining by 57.7%. 5. In the discriminant analysis used to clarify the major factors affecting the selection of delivery facilities, the 16 significant variables were regarded as independent variables, and the type of delivery facilities was considered a dependent variable. The stepwise method was applied to the analysis. Detected discriminant variables were the facilities for prenatal care, scale factor, personal service factor, urgency factor, convenience factor, reputation factor, experience factor, gestational period, types of delivery, frequency of miscarriage, age and income. These 12 discriminant variables were tested, with reference to discriminant prediction, on their importance in the choice of the delivery facility, by the discriminant functional formula. The test showed a hit-rate of 67.7%. The results suggest that general characteristics, obstetrical characteristics, and motivations for selecting the delivery facilities differ significantly according to the types of the delivery facilities. This study implies that all types of delivery facilities should attempt to acommodate characteristics and motivations of pregnant women. The facilities should be prepared to increase their patients satisfaction with required medical conditions by improving service and responding to the pregnant women's preferences.

A Case Study on the Clinical Application of Lee Silverman Voice Treatment-BIG (LSVT-BIG) Program for Occupational Performance and Motor Functions of Stroke Patients (뇌졸중 환자의 작업수행과 운동기능을 위한 Lee Silverman Voice Treatment-BIG(LSVT-BIG) 프로그램의 임상적용에 대한 사례연구)

  • Objective : The purpose of this study was to examine the changes in work performance and motor function of stroke patients in the Lee Silverman Voice Treatment-BIG (LSVT-BIG) program and to confirm its clinical applicability. Methods : Two stroke patients underwent the LSVT-BIG program for a total of 16 sessions (60 minutes per session and, four days a week for four weeks). To assess any changes between before and after the intervention, the Canadian Occupational Performance Measurement (COPM), Berg Balance Scale (BBS), Timed Up and Go (TUG), Functional Reaching Test (FRT), Manual Function Test (MFT) were used. Differences in scores between before and after the intervention were analyzed. Results : The performance and satisfaction of occupational performance increased after the intervention in both subjects. The performance time of the TUG decreased to 0.91, 8.42 seconds for each subject, increasing the walking speed. In FRT distance change, the subject increased in both the affected side and unaffected side. The BBS score increased by 3 points in one subject and by 6 points in the other, indicating improved balance. In addition, in the MFT score, subject A showed an improvement of 1 point on the unaffected side, and subject B showed an improvement of 1 point on the unaffected side and 3 points on the affected side. Conclusion : We confirmed the applicability of the LSVT-BIG program as a new intervention technique for stroke patients. Future, complementary research on the effects of the LSVT-BIG program on stroke patients will be needed.

The Case Study of Reflective Practice of the Liberal Dance Class in University (대학 교양무용 수업의 반성적 실천 사례)

  • The purpose of this study is to research the practical cases of the reflective curriculum design and the improvement of the dance courses as a general education at University. This study deals with the recreation dance program with 40 students participating for 15 weeks. This study is qualitative research to explore the practical meaning and value the process on the basis of the interviews and reports of the students and the class evaluation of the instructors. The results were as follows: First is the experience of 'thinking dance'. Not just a functional movement, but moreover, they could experience the attraction, sensitivity and culture of the dance altogether. Second is the experience of 'communicating dance'. They experienced the sociability from the relationship with new partners, the overcome of the passivity (shyness) and the manner of consideration. The third is the experience of 'creating dance together'. As a member of a group, they could learn the integrity, responsibility and satisfaction in the middle of cooperative task performance. The view from the teacher regarding the class reflective assessment is as followed: The importance of dance instruction is to improve the expressive personality and potentiality of the students. Forcing the students to follow the basic choreography could be exhaustive.

