The purpose of this study is to analyse statistically the relation between medical service quality and managerial performance. And then the way of analysis is the regression analysis that independent variable is service quality, dependent variable is the volume of revenue, the number of patients and the rate of beds utility, and dummy variable is the number of beds, ownership and region. The sample hospitals were the 113 hospitals on general hospitals more than 300beds which were consisted of 20 public hospitals, 41 corporate hospitals and 52 college hospitals, and also distributed 67 hospitals on big city and 46 hospitals other city. The sample hospitals were selected from the Korean Hospital Association and the data of the year 2003 and 2004. The collected data was analyzed using the SPSSWIN 10.0 version, and the study hypothesis was tested using regression analysis. The findings of this study are summarized as follows. First, as a result of analysing hypothesis 1. In the study of the relationship between the service quality and the revenue, it was verified that the more service quality in last year became the more revenue in the year. But the dummy variable, ownership and region, rarely related to hospital revenue. Therefore it means that the more service quality is connected to the more revenue on the large number of beds hospitals. Second, as a result of analysing hypothesis 2 & 3. In the study of the relationship between the service quality and the number of patients, it was verified that the more service quality in last year became the more outpatients in the year. But there was no verified to inpatients. It seems to be the reason why a hospital has operated the fixed number of beds approved by the public office. So there are no free to expand beds according to the number of inpatients as much as inpatients are increasing. Third, as a result of analysing hypothesis 4 & 5. In the study of the relationship between the service quality and the rate of beds utility, it was verified that the more service quality in last year became the shorter of average stay of length in the year. Especially it has influenced much more on a hospital which was the large number of beds, the corporate and the college, but the region. But it was denied that the more service quality became the more the beds turnover. As a result of this study, it shows that the service quality in the last year has importantly influenced on a hospital managerial performance in the year. Estimating the service quality of each hospital, most patients have selected the hospital they want. And the hospital need to keep the number of patients for the proper management. So this result of the study means that a hospital must improve the service quality for keeping efficient management.
Objectives: The purpose of this study was to investigate factors influencing the health status and life satisfaction of elders in welfare facilities. Methods: The subjects of this study were selected randomly among those without cognitive impairment from free (140 persons) and charged (140 persons) welfare facilities in the Yeongnam area. Data was analyzed using t-test and stepwise multiple regression. Results: Health status and life satisfaction were 2.52 and 1.98, respectively, in the elders from free welfare facilities, and 2.67 and 2.08 respectively, in the elders from charged welfare facilities. In those from free facilities, life satisfaction and motivation for getting into the welfare facility were the influencing factors of health status. In those from charged facilities, life satisfaction, gender, motivation for getting into the welfare facility, limited service such as physiotherapy, age, and lack of staff and professionalism were the influence factors of health status. In those from free facilities, health status, relationship conflict with fellow elders, lack of staff and professionalism, insufficient facilities and inadequate environment, and indifference of sons and daughters were the influence factors of life satisfaction. In those from charged facilities, health status, education and age were the influence factors of life satisfaction. Conclusions: It was found that both health status and life satisfaction of elders in charged welfare facilities were higher than those in free welfare facilities.

For advanced healthcare services, a variety of agents should maintain reliable connections with the manager and communicate personal health and medical information. The ISO/IEEE 11073 standards provide convenient interoperability and the optimized exchange protocol (OEP) supports efficient communication for devices. However, the standard does not specify secure communication, and sensitive personal information is easily exposed through attacks. Malicious attacks may lead to the worst results owing to service errors, service suspension, and deliberate delays. All possible attacks on the communication are analyzed in detail, and the damage is specifically identified. In this study, novel secure communication schemes over the 20601 OEP are proposed by introducing an authentication process while maintaining compatibility with existing devices. The agent performs a secure association with the manager for mutual authentication. However, communication with mutual authentication is not completely free from attacks. Message encryption schemes are proposed for concrete security. The authentication process and secure communication schemes between the secure registered agent (SRA) and the secure registered manager (SRM) are implemented and verified. The experimental analysis shows that the complexities of the SRA and SRM are not significantly different from those of the existing agent and manager.
