요통 환자와 정상인의 자세에 따른 체간 위치 오류 비교 (The Comparison of Trunk Repositioning Errors in Individuals with and without Low Back Pain at Different Postures)

  • Purpose : The purpose of this study was to compare trunk repositioning errors between subjects with and without low back pain in sitting and standing. Methods : Total 81 participants were recruited who consisted of 41 subjects with low back pain and 40 normal subjects. The subjects were instructed to replicate the predetermined target positions of the trunk toward upright and $30^{\circ}$ flexion in sitting and standing. During each of movement, digital inclinometer was used to measure the angular movement of $T_{12}$ spinal process. Repositioning error was calculated as the absolute difference between the predetermined target positions and replicated target positions. Results : In subjects with low back pain, upright repositioning error was $1.26^{\circ}{\pm}0.14^{\circ}$ in sitting and $1.55^{\circ}{\pm}0.24^{\circ}$ in standing, and $30^{\circ}$ flexion repositioning error was $3.23^{\circ}{\pm}0.33^{\circ}$ in sitting and $5.50^{\circ}{\pm}0.50^{\circ}$ in standing. In subjects without low back pain, upright repositioning error was $1.38^{\circ}{\pm}0.15^{\circ}$ in sitting and $1.67^{\circ}{\pm}0.18^{\circ}$ in standing, and flexion repositioning error was $2.61^{\circ}{\pm}0.28^{\circ}$ in sitting and $3.70^{\circ}{\pm}0.52^{\circ}$ in standing. It was demonstrated that flexion repositioning error increased significantly in standing position. In subjects with low back pain, $30^{\circ}$ flexion repositioning error was significantly higher in standing than in sitting. Conclusion : The repositioning error of subjects with low back pain increased during flexion and it implies that some aspects of proprioception are decreased in subjects with low back pain. Therefore, it will be emphasis that a clinical trial to increase the trunk flexion stability of subjects with low back pain in standing.

작업영역의 해석적 생성을 위한 2자유도 동작의 동작범위 측정 (Measurement on range of two degrees of freedom motion for analytic generation of workspace)

  • To generate workspace analytically using the robot kinematics, data on range of human joints motion, especially range of two degrees of freedom motion, are needed. However, these data have not been investigated up to now. Therefore, in this research, we are to investigate an interaction effect of motions with two degrees of freedom occurred simultaneously at the shoulder, virtual hip(L5/S1) and hip joints, respectively, for 47 young male students. When motion with two degrees of freedom occurred at a joint such as shoulder, virtual hip and hip joints, it was found from the results of ANOVA that the action of a degree of freedom motion may either decrease or increase the effective functioning of the other degree of freedom motion. In other words, the shoulder flexion was decreased as the shoulder was adducted or abducted to $60^{\circ}C$TEX>or abducted from $60^{\circ}C$TEX>to maximum degree of abduction, while the shoulder flexion increased as the joint was abducted from $60^{\circ}C$TEX> to $60^{\circ}C$TEX> The flexion was decreased as the virtual hip was bent laterally at the virtual hip joint, and also did as the hip was adducted or abducted from the neutral position. It is expected that workspace can be generated more precisely based the data on the range of two degrees of joint motion measured in this study.

노인의 직립자세역학과 체력과의 관계 (The Relationship between Standing Posture Biomechanics and Physical Fitness in the Elderly)

  • The purpose of this study was to find the relationship between standing posture biomechanics and physical fitness in the elderly. Physical fitness variables and postural variables for 227 (140 women and 87 men) elderly individuals were tested. Physical fitness tests (Korean Institute of Sports Science, 2012) included 3m sit, walk, and return, grip test, 30 second chair sit and stand, sit and reach, figure 8 walks, and 2 minute stationary march. Postural biomechanics variables included resting calcaneal stance position (RCSP), shoulder slope, pelvic slope, knee flexion angle, leg length difference, thoracic angle, and upper body slope. In statistical analysis, multiple regression was conducted by using stepwise selection method via SAS (version 9.2). Analysis for both men and women revealed significant relationships between physical fitness and age, upper body slope, knee flexion angle, leg length difference. Pelvic and thoracic angle were only related to figure 8 walking and sit and reach in women, while RCSP and shoulder slope had no relationship with any physical fitness variables.

