Fire Acupuncture for Varicose Veins in the Lower Extremities: A Review of Randomized Controlled Trials

  • Varicose veins have a substantial burden on individuals' aesthetics and overall quality of life. The current body of research on Korean medical interventions for varicose veins is limited in Korea, and exploration into clinical trends is lacking. This review specifically analyzed fire acupuncture, which is an understudied intervention in Korea that demonstrated efficacy in alleviating pain and paralysis while promoting tissue regeneration. A comprehensive investigation of the China National Knowledge Infrastructure database was conducted, utilizing terms such as "varicose vein," "fire needle," "fire acupuncture," "burning acupuncture," "burning needle," "huǒzhēn," "xiàzhī jìngmài," and "jìngmài qūzhāng." This review included 14 studies investigating the role of fire acupuncture in treating varicose veins. We extracted variables, including age, sex, disease duration, intervention method, treatment frequency, treatment period, acupoints, and outcomes. Fire acupuncture appeared as the primary intervention method, followed by general acupuncture. The protruding part of varicose veins is the main treatment area for fire acupuncture, while the acupoints BL57 and ST36 were most frequently utilized for general acupuncture. Treatment results revealed significant improvement in all studies. However, there are limitations, such as scarce studies, limited side effects studies, and potential bias. Consequently, extensive and well-structured future studies are necessary to demonstrate the role of fire acupuncture in varicose veins.

The Study of Oriental Medicine Music Therapy Applied to the Acupuncture of Pathogenic Fire (화열병(火熱病) 침구치료 시 적용 가능한 한방음악치료에 관한 연구)

  • Objectives : The purpose of this study is to introduce oriental medicine music therapy during the acupuncture of pathogenic fire. Methods : We investigated the acupuncture points of pathogenic fire and the methodology of oriental medicine music therapy. Results : During the acupuncture of pathogenic fire, patients listen to proper music and after withdrawing of acupuncture, patients play the musical instrument. The patients with the case of fire due to affection by exopathogen, the method of dispelling superficial fire can be used through playing percussion instruments. The case of patients with the fire of five organ, the Five Phases music can be used properly. Conclusions : This study describes oriental medicine music therapy has a good possibility as a treatment of pathogenic fire with acupuncture.

Study on the Principles of Prescriptions of Fire and Heat of Saam Acupuncture (사암침법(舍岩鍼法)의 화열(火熱)에 대한 연구)

  • Objectives : Saam acupuncture has evident principles of prescription based on acupuncture tonification and purgation of Nan-gyeong but many prescriptions of Saam acupuncture are not typical patterns. This paper studies principles of prescriptions of the king fire and the premier fire. Methods : Through investigation of many medical documents concerning king fire and the premier fire, the author explained of physiological and pathological principle of king and the premier fire, also explained of interacting mechanism through five element. And The theoretical authorities of analysis are Nan-gyeong, works of Joo dan-gye and Li dong-won. Results & Conclusion : 1. The sufficiency and the control of the water is the key to treat the disease by fire. Considering the attack and the treatment of disease, heart and liver give rise to fire in its early stage and kidney and lung are the key to treatment. 2. Consequently the pathology of the disease from fire is corresponding to the theory- Yang Sufficient, Yin Deficient- of Joo dan-gye. It seems that the theory of Joo dan-gye influenced on the medical theory of Saam Acupuncture. 3. On that account I believe that the study of medical theories of the Geum and Won Dynasty that various medical theories had appeared is very available for Saam Acupuncture. At present global warming is accelerating because environmental pollution has been increasing. Therefore the damage of fire to human body will be more and more severe and extensive. Because in the future medical plants will not be free from environmental pollution, acupuncture will be very confidential treatment.

