Development of a Phlegm Pattern Questionnaire for Functional Dyspepsia and Analysis of Its Reliability and Validity (기능성소화불량에 대한 담음 변증설문지 개발과 신뢰도 및 타당도 검정)

  • Objectives: The aim of this study was to develop a Phlegm Pattern Questionnaire for Functional Dyspepsia (PPQ-FD) by modifying a previously developed Phlegm Pattern Questionnaire (PPQ) and to verify its reliability and validity. Further objectives were to obtain the optimal cut-off value for the PPQ-FD for standardization and for clinical use. Methods: The PPQ-FD was developed by extracting the major symptoms of the phlegm pattern in functional dyspepsia and by using the Delphi method to administer a requested importance survey to experts. The reliability, validity, and optimal cut-off value of the PPQ-FD were analyzed by enrolling a total of 60 subjects in this study. Thirty patients were diagnosed with both functional dyspepsia and phlegm pattern and thirty patients were diagnosed with only functional dyspepsia. All participants were requested to fill out the PPQ-FD. Results: No statistically significant differences were detected in the two groups for sex distribution, age, or body mass index. Five of the survey questions negatively affected its reliability; therefore, we decided to exclude those five questions on further inspection. The Cronbach's α coefficient of the revised PPQ-FD was 0.853, and clinical validity was verified. Construct validity was analyzed by factor analysis and identified four factors. Statistically significant positive correlations were found between the revised PPQ-FD and other dyspepsia scales, such as the SQDQ, SSQD, VAS, NDI-K, and FD-QoL scales. The VAS had particularly strong positive correlations with the PPQ-FD. Conclusions: The PPQ-FD developed in this study has fundamental reliability and validity for use as a pattern-diagnosis questionnaire. The PPQ-FD can help to diagnose the phlegm pattern in patients with functional dyspepsia.

  • Two experiments were conducted to investigate the effect of feeding Monascus Purpures(MP) on the growth performance of broiler chicks and egg qualify in laying hens. In Expt 1, MP were supplemented with 0. 0.5, 1.0, 2.0, 4.0% in basal diets. Weight gain, feed intake, feed conversion were examined for five weeks and blood cholesterol, protein were measured at the end of experiment. In Expt 2, diets supplemented with 0, 0.5, 1.0, 2.0, 3.0% MP were fed for two weeks. Egg qualify and yolk cholesterol were weekly measured. In Expt 1, Basal diets containing CP 21.5, 19%, ME 3,100, 3,200 kcal/kg were fed ad libitum for starting and finsihing period, repectively. In Expt 2, hens diet contained CP 16%, ME 2,800 kca1/kg and fed for two weeks. In Expt 1, weight gain of birds fed 2.0% MP was maximized at five weeks of age. Feed efficiency was not statistically different, but tended to improve in 1.0, 2.0% MP supplemental groups compared with control. Total serum cholesterol tended to be lower in 2.0% MP supplemental group but was not statistically different. In Expt 2, albumen height and Haugh unit of birds fed 1.0 and 2.0% MP were higher than other treatments, but was not statistically different. Yolk cholesterol of hens fed MP diets showed no significance.

Microbiological Effect of Hand Safety after Hand Washing Education for Preschool Children in a Day Care Center (미취학 어린이의 손씻기 교육에 따른 미생물학적 실태 분석)

  • The objective of this study is to monitor the microbiological effect on preschool children's hand safety according to educational methods of hand washing. The subjects were ten children aged three to five. The analyses of the microbiological effect were made before band washing education, after one-week video demonstrations of hand washing education, and after one-week video demonstrations of hand washing education combined with practice. The results were as follows. Total plate count, filamentous fungi, coliform, and Staphylocuccus aureus were detected from children's hands just before band washing education. According to the two-week hand washing education, such education was found effective in keeping the children's hands clean and safe. In conclusion, repeated and more consistent training of band washing would be important for children, especially aged three to five because most of the habits and behavior patterns were developed in these age groups and the effects were persistent during their future life.