PNF 하지패턴이 편마비 환자의 반대편 하지 측부 근육 활성도에 미치는 영향 (The Effect of Proprioceptive Neuromuscular Facilitation Exercise Patterns Targeting the Lower Extremity on Lateral Muscle Activity of the Opposite Lower Extremity in Chronic Hemiplegia Patients)

  • Purpose: The purpose of this study was to investigate the influence of opposite lower extremity lateral muscle activation by proprioceptive neuromuscular facilitation (PNF) exercise targeting the lower extremities. Methods: Nineteen patients with chronic hemiplegia volunteered to participate in this study. PNF flexion, abduction, and internal rotation patterns; initial, end range, and extension patterns; abduction and internal rotation patterns; and initial and end range patterns were applied to the dominant lower extremity. Activation of lateral muscles (multifidus, gluteus medius, tensor fascia lata, and peroneous longus) of the paralyzed leg was then measured by electromyography (EMG). Results: There were significant differences in lateral muscle activation, depending on the PNF pattern applied, with the differences more significant in flexion, abduction, internal rotation, and end range patterns. Conclusion: PNF flexion, abduction, and internal rotation patterns can improve lateral muscle activation of one leg in the standing position in the gait cycle.

턱 당김 운동이 무릎 굽힘폄의 근 수행력에 미치는 영향 (The Effect of Muscle performance on knee flexion / extension by chin in exercise)

  • Background: The purpose of the present study is to report the effect of muscle performance on knee flexion and extension by chin in exercise. Methods: This study was Participated in 10 healthy subjects. For conducting the chin-in exercise, subjects are lay down with supine position. Using the rounded towel, subjects was performed contraction of longus colli and longus capatis muscle by push the towel. Chin-in exercise was conducted 3 times a day, 4 times a week for 4 weeks. By using the Biodex system4, We measured absolute muscle strength, relative muscle strength, total exercise quantity and average rate of production in knee flexion and extension. The data was analyzed by the repeated-measure ANOVA for comparing before, after exercise 2 weeks, after exercise 4 weeks changes of factors. Results: After chin-in exercise, there was significant difference of before, after 2 weeks and 4weeks results in absolute muscle strength, relative muscle strength, total exercise quantity, average rate of production, agonist/antagonist ratio(p<.05). Conclusion: As a results of this study, chin-in exercise may help to improve muscle ability of knee joint activation and knee joint action performance.

Relationship of trunk muscle atrophy and provocation position in patients with chronic low back pain

  • Objective: The purpose of this study is to compare chronic low back pain patients' pain provocation position so as to identify the relevance with lumbar stabilizing muscles atrophy and pain provocation position. Design: Cross-sectional study. Methods: Fifty five chronic low back pain patients were participated in this study. Subjects were eligible for study participation if they were 35-55 years old and had experienced low back pain for more than 3 months. Subjects were questioned about pain and pain provocation test were done. And then they were inspected their cross sectional area (CSA) of lumbar muscles (erector spinae, iliopsoas, and multifidus) by using computed tomography. Analyze the relevance through the result data with painful area, aspect of pain and pain provocation position. Results: CSA of erector spinae showed significant decrease on ipsilateral extension position (p<0.05). Iliopsoas muscle showed significant decrease on contralateral position (p<0.05). Multifidus showed significant decrease on the position of contralateral extension and contralateral flexion (p<0.05). Conclusions: Based on the results of our study, it may be possible to evaluate muscle atrophy by assessing causing position.

엉덩관절 가동범위에 따른 중간볼기근의 근활성도 비교 (Comparison of Gluteus Medius Muscle Electromyographic activity at different hip angles and motions)

  • Background : The purpose of this study was to investigate the electromyographic(EMG) amplitude of the gluteus medius muscle bilaterally at various hip angles and motions(flexion, extension and abduction). Methods : Thirty young with a mean(SD) age of 21.03(1.69) years performed hip motions(flexion, extension and abduction) while the surface EMG activity of the gluteus medius muscles was recorded bilaterally. Subjects were instructed to use the right lower limb during moving 3 different direction. Differences in EMG amplitudes in according with movement direction and angle were assessed by use of paired t-test analyses of variance for the right and left lower extremities. The alpha level was set at .05. Results : Gluteus medius muscle EMG activities in the both side were significantly greater for abduction at angles of 30 degrees than those for other motions at other angles. Conclusion : Hip abduction at angles of 30 degrees on the standing position may be effective in activating the gluteus medius muscle.