A Review of Fire Needling on Frozen Shoulder: Focusing on Chinese Journals (동결견의 화침치료에 대한 임상논문 고찰: 중국논문을 중심으로)

  • Objectives : The objective of this study is to find out the method and effects of fire needling on frozen shoulder in China. Methods : We searched journals using the China National Knowledge Infrastructure(CNKI) and PubMed. The keywords were a combination of "fire needling", "fire needle", "burning acupuncture", "frozen shoulder", "adhesive capsulitis", "periarthritis". Results : There were 23 studies finally selected, 7 were case control studies and 16 were case series studies. There were 9 out of 14 main meridians and 31 kinds of acupoints. The most frequently adopted meridians were LI, SI and TE. The most frequently used acupoints were $LI_{15}$, $SI_9$ and $TE_{14}$. Adjacent points were used more often than distant points. They usually heated the needle before insertion and needle retention was mostly not done. 7 case control studies showed that fire needling reported better results than the filiform needle or electro-acupuncture treatment. 15 case series studies reported a significant improvement in fire needling on frozen shoulder patients. Conclusions : There have been many studies regarding fire needling on frozen shoulder in China. Further studies should be required and these can be applied to clinical practices in Korea.

The Safety Assessment of Fire needling (화침의 안전성 평가에 관한 고찰)

  • Objectives : Fire needling has been applied as the treatment for various diseases and been getting much attention from Oriental medicine due to its excellent effectiveness as the results of clinical studies have reported. However, the research findings on the safety of treatment method, materials for the Fire needling needle materials and the possibility of burn injury during the procedure are still insufficient. Methods : A thermo imaging camera was used to confirm the temperature distribution on acupuncture needle and the treatment area during the fire needling therapy. Then the degree of thermal injury was observed by H&E stain and TUNEL assay. In addition, in order to assess the safety of acupuncture materials, we conducted MTT assay using a L6 cell line. Results : The average temperature of the skin surface was observed at $47{\sim}51^{\circ}C$ after classic fire needling and $30^{\circ}C$ after warming fire needling. Warming fire needling therapy does not induce a burn on the tissue and a third degree burn was observed locally in the muscle and skin layers after classic fire needling treatment. This confirms that hwa-acupuncture therapies do not cause major burns. According to the safety assessment test result, no cytotoxicity was detected in the warming fire needling materials. This confirms the safety of the acupuncture materials Conclusions : Various research results on the biological safety of fire needling. Since fire needling therapy induces a burn locally without leaving any scar, and as other results indicate, it is considered a safe treatment method.

Systematic Review of Fire Needling or Warm Needling Treatment for Ankle Sprain

  • The effectiveness of fire needling or warm needling treatment in clinical studies for the treatment of ankle sprains was reviewed using 4 international (PubMed, Cochrane library, EMBASE, CNKI) and 5 Korean databases (NDSL, RISS, KISS, OASIS, KTKP). Randomized controlled trials, that performed fire needling or warm needling treatment for ankle sprains until October, 2018 were retrieved (n = 8). All studies were performed in China, and 7 out of 8 studies were published within the last 5 years. There were 4 studies that used fire needling treatment, 3 studies used warm needling treatment, and 1 study used fire and warm needling treatment. The ashi-points and gallbladder meridian were the most frequently selected acupoint and meridian each. All intervention groups in the 8 studies showed statistically significant beneficial effects compared with control groups. The results of this study could provide preliminary data as the basis for well-designed randomized controlled trials on fire needling or warm needling treatment for ankle sprains.

A Systematic Review and Meta-Analysis of Fire Needling Treatment for Knee Osteoarthritis: Focused on Comparative Studies with Manual Acupuncture Treatment during Recent Five Years (퇴행성 슬관절염의 화침 치료에 대한 체계적 문헌 고찰 및 메타분석: 최근 5년간의 호침 치료 비교 연구를 중심으로)

