A Study on the Perception of Pit and Fissure Sealant using Unstructured Big Data (비정형 빅데이터를 이용한 치면열구전색(치아홈메우기)에 대한 인식분석)

  • Background: This study aimed to explore the overall perception of pit and fissure sealants and suggest methods to revitalize their current stagnation. Methods: To determine the social perception of the change in coverage policy for pit and fissure sealants, we categorized them into five time periods. The first period (December 1, 2009 to November 30, 2010), the second period (December 1, 2010 to September 30, 2012), the third period (October 1, 2012 to May 5, 2013), the fourth period (May 6, 2013 to September 30, 2017), and the fifth period (October 1, 2017 to December 31, 2022). We utilized text mining, an unstructured big data analysis method. Keywords were collected and analyzed using Textom, and the frequency analysis of the top 30 keywords, structural features of the semantic network, centrality analysis, QAP correlation analysis, and co-occurrence analysis were conducted. Results: The frequency analysis showed that the top keywords for each time period were 'Cavities', 'Treatment', and 'Children'. In the structural features of the semantic network of pit and fissure sealants by time period, the density index was found to be around 1.00 for all time periods. The QAP correlation analysis showed the highest correlation between the first and second periods and the fourth and fifth periods with a correlation coefficient of 0.834. The co-occurrence analysis showed that 'cavities' and 'prevention were the top two words across all time periods. Conclusion: This study showed that pit and fissure sealants are well accepted by the society as a preventive treatment for caries. However, the awareness of health education related to these sealants was found to be low. Efforts to revitalize stagnant pit and fissure sealants need to be strengthened with effective education.

Seismic responses of a metro tunnel in a ground fissure site

  • Shake table tests were conducted on scaled tunnel model to investigate the mechanism and effect of seismic loadings on horseshoe scaled tunnel model in ground fissure site. Key technical details of the experimental test were set up, including similarity relations, boundary conditions, sensor layout, modelling methods were presented. Synthetic waves and El Centro waves were adopted as the input earthquake waves. Results measured from hanging wall and foot wall were compared and analyzed. It is found that the seismic loadings increased the subsidence of hanging wall and lead to the appearance and propagation of cracks. The values of acceleration, earth pressure and strain were greater in the hanging wall than those in the foot wall. The tunnel exhibited the greatest earth pressure on right and left arches, however, the earth pressure on the crown of arch is the second largest and the inverted arch has the least earth pressure in the same tunnel section. Therefore, the effect of the hanging wall on the seismic performance of metro tunnel in earth fissure ground should be considered in the seismic design.

Study of the cumulative dental caries rate of pit and fissure sealant in community public health program: 3 years follow-up (3년간 추적연구에 의한 지역사회 치면열구전색사업의 누적치아우식발생률에 관한 연구)

  • Objectives: The purpose of the study was to investigate the cumulative dental caries rate of pit and fissure sealant in community public health program from 2005 to 2008 follow-up. Methods: The subjects were 4,768 students of 34 elementary schools in Gimcheon. The survey included direct oral examination and cumulative retention rate of pit and fissure sealant from 2005 to 2008. Data were analyzed using PASW statistical package version 18.0 and the level of significance was set at p<0.05. Results: The cumulative retention rate of sealant was 18%, 25%, and 33% after 1 year, 2 years, and 3 years, respectively. In addition, the cumulative retention rate of lower teeth was higher than that of upper teeth. Cumulative dental caries rate increased in sealed group and no sealed group. The cumulative dental caries rate decreased owing to tooth brushing frequency in sealed group and no sealed group. Conclusions: Regular and timely tooth brushing can maintain good oral health condition after tooth sealing. The school based oral health education can prevent loss of sealant and secondary caries.

A COMPARATIVE STUDY OF MICROHARDNESS OF DUAL-CURE AND LIGHT-CURE PIT AND FISSURE SEALANT (이중중합 치면열구 전색제와 광중합 치면열구 전색제의 미세경도에 관한 비교연구)

  • Due to the various reason, sealing of pit & fissure might be imperfect. One of these reason can be the fracture of sealant material because of the low hardness value of sealing material. The purpose of this in vitro study was to evaluate the microhardness of two different curing type pit and fissure sealants: Dual-cure and Light-cure. The result from the present study can be summarized as follows: 1. All pit and fissure sealants that used in this study showed statistically significant difference in their microhardness of upper and lower surface. (P <0.05) 2. Except of lower surface of teethmate, microhardness of 40-second curing sealant was statistically higher than that of 20-second curing sealant. (P <0.05) 3. In comparison of sealants, microhardness of dual-cure sealant was statistically higher than that of light-cure sealant. Above results suggest that the use of dual-cure sealant and longer curing time are recommended.

