  • A foreign body may present diagnostic difficulties. If the foreign body has been in existence for a long time, the diagnosis may be even more complex. This report shows an unusual foreign body that turned out to be the cap of a ball point pen.

An Intradiscal Granuloma Due to a Retained Wooden Foreign Body

  • We report a patient with a wooden foreign body granuloma in the intervertebral disc space being symptomatic 17 years after a paraspinal penetrant trauma. According to the our result of the search for wooden foreign body granulomas, this is the first case suffered from a wooden foreign body granuloma in the intervertebral disc space that reported in the literature. In this report, we emphasized the importance of rigorous examination and follow up in paraspinal wooden penetrant traumas.

  • The risk of foreign body by small button-disc battery in children has been increasing because the batteries are widely used in electronic device and plaything and easily contact with children. Foreign body of mercury type button-disc battery, when lodged in the esophagus, leak a caustic solution which causes a rapid necrosis of esophageal mucosa. So it should be removed immediately when the radiologic diagnosis is made. We experienced a case of esophageal foreign body by mercuric disc battery in a 14-month-old male. This case will be discussed with literature review.

기도 및 식도 이물에 대한 임상적 고찰 (Clinical Analysis of Upper Aerodigestive Tract Foreign Body)

  • Background and Objectives: Foreign bodies of upper aerodigestive tract are common problem for primary care physicians. Delayed diagnosis or failure of removal might cause fatal problemsand complications. Therefore proper diagnosis and management is imperative. In this study, we described clinical features of upper aerodigestive tract foreign body, and analyzed efficacy of different management modality. Materials and Methods: 250 cases of foreign bodies in the esophagus and trachea, between Jan. 1998 through Jan. 2009 has been retrospectively analyzed. A total of 24 cases and 226 cases had been found each as airway foreign bodies and esophageal foreign bodies. The clinical features are described and treatment outcomes, prognosis, and rate of complications of each management modality have been compared. Results: In airway foreign bodies, ventilating bronchoscopy yielded better results, 19 success out of 19 trials than fiberoptic bronchoscopy, 3 success out of 5 trials. Hospitalization days after removal of foreign body didn't show difference between two treatment modalities, although patients who had ventilating bronchoscopy had gone through general anesthesia. And there was no complication after removal of foreign body. In esophageal foreign bodies, rigid esophagoscope yielded better results, 99% of successful removal rate, compared to the EGD, only 78% of successful removal rate. There was no difference of hospitalization days between two modalities. And complication rate was even low in patients who had done rigid esophagoscopic foreign body removal. Conclusion: In upper aerodigestivetract foreign body. Rapid diagnosis and successful foreign body removal is important. Removal by rigid scope(ventilating bronchoscope, rigid esophagoscope) revealed less failure in both airway and esophageal foreign bodies.

갑상선을 침범한 후두의 이물 (A Case of Foreign Body in Larynx Involving Thyroid)

  • Pharyngeal and laryngeal foreign bodies are common problem at ENT clinic. Removal of a foreign body is mostly simple with the manipulation of endoscope and various instruments. Rarely, migration of swallowed sharp foreign body makes complication. We describe a 74-year-old patient diagnosed with foreign body in larynx involving thyroid. In this case, we present this disease and the treatment course which has been successfully treated by surgical removal and conservative treatment. Furthermore, we discuss its symptoms, physical examinations through literature review.

  • The purpose of this study was to introduce the radiography for the natural course and clinical diagnosis of foreign body ingestion and aspiration, to help diagnosis and treatment, to evaluate the accuracy of radiographic images of pediatric patients. A 2 to 7 year-old patient who ingested a foreign body was ingested and aspirated with foreign substances such as coin, cloth pin, earring, baduk stone, and hairpins, and chest and abdomen of the plain radiography. The pediatric patient who ingested and aspirated the foreign body of the coins, the clothespins, the earrings, the stones, and the hairpins were examined by chest and abdomen of the plain radiography and fluoroscopic images. The radiography examination can be combined to effectively cope with the treatment and the treatment of the foreign substance removal. It can be applied to the diagnosis of foreign body in pediatric patient's clinic and appropriate treatment and treatment direction.

