Evaluation of High-Viscosity Grouting Injection Perfomance for Reinforcement of Rock Joint in Deep -Depth Tunnels (대심도 터널 암반 절리 보강을 위한 고점도 그라우팅 주입 성능 평가)

  • This study aimed to develop high-efficiency grouting techniques under deep-depth conditions by experimentally verifying the applicability of various injection materials. Particle size analysis and injection model experiments were conducted with Ordinary Portland Cement (OPC) and Micro Cement (MC) to evaluate the injection performance of each material. Using Barton's Cubic Network theory, the rock fracture spacing was calculated for domestic deep-depth standards, specifically below 40 meters underground. The analysis of particle size passability under selected conditions showed that MC could pass through the rock fracture gaps, while OPC could not. According to the results of the injection model experiments using experimental devices and area calculation software, OPC failed in injection due to its larger particle size, whereas MC was capable of injection even under high-viscosity conditions. Based on these findings, the study quantitatively and visually derived the applicability of grouting materials under deep-depth conditions, and high-viscosity MC material is expected to be effective for waterproofing enhancement in deep-depth rock fracture surfaces.

  • The objective of this paper is to evaluate the material properties of SA312 TP316 and SA312 TP304 stainless steels and their associated welds manufactured for safety injection system of Yonggwang 3,4 nuclear generating stations. A total of 62 tensile tests and 46 fracture toughness tests were conducted and the effects of various parameters such as pipe size, crack plane orientation, tests were conducted and the effects of various parameters such as pipe size, crack plane orientation, test temperature, welding on material properties were discussed. Test results show that the effect of test temperature on fracture toughness was significant while the effects of pipe size and crack plane orientation on fracture toughness were negligible. Fracture toughness of the weld metal was in general higher than that of the base metal.

Effects of Pharmacopuncture and Danggwisu-powder for Lateral Malleolus Avulsion Fracture: a Case Report (족외과 견열골절 환자에서 약침치료, 당귀수산을 통해 호전된 1례 증례보고)

  • Objectives : The purpose of this study is to report the clinical effect of Korean medical treatment, specifically Homnis Placenta pharmacopuncture, bee venom injection and Danggwisu-powder, on a patient suffering from a lateral malleolus avulsion fracture. Methods : A 63-year-old female patient suffering from right ankle pain due to lateral malleolus avulsion fracture was treated with Homnis Placenta pharmacopuncture, bee venom injection, and herbal medicine(Danggwisu-powder). from May 20th to June $16^{th}$ of 2015. The change in patientcondition was measured with the American Orthopedic Foot and Ankle Society Ankle-Hindfoot Scale, the verbal numerical rating scale, and via the ankle's range of motion. Results : After treatment, all obtained results showed improvement. The American Orthopedic Foot and Ankle Society Ankle-Hindfoot Scale increased from 18 to 71, the verbal numerical rating scale changed from 8 to 3, and the ankle's range of motion improved. Conclusions : This study suggests that Korean medical care focused on Homnis Placenta pharmacopuncture, bee venom injection and Danggwisu-powder can be effective to treat lateral malleolus avulsion fracture.

Treatment for Acute Stage Complex Regional Pain Syndrome Type II with Polydeoxyribonucleotide Injection

  • Complex regional pain syndrome (CRPS) type II is a syndrome that develops after nerve injury. Symptoms may be severe, and vary depending on the degree of sympathetic nerve involvement. As yet, there is no satisfactory treatment. We report the case of a female patient who had an L5 left transverse process fracture and an S2 body fracture, who developed symptoms of CRPS type II in her left lower leg that were aggravated during ambulation in spite of absolute bed rest for one month after the trauma. Several treatments, including bed rest, medication, and numerous nerve blocks were attempted, but the pain persisted. We finally tried injection of polydeoxyribonucleotide (PDRN) solution at the left L5 transverse process fracture site because we knew of the anti-inflammatory effect of PDRN. One day after this treatment, her symptoms had almost disappeared and three days later, she was discharged. We will also further discuss the possibility of using PDRN solution for the treatment of CRPS.

  • The effect of the temperature, the fatigue and the test speed on DEN(double edged notch) specimen which was made by the pp-rubber composites during fracture was stuied. DEN specimen was made on PP-rubber composites through the injection molding. With increasing temperature the fracture strength is linearly decrease and the fracture energy is first increase by $0^{\circ}C$ and after that decrease. In the same temperature the fracture strength during increasing the notch radius is hardly increase. The fracture behaviour at low and high test speed is different entirely. At high test speed plastic region is small and fracture behaviour was seen to brittle fracture tendency. The deformation mechanism of polypropylene-rubber composites during fracture was studied by SEM fractography. A strong plastic deformation of the matrix material ahead of the notch/crack occured. The deformation seem to be enhanced by a thermal blunting of the notch/crack.

Botulinum toxin as adjunct therapy in surgical management of a periprosthetic scapular spine fracture: a case report

  • Six months after undergoing reverse shoulder arthroplasty (RSA) a 73-year-old woman sustained a periprosthetic scapular spine fracture following a fall. She was treated with open reduction and internal fixation (ORIF), followed by botulinum toxin injection into the deltoid muscle to temporarily minimize strain at the fracture. Fracture union was achieved by 3 months, with excellent clinical function more than 1 year following fracture fixation and full resolution of deltoid function. Scapular spine fracture following RSA can be treated with ORIF and temporary deltoid paralysis using botulinum toxin in the immediate postoperative period to safely support fracture healing.