A Study on Information Users' Needs and Information Seeking Behavior of Doctoral Researchers in Digital Age (디지털 환경에서 학술연구자들의 정보요구 및 이용행태에 관한 연구 - 인문사회분야와 과학기술분야의 비교를 중심으로 -)

  • In the digital environment where the Internet and IT change rapidly, researchers have utilized scientific information by relying on more complex and various media than before. This study performed a group interview with thirteen questions derived from five categories in order to understand information needs and information seeking behaviors of Korean doctoral researchers in this information environment. The five categories are (1) identifying information resource usage pattern, (2) attitude towards information resources and reasons for use, (3) IT skills and information literacy, (4) physical library use vs electronic access, and (5) attitudes toward the digital library. In particular, this study focused on analyzing similarities and differences of two groups of researchers selected equally in the humanities and social sciences and in science and technology by doing the interview. The research showed that there are no differences in the goal of using scientific information, information literacy, the ratio of using written materials to electronic resources, and routes of acquiring research ideas or knowledge, but there are differences in materials used for studies or researches and in favorite scientific portal sites.

2008 Dietary Goals and Dietary Guidelines for Korean Adults (2008 한국 성인을 위한 식생활목표와 식생활지침)

  • Recently the Ministry of Health and Welfare, Republic of Korea, announced the “Dietary Guidelines for Korean Adults (DGKA)”, which includes ten Dietary Goals, six Dietary Guidelines, and twenty-three Action Guidelines. DGKA are developed as the revision of the 2003 Dietary Guidelines for Koreans, targeting adult population. Dietary Guidelines are developed for general purpose as well as for different age groups. They are revised periodically to accommodate changes in diet and health problems of the population. The process of developing new DGKA can be summarized as 1) selection of focus areas, 2) analysis and review of available data for each area selected, and 3) derivation of guidelines based on the analyzed data, and 4) finalizing the guidelines after open discussions among the experts and general public. Five focus areas were selected by examining the Nutrition Goals of the Health Plan 2010 of Korea, soliciting proposals from the experts in the related fields, and reviewing existing and international guidelines. Five areas selected were 1) adequate intake of nutrients and foods, 2) balance of energy intake and physical activities, 3) alcohol intake, 4) food security and nutrition service, and 5) food safety. Adequacy of nutrient and food intakes of the Korean adult population was assessed using 2005 Korea National Health and Nutrition Examination Survey (KNHANES) data. Newly developed Dietary Reference Intakes for Koreans were used as reference values to assess the prevalence of inadequacies and excesses in nutrient intakes. Energy balance was examined with energy intake of 2005 KNHANES survey and results of physical activity questionnaire in the survey. Alcohol intake was also examined using 2005 KNHANES results of dietary intakes as well as the results of questionnaire survey on alcohol intakes. Food security, nutrition services, and food safety were analyzed using various government data and published results on the issues. Ten Dietary Goals and six Dietary Guidelines were developed after data analysis and were subjected to reviews of experts and general public. The final DGKA are: 1) Eat a variety of foods from each food group, 2) Increase physical activity and maintain healthy weight, 3) Eat proper amount of clean foods, 4) Avoid salty foods and try to eat foods with bland taste, 5) Avoid foods with high fat contents and deep-fried foods, and 6) When you drink alcohol, limit the amount. Twenty-three action guidelines are developed in order to achieve these guidelines in actual diet and life among the population. The government is disseminating the guidelines with “337” slogan and emblem. “337” indicates everyone should practice “3” guidelines of promoting good eating practice, “3” guidelines to limit or decrease in your diet, and you should practice them for “7” days a week. The guidelines will be useful in promoting healthy food habits and good nutritional status which will result in decrease nutrition related health problems in Korea.