치과 치료와 관련된 기도내 이물질 흡인 (Foreign body aspiration during dental procedure)

  • Objective : The aim of this study was to investigate risk factor, precaution and treatment of aspirated foreign body during dental procedure. Material and Methods : Twenty cases of accidental aspiration of the foreign body, which removed by bronchoscopy at the Asan Medical Center between 2008 and 2012, were analyzed retrospectively. Results : Ten cases of accidental aspiration were occurred during dental procedure. Symtoms include cough(65%), dyspnea(50%), sputum(25%) and wheezing(25%). The most common location of foreign body was right bronchial tree(50%), left bronchial tree(45%) and carina(5%). Patients risk factors were chronic obstructive pulmonary disease, lung cancer, pulmonary tuberculosis, esophageal cancer and vegetative state. Conclusion : Accidental aspiration or swallowing of dental instrument or material is not uncommon accidents in dental practice. Most foreign bodies enter into gastrointestinal tract spontaneously. But aspiration into broncho-trachea can be more serious events and must be treated as an emergency situation. Prompt emergency treatment and removal of the foreign body is necessary to avoid complication. Dentists must have knowledge about the precaution and be ready to deal with foreign body aspiration during dental procedures.

Removal of a Bronchial Foreign Body by Bronchoscopic Cryotherapy: A Case Study

  • A foreign body in the airway can be a potentially life-threatening event. The diagnosis and treatment of foreign bodies in the airway are a challenge for otolaryngologists. Despite the improvements in medical care and public awareness, approximately 3,000 deaths occur each year from foreign body aspiration. A high degree of vigilance is required to ensure prompt treatment and avoid the complications of foreign body aspiration. The author encountered a case of a 77-year-old female patient who had aspirated an unknown foreign body that was fixed in her main bronchus. An initial attempt was made to remove it with a flexible bronchoscope but failed due to the patient's hypoxemic state during the procedure. Under general anesthesia, a rigid bronchoscopic examination was performed, but it was difficult to approach the object due to the bronchus curvature. Instead, a cryotherapy instrument of bronchoscopy was applied. The foreign body was frozen and removed to the carina, where a laryngoscope and laryngeal forceps were used to remove it.

경성 비내시경을 이용한 기관이물 및 육아조직 제거 2례 (Removal of Endotracheal Granulation Tissue and Foreign Body via Tracheostoma used by Rigid Nasal Endoscope)

  • Previously, we used a rigid bronchoscope in removal of endotracheal granulation tissue and foreign body. But these method has poor visual field and difficulty in handling of the instruments, therefore there were restriction in removing the endotracheal granulation tissue and foreign body. Recently we underwent one case each of endotracheal granulation tissue and foreign body causing dyspnea and removed them by right angled forceps under visualization via nasal rigid endoscope inserted through the tracheal stoma. We suggest this method for removal of tracheal foreign body, granulation tissue and excision of tumorous condition in patients with tracheocutaneous fistula.

중간 기관지 내 이물의 수술적 치료 (Surgical Management of Occult Foreign Body in the Bronchus Intermedius)

  • Occult bronchial foreign body is that long-standing foreign body lodge in bronchial tree. Occult bronchial foreign bodies arc rare in adults, whereas tracheobronchial aspiration of foreign bodies occurs commonly in children. A 65-year-old man with chronic cough, sputum production, and fever was transferred for treatment of right middle and lower lobc collapse and obstructive pneumonitis as evidenced by imaging studies. The patient was treated with right middle-lower bilobectomy because fiberoptic bronchoscopic removal of the foreign body failed. We report this case with review of literatures.
