A Study on the Improvement of Response System through the Case of Heavy Rain Disaster Response (폭우재난 대응 사례를 통한 대응체계 개선방안 연구)

  • Purpose: The Ministry of Employment and Labor has been working hard to ensure the safety of workers due to heavy rain during natural disasters as the responsible ministry in charge of preventing industrial accidents and health problems for workers. Accordingly, the Ministry of Employment and Labor intends to analyze actual cases of responding to heavy rain disasters and suggest ways to improve the response system. Method: An emergency response system implemented to respond to heavy rain disasters with an internal expert group composed of those in charge of disaster work at headquarters, local government offices, and Korea Occupational Safety and Health Agency, and an external expert group composed of professors, consulting representatives, and disaster managers from other ministries. Contents on self-inspection by industry, workplace inspection, use of serious siren, safety management and restoration work guidance were reviewed. Result: First of all, it is necessary to check the regular contact system from time to time, and it is also necessary to prepare and distribute detailed self-checklists for each industry. In addition, it is necessary to check the implementation of self-inspection when inspecting workplaces, and it seems necessary to have measures to increase the readability of information notified through serious disaster sirens. In addition, since safety work is done in the form of a contract, it seems necessary to prepare specific safety guidelines. Conclusion: In order to protect the lives of workers due to seasonal harm and risk factors, unlike the passive coping methods of the past, abnormal weather should not be regarded as an unexpected situation, and it should be actively and preemptively responding beyond the conventional framework.

  • Posttraumatic stress disorder (PTSD) is well known to have a limited response to drug treatment. Many recently published clinical care guidelines recommend trauma-focused psychotherapies such as cognitive processing therapy (CPT) and prolonged exposure therapy (PE) as first-line treatment and medication such as serotonin reuptake inhibitors and venlafaxine as second-line treatment. Current review introduces the session composition and contents of CPT and presents various CPT studies that show therapeutic effect for civilian and veterans/military with PTSD. In order for clinicians to help effectively patients with PTSD, it is necessary to learn and actively use evidence-based trauma-focused psychotherapies including CPT and PE.

Current Status of Clinical Practice for Gout in Korean Medicine - On-line (Web-Based) Survey (통풍의 한의임상진료현황 - 온라인(웹기반) 설문조사)

  • Objectives : The purpose of this study was to investigate the current status of clinical practice for gout in Korean Medicine (KM). Methods : On-line survey was adopted for this study, targeted at KM Doctors who were registered in the Association of Korean Medicine. The questionnaire included the general status of treatment, pattern identification and treatment method according to three stages given: acute, chronic and asymptomatic hyperuricemia stage. Results : Data from a total of 384 respondents was analyzed. Participants who responded most were in their 40s, working in KM clinics located in Seoul. Acupuncture and herbal medicines were the top priority treatments in preparing recommendations in clinical practice guidelines for gout, and where clinical trials were considered essential. As of the past year, the average number of first visits per month was 86.7%, and the average treatment duration was less than 1 month, accounting for 72.9%. As for the diagnostic method used for gout patients, the "diagnosed by clinical pattern" response was the highest, and the "blood and urine test" response was the highest as the diagnostic equipment used. As for the evaluation scale being used, the "VAS/NRS" response was the highest. Regardless of the gout stage, acupuncture was the most commonly used treatment method for gout patients, and bee venom pharmacopuncture was the highest for the pharmacopuncture used. In the case of herbal medicine, it was found that a wider variety of herbal medication uninsured in health insurance and herbal prescription were used for the staged treatment of gout than herbal medication in national health insurance coverage. In clinical practice, "acupuncture treatment three times a week" was the most common, and the "acupoints in the limb" were the most frequently used; LR3 and SP3 were the most frequently used acupoints. Conclusions : It is suggested that this survey should be helpful to develop clinical practice guideline for gout that reflects actual clinical practice.

