Diabetic patients have an increased risk of burn injuries on foot. Because of their diabetic neuropathy, they could contact with hot water or warming device without being aware of it. Split-thickness skin graft (STSG) is successful in treatment of various wound types; however, donor site wounds are sometimes problematic, and complications such as pain and impaired healing often occur. Although, donor site wounds in healthy young individuals can rapidly heal without complications, the wound-healing capacity of elderly patients or those with a comorbidity has been reported to be low. The dermatome is the most commonly used tool because it can harvest a large skin graft in one attempt. However, it is difficult to harvest tissues if the area is not flat. Furthermore, because the harvested skin is usually rectangular, additional skin usually remains after skin grafting. Therefore, use of razor blade and fragmented STSG on a large defect area is advantageous for harvesting a graft with a desired size, shape, and thickness. From January 2018 to July 2018, fragmented STSG was used in 9 patients who suffered from burn induced open wound on foot with diabetic neuropathy. With this approach, healing process was relatively rapid. The mean age of patients was 70 (57~86 years) and all of 9 patients had diabetes mellitus type 2. In all patients, the skin graft on the defect site healed well and did not result in complications such as hematoma or seroma.
Background: It is essential to investigate the functionality of the newly developed knobble shoe. Purpose: The purpose was to explore how the body shape and balance change when wearing knobble shoes. Methods: Nine subjects wearing knobble shoes after walking for a month were analyzed. Results: For knobble shoes, we created various small air pockets in the insole to stimulate the soles of the feet to induce body changes. In particular, it was to promote the arch part of the foot by making a large lump inside and outside the center of the foot's arch. After wearing knobble shoes and using them for 30 days, the average blood sugar decreased by 31.1 mg. The lowering range was 5 to 120 mg. In one patient, the result of the hemoglobin test decreased by 7.5% to 6.5%. In the shoulder tapping test for a body balance test, seven subjects leaned to the right in a barefoot state, and two people to the left. However, after wearing shoes, all of them maintained equilibrium. knobble shoes fit, and balance was all stable. There were two subjects with a short left and four subjects with a short right. Conclusions: Blood sugar decreased after wearing the knobble shoes. The deformed shape of the body was restored to its original state by the knobble supporting the foot arch area. The balance of the sole directly affects the overall posture.

The purpose of this study was to investigate the test-retest of plantar pressures using the F-Scan system over speeds and plantar regions. 6 healthy female subjects in 20's were recruited for the study. Plantar pressure measurements during locomotor activities can provide information concerning foot function, particularly if the timing and magnitude of the loading profile can be related to the location of specific foot structures such as the metatarsal heads. The Tekscan F-Scan system consists of a flexible, 0.18mm thick sole-shape having 1260 pressure sensors, the sensor insole was trimmed to fit the subjects' right. left shoes - sneakers shoes & dress shoes. It was calibrated by the known weight of the test subject standing on one foot. The Tekscan measurements show the insole pressure distribution as a function of the time. This finding has important implications for the development of plantar pressure test protocols where the function of the forefoot is important. According to the result of analysis it is as follows 1) Center of force trajectory in women's dress shoes display direct movement, compare with center of force trajectory in Sneaker shoes displays a little bit curved slow pronation movement. Sneaker shoes in forefoot part display very quick supination movement, therefore, this shoes effects negative effectiveness for ankle's stability Considering center of force trajectory analyzing the more center of force close straight line, the more movement can be quick movement for locomotion. For foot pressure distribution, center of force trajectory in locomotion is better to curved trajectory with pronation movement. So sneaker shoes style is good shoes considering center of pressure distribution trajectory compare with women's dress shoes. 2) Women's dress shoes increased peak pressure in medial, this is effected by high hill's height. The more increased women's dress shoes's height, the more women's peak pressure will increase, pronation can increase compare with before. Supination movement increase, this focused pressure in lateral, also, supination increased more. If the supination movement increased, foot pressure focused in lateral, therefore, it is appeared force distribution in gait direction. This is bad movement in foot's stability. 3) Women's dress shoes in landing phase displayed a long time, this is when women's dress shoes wear, gait movement is unbalance, so, landing phase displayed a long time. For compensation in gait, swing phase quick movement. 4) Women's dress shoes displayed peak pressure distribution in lateral of rearfoot part, Sneakers shoes displayed peak pressure distribution in medial of forefoot part. Its results has good impact absorption compare with women's dress shoes. In forefoot part, sneakers shoes has good propulsive force compare with women's dress shoes.
Objective: The purpose of this study was to determine the knee and ankle joint kinematics and kinetics by comparing downhill walking with valley-shape combined slope walking. Method: Eighteen healthy men participated in this study. A three-dimensional motion capture system equipped with eight infrared cameras and a synchronized force plate, which was embedded in the sloped walkway, was used. Obtained kinematic and kinetic parameters were compared using paired two-tailed Student's t-tests at a significance level of 0.05. Results: The knee flexion angle after the mid-stance phase, the mean peak knee flexion angle in the early swing phase, and the ankle mean peak dorsiflexion angle were greater during downhill walking compared with valley-shape combined slope walking (p < 0.001). Both the mean peak vertical ground reaction force (GRF) in the early stance phase and late stance phase during downhill walking were smaller than those values during valley-shape combined slope walking. (p = 0.007 and p < 0.001, respectively). The mean peak anterior GRF, appearing right after toe-off during downhill walking, was also smaller than that of valley-shape combined slope walking (p = 0.002). The mean peak knee extension moment and ankle plantar flexion moment in late stance phase during downhill walking were significantly smaller than those of valley-shape combined slope walking (p = 0.002 and p = 0.015, respectively). Conclusion: These results suggest that gait strategy was modified during valley-shape combined slope walking when compared with continuous downhill walking in order to gain the propulsion for lifting the body up the incline for foot clearance.

