  • Purpose : Mothers and primary encounter and Korean traditional Gynecology clinician's perspective on breastfeeding, the importance and general knowledge, familiar with the breastfeeding-related issues to help resolve the maternal education and treatment. treatment options for a specific note down the need for Maternity and think that the existing studies and articles about breast diseases with reference to the view of the contemplated. Method : Maternity and breast disease with reference to research and review papers. Results : The first, from the past to encourage nursing mothers to eat a lot of nursing was given recently is changing the perception of adequate calories. and second, the posture of breastfeeding in prevention of mother's fatigue and pain and the baby's milk intake should be considered effective. The third, breast feeding is heavier than normal because of the surrounding muscle tissue in the breast to support the training may be helpful to. At last is emotional stability, and adequate nutritional intake and their milk is not enough awareness and interest in and education about breast-feeding mothers to breastfeed will help you look. Conclusion : Prenatal care program in order to facilitate breastfeeding and breast-feeding from the breast managed Postpartum Care Program consists of a map for the breast-related discomfort in advance to lead to prevention and to increase women's satisfaction seems to be.

제왕절개 분만 수유부의 모유, 티아민, 리보플라빈의 분비량과 영아의 섭취량 (The Yields of Volume, Thiamin and Riboflavin from the Milk of Cesarian-Section Women, and Their Ingestion in Infants)

  • The present study was conducted to investigate the yields of volume, thiamin and riboflavin from the milk of twenty-five Cesarean-section women, and their ingestion in infants. We measured the consumed volume of human milk of infants by the test-weighing method, and thiamin riboflavin contents were determined by fluorescence spectroscopy during the first 7 days postpartum. Thiamin contents of the milk averaged 24.3, 24.0, 27.4 and 30.2ug/100g. Thiamin and riboflavin contents of the milk both increased during the lactation ps mod(p<0.05). The first secretion of breast milk occurred from the 4th to 5th day postpartum in Cesarean-section women. The consumed volume of human milk was 63, 81, 104 and 133g/day at 4, 5, 6 and 7 days postpartum and the consumed volume of formula milk was 58, 176, 292, 323, 308, 321 and 318g/day at 1, 2, 3, 4, 5, 6 and 7 days postpartum and the consumed volume of formula from the human and formula milk were 26.6, 91.0, 151.0, 172.5, 169.2, 175.9 and 172.9ug/day. Total daily riboflavin ingestion from the human and formula milk were 26.6, 91.0, 151.0, 172.5, 169.2, 175.9 and 172.9ug/day. Total daily riboflavin ingestion from the human and formula milk was 48.7, 166.8, 277.6, 318.2, 312.0, 327.0 and 335.7ug/day at 1, 2, 3, 4, 5, 6 and 7 postpartum. These results suggest that the milk secretion of Cesarean-section women is delayed a few days and the yield volume of milk is less than that of normal delivery women, but thiamin and riboflavin contents of the milk are not different from those of milk of normal-delivery women.

Comparison of Breast Feeding Trends in Urban Versus Rural Areas: Recommendations to Improve Breast Feeding in Rural America

  • Breast feeding can play a very important role in the development of strong, healthy children. Many studies over the years have shown that breast milk provides defense against common ailments of childhood such as otitis media, gastrointestinal distress, and atopic diseases (allergies). For these reasons the American Academy of Pediatrics recommends breast feeding for at least the first twelve months of an infant's life. Goals of the Health People 2010 Initiative include that at least 75% of mothers will be breast feeding upon discharge from the hospital and at least 50% will be still nursing at six months post-partum. Currently, about 60% of new mothers initiate breast feeding and about 26% are still breast feeding at six months. However, research has shown that breast feeding trends are below these averages in rural areas of the United States. This may be due in part to lack of breast feeding knowledge and teaching in rural areas. Rural hospitals and birthing centers have not initiated many of the breast feeding promotional programs, such as the Baby Friendly Hospital Initiative and Best Start, that have been successful in improving breast feeding trends in urban areas. Often new mothers who live in rural areas do not have access to a lactation consultant to help them with proper follow up. This paper will examine these concerns and propose recommendations to improve breast feeding in rural areas.

  • In Korea, breast-feeding practice has been decreased since 1980's. The purpose of this study was to determine the factors that influence breast-feeding practice. The total numbers of study subjects were 139 mothers in their postpartum one week. The study was carried out for the period of three months from June 17 to August 26, 1998. The tool for breast-feeding problem was developed by Kearney et. al(1990). The data were analyzed by SAS and the results were as follows ; 1. The reported breast-feeding problems were nipple pain, demand of frequent feeding, prolonged feeding, milk leakage, experience of difficulty in breast feeding. 2. Breast-feeding problem defined were prolonged feeding, baby's whimpering after breast-feeding, nipple pain, experience of difficulty in breast feeding, difficulty of breast feeding due to mother's general discomfort.

