Median Nerve Entrapment Syndrome Due to Adhesion of Laceration Wound by Suicidal Attempt -A Case Report (자살시도로 인한 손목 열상 후 유착에 의해 발생한 수근부 정중신경포착증후군 치험례)

  • Purpose: Median nerve entrapment syndrome within carpal tunnel is usually called carpal tunnel syndrome and it is the most common form of peripheral nerve entrapment syndrome. Many factors such as diabetes mellitus, hypothyroidism, hormonal replacement theraphy, corticosteroid use, rhematoid arthritis and wrist fractures may cause carpal tunnel syndrome. To the best of our knowledge, this is the first case report of median nerve entrapment syndrome due to adhesion of laceration wound after suicidal attempt. Methods: A 28-year-old woman presented with a sensory change and thenar hypotrophy on her left hand. On her history, she attempted suicide by slashing her wrist. Initial electromyography (EMG) showed that the nerve conduction velocities of median nerve was delayed. Therefore, we performed surgical procedures. When exploration, Fibrous scar tissue observed around the median nerve but nerve had not been injured. Transcarpal ligament was completely released and adjacent fibrous tissue was removed to decompress the median nerve. Results: The postoperative course was uneventful until the first year. Opposition difficulty and thenar hypotrophy were improved progressively after the surgery. Sensory abnormality was slowly improved over one year. Conclusion: We report a case of median nerve entrapment syndrome that was caused by adhesion of laceration wound after suicidal attempt. This is an unusual cause of median nerve entrapment syndrome, the symptoms were relieved after transcarpal ligament release and fibrous scar tissue removal.

Morphological Development and Histology of Multiple Shoots and Microbulbs of Garlic Cultured in Bioreactors (생물반응기에서 배양된 마늘 신초와 Microbulb의 형태발달 및 조직관찰)

  • Multiple shoots of garlic (Allium sativum L.) were propagated in bioreactors containing MS medium supplemented with 2% sucrose for 3 weeks. Microbulbs were induced on MS medium supplemented with 0.1mg/L NAA and 11% sucrose for 9 weeks. For multiple shoot proliferation, leaves in the shoot must be removed before cultures. When the multiple shoots were cultured without removal of leaves, more than 90% of hyperhydricity and no microbulb formation were observed. Histological observation also indicated irregular size and shape of the cells in hyperhydricity of the shoot. Microbulbs were strarted to form from the shoot after 7 weeks of culture by protuberance of adventitious shoot buds followed by inner periclinal divisions and simultaneous anticlinical division in the epidermis of meristematic bulge. Analysis of ploidy level indicated no phenotypic variations in both multiple shoots and microbulbs induced from the mother plant, suggesting genetic homogeneity among the regenerants.

Malignant Thrombi of Inferior Vena Cava from Renal Cell Carcinoma -4 cases reports- (신세포암에 의한 하대정맥 악성 혈전 -치험 4례-)

  • Renal cell carcinoma involves the inferior vena cava(IVC) in approximately 5-10% of the patients. Presently surgical extirpation is the only form of therapy that can result in cure. Circulatory arrest with cardiopulmonary bypass is an operative technique that recently has been used to assist in resection of tumors that extend into the vena cava above the level of hepatic veins. We performed removal of tumor thrombi of IVC in 4 patients. All of them who had the renal cell carcinormas with infrahepatic vena caval extension were performed by standard surgical technique without cardiopulmonary bypass. But in one patient, inferior vena caval thrombectomy was done using circulatory arrest because of the recurred extension of the tumor thrombi within the vena cava above the insertion of the hepatic vein. All patients were recovered without any significant problems.

Analysis of PICC Inserted Patient Data in a Hospital by IV CNS-Driven Intervention (정맥주입 전문간호사가 삽입한 말초삽입형 중심정맥관(PICC) 사용 결과에 대한 후향적 분석)

  • Background : Intravenous(IV) access is becoming an increasingly important part of health care today. The current drive for clinical effectiveness and cost-effective health care serves to increase the need for reliable vascular access. Venous access devices were developed to overcome problems associated with limited peripheral access and frequent venipuncture in patients with long-term therapy. Although the peripherally inserted central catheter(PICC) have become popular during recent years in USA, its procedure is rare in Korea. Purpose : The goal of this study was to analyze the PICC inserted patient data by IV CNS intervention. Method : A Total of 62 PICCs were inserted into 51 patients by the IV CNS during a 10-month period form November, 14, 2002, to October 2, 2002. Data was obtained retrospectively through chart review. Result : The patient population included 34(54.8%) men and 28(45.2%) women, with a mean age 50.6 years. The main indication for PICC placement was to access vein in poor peripheral venous status(40.3%). The mean served interval for PICC insertions was 16.7 days(range, $2{\sim}61$ days). The reasons for removal were completed therapy in 18 cases(29.0%), patient death in 13 cases(21.0%), and mechanical or functional PICC problem in 10cases(16.1%). The three PICCs removed for presumed infection, and one had only positive tip cultures(0.2%). Conclusion : PICCs are rapidly growing popularity and required an extended course of IV therapy.