The footwear such as socks and shoes is the clothing which are necessary to our life. In this study, clothing comfort of the footwear was carried out by physiological responses and subjective evaluation. As a result, the effect to walking comfort by the height of a heel in female shoes became clear. Then suitability evaluation system of shoes was proposed from the measuring results. On the other hands, the clothing comfort of the hosiery was evaluated from ECG analysis. By this analysis, it became clear that the factor which influences the clothing comfort of the hosiery was the clothes pressure in the hosiery. In the future, plane shape of the foot and solid shape must be considered in order to design the hosiery.

A 3D body scan measurement (SM) is used as an alternative to physical measurements (PM) as the information for designing industrial products. This study compared the mean difference (MD) between SM and PM by gender and analyzed the causes of the difference. The data used in this study were the scan measurements and physical measurements of adults aged twenty to seventy years old of the fifth Size Korea survey. The results of this study are as follows: 1. The comparison of MD between men and women for all subjects: The measurement of the significant differences between men and women were height, neck base girth, chest girth, under-bust girth, waist girth, armscye girth, back length, and foot length. The causes of difference are the difference of body shape. 2. The comparison of MD between men and women by BMI groups: Many measurements had significant differences between men and women at normal weight and overweight but underweight. Some measurements had significant differences only at a specific BMI group because the body shape difference between men and women is revealed clearly in the group. The comparison of MD between men and women by age groups: The measurements that show significant difference at more than four age groups were neck girth, chest girth, under-bust girth, waist girth, armscye girth, and foot length. The height and abdomen girth had a significant difference in the age range of 20's and 30's. There were measurements that increase MD with an increase in age; under-bust girth and lateral shoulder length for women and lateral shoulder length for men. This comparison of MD between men and women provide the correct guidelines for the use of SM.

The study of bipedal robot is towards similar shape and function with human. In this paper, we propose a human-like walking pattern compatible to the flexible foot with toe and heel structure. The new walking pattern for a bipedal robot consists of ZMP, center of mass (CoM), and ankle trajectory and is drawn by considering human kinesiology. First, the ZMP trajectory moves forward without stopping at a point even in the single support phase. The corresponding CoM trajectory to the ZMP one is derived by solving differential equations. As well, a CoM trajectory for the vertical axis is added by following the idea of human motion. The ankle trajectory closely mimics the rotational motion of human ankles during taking off and landing on the ground. The advantages of the proposed walking pattern are demonstrated by showing improved stability, decreased ankle torque, and the longer step length capability. Specifically, it is interesting to know that the vertical CoM motion is able to compensate for the initial transient response.
The first metatarsophalangeal joint injury is common in professional soldiers and athletes. But this was rarely reported. A professional soldier has varus instability in the first metatarsophalangeal joint due to hyperextension. In the MR Imaging, weavy appearance in lateral collateral ligament and high signal change in plantar plate was shown. So he has surgical treatment using reconstructive procedure. At first, $4^{th}$ extensor digitorum longus tendon was splitted longitudinally and harvested, second triangular shape reconstruction on lateral joint line was done using harvested tendon. One year later, fifteen degrees was limited compared with intact side. Reconstruction using $4^{th}$ extensor digitorum longus tendon in traumatic dynamic hallux varus was good method.
A 5 day-old Warmblood foal was presented with a quarter crack due to the dam accidentally stepping on his left hind foot. Physical examination revealed the vertical crack included the coronary band of the medial side of the left hind hoof (quarter crack) with the lower pastern avulsion wound. Different from adult horses, the foot of the 5 day-old foal lacked adequate hoof wall thickness for stainless steel wire insertion and hoof size for a reinforcing fabric to be applied. Therefore, the authors decided to stabilize the edges of the cracked wall only by the polymethyl methacrylate (PMMA) composites without wiring and a reinforcing fabric application; and gluing a plastic shoe that was reshaped to protect the lower pastern and to spread pressure to other parts of the hoof when bearing weight. Disinfection and a supportive bandage were applied daily for 2 weeks. A month after the hoof crack accident, lameness had diminished. The quarter crack in this foal was treated successfully by the application of the composites three times with a reshaped gluing shoe, with approximately a month interval over the 4 months. After 4 months, it presented a successful functional recovery and needed no further treatment. When followed up 8 months after the accident, it revealed successful outcomes without hoof shape distortion and unbalanced hoof/limb growth.
During bone tumor resection, many cases require medial malleolar osteotomy to achieve adequate access to the operative field. Various osteotomy methods have been developed to address this issue, including oblique, transverse, reverse V-shape, and step-cut osteotomies. However, medial malleolar osteotomy has several drawbacks, such as the excessive disruption of the joint surface, unstable screw fixation when fixing the medial malleolus, and iatrogenic medial ankle joint arthritis due to articular displacement during the reduction of the osteotomy site. In addition, there is a possibility of injury to the posterior tibial artery, tibial nerve, or posterior tibialis tendon if the osteotomy range is too aggressive. Therefore, the authors propose a new osteotomy method, which has shown promising clinical results, namely, partial posterior medial malleolar osteotomy. This method minimizes articular involvement and provides adequate access to the operative field during talar body bone tumor resection.