Medical Dispute Arbitration Law had been debated on its legislation several times since Korean Medical Association's submission of the bill to the National Assembly in 1988, eventually in December, 2009, passed the National Assembly Standing Committee and was laid before the Legislation and Judiciary Committee, and thus its legislation is now near at hand. During the long process, it has provided a hot issue with our society. And yet, Medical Dispute Arbitration Law has differed considerably in legislative content depending on the main body of proceeding the enactment, which subsequently was given the mixed comments of 'Act on Malpractice-related Damage Relief' or 'Medical Indemnity Act', and this legislative bill also cannot be free from this debate. It is desirable that medical disputes between doctors and patients be resolved through conciliation between the parties concerned. But, because reaching a compromise is difficult owing to deep emotional conflicts between the parties, difficulties in investigating a cause and requiring a high amount of settlement money, etc., it is inevitable to seek a resolution by third party intervention. By the way, such an arbitration by third party is based on the compromise of the interested parties and thus has a limitation of not being able to satisfy both parties completely. Therefore, the legislative bill made for arbitration of medical disputes between the parties will have to prepare an institutional system for the parties to easily understand and accept. Also, problems occurred in the legislative bill will have to be corrected through an in-depth discussion in order for the legislative bill to work as an effective system.
Objectives: In Korea, the top 10% of Medical Aid recipients represent nearly 60% of total payment, with the costs for those disabled for over 365 days representing approximately 30% of total payment. The purpose of this study was to compare Medical Aid use of the disabled with non-disabled recipients, and to identify contributing factors to the total payment in the top 2% of recipients identified as Medical Aid overusers. Methods: Subjects (n=2,211) selected were ${\geq}18$-years-of-age and received >1000 days of co-payment-free type I Medical Aid. Case managers (n=200) conducted interviews in December 2006, and collected data from Health Insurance Review & Assessment Service. Amounts over the 9 months from January September 2006 were analyzed descriptively and using Chi-square, ANCOVA, and robust multiple linear regression. Results: Disabled individuals (mean age 61.3 years) composed 36.6% of subjects; 44.8% of the disabled were male. On a monthly basis per capita, the disabled group averaged 10.5 outpatient days, total payment of 523,000 Korean Won(₩), inpatient payment of ₩359,000, and outpatient payment of ₩183,000. All values exceeded the monthly average for non-disabled individuals. Contributing factors were identified as male gender (₩82,000), elementary school or lower educational level (₩64,000), residence in a small city (₩82,000), lack of family support (₩61,000), kidney disability (₩673,000), intellectual disability (₩151,000), and multiple disabilities (₩119,000). Conclusions: The identification of contributing factors to Medical Aid use by those defined as disabled supports the adoption of comprehensive alternative policies such as strengthening of education and consultation services, provision of alternative facilities, and promotion of self-care.

This is the study on the current status and system of the elderly medical and welfare facilities with the subjects in Busan. I investigated the problems related to those by analysing the current status and implementing a survey and then suggested a new facility system as a salvation. At present, the elderly medical and welfare facilities can not receive variety of inmates and also they are operated separatedly so the elderly is not able to receive special treatment and nursing at the same time in one facility. Some nursing facilities can give special treatment by connecting specific hospital when it is necessary, but the connection is not activated than our expectancy at the present. On the base of these problems, this study suggested 'one-stop service system' that can afford to give nursing and special treatment in a place ranging wider areas as the solution. The five types of facility system by integrating the results of survey are as followings. 1) Free special nursing facility + The geriatric hospital. 2) The profit special nursing facility + The geriatric hospital. 3) The geriatric hospital + The hospital rooms with silver town residence type. 4) The actual expense special nursing facility + The geriatric hospital + The hospital rooms with silver town residence type. 5) The geriatric hospital + The residential welfare service. It seems that the preparation for the elderly is urgent when considering the facilities related to those are not special any more because every one is supposed to use them some day. The kinds and the degree of elderly's diseases will be more duplicated and significant afterwards. Therefore it is expected that more various and specialized facilities are to be required.
Our Constitution obliges the state to protect the health of the people, and the Medical Law, which embodied Constitution, sets out in detail the matters related to open the medical institution and one of them is to prohibit the operation of multiple medical institutions In the past, there was a provision stipulating the same purpose. But because the Supreme Court interpreted that several medical institutions could be opened if the medical treatment was not made at the additional medical instition which was opened in the another doctor,s license, multiple medical institutions could be opened and operated. However, some health care providers opened the several medical institutions to another doctor's license just by the excuse of the business management and then did illegal medical cares like the unfair luring of patients, overtreatment, and commition treatment for more profits. So, the health rights of the people came to be infringed on. Accordingly, lawmakers amended the Medical Law for medical personnel not to open and to operate more than one medical institution. As the amended medical law prohibited a medical personnel to open multiple medical institution, some medical personnels insisted that the amended medical law is unconstitutional under which they could not be able to open and operate medical institutions on based on free investment and bring out the benefits of network hospitals. But the regulation to prohibit multiple institutions does not apply only to a medical personnel. Many other experts like lawyer and pharmacist can open only one office under such a restriction. If the regulation goes out of force, the procedure that multiple medical institutions should be opened and operated in the capacity as a medical corporation or a non-profit corporation does not have to be followed. And we should keep in mind that the permission for medical personels to open multiple medical institutions could lead virtually to commercial hospital. If in the nation with a very low rate of public medical service, If only a few medical personnels with capital own many medical institutions and operate commercially them, this could cause a falling-off in quality of medical service, ultimately infringe on the health rights and the life right of the people.