Structural characterization of calmodulin like domain of ryanodine receptor type 1

  • Ryanodine receptor (RyR) is one of the two major $Ca^{2+}$ channels in membranes of intracellular $Ca^{2+}$ stores and is found in sarcoplasmic reticulum (SR), endoplasmic reticulum (ER). RyR1 is also the major calmodulin-binding protein of sarcoplasmic reticulum membranes. Residues 4064-4210 in the RyR1 polypeptide chain has similar primary sequence with calmodulin (CaM) and was designated as CaM-like domain (CaMLD). When expressed as a recombinant peptide, CaMLD showed several CaM-like properties in previous studies. Still, previous studies of CaMLD were focused on protein-protein interactions rather than its own properties. Here, we studied the expression of CaMLD and its sub-domains corresponding to each lobe of CaM in Escherichia coli. CaMLD could be obtained only as inclusion body, and it was refolded using urea solubilization followed by dialysis. Using spectroscopic approaches, such as NMR, circular dichroism, and gel filtration experiment, we found that the refolded CaMLD exists as nonspecific aggregate, even though it has alpha helical secondary structure. In comparison, the first half of CaMLD (R4061-4141) could be obtained as natively soluble protein with thioredoxin fusion. After the removal of the fusion tag, it exhibited folded and helical properties as shown by NMR and circular dichroism experiments. Its oligomeric status was different from CaMLD, existing as dimeric form in solution. However, the second half of the protein could not be obtained as soluble protein regardless of fusion tag. Based on these results, we believe that CaMLD, although similar to CaM in sequence, has quite different physicochemical properties and that the second half of the protein renders it the aggregative properties.

Preparation and Characterization of Liquefied Ibuprofen Using Self-Microemulsion Drug Delivery System (SMEDDS) (자가미세유화를 이용한 이부프로펜 액상제제의 제조와 특성)

  • Ibuprofen (IBU), is a non-steroidal anti-inflammatory drug, used to treat rheumatoid arthritis, removal of fever and mild to moderate pain. Because of small dosage and very low accumulation in the body, IBU has been used to heal children's fever. However, IBU was very low solubility in a low pH and water (in water $0.03{\sim}2.5$ mg/ml). A nanoemulsion containing IBU by means of self-microemulsion drug delver system (SMEDDS) was prepared in order to enhance the solubility of IBU. The SMEDDS was composed of cosurfactant, oil and surfactant The solubility of IBU in various components such as cosurfactant, oil and surfactant was examined. $Carbitol^{\circledR}\;(386.99{\pm}20.5\;mg/ml)$ as a cosurfactant, $Labrafil^{\circledR}$  M1944CS $(90.16{\pm}1.60mg/ml)$ as an oil and $Cremopher^{\circledR}$  RH-40 $(239.01{\pm}2.8\;mg/ml)$ as a surfactant were used in this study for preparing SMEDDS. Optimized formulation of SMEDDS was obtained by phase diagram which express the section of nanoemulsion formation. The SMEDDS containing IBU had higher dissolution rate than conventional IBU sirups. Thus the SMEDDS was a potential candidate of stable conventional and effective oral dosage form for IBU.

Community Characteristics and Assessment of Water Quality Impact by Plants at Flooded Area (저수지역 식물의 군집특성 및 수질영향 평가)

  • This study carried out submerged area due to Dam construction in the near future. It includes species classification of plant, survey of community structure, examination of pollutant load and assessment of water quality impact. The vascular plants of this area are listed 224 taxa; 64 families, 168 genera, 193 species, 30 varieties and 1 form. This study area is classified into total 21 communities, most community was consist of grass vegetation. Among the communities, Erigeron annuus ($869,286m^2$, 22%) community was dominant and Erigeron annuus-Avena fatua comminity (16%) was subdominant until May, and then Erigeron canadensis community occupied most area to $1,774,985m^2$ (32%) from May to July. For the evaluation of water quality impact due to submerged macrophyte, nutrient release test was conducted both dead body macrophyte and living body macrophyte. The results of release test show that T-N is not released at dead body macrophyte, but it is released at living body macrophyte, especially living body Artemisia priceps var. orientalis shows 1.436mgN/g. At release test of dead body macrophyte, T-P release rate of Erigeron annuus shows 0.500mgP/g at the top of them and it also shows 0.436mgP/g at Erigeron annuus of living body macrophyte. T-N load of submerged macrophyte shows 0.76% by comparison of total load on watershed and T-P load of that shows 3.61%. In case of removal macrophyte for reduction of pollutant load in submerged area, T-N load of submerged macrophyte changes from 0.76% to 0.15% by comparison of total load on watershed and T-P load of that changes from 3.61% to 0.72%.

