Development of Numerical Tool considering Interfacial Fracture Behavior in Repaired RC Structure (보수.보강된 RC 구조물의 경계면 파괴를 고려한 수치해석 기법 개발)

  • In this study, a numerical simulation that can effectively predict the interfacial fracture behavior in repaired structures is developed using the axial deformation link elements. In repaired structures, concrete and interface are considered as quais-brittle materials, and steel plate as a repair material and reinforcement are modeled as elasto-plastic materials. The behavior of repaired reinforced concrete structures under flexural loading conditions is numerically simulated, and compaired with experimental results. The strengthening effect according to the length and thickness of the repair material is studied and rip-off, debonding and rupture failure mechanism of interface between substrate and repair materials are detected. It is shown that the interface properties affect on the mechanical behavior of repaired structures. Therefore, the developed numerical method using axial deformation link elements can be used for determining the strengthening effects and failure mechanism of repaired structures.

  • A thyroid cartilage fracture is a rare entity and can be overlooked easily. Such cases are difficult to diagnose, and assessment and treatment guidelines are difficult to determine. CT of the neck region may be useful when acute airway intervention is not required or when more information regarding the neck's anatomy is required for management decisions. We describe a case of a thyroid cartilage fracture with displacement. In the emergency department (ED), neck CT and fiberoptic nasopharyngoscopy were used to assess the status of the patient's (a male) vocal chords immediately. He remained unable to phonate continuously. After an immediate assessment, we decided to use steroid and conservative therapy. The patient had a good recovery and was without symptoms one month after injury. There is no question that early surgical repair of neck injuries affords the best results for airway and voice patency in most cases however, we suspect that surgical repair is not needed in all cases. Early recognition and an accurate therapy plan for a thyroid fracture with displacement are essential. Therefore, the emergency physician's immediate and careful decision based on endoscopy and neck CT is important for the patient's long-term recovery.

Usefulness of a Transconjunctival Approach in the Reconstruction of the Medial Blow-Out Wall Fracture

  • Background: A transcaruncular approach is typically used for reconstructions of medial wall fractures. However, others reported that a transconjunctival approach was conducive for securing an adequate surgical field of view. In this study, we aimed to examine the extent of repair of medial wall fracture via a transconjunctival approach. Methods: We retrospectively reviewed the medical records of 50 patients diagnosed as having medial wall fracture via preoperative computed tomography and who underwent surgery between March 2011 and February 2014. The fracture location was defined by dividing each of the anterior-posterior and superior-inferior distances into three compartments. Results: A transcaruncular approach was used in 7 patients, while the transconjunctival approach was performed in the remaining 43 patients. The transconjunctival approach enabled a relatively broad range of repair that partially included the front and back of the medial wall, and was successful in 86% of the entire study population. Conclusion: It is known that more than 50% of total cases of the medial wall fracture occur mainly in the middle-middle portion, a majority of which can be reconstructed via a transconjunctival approach. We used a transconjunctival approach in identifying the location of the fracture on image scans except for cases including the fracture of the superior portion in patients with medial wall fracture. If it is possible to identify the location of the fracture, a transconjunctival approach would be an useful method for the reconstruction in that it causes no damages to the lacrimal system and is useful in confirming the overall status of the floor.

Effects of Diet of Korean Safflower(Carthamus tinctorious L.) Seed Powder on Bone Tissue in Rats during the Recovery of Rib Fracture (토종홍화씨의 급여가 실험동물의 늑골골절 회복중 골조직에 미치는 영향)

  • This study was carried out to investigate the effects of diet of Korean safflower(Carthamus tinctorious L.) seed powder on bone tissue during the recovery of rib-fracture in rats. Male Sprague-Dawley rats of 10 weeks old, weighing 370$\pm$5g, were divided into two groups including the control group(C group, AIN-76 semipurified diet) and safflower seed group(S group, AIN-76 semipurified diet+10% safflower seed powder) and were fed experimental diets for 12 days after adaptation period. After this period, the 9th right rib was fractured surgically and sham-operation was also performed. Rats were fed with experimental diets for up to 30 more days after rib-fracture. The degree of bone repair was evaluated during the recovery period at the 8th, 11th, 16th, 21st, 30th days after the surgical operation by microscopic observation of the fractured rib tissue. In callus formation, the portion of hyaline cartilage was noticably higher in S group than C group. The intracatilagenous ossification was observed at the 8th day in S group, but at 11th day in C group. The intramembranous ossification in callus was widely found over the 8th day to the 11th day in S group, but it was shown over the 11th day to the 16th day in C group. Bone resorption was also occured more rapidly in S group as indicated by large numbers of osteoclasts observed. At the 30th day, most of trabecular bones were disappeared in S group, whereas still shwon in C group over wide ranges of fractured ribs. These results imply that the supplementation of Korean safflower seed powder influences in the recovery of bone fracture by accelerating the process of bone repair.