Effects of Death Preparation Education on Awareness of Hospice Palliative Care and Withdrawing Life Sustaining Treatment in City Dwellers (죽음준비교육이 일반인의 연명치료중단 및 호스피스·완화의료에 대한 인식에 미치는 효과)

  • Purpose: This study was done to analyze how a death preparation education program have the effects on awareness of hospice palliative care and withdrawing life sustaining treatment in older adults. Methods: This study employed a non-equivalent control group design among quasi-experimental designs. The experimental group was comprised with 35 adults and the control group with 40 adults. The death preparation program consisted of five two-hour sessions and was administered once a week for five straight weeks. Data were analyzed by descriptive statistics, t-test, ${\chi}^2$ test, Fisher's exact test, and ANCOVA using SPSS version 18.0. Results: The mean age of the participants was 66.2 years. A significant difference between the experimental and control groups was observed in withdrawing life sustaining treatment (F=3.380, P=0.040). However, no significant difference was found in awareness of hospice palliative care (F=0.163, P=0.850). Conclusion: The study results indicate that death preparation training could positively affect people's awareness of withdrawing life sustaining treatment. More studies should be conducted to explore effects of death preparation education for all ages and help people better understand hospice palliative care.

A Comparative Study on the Degree of Alcoholism Recovery, Quality of Life, Empowerment and Self-esteem Based on Different Abstinence Periods of Alcohol use Disorder (알코올 사용장애자의 단주기간에 따른 알코올의존 회복, 삶의 질, 임파워먼트, 자아존중감 비교 연구)

  • This study investigated the differences in sociodemographic variables, alcoholism recovery, quality of life, empowerment and self-esteem based on different abstinence periods of alcohol use disorder. The study population from seven different regions in nationwide were 207 individuals with alcohol use disorder who had been abstinent. According to previous studies, 207 individuals were classified into different abstinence periods: less than one year, more than one to five years, and more than five years. As a result, the sociodemographic variables affecting differences in groups were gender, age, marital status, and economic activity. In addition, the group that maintained abstinence from alcohol for more than one year showed significant differences in alcoholism recovery, quality of life, empowerment, and self-esteem than those that maintained abstinence from alcohol for less than one year. Regardless of abstinence periods, alcoholism recovery, quality of life, empowerment, and self-esteem showed significant positive correlations. Therefore, the results of this study identified the factors influencing long-term abstinence and effects and suggested the need for alcohol-dependent recovery programs or therapeutic interventions for individuals who abstained from alcohol use.

A Validation Study of the Korean Version of the Workplace Intergenerational Climate Scale(K-WICS) (한국판 세대친화적 조직문화척도(K-WICS) 타당화 연구)

  • Due to recent demographic changes, employees from diverse generations now work together in organizations. Thus, there is a need for research on intergenerational cooperation. However, the lack of valid and reliable measures to capture intergenerational climate in the workplace is an obstacle to research. Therefore, we translated the Workplace Intergenerational Climate Scale(WICS) into Korean and validated it with a sample of 1,052 Korean full-time employees. Firstly, we conducted an exploratory factor analysis by using sample 1(N = 460) and revealed a five-factor solution. Secondly, the confirmatory factor analysis(sample 2; N = 592) showed a good model fit of the correlated five-factor model. Thirdly, the scale's discriminant and convergent validity was supported by negative correlations with four types of existing ageism scales and by positive correlations with trust, organizational commitment, work engagement, psychological safety, intention to remain, job satisfaction, and communication satisfaction. Moreover, it further demonstrated significant incremental validity in predicting positive outcome variables even when controlling for pre-existing agism scales. Lastly, we confirmed strict measurement invariance of the scale between the age groups(below 40 versus above 40). The findings support the reliability and validity of the Korean version of WICS among Korean employees. The scale will be broadly applied to measure intergenerational climate of organizations and provide practical implications for HR management.