Evaluation of Fishmeal Supplement with Net Nitrogen Flux by the Portal-drained Viscera and the Liver in Mature Sheep

  • The objective of this study was to evaluate the net flux response of nitrogen compounds (alpha-amino N, ammonia N, urea N, essential amino acids) across the portal-drained viscera (PDV), liver and total splanchnic tissues of mature wethers to increasing level of dietary fishmeal (FM) supplementation. Four wethers (average body weight, 64 kg) with chronic indwelling catheters into the portal, hepatic and mesenteric veins and the abdominal aorta were used in a 4${\times}$4 Latin square design. A basal diet consisting of 0.7 hay and 0.3 concentrate was fed twice daily with a fixed amount at 1.4 times maintenance energy (1.3 kg/day on a dry matter basis). The supplementation proportion of FM as treatment was 0, 0.03, 0.06 and 0.09 to the amount of the basal diet to contain 119, 137, 154 and 170 g crude protein per kg dietary dry matter, respectively. Blood flows through PDV and liver did not differ (p>0.05) among the treatments. Both net PDV release and hepatic uptake of alpha amino acid N increased linearly (p<0.05) in response to increased dietary FM, which resulted in similar total splanchnic release of alpha-amino N among the treatments. Similarly, increased dietary FM increased net PDV absorption and hepatic removal of ammonia N linearly (p<0.05). Hepatic synthesis and total splanchnic release of urea N increased linearly (p<0.01) with increased dietary FM, but PDV uptake of urea N did not respond to increased dietary FM. Linear regression equations between the increases in FM N intake and PDV net flux indicated that 0.34 and 0.30 of FM N was absorbed in the form of alpha-amino N and ammonia N, respectively. The results demonstrated that FM supplementation provides more alpha-amino N than ammonia N to the liver, but the alpha-amino acid N absorption is less than the expected metabolizable protein N from FM supplementation.

Behavioral Characteristics of Bos indicus Cattle after a Superovulatory Treatment Compared to Cows Synchronized for Estrus

  • The intensity and duration of sexual behavior in Bos indicus was assessed through the continuous observation of sexual receptivity. Two groups of cows were formed: only synchronized (n = 50) and other group further superovulated (n = 20). An intravaginal implant that released progesterone over 9 d was used. After removing the implant, 25 mg of $PGF_{2{\alpha}}$ was administered. In the superovulated group, the administration of 280 mg (Follicle stimulant hormone) FSH-P1 per cow with a decreasing dosage over 4 d was utilized. In both groups, behavioral observations began at the moment of implant removal. Sexual behavior was analyzed using a Kruskal-Wallis test to compare the mean of hours in estrus, effective mountings and number of mounts/hour during estrus. A nonparametric survival analysis was performed using the time in two ways: i) when an event happened it was placed in a 24 h timeframe and, ii) the time of observation in continuous form (96 h) assessing the difference between curves by the log rank test Chi-square. The only significant difference was the number of mounts/h during receptivity (p<0.05). In the superovulated group three periods of sexual activity during the day were identified, with these events being of greater frequency and duration than the synchronized group (p = 0.02); besides, the superovulated group began estrus before the synchronized group (p = 0.0035) when using the total period. In a simulation study, when the number of observations went from two (06:00-18:00) to three periods (06:00, 12:00 and 18:00) cows detected accurately (<6 h after the onset) increased more than 20%. The results show that superovulated cows presented greater intensity and duration of sexual activity in contrast with only synchronized animals.

Performance Evaluation of Recycled Aggregate Concrete Block Reinforced with GFRP (GFRP로 보강된 순환골재콘크리트 블록의 성능평가)

  • Precast concrete blocks are used mainly for score protection, slope protection and riverbed structure protection, etc. Because these concrete blocks are exposed to water or wetting environments, the steel rebar used as reinforcements in concrete blocks can corrode easily. Corrosion of the steel rebar tends to reduce the performance and service life of the concrete blocks. In this study, Glass Fiber Reinforced Polymer(GFRP) rebar, which does not corrode, was applied instead of a steel rebar to prevent performance degradation of the blocks. Recycled concrete aggregate and high early strength cement(HESC) were used in the concrete mix for field applicability. The experiment results showed that the workability and form removal strength of the recycled aggregate concrete using HESC showed comparable results to normal concrete and the compressive strength at 28 days increased by about 18% compared to normal concrete. The load resistance capacity of the recycled aggregate concrete blocks reinforced with a GFRP rebar increased by approximately 10~30% compared to common concrete block.