  • When constructing subsurface structures and/or wells, the precise estimates of hydraulic parameters must be obtained for operating safety and/or for developing necessary quantity of groundwater. Hydraulic conductivity is mainly subjected to the characteristics of fracture network in the fractured media such as fracture orientation and angle, fracture aperture and frequency, fracture length, interconnectivity of fractures, and filling material, feature of fracture plane. In this study we conducted water injection test at afferent depths on six boreholes drilled in granite of Mt. Geumjeong. hydraulic conductivity was calculated using Moye and Hvorslev methods. The relation between hydraulic conductivity and fracture frequency data obtained from acoustic televiewer and core log were analyzed. The result shows that the correlation between the hydraulic conductivity and the fracture frequency data obtained from acoustic televiewer is better than that with the core log.

  • We simulated the fault reactivation experiment conducted at 'Main Fault' intersecting the low permeability clay formations of Mont Terri Underground Research Laboratory in Switzerland using TOUGH-FLAC simulator. The fluid flow along a fault was modelled with solid elements and governed by Darcy's law with the cubic law in TOUGH2, whereas the mechanical behavior of a single fault was represented by creating interface elements between two separating rock blocks in FLAC3D. We formulate the hydro-mechanical coupling relation of hydraulic aperture to consider the elastic fracture opening and failure-induced dilation for reproducing the abrupt changes in injection flow rate and monitoring pressure at fracture opening pressure. A parametric study was conducted to examine the effects of in-situ stress condition and fault deformation and strength parameters and to find the optimal parameter set to reproduce the field observations. In the best matching simulation, the fracture opening pressure and variations of injection flow rate and monitoring pressure showed good agreement with field experiment results, which suggests the capability of the numerical model to reasonably capture the fracture opening and propagation process. The model overestimated the fault displacement in shear direction and the range of reactivated zone, which was attributed to the progressive shear failures along the fault at high injection pressure. In the field experiment results, however, fracture tensile opening seems the dominant mechanism affecting the hydraulic aperture increase.

Evaluation of grout penetration in single rock fracture using electrical resistivity

  • In this study, a new approach using electrical resistivity measurement was proposed to detect grout penetration and to evaluate the grouting performance for such as waterproof efficiency in single rock fracture. For this purpose, an electrical resistivity monitoring system was designed to collect multi-channel data in real time. This was applied to a system for grout injection/penetration using a transparent fracture replica with various aperture sizes and water-cement mix ratio. The electrical resistivity was measured under various grout penetration conditions in real time, which results were directly compared to the visual observation images of grout penetration/distribution. Moreover, the grouting success status after the curing process was evaluated by measuring the electrical resistivity in relation to changes in frequency in fracture cells where grout injection and penetration were completed. Consequently, it was determined that the electrical resistivity monitoring system could be applied effectively to the detection of successful penetration of grouting into a target area and to actual field evaluation of the grouting performance and long-term stability of underground rock structures.

Effect of Surgical-Site, Multimodal Drug Injection on Pain and Stress Biomarkers in Patients Undergoing Plate Fixation for Clavicular Fractures

  • Background: Surgical-site, multimodal drug injection has recently evolved to be a safe and useful method for multimodal pain management even in patients with musculoskeletal trauma. Methods: Fifty consecutive patients who underwent plating for mid-shaft and distal clavicular fractures were included in the study. To evaluate whether surgical-site injections (SIs) have pain management benefits, the patients were divided into two groups (SI and no-SI groups). The injection was administered between the deep and superficial tissues prior to wound closure. The mixture of anesthetics consisted of epinephrine hydrochloride (HCL), morphine sulfate, ropivacaine HCL, and normal saline. The visual analogue scale (VAS) pain scores were measured at 6-hour intervals until postoperative hour (POH) 72; stress biomarkers (dehydroepiandrosterone sulfate [DHEA-S], insulin, and fibrinogen) were measured preoperatively and at POH 24, 48, and 72. In patients who wanted further pain control or had a VAS pain score of 7 points until POH 72, 75 mg of intravenous tramadol was administered, and the intravenous tramadol requirements were also recorded. Other medications were not used for pain management. Results: The SI group showed significantly lower VAS pain scores until POH 24, except for POH 18. Tramadol requirement was significantly lower in the SI group until POH 24, except for POH 12 and 18. The mean DHEA-S level significantly decreased in the no-SI group ($74.2{\pm}47.0{\mu}g/dL$) at POH 72 compared to that in the SI group ($110.1{\pm}87.1{\mu}g/dL$; p = 0.046). There was no significant difference in the insulin and fibrinogen levels between the groups. The correlation values between all the biomarkers and VAS pain scores were not significantly different between the two groups (p > 0.05). Conclusions: After internal fixation of the clavicular fracture, the surgical-site, multimodal drug injection effectively relieved pain on the day of the surgery without any complications. Therefore, we believe that SI is a safe and effective method for pain management after internal fixation of a clavicular fracture.