Review of Assessment Criteria for Sustainable Outdoor Space Responding to Climate Change (기후위기 대응 외부공간의 지속가능성 평가지표의 검토 및 고찰)

  • This study examines the assessment systems of LEED v4.1, an eco-friendly building evaluation system in the United States, and SITE v2, an assessment system for sustainable outdoor spaces, and then compares and examines them from the perspective ofrelevant laws and institutional guidelines and standards in Korea. The conclusion is as follows. First, the US eco-friendly certification system basically not only establishes an independent evaluation system according to the field of expertise, but also provides a sustainable city and community through response to the climate crisis and the comfort of the external space environment. It can be evaluated that securing the quality of life of healthy and happy city dwellers is the top priority. Second, Korea's Green Building Certification System (G-LEED) was basically based on the American LEED system, but it was judged that there was a fundamental difference. It was judged that there is a limitation in not being able to achieve an integrated approach through the participation of various expert groups and stakeholders, but also in the accumulation of more scientific and reliable data and information through the application of cutting-edge information and communication equipment. Third, in the case of external space in Korea, a sustainable assessment system has not been established, and not only is it dispersed in various legal and institutional guidelines, but also its effectiveness is judged to be very low. Therefore, it is judged that it is urgent to introduce and secure the applicability of SITES v2, a sustainable outdoor space assessment system in the United States. It was judged that the effectiveness should be secured through the upward adjustment of the minimum Ecological Area Ratio.

A Study Comparing Public and Medical Librarians' Perceptions of Evaluation Guidelines for Health & Medical Information (건강정보원 평가기준에 대한 공공도서관 및 의학도서관 사서간 인식비교 연구)

  • Providing reliable and high quality information sources will be one of the basic skills of librarians in the future. Therefore, this study proposed evaluation criteria for health-related information sources based on a survey of public and medical librarians. As a result, a total of 21 items were selected as evaluation items, in three groups. The first, the health information content group, had 13 evaluation items, including accuracy, recency, medical expertise, regular updates, consideration of audience, objectivity, ease of understanding, plain (non-scientific or technical) language, completeness, relevance to the topic, verifiability, citation of information sources, and specification of precautions or warnings. The second group, the health-information sources group, had 5 evaluation items including clarity of health information for achieving its purpose, clarification of the responsibility of health information, compliance to the privacy policy, fairness of health information providers, and ethics of health information providers. The third group was the health-information website design group, and featured 4 evaluation criteria: ease of access, search capabilities, website ease of use, and query-response services.

  • In this paper, we review the regulations relating to railway safety and station workers interviewed, eight urban rail accidents decreased operating agencies through the prevention research currently being conducted into the (Seoul Metropolitan Rapid Transit Corporation City, Seoul Metro, Incheon traffic mortality, etc.) the root cause for the lack of aggressive prevention measures performed on the difficulties related regulations, organizational culture and work environment, prevention of negative operating authority derived in three improvements were obtained through this direction. First proposed urban railway safety ever buy Conduct production, utilization and operating agencies college safety assistant suggests directions for safety training, organizational culture and work environment improvements in senior management. Finally, active accident prevention campaigns, and provide direction to increase the educational participation of guests. Improvement for such accidents would decrease is thought to play a role in the prevention of future operating agency established when the guidelines.

Development and Evaluation of Multilingual Education Materials to Promote Immigrant Women's Adaptation to Pregnancy (이주여성의 임신적응증진을 위한 다국어 교육자료 개발과 평가)

  • Purpose: This study was done to develop and evaluate multilingual education materials to promote health and adaptation to pregnancy for immigrant women in Korea. Methods: This study had three procedures: First, contents of the education materials were developed according to pregnant women's needs, literature reviews, and group discussion. Details in contents were constructed based on Roy's adaptation model; Second, validity verification and translation of education materials were accomplished; Third, evaluation of the education materials was done through a survey of immigrant women. Results: The education materials were developed in six languages (Korean, English, Chinese, Vietnamese, Filipino, and Cambodian). The title is "Healthy mother, happy family: eight-step guide to a healthy pregnancy". It was composed of an eight-step guide to healthy pregnancy, self-examination check list, websites for childbirth education and information, and guidelines on education materials in a brochure. In the evaluation, the average response score for the questionnaire items was high (3.23 on a four-point Likert scale). The average score for Cambodian immigrants was the highest of all immigrant women. Conclusion: Multilingual education materials developed in this research will help community health nurses to manage pregnancies for immigrant women and will be useful for health education for these women.