Breast Milk-Transmitted Cytomegalovirus Infection in Preterm Infants

  • Purpose: The purpose of this study is to describe the rate of cytomegalovirus (CMV) virolactia, and the prevalence of breast milk (BM)-transmitted postnatal CMV infection among premature infants after freeze-thawing (FT) and Holder pasteurization (HP) of breast milk. Methods: This is a single-center, retrospective study of 312 infants born at less than 32 weeks of gestation, or with a birth weight less than 1,500 g from January 2013 to June 2017. All infants were screened for CMV-specific immunoglobulin (Ig) G and IgM at birth. Initial CMV specific polymerase chain reaction (PCR) and CMV culture were performed on mothers' BM and babies' urine within the first 21 days of life. FT and HP of BM was used to prevent the transmission of CMV. For the surveillance of postnatal CMV infection, CMV culture and CMV specific PCR of urine from babies were repeated one to two months after the initial screening. Screening for viremia and viruria was performed if postnatal CMV infection was suspected. Results: Among 178 BM samples obtained from mothers of CMV-IgG-seropositive infants, 80 (44.9%) were CMV PCR positive. CMV deoxyribonucleic acid (DNA) was detected in five of the 22 BM samples (22.7%) obtained from the mothers of CMV-IgG seronegative infants. When CMV DNA load in BM was measured before and after HP, various results were shown. Sixty-three infants out of 232 (27.2%) were evaluated for postnatal CMV infection and four infants out of 63 (6.3%) were infected. Conclusion: Interventions to prevent BM-transmitted CMV infection can reduce the chance of postnatal CMV infection, but not completely eliminate it.

채식을 하는 수유부의 수유기간별 모유분비량과 수유양식에 관한 연구 (A Longitudinal Study on Human Milk Volume and Lactational Performance of Korean Lacto-ovo-vegetarians)

  • The longitudinal changes on human milk volume per day and lactational performance of 23 Korean lacto-ovo-vegetarians(primiparae=11, multiparae=12) from 0.5 to 5 months after parturition have been studied by test weighing method. The human milk volume per day tended to increase during lactation. The mean volume (SD, ml/day) at 0.5, 1, 2, 3, 4, 5 months were 502(102), 692(127), 697(100), 684( 125), 757(52), 703(70), respectively. The overall mean volume was 661$\pm$132ml/day. The peak milk volume during the lactation was observed at most lactating women at the 1st month. The distributions of the individual mean volume during the first 5 months of lactation were found over 750m1(8.7% ), 650-750ml(47.9% ), 550-650m1(2l.7% ), and 450-550ml(2l.7% ) The number of feeding per day was 8.1 ($\pm$1.0) at 0.5 months postpartum, which was consistently decreased. However, the mean volume per feeding was increased from 62($\pm$9.5)ml at 0.5 month to 112($\pm$13.3)ml at 5 months postpartum. The milk volume was correlated with the peak volume. and maternal age. but not with weight before delivery. maternal height and birth weight. No differences of milk volume and lactational performance between facto-ovo-vegetarian and nonvegetarian women were observed. This study suggests that the human milk volume cited in the recommended dietary allowances for Koreitns is over estimated.

Anemia and Iron Deficiency according to Feeding Practices in Infants Aged 6 to 24 Months

  • The prevalence of iron deficiency in later infancy and the toddler years(25% to 40% at 1 year of age) has not decreased remarkably , except in Western countries. The purpose of this study was to 1) determine the relationship between current feeding practices and iron status, and 2) assess compliance to infant feeding instructions. Two groupsof infants were examined. The first group of 302 infants aged 6 to 24months was seen at a well baby clinic while the second group of 135 infants of the same age group was assessed by venipuncture. Cutoff values for laboratory tests were as follows ; hemoglobin<11g/dL, mean corpuscular volume (MCV) <72fl ; red cell distribution width(RDW)>15% ; serum ferritin level<10ng/ml ; and transferrin saturation (serum iron(TIBC)<10%. The diagnosis of iron deficiency anemia (IDA) was made when a low hemoglobin level was associated with either low ferritin orlow transferrin saturation . Of the 302 children brought to the well baby clinic , 12.3%(n=37) were found to have anemia (hemoglobin<11.0/dL). In terms of children grouped according to feeding practices, it was found that children with anemial comprised 32.0% (24/75) of the prolonged breast-fed group (Group A), significantly more than the 4.0%(7/176) of the artificial milk feeding group(Group B). and 3.9%(2/51) of the switched from breast milk to iron -fortified weaning foods group(Group C).Among the 107 children with IDA , iron deficiency in 105 children(98.1%) was suggested by their dietary histories ; exclusive or prolonged breast-feeding for more than 6 months without iron fortification in 98 infants ; cow's milk consumption> 500ml/day without iron fortification during infancy(n=12), or >800ml without iron-fortified foods after infancy(n=15) ; and the use of unfortified forumula or unbalanced diets, mainly limited to rice gruel. Despite the relatively high (79.6%) motivation on the part of the infants mothers and supervison by professional personnel, the poor results in the infants receiving iron fortified foods were due to poor compliance(85.75). Among the mothers of 98 IDA patients who were contacted by telephone , it was revealed that 29% did not give the oral iron preparation for more than 2 months. Furthermore, negligence or disregard by the parents occurred in 14% of the case , discontinuance of the oral iron preparation by the parents due to side effects occurred in 6%, and the children's refusal or poor oral intake and no further trial occurred in 6%. The dietary history of a large group of infants was highly predictive of their risk for anemia . Continued consumption of breast milk until the age of 1 year is not warranted unless iron-fortified foods are given concomitantly. Because there is a problem with compliance, more successful and safe strategies for preventing iron deficiency woold included dual coverage in the from of therapeutic iron supplementation as well use of iron-fortified foods for teddlers who are at risk of iron deficiency.