Posterior Lumbar Interbody Fusion Versus 360° Fixation in Degenerative Lumbar Diseases (퇴행성 요추 질환에서 후방경유 추체간 유합술과 360° 고정술의 비교)

  • Objectives : The goal of operation for degenerative lumbar diseases is to relieve radiculopathy and low back pain and to prevent further degeneration. The authors analyzed the surgical results of posterior lumbar interbody fusion(PLIF) and $0^{\circ}$ fixation to evaluate the proper treatment policy in spinal stenosis, degenerative spondylolisthesis and low grade isthmic spondylolisthesis. Material and Methods : The authors performed PLIF on 92 patients and $0^{\circ}$ fixation on 138 patients with spinal stenosis, degenerative spondylolisthesis and low grade isthmic spondylolisthesis. We retrospectively studied clinical outcomes and subjective satisfaction of these patients by several criteria such as visual analog scale(VAS), Prolo's economic and functional outcome scale, medication usage after operation and questionaire for overall outcome. Result : Pre- and postoperative VAS on back pain and leg pain showed decrease of pain from 6.5, 6.7 to 2.2, 2.4 in PLIF group and from 7.0, 7.2 to 2.5, 2.7 in $0^{\circ}$ fixation group. Excellent and good outcomes on Prolo's scale were 81.5% in PLIF group and 82.6% in $0^{\circ}$ fixation group. Medication usage after operation was reduced in 79.3% of PLIF group and in 78.3% of $0^{\circ}$ fixation group. Patients' self-reported overall success of their procedure showed 82% in PLIF group and 84% in $0^{\circ}$ fixation group. Conclusion : Both PLIF and $0^{\circ}$ fixation showed good outcomes and provided biomechanically stable fusion in spinal stenosis, degenerative spondylolisthesis and low grade isthmic spondylolisthesis. Therefore, only PLIF seems necessary and considered a proper surgical treatment for these disorders.

Diagnosis and Treatment of Chronic Medial Ankle Instability (만성 내측 족관절 불안정성에 대한 진단 및 치료)

  • Purpose: We investigated the short term results of medial deltoid ligament repair after diagnosis of chronic medial ankle instability. Materials and Methods: 262 military patients with ankle instability symptom were evaluated between May 2007 and December 2009. We diagnosed 29 chronic medial ankle instability cases with medial drive through sign under arthroscopy, radiologic findings and physical exam, treated with deltoid talo-navicular band repair using anchor suture. We used the American orthopedic foot and ankle society ankle-hindfoot score (AOFAS), visual analog scale (VAS) and ankle functional satisfactory scores were carried out. Results: 29 patients (11.1%) has chronic medial ankle instability, mean AFOAS score, VAS was improved from 65.4(range; 43-83), 6.0(range; 4-10) to 82.0(range; 60-100), 3.2(range; 1-7). Patients satisfaction were excellent 13(44.8%), good 11(37.9%) and poor 5(17.2%). Two cases was recurred and revised with allo-tendinous reconstruction. Conclusion: We underwent surgery to repair the talo-navicular ligament for chronic medial ankle instability, and about 83% of satisfactory results were obtained.

Comparison of Arthroscopic versus Mini Open Repair in Medium and Large Sized Full Thickness Rotator Cuff Tear - Short Term Preliminary Results - (중 대 범위 전층 회전근 개 파열에서 관절경 하 봉합술과 소 절개 봉합술의 비교 - 단기 추시 예비 결과 -)

  • Purpose: To compare the results of a miniopen repair with those of complete arthroscopic repair in medium and large sized full thickness rotator cuff tears. Materials and Methods: The thirty four(34) patients with medium and large sized complete rotator cuff tear were observed, Group I (complete arthroscopic repair) were 13 cases and group II (miniopen repair) were 21 cases. The tear sizes are from 1cm to 5cm. The average follow up periods are 24(range;12$\~$36) and 28(range; 12$\~$36) months. Subjective pain was evaluated with VAS (visual analogue scale) in rest state. ADL (Activity of Daily Living) and UCLA scoring system were used to evaluate clinical results. Results: At last follow-up periods, pain and functional scores were improved but they had not been shown statistical significance (p>0.05). In the group I and group II, there are no significant difference in VAS, ADL; UCLA score, satisfaction (p>0.05). Conclusions: In medium and large sized full thickness rotator cuff tears, there are no significant clinical results between the arthroscopic and miniopen group.