A Comparative Study on Trends and Efficacy of Erythropoietin Stimulating Agents in Patient Receiving Peritoneal Dialysis (복막투석 환자의 빈혈 관리에 있어 에리스로포이에틴 자극제의 사용현황 및 비교평가)

  • Objectives: This was to evaluate the current usage of three erythropoietin stimulating agents (ESA) and their efficacy for management of anemia in peritoneal dialysis (PD) patients with chronic kidney disease. Methods: It was a retrospective comparative study through review of electronic medical records of chronic kidney disease patients undergoing PD at a tertiary teaching hospital from January 1998 to June 2013. Results: Average administration frequency was 1.66 times/week in EPO group, 0.75 times/week in DA group, and 0.19 times/week in MPG-EPO group. At the first 4 weeks, there were significant differences in mean hemoglobin levels between EPO and DA groups ($9.25{\pm}1.28g/dL$, $10.02{\pm}0.95g/dL$ each, p = 0.018) and also in hemoglobin response rates (10.0%, 45.2% each, p = 0.008), but since after 4 week, there had been no significant differences. There also showed no significant differences in achievement of hemoglobin target between the two groups. When converted to MPG-EPO in EPO/DA groups, there showed a slight increase in hemoglobin levels of both groups. MPG-EPO was the highest compared with two other drugs by the average cost based on the average weekly dose. Conclusion: EPO, DA, and MPG-EPO showed similar effects in treatment of anemia of PD patients based on hemoglobin target range (11.0~12.0 g/dL) which NFK-K/DOQI guidelines suggest. Though the average cost of MPG-EPO was higher than the other two drugs, the number of PD patients using MPG-EPO has increased and it is thought that long half-life and low administration frequency of MPG-EPO have improved the compliance of PD patients who have to self-administrate.

  • In order to protect the lives and property of citizens, the central and local governments are responding by enacting municipal ordinances and regulations as the frequency of extreme weather conditions due to climate change increases and intensity increases gradually. Accordingly, the basic contents and strategies of domestic and foreign policies to cope with cold and heat waves were reviewed, referring to measures suitable for application to the Daegu metropolitan area. In addition, it is intended to provide a policy alternative to Daegu metropolitan area to minimize damage from extreme weather by identifying the current status, characteristics, and future prospects of extreme weather in Daegu metropolitan area. Since the damage caused by the cold wave in Daegu area is not as great as that of other regions, it is urgent to come up with cold wave measures for the health and transportation sectors, and to come up with measures against the heat wave as the damage caused by the heat wave is the most serious in the country. Also we will identify spatial characteristics so that the districts and counties with high vulnerability to extreme weather can be identified and implemented first, and present civic life-oriented facilities and civic action guidelines to overcome cold and heat waves.

Biosafety of Microbiological Laboratories in Korea (우리나라 미생물 실험실의 생물안전현황)

  • Objectives : The biosafety level (BSL) practiced in microbiology laboratories in Korea according to the laboratory biosafety manual published by the World Health Organization (WHO) was evaluated using the data obtained by a survey. Methods : Under the advise of Clinical Laboratory Physicians, 144 types of microorganisms were screened based on the guidelines of biosafety in microbiological and biomedical laboratories published by the US Center for Disease Control and Prevention and classified into 1-4 risk groups. A questionnaire containing 21 questions in 5 areas was developed using the biosafety manual by published WHO. Of the 1,876 different organizations sent the survey, 563 responded to the survey (response rate: 30.0%). The species of microoganisms handled by as well as the biosafety level in microbiology laboratories were analyzed. Results : There were 123 species of microorganisms handled in microbiology labs in Korea. The BSL required in 512 microbiology labs was answered by the survey responders as the first grade in 33 labs (6.4%), 2nd in 437 (85.4%), 3rd in 42 (8.2%), and 4th in none. The average number of items satisfied was 12.2, showing only a 57.9% satisfactory rate and normal distribution. Conclusions : The state of overall observance of BSL in most microbiology labs of Korea was evaluated as lagging compared with the standard set up by WHO. Therefore, the Korean government need to produce and distribute a biosafety manual in microbiology laboratories and make efforts to prevent this threat through measures such as training in biosafety in microbiology labs.
