Meningococcal infections can be associated with abnormalities of the complement system, which contains 5 terminal complement proteins. Furthermore, deficiencies in 1 of these 5, complement component 7 (C7), leads to the loss of complement lytic function, and affected patients show increased susceptibility to recurrent meningococcal meningitis and systemic Neisseria gonorrhoeae infection. In September 2003, an 11-year-old female patient presented at our outpatient department with high fever, lower leg pain, headache, and petechiaes. She rapidly progressed to coma but later achieved full recovery due to prompt treatment. Her final diagnosis was meningococcal sepsis and arthritis. Her elder brother also had a similar bacterial meningoencephalitis history, which encouraged us to perform analyses for complement component and gene mutations. Resultantly, both the brother and sister were found to have the same mutation in the C7 gene. Subsequently, vaccinations of the meningococcal vaccine meningococcal vaccine ($Menomune^{(R)}$) were administered. However, in September 2006, the brother expired due to acute micrococcus meningoencephalitis. At present, the 16-year-old female patient is healthy. Here, we report a Korean family with a hereditary C7 deficiency with susceptibility to meningococcal infections due to C7 gene mutation.
The WIPP (Waste Isolation Pilot Plant), located 42km east of Carlsbad, New Mexico (NM), in bedded salt 655m below the surface, is a mined repository constructed by the US DOE for the permanent disposal of transuranic (TRU) wastes generated by activities related to defence of the US since 1970. Its historical disposal operation began in March 1999 following receipt of a final permit from the State of NM after a positive certification decision for the WIPP was issued by the EPA in 1998, as the first licensed facility in the US for the deep geologic disposal of radioactive wastes. The CCA (Compliance Certification Application) for the WIPP that the DOE submitted to the EPA in 1966 was supported by an extensive performance assessment (PA) carried out by Sandia National Laboratories (SNL), with so-called 1996 PA. Even though such PA methodologies could be greatly different from the way we consider for HLW disposal in Korea largely due to quite different geologic formations in which repository are likely to be located, a review on lots of works done through the WIPP PA studies could be the most important lessons that we can learn from in view of current situation in Korea where an initial phase of conceptual studies on HLW disposal has been just started. The objective of this art report is an overview of the methodology used in the recent WIPP PA to support the US DOE WIPP CCA and some relevant results completed by SNL.
Objectives : The aim of this study was to examine the level of oral health knowledge, oral health importance, oral health status and oral health interest of the social workers engaged in child care facilities to analyze the influential factors on oral health promotion activities and provide the basic data for oral health promotion activities. Methods : The subjects in this study were those social workers engaged in child care facilities from May 15th to May 25th in 2011. The self-report questionnaires were distributed and withdrawn and then a total of 205 papers from the subjects that participated in this study were analyzed for the final analysis. Results : 1. Oral health knowledge had $12.16{\pm}1.66$ in a full score of 15 and oral health importance, oral health status and oral health interest were $1.12{\pm}0.36$, $2.80{\pm}0.50$ and $2.14{\pm}0.8$ respectively in a full score of 5. 2. According to the general characteristics, the difference analysis of cognition-recognition factors showed that singles had significantly higher oral health knowledge married couples (p<0.05) and the social workers unusing auxiliary oral hygiene devices had significantly higher oral health interest than those using them(p<0.01). 3. The mean of oral health promotion was $3.28{\pm}0.51$, characteristics of detailed specific-area were as follows ; the toothbrushing was $3.77{\pm}0.57$, the use of fluoride was $2.67{\pm}0.98$, periodic dental screening and treatment were $2.98{\pm}1.09$ and eating habits was $4.14{\pm}0.57$. These results generally showed tooth-brushing and eating habits show a little higher figures. 4. Considering the factors oral health promotion behaviors of social workers engaged in child care facilities, the higher oral health interest was, the higher oral health promotion behavior was. Conclusions : It seems that social workers engaged in child care facilities are required to pay attention to the importance of oral health and learn oral health knowledge, which will lead to much better improvement of oral health for children brought up by them.