In recent years the combination of IT and BT for U-Healthcare Medical Services has attracted popular attention as an alternative for solving health problems due to aging issue. Active oxygen as free radicals causes unstable state of the body resulting to other disease on which approximate 90% of the diseases are associated with active oxygen. The severity is even higher along with increasing age. Therefore, there is a need for systematic management of active oxygen saturation for the elderly and chronic patients. In this paper, the data about active oxygen measurements from the body measurement is sent to the data storage of medical facilities by the service provider. Medical information is measured by devices provided complying with the standard, support and information using Bluetooth communication. Users can check their health status themselves using the proposed system through 3G/4G mobile devices like smart phone to medical institutions in which the smart phone act as a gateway for medical data transferred from patient to medical institutions and vice versa. Users can get and experience a diagnosis and effective U-Environmental Health Services from Medical institutions anywhere using the proposed built system.
In Korea, as the elderly population is growing, the quality of life of the rural elderly is becoming a major concern. By the way, social welfare services is less available and accessible to the rural elderly. And we have very limited information about community -oriented welfare services for the elderly. The lack of social welfare services in rural area resulted from mainly geographic isolation and economic deprivation. So, the present research aimed at; 1) to explore what the social welfare service is benefited from governmental or local assembly. 2) to explore what kind of community services is provided for the rural village and elderly. 3) to appear the political propose for the rural elderly. In South Korea, The Rural Development Administration currently operate 'villages which is supported special programs for the improvement of QOL of rural elderly' in 110 villages. It is a model governmental welfare service for rural area. For the purpose, the survey data is gathered from community level data per village (107 villages), individual survey data(881people) who live in the village and qualitative data. Two kinds of quantitative data is combined to form a data. The statistical methods used for data analysis are descriptive statistics, t-test and ANOVA. The major findings of this study were as follows : It was founded that the majority family type of the rural elderly is elderly-only households(75%). In case of poor elderly, they have very limited social insurance benefit and they can not get medical services with satisfaction. The result show that the welfare facilities per village and welfare service for the elderly is extremely low while the needs of welfare service is greate high. A distinctive characteristics in rural villages is that they receive a lot of services from private sectors, like as Women Farmers Union, Adult Union. They operate voluntary welfare services related to food supporting, education for the elderly, free haircut services and so on. In conclusion, the community care services from private sector has specialty in rural area. We conclude it is a distinguishing characteristic of rural community.

Purpose: This study was to evaluate the utilization of health care service and to provide supportive data for health care policy making in one urban area in Korea. Method: This study tested the significance of public health service using the database of an university hospital and public health center from Feb. 2000 to Dec. 2004. Data were analyzed by multidimensional analysis and data mining technique and produced the information on the classification of utilization characteristics by main disease and the total cost of use and disease association with the users of the public health center. Results: The Results were as follows: 1) Top 10 diseases in the area accounted for 22.4% of total frequency for the most recent 5 years in university hospital, while 59.0% in public health center. 2) There were significant correlations between university hospital and public health center user's insurance type and place of residence: It showed higher use of public health center for free service beneficiaries residing in Seoul than residents in nearby or local area. The medical insurance types for hospital users were more various than those for public health center users. 3) The use of hospital for patients of hypertension, diabetes mellitus and hyperlipidemia was tended to concentrate in mostly autumn and winter since August 2000, while the cost of using public health center for those patients has been steadily reduced since July 2000. 4) As a result of cluster analysis, there were classified into three homogeneous groups according to the total cost of using public health service, age, and the frequency of use. 5) The association analysis on patients with chronic disease in public health center produced a detailed information on accompanying diseases related to the incidence rate of disease of high frequency due to aging, information on drug abuse and immune disease. Conclusion: The health care policy for local community should be evaluated continuously. And the policy to build an integrated data warehousing by public health indicator system and to enhance the faithfulness of data is required.