The Results of Primary Repair in Acute Injuries of the Posterior Cruciate Ligament (후방 십자 인대 급성 손상에서 일차 복원술의 결과)

  • Background: We analyze the functional evaluation, posterior stability after surgery of acute injuries in the femoral detachment and tibial avulsion fracture of posterior cruciate ligament. Materials and Methods: Twenty two patients who underwent primary repair were followed more than one year after operation (mean follow-up 33.7 months). The patients were evaluated with KT 2000 arthrometer and posterior stress test for posterior stability. Lysholm knee score was used for knee function. Results: Lysholm score revealed 85.3 in femoral detachment group and 91.1 in tibial avulsion fracture group (P<0.05). Posterior displacement of the posterior stress test showed 17.2 mm at initial injury and 8.6 mm at last follow up in femoral detachment group, 16.8 mm at initial injury and 7.1 mm at last follow up in tibial avulsion fracture group. There revealed the tendency of the improved posterior stability of the tibial avulsion group compared with femoral detachment group, but there showed no statistical significance in KT 2000 arthrometer and posterior stress x-ray. Conclusion: Functional results of tibial avulsion fracture group revealed the better outcome compared with femoral detachment group, but the degree of posterior stability in tibial avulsion fracture group showed no statistical significance in posterior stability.

  • Isolated lateral collateral ligament injury has been reported as avulsion fracture of attachment site or intrasubstance tear. The treatment of avulsion fracture of lateral collateral ligament was primary repair with anchor or staple. There are some reports about loosening or migration of staple which used in repairing meniscus of knee. There is, however, no report about loosening or migration of staple which was used for lateral collateral ligament repair. We report a case of migrated staple that was used for lateral collateral ligament repair, which was migrated to intraarticular posterolateral corner of the knee.

  • In clinical practice, recurrent Achilles ruptures have been noted to occurr at the original ruptured site. However, reports of new developed fresh rupture of the Achilles tendon in other sites are is extremely rare. Our report is about one uncommon case of a traumatic calcaneal tuberosity avulsion fracture following augmented repair, which was performed using the Krackow locking loop technique. We performed open reduction and intra-osseous fixation using a suture anchor. This procedure was done through the primary longitudinal incision for the calcaneal avulsion fracture fragment. After 6 months of follow-up, our patient has achieved a complete functional recovery and he can normally perform daily and work-related tasks without pain.

  • The Wnt signaling pathway is well known to play major roles in skeletal development and homeostasis. In certain aspects, fracture repair mimics the process of bone embryonic development. Thus, the importance of Wnt signaling in fracture healing has become more apparent in recent years. Here, we summarize recent research progress in the area, which may be conducive to the development of Wnt-based therapeutic strategies for bone repair.

Repair bond strength of resin composite to bilayer dental ceramics

  • PURPOSE. The purpose of this study was to investigate the effect of various surface treatments (ST) on the shear bond strength of resin composite to three bilayer dental ceramics made by CAD/CAM and two veneering ceramics. MATERIALS AND METHODS. Three different bilayer dental ceramics and two different veneering ceramics were used (Group A: IPS e.max CAD+IPS e.max Ceram; Group B: IPS e.max ZirCAD+IPS e.max Ceram, Group C: Vita Suprinity+Vita VM11; Group D: IPS e.max Ceram; Group E: Vita VM11). All groups were divided into eight subgroups according to the ST. Then, all test specimens were repaired with a nano hybrid resin composite. Half of the test specimens were subjected to thermocycling procedure and the other half was stored in distilled water at $37^{\circ}C$. Shear bond strength tests for all test specimens were carried out with a universal testing machine. RESULTS. There were statistically significant differences among the tested surface treatments within the all tested fracture types (P<.005). HF etching showed higher bond strength values in Groups A, C, D, and E than the other tested ST. However, bonding durability of all the surface-treated groups were similar after thermocycling (P>.00125). CONCLUSION. This study revealed that HF etching for glass ceramics and sandblasting for zirconia ceramics were adequate for repair of all ceramic restorations. The effect of ceramic type exposed on the fracture area was not significant on the repair bond strength of resin composites to different ceramic types.

A Case of Transorbital Orbital Floor Fracture Repair Assisted Endonasal Balloon Catheter (비내 풍선 카테터 유도하 경안와 접근법을 이용한 안와 하벽 골절 정복 1예)

  • It is important to choose which approaches should be used to correct the fracture if the operation is indication for surgery in patients with orbital floor fracture. Transorbital, transantral, and endonasal approaches are methods for correcting the orbital floor fracture. The approach needs to be considered the location of the fracture, the degree, the severity of the fracture, the least remaining patient's disability, and preference of the surgeon. We report a case of orbital floor fracture using transorbital approach and endonasal catheter ballooning to resolve the limitations of transorbital approach alone.