The Analysis of Normal Values on the Ocular Protrusion and Height of Palpebral Fissure on Women University Students in Korean (한국 여자 대학생들의 검열 폭과 안구돌출의 정상 값 분석)

  • The purpose of this study was undertaken to determine the analysis of normal values of height of palpebral fissure and Hertel exophthalmometry in Korean woman university students. These measurements were taken from 118 health subjects aged 18-21 years in Korea. In 18-21 years, the mean values of height of palpebral fissure were $6.967{\pm}1.599mm$, $7.172{\pm}1.502mm$, $8.015{\pm}1.44mm$ and $7.1{\pm}1.224mm$, respectively. However, In 18-21 years, the mean values of protrusion were $12.50{\pm}2.33mm$, $12.343{\pm}2.634mm$, $12.773{\pm}1.974mm$ and $13.036{\pm}2.23mm$, respectively. As a results, according to increase the aging, the palpebral fissure height and ocular protrusion was more increase. But in 21 ages the palpebral fissure was some decrease. Also in 19 ages the protrusion was some decrease. On the other hand, the right and left eye have not statistically significant.

An Effect of Incremental Dental Health Care Program in School Dental Clinic (학교구강보건실 계속구강건강관리사업의 효과)

  • In 2001, as a subject of this study, the first grade 165 kids in Yomju elementary school had been guided in pit and fissure sealant, fluoride rinse, group tooth brushing, and Dental health education as a part of oral disease preventive program. From the data, this author has estimated incremental dental health care program in school dental clinic in order to make it more effective and enlarge it. For that purpose, the program has been continued at six month intervals for two years. The retention condition of pit and fissure sealant in first molar and DMF rate had been investigated. The conclusions are as follows: 1. Full and partial retention of pit and fissure sealant was measured as 80.69% in maxillary right first molar, 83.20% in maxillary left first molar, and 86.72% in mandibular right first molar, and 86.28% in mandibular left first molar. 2. Retention of pit and fissure sealant in first molar was measured as 76.55% in maxillary right first molar, 81.03% in maxillary left first molar, 80.65% in mandibular left first molar, and 82.03% in mandibular right first molar. 3. Among Yomju elementary school students, DMF rate was measured as 8.7%, and DMFT index as 1.03. However, in Yangdong elementary school students the former was measured as 13.8% and the latter as 1.76. When the DMF rate difference between Yomju and Yangdong elementary school kids was considered, the oral health condition of the former was much better than that of the latter because the former had received incremental dental health care program for two years and on the other hand, the latter had not. So it is necessary that we should enlarge school dental clinic, improve and keep students' oral health.

Enamel pretreatment with Er:YAG laser: effects on the microleakage of fissure sealant in fluorosed teeth

  • Objectives: The purpose of this in vitro study was to evaluate the microleakage and penetration of fissure sealant in permanent molar teeth with fluorosis after pretreatment of the occlusal surface. Materials and Methods: A total of 120 third molars with mild dental fluorosis were randomly divided into 6 groups (n = 20). The tooth surfaces were sealed with an unfilled resin fissure sealant (FS) material. The experimental groups included: 1) phosphoric acid etching (AE) + FS (control); 2) AE + One-Step Plus (OS, Bisco) + FS; 3) bur + AE + FS; 4) bur + AE + OS + FS; 5) Er:YAG laser + AE + FS; and 6) Er:YAG laser + AE + OS + FS. After thermocycling, the teeth were immersed in 0.5% fuchsin and sectioned. Proportions of mircoleakage (PM) and unfilled area (PUA) were measured by digital microscope. Results: Overall, there were significant differences among all groups in the PM (p = 0.00). Group 3 showed the greatest PM, and was significantly different from groups 2 to 6 (p < 0.05). Group 6 showed the lowest PM. Pretreatment with Er:YAG with or without adhesive led to less PM than bur pretreatment. There were no significant differences among groups in PUA. Conclusions: Conventional acid etching provided a similar degree of occlusal seal in teeth with fluorosis compared to those pretreated with a bur or Er:YAG laser. Pretreatment of pits and fissures with Er:YAG in teeth with fluorosis may be an alternative method before fissure sealant application.