체지방 조직 및 초유중에 잔류되어 있는 다이옥신 함유도에 관한 연구 (The Concentrations and TEQ Levels of PCDFs and PCDDs in Human Adipose Tissue and First Breast Milk of Korean)

  • 우리 나라 사람들의 체지방 조직과 초유중에서, 다이옥신의 잔류 농도를 HRGC/HRMS를 이용하여 동위원소희석법으로 측정하였다. 체지방 조직 시료는 서울에 있는 모병원에서 채취하였으며, 체지방 조직에서의 PCDFs의 총농도와 TEQ 농도는 각각 5.812 pg/g, 1.485 pgTEQ/g 이었고, PCDDs의 총농도와 TEQ 농도는 각각 26.648 pg/g, 1.176 pgTEQ/g 이었다. 서울대학 병원으로부터 받은 산모 20명의 초유에 함유된 다이옥신의 잔류농도는 PCDFs의 총 농도가 7.019 pg/mL, TEQ 농도로 환산하면 0.177 pgTEQ/mL 이었고, PCDDs의 총 농도와 TEQ 농도는 각각 14.224 pg/mL, 0.693 pgTEQ/mL이었다. 체지방 조직과 모유에서의 다이옥신 이성질체들의 분포는 PCDDs가 PCDFs보다 상대적으로 높게 나타났으며, 그중 OCDD가 가장 높은 농도로 포함되어 있다.

  • 식품과민반응의 선천적 이력(natural history)과 방어의 이해는 식품 알레르기 환자의 관리에 매우 중요하다. 이러한 주제에 관하여 많은 연구내용들이 완전히 일치하지는 않지만 데이터에서 몇 가지 분명한 교훈을 얻을 수 있다. 첫째, 식품 알레르기는 매우 일반적인 것이다. 둘째, 식품 알레르기의 거의 대부분은 생후 1~2살에 시작한다. 셋째, 모든 식품 알레르기는 일반적으로 긍정적인 결과에 대한 예외가 있지만 증가하고 있는 실정이다. 넷째, 식품 알레르기는 대부분의 어린이들이 시간이 지나면서 없어지지만 일부는 호흡기 알레르기로 발달하여 종종 아토피질환의 시초가 된다. 마지막으로 적어도 몇 가지 식품 알레르기는 유아기와 어린이 시기에 주요 식품알레르겐들을 피함으로써 막을 수 있다.

수유방법별 관련요인 및 영아의 성장비교 (The study on related factors of feeding type and comparison of development between breast-fed and formula-fed infants)

  • This study presents results of surveys conducted Seoul and Kangreung public health center using structured questionnaire developed by researchers to identify factors that were relevant to the method of feeding. To compare the infant's development between the breast-fed infants and formula-fed infants, infant's height, weight, triceps skinfold during 1month and 4months were cheked. The results were as follows : 1. At postpartum, 59.1% infants had formula feeding, while 22.6% had breast feeding. At 1 month old, 49.6% infants had formula feeding, 27.8% had breast feeding. At 4 months old, 60.9% infants had formula feeding, while 26.1% had breast feeding. 2. 'Lack of breast milk' was the predominant reason for formula feeding. 3. The sujects didn't enough eat not only rice and seaweed soup but also any other specific foods during breast feeding period. 4. Factors that affected the method of feeding were the patterns of delivery, mother's height & weight, first baby feeding type(at post partum), infant sex, mother's age, preparation of breast feeding, first baby feeding type, regular clinic visit (at 4months old). 5. The birth weight and height were correlated with mother's weight and height. 6. There were no significant different on infant's weight, height, triceps skinfold between breast-fed infants and formula-fed infants.