Factors for Survival and Complications of Malignant Bone Tumor Patients with a Total Femoral Replacement (대퇴골 전치환술 받은 악성 골종양 환자의 생존인자와 합병증)

  • Purpose: Total femoral replacement (TFR) is an extreme form of limb salvage. Considering the rarity of this procedure, reports have focused on the complications and a proper indication is unclear. This study analyzed 36 patients with TFR who were asked the following: 1) prognostic factors related to survival in patients who underwent TFR with a tumoral cause; 2) overall implant and limb survival; 3) complications, functional outcome, and limb status for patients surviving for more than 3 years. Materials and Methods: According to the causes for TFR, 36 patients were categorized into three groups: extensive primary tumoral involvement (group 1, 15 cases), tumoral contamination by an inadvertent procedure or local recurrence (group 2, 16 cases), and salvage of a failed reconstruction (group 3, 5 cases). The factors that may affect the survival of patients included age, sex, cause of TFR, and tumor volume change after chemotherapy. Results: The overall five-year survival of the 36 patients was 31.5%±16.2%. The five-year survival of 31 patients with tumoral causes was 21.1%±15.6%. The five-year survival of 50.0%±31.0% in patients with a decreased tumor volume after chemotherapy was higher than that of increased tumor volume (p=0.02). The five-year survival of 12 cases with a wide margin was 41.7%±27.9%, whereas that of the marginal margin was 0.0%±0.0% (p=0.03). The ten-year overall implant survival of 36 patients was 85.9%±14.1%. The five-year revision-free survival was 16.6%±18.2%. At the final follow-up, 12 maintained tumor prosthesis, three underwent amputation (rotationplasty, 2; above knee amputation, 1), and the remaining one had knee fusion. Among 16 patients with a follow-up of more than three years, 14 patients underwent surgical intervention and two patients had conservative management. Complications included infection in 10 cases, local recurrences in two cases, and one case each of hip dislocation, bushing fracture, and femoral artery occlusion. Conclusion: Patients showing an increased tumor volume after chemotherapy and having an inadequate surgical margin showed a high chance of early death. In the long-term follow-up, TFR showed a high infection rate and the functional outcome was unsatisfactory. Nevertheless, this procedure is an inevitable option of limb preservation in selected patients.

Posttransplant Lymphoproliferative Disorder in Pediatric Liver Transplantation: Samsung Medical Center Experience (소아 간이식에서 Posttansplant Lymphoproliferative Disorder (PTLD): 삼성서울병원의 경험)

  • Purpose: In a retrospective study for the pediatric patients who underwent liver transplantation in the past 6 years at Samsung Medical Center, the clinical features of 5 patients with posttransplant lymphoproliferative disorder (PTLD) were analyzed. Methods: Between June 1996 and June 2002, 41 pediatric patients underwent liver transplantation. Seven of them died in the postoperative period. Thirty-five including one patient who died of PTLD were finally reviewed. Patients were divided into two groups: high risk group, EBV naive recipients of EBV-positive grafts; low risk group, the patients other than those in high risk group. The authors reviewed age at operation, immunosuppressive agent, postoperative duration until diagnosis, postoperative duration until EBV seroconversion, presence of treatment against rejection, and presenting symptoms of PTLD. Results: Five of 41 patients (12.2%) developed PTLD. All of them belonged to high risk group, and the incidence of PTLD in high risk group was 31.3% (5/16). The mean age at operation was 10.8 months old and the mean duration between operation and diagnosis for PTLD was 9.8 months. Primary EBV infection developed after a median of 6 months after transplantation. One patient was diagnosed as laryngeal and gastrointestinal PTLD and the other four, gastrointestinal PTLD. The following symptoms and signs were seen in the patients: anemia (100%), hypoalbuminemia (100%), fever (80%), diarrhea (80%), gastrointestinal bleeding (80%), and anorexia (60%). Conclusion: PTLD is one of the major complications after pediatric liver transplantation, especially in the group of high-risk recipients. Anemia, hypoalbuminemia, fever, diarrhea and gastrointestinal bleeding were features that are characteristic of PTLD. The common features of PTLD development were: (i) EBV-positive donors placed into EBV naive recipients, (ii) primary EBV infection about 6 months after transplantation, (iii) young age, about 1 year old at operation, and (iv) the requirement for intensive posttransplant immunosuppression.