Comparison of the Effects of Abdominal Bracing Exercises and Abdominal Hollowing Exercises on Lumbar Flexibility and Pulmonary Function in Healthy Adults

  • Background: Abdominal bracing exercise (ABE) and abdominal hollowing exercise (AHE) improve the lumbar flexibility and pulmonary function in various patients, yet the efficacy of ABE or AHE have not yet been evaluated. Objects: The purpose of this study was to compare the lumbar flexibility and pulmonary function during both ABE and AHE in healthy adults. Methods: The study included 40 healthy adults, who were randomly divided into the experimental group and control group, each with 20 subjects. All subjects performed ABE (experimental group) and AHE (control group). The lumbar flexibility such as trunk flexion test (sitting and standing position) and schober test and pulmonary function such as the spirometer including forced vital capacity (FVC) and force expiratory volume in one second ($FEV_1$) and chest circumference measurement (middle and lower chest) were measured, respectively. Two-way repeated analysis of variance was used to compare the lumbar flexibility and pulmonary function, respectively. Results: No significant effects of lumbar flexibility were observed on trunk flexion test from the sitting position (P=.478) and standing position (P=.096) in the ABE than in the AHE. However, the length of ABE was longer significantly than it of AHE (P=.024). No significant effects of lung function were observed on the FVC (P=.410) and $FEV_1$ (P=.072) in the ABE group than in the AHE group. And also, no significant effects of chest circumference measurement were observed on the inspiration (P=.468) and expiration (P=.563) in middle chest circumference and inspiration (P=.104) and expiration (P=.346) in lower chest circumference. Conclusion: This study indicated that the ABE is only more effective in lumbar flexibility by lumbar length difference than AHE in healthy adults.

디지털경사계를 사용한 체간재위치오류 검사의 신뢰도 분석 (Analysis of Intrarater and Interrater Reliability of Trunk Repositioning Error Test using a Portable Digital Inclinometer)

  • Purpose: A cost effective tool for the clinical measurement of trunk reposition sense is clearly needed. This study was to analyze intrarater and interrater Reliability of trunk repositioning error (TRE) test which assesses trunk position sense using a portable digital inclinometer. Methods: Twenty four normal healthy subjects were recruited. TRE was measured using a portable digital inclinometer. A digital inclinometer (Acumar-ACU360; Lafayetter Instrument) with precision to $1^{\circ}$ was placed on skin over the spinous process from first to second thoracic vertebra (T1-T2) and secured with double-sided tape. TRE test during sitting forward and lateral flexion movement was assessed. When they reached a point approximately 50% of full trunk flexion range, the examiner instructed the subjects to stop and told them. This was the target position that they should try to reproduce exactly. Each subject performed six trials. Results: ICC (2,1) for intrarater reliability (with-day and between-day) of TRE test in sagittal and frontal plane of movement was 0.75 and 0.78 (excellent reliability). Interrater reliability was 0.66 in sagittal and 0.64, frontal plane (fair to good reliability). However, there were poor correlations between an average of TRE test in sagittal and frontal plane. Conclusion: TRE test using a portable digital inclinometer demonstrated good to excellent reliability. The device may be a cost effective clinical measurement for trunk reposition sense measurement.

위치/힘 피드백이 가능한 외골격 구조의 데이터 글로브 개발 (Development of Exoskeleton-Type Data Glove for Position/Force Feedback)

  • 본 연구에서는 사용자의 손에 장착하여 손의 움직임을 측정하고 힘의 반영이 가능한 새로운 형태의 데이터 글로브(data glove)를 제안한다. 본 연구의 데이터 글로브는 인간의 외골격 구조의 분석이 기반하고 있으며 하나의 손가락 모듈은 4절기구의 조합을 통하여 1자유도로 구동이 되도록 고안되어 있다. 데이터 글로브는 펴기(extension)와 구부리기(flexion)를 할 수 있으며 내전(adduction)/외전(abduction)을 위해서 두 개의 유니버설 관절을 이용한 새로운 metacarpal joint 메커니즘을 고안하였다. 동 데이터 글로브의 유효성을 평가하기 위하여 검지손가락을 위한 구동회로와 센서를 포함한 전체 시스템을 제작하였으며 가상공간에 동적 시뮬레이션을 통해서 나타낸 물체를 조작하는 실험을 수행하였다.