  • Objectives : The purpose of this study is to review the effectiveness of fire needling treatment for knee osteoarthritis in comparison with manual acupuncture treatment. Methods : Through four foreign online databases (PubMed, Cochrane library, EMBASE, and CNKI) and five domestic online databases (NDSL, RISS, KISS, OASIS, and KTKP), we searched for clinical studies that performed fire needling treatment for knee osteoarthritis until May 10, 2019. Only randomized controlled trials were selected and we assessed the risk of bias according to the Cochrane RoB criteria. This review examined the selected studies into first author, publication year, sample size, outcome measurements, results, acupoints, treatment time & period and so on. Results : A total of 7 RCTs were selected in this review and all were conducted in China. Treatment period of more than 4 weeks and treatment visits of 10 to 20 times were the most common. EX-LE4 and ST35 (=EX-LE5) acupoints were most frequently selected in treatment. Among the evaluation indexes, a total efficacy rate was used the most. Most of fire needling groups showed more significant results compared with the manual acupuncture groups statistically. Conclusions : All studies showed that fire needling treatments for knee osteoarthritis were more effective than manual acupuncture treatments statistically. Therefore, the results of this study could be utilized as a preliminary data for another clinical research on fire needling treatment for knee osteoarthritis. However, further well-designed randomized controlled trials will be needed to develop sufficient evidence about the effectiveness and safety of fire needling treatment for knee osteoarthritis in the future.

The Comparative Study on the Effect of Fire Needling Therapy and General Acupuncture with Other Korean Traditional Medical Treatment for the Patient with Lumbar Herniated Intervertebral Disc: A Randomized, Assessor Blinded, Two Arm Trial (요추 추간판 탈출증 환자에 대하여 기타 한방치료를 병행한 가열식 화침과 일반침의 효능 비교 연구: 무작위배정, 평가자 맹검, 평행군 설계)

  • Objectives : The purpose of this study is to compare the efficacy of fire needling therapy and general acupuncture for the patient with lumbar herniated intervertebral disc. Methods : We observed 20 lumbar herniated intervertebral disc patients from March, 2014 to October, 2015. This study was randomized and assessor blinded. The patients were divided into two groups: one group(group A) was treated by general acupuncture only, and the other group(group B) was treated by fire needling therapy and general acupuncture. We evaluated the treatment effect experienced by each group using the visual analogue scale(VAS) and Oswestry disability index(ODI). Results : 1. In group A, VAS and ODI from before treatment to treatment 4, 8, 12, 16 were significantly improved in statistics. 2. In group B, VAS from before treatment to treatment 4, 8, 12, 16 was significantly improved, but ODI was significantly improved in statistics only from before treatment to treatment 12, not in treatment 4, 8, 16. 3. As a result of comparisons between group A and B, there were no statistical significance in VAS and ODI. Conclusions : This study was not able to determine statistical significance between the group treated by general acupuncture only and the group treated by fire needling therapy and general acupuncture, so further studies are required in the future.

  • Saam acupuncture has an evident principles of prescription based on acupuncture tonification and purgation of Nan-gyeong but many prescriptions of Saam acupuncture are not typical patterns. Therefore it is difficult to understand principles of those prescriptions. Understanding the principles of any prescription means understanding the physiology and pathology of that prescription and it have an important role in selecting the best prescription and taking the most effective treatment. The damage of fire to mentality is very intensive and severe. The prescriptions of the king fire and premier fire have a good effect on diseases by fire. The fire of human body are two types, the king fire and the premier fire but the water of human body is only one and then inferior to fire. The fire superior to the water is unstable and the fire influences rapidly and intensively on four elements, wood, earth, metal, water. So the control of the water is the key to treat the disease by fire. Considering the attack and the treatment of disease, heart and liver are attacked with a disease by fire in its early stage and kidney and lung are the key to treatment. Consequently the pathology of the disease by fire is corresponding to the theory- Yang Sufficient, Yin Deficient- of Joo dan-gye. It seems that the theory of Joo dan-gye influenced on the medical theory of Saam Acupuncture. Case Report Combined Therapy of Acupuncture and Fire Needling is Effective to Treat Frozen Shoulder (일반침과 화침의 병용 요법의 동결견 환자에 대한 응용 증례 보고)

  • Fire needling is an acupuncture procedure involving the swift pricking of diseased part with a red hot needle. The purpose of the present study is to report the therapeutic effect of acupuncture and fire needling combined therapy on the frozen shoulder patients. Each of patient received combined therapy of acupuncture and fire needling. Fire needling was carried out 2 or 3 times a week. Shoulder pain was evaluated by measuring visual analoge scale (VAS), range of movement (ROM), and global assessment and grade. All of the patinets treated with combined therapy exhibited decreasing of VAS and grade, increasing of ROM and global assessment.