Changes in the amount of adhesion of Streptococcus mutans to pit and fissure sealant incorporating cerium oxide nano particles(CNPs) (세륨옥사이드나노입자(Cerium oxide nano particles: CNPs)를 함유한 치면열구전색재의 Streptococcus mutans 부착량 변화)

  • Objectives: The aim of this study was to investigated the surface roughness and change in the amount of adhesion of Streptococcus mutans to the commercial pit and fissure sealant containing cerium oxide nano particles(CNPs). Methods: The CNPs was incorporated into a commercial pit and fissure sealant at 0-4.0 wt%. Disk Specimens (ϕ 10 mm × 2 mm) were prepared by light polymerization the front and back for 40s. Average surface roughness was measured and Streptococcus mutans adhesion was observed under confocal laser scanning microscopy (CLSM) after 24 hour. Data were statistically analyzed by one-way ANOVA and Tukey HSDa post-hoc test. Results: Difference of the surface roughness(Ra) between groups was not statistically significant in both non CNPs group and CNPs group(p>0.05). In CNPs group, the amount of S. mutans adhesion was significantly different between control group and decreased in order of CNPs 4.0, CNPs 0.5, CNPs 1.0 and CNPs 2.0(p<0.05). Conclusions: Within the limitation of this study, these aspects of oral bacteria performances suggest potential usefulness of the CNPs incorporation, especially CNPs 1% and 2%, in pit and fissure sealant for inducing antibacterial effect.

THE FISSURE PENETRATION AND MICROLEAKAGE OF PIT AND FISSURE SEALANT WITH MECHANICAL PREPARATION (기계적 삭제방법을 이용한 치면열구전색제의 열구 침투도 및 미세누출)

  • Mechanical preparation has been introduced to provide the sealant retention. The objective of this study was to compare the fissure penetration and the microleakage of pit and fissure sealant using mechanical preparation(mechanical preparation + acid etching) and acid etching only. An additional objective of this study was to compare the fissure penetration and the microleakage of unfilled and filled sealant in both methods. Sixty human premolars extracted for orthodontic purpose were selected. Thirty teeth were acid etched alone and remaining thirty teeth were prepared with a $\frac{1}{4}$ round bur and then acid etched. One-half of teeth in each surface treatment method were sealed with unfilled sealant and the other half were sealed with filled sealant. All of the teeth were thermocycled for 1200 cycles at $5^{\circ}C\;and\;55^{\circ}C$ and immersed in 5% methylene blue for 24 hours. Each tooth was sectioned bucco-lingually at mesial pit and distal pit and examined under a Measurescope. In the case of mechanical preparation, fissure penetration of sealant was significantly increased compared with the case of acid etching only(P < 0.05). The filled and unfilled sealant using mechanical preparation showed significantly decreased microleakage when compared with the unfilled sealant using acid etching only(P < 0.05). No differences were found in fissure penetration and microleakage between unfilled and filled sealant in both methods. Taken together, the results of this study suggest that mechanical preparation and filled sealant are recommended when placing pit and fissure sealant. However, further clinical studies should be performed in regard to microleakage.

수소화물의 파괴가 압력관 재료들의 파괴인성에 미치는 영향

  • 본 연구에서는 수소화물의 파괴가 일어나는 조건과 수소화물의 파괴가 압력관 재료의 파괴인성에 미치는 영향에 대하여 알아보았다. 사용된 재료들은 상용 캐나다 압력관 재료와 러시아에서 개발된 E635, El25라는 재료로서 균열진전방향이 압력관의 축방향을 향하도록 CT 시편을 제작하여 파괴시험을 하였다 상온에서 파괴인성은 캐나다 압력관재료, El25, E635의 순서로 크게 나왔다. 100ppm의 수소를 장입한 경우 상온에서의 파괴인성은 크게 감소하여 세 재료들 모두 비슷한 평평한 J-R 커브를 보였으며, 캐나다 재료와 E63S의 파면에서 관찰되는 fissure의 간격은 150~200$\mu\textrm{m}$로 수소화물의 간격과 비슷하였으며 tunnelling 현상이 일어났다. El2S는 불규칙한 짧은 fissure들이 관찰되었다. 20$0^{\circ}C$에서는 수소화물의 파괴에 의한 fissure들이 관찰되지 않았으며 수소화물이 파괴인성에 영향을 미치지 않는 것으로 나타났다. El25는 상온과 20$0^{\circ}C$에서의 J-R커브가 비슷하게 나타났다.