This is a correlational study to investigate the degree of expectation regarding aging, depression and health promotion lifestyle and to identify the factors influencing health promotion lifestyle among health checkup examinee. The subjects of the study were 143 health checkup examinee who visited the M medical center located in G Metropolitan city. Data were collected from July 5 to 21, 2016 using the structured questionnaire of self-report form. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation, and hierarchical regression. As a result, health promoting lifestyle were correlated with expectation regarding aging positively, and with depression negatively. The factors influencing the health promotion lifestyle were education level, perceived health status, time to move to health care facilities, expectation regarding aging, and depression. With controlled general characteristics, the level of health promotion lifestyle was better explained by the addition of the independent variables of expectation regarding aging, and depression, and the explanatory power of the final model was calculated as 27.0%. Therefore, in order to improve health-promoting lifestyle, it is necessary to consider cognitive factors (knowledge, expectation regarding aging) and psychological factor (depression) besides physical approach.
The purpose of this study is to examine the effects of care-giving stress of dementia elderly on the caregiver's suicidal ideation. In addition, mediating effect of self-efficacy was examined. In order to accomplish these purposes, a total of 336 who are caregivers of dementia elderly using self-report questionnaires. In this study, 325 elderly patients were used for final analysis. Date were analyzed by frequency and descriptive analysis, regression model analysis with SPSS 22.0. The analysis result of dementia elderly caregiver's symptoms showed that prevalence of suicidal ideation were 32.6%. Regression analysis was conducted under the control of dementia elderly's disease period, ratings, general characteristics and age, sex, educational background, occupation status, health status, living standard level of caregivers who were the subject of the survey. As a result, it was confirmed that the care giving stress had a statistically significant effect on suicidal ideation. Also, the self-efficacy of caregivers was found to partial mediate the relationship between care-giving stress and suicidal ideation(***p<.001). Based on this result, I suggested the various policies and the necessity of social welfare intervention to restore the mental health of caregivers of the elderly with dementia.
The purpose of this study was to assess the level of awareness regarding the relationship between systemic diseases and oral health and the importance of education related to this association among dental patients, as well as to identify factors that affect the awareness of the relationship and the perceived need for education. This study was conducted from June 2016 to February 2017. Subjects of this study included outpatients (20-60 years old) from a dental clinic, and only patients who agreed to the survey were included. A total of 110 subjects were included, but the final analysis was based on 92 questionnaires, excluding questionnaires that were inaccurate. Survey questionnaires were completed by self-report and face-to-face interviews with a dental hygienist. The questionnaires covered basic information, awareness of the association between general and oral health, and experience with, and need for education related to, this association. Among the subjects, 48.9% were aware of the association between systemic diseases and oral health. A total of 39.1% of subjects had received education regarding the relationship between systemic diseases and oral health. Subjects who visited dental clinics regularly were 3.94 times (95% confidence intervals [CI]: 1.21-12.84) more likely to be aware of the association between oral health and disease compared with subjects who made only irregular visits to dental clinics. In addition, experience or education was significantly associated with awareness of the relationship between oral health and disease (odds ratio [OR]: 4.64, 95% CI: 1.54-13.93) and the need for education (OR: 3.98, 95% CI: 1.20-13.12). Thus, the dental professionals should provide education on the relationship between oral health and systemic disease in dental clinics to improve patients' awareness and oral health behaviors. These results can be used to strengthen education in the dental clinic.
Digital imaging detectors can use a variety of detection materials to convert X-ray radiation either to light or directly to electron charge. Many detectors such as amorphous silicon flat panels, CCDs, and CMOS photodiode arrays incorporate a scintillator screen to convert x-ray to light. The digital radiography systems based on semiconductor detectors, commonly referred to as flat panel detectors, are gaining popularity in the clinical & hospital. The X-ray detectors are described between a-Silicon based indirect type and a-Selenium based direct type. The DRS of detectors is used to convert the x-ray to electron hole pairs. Image processing is described by specific image features: Latitude compression, Contrast enhancement, Edge enhancement, Look up table, Noise suppression. The image features are tuned independently. The final enhancement result is a combination of all image features. The parameters are altered by using specific image features in the different several hospitals. The image in a radiological report consists of two image evaluation processes: Clinical image parameters and MTF is a descriptor of the spatial resolution of a digital imaging system. We used the edge test phantom and exposure procedure described in the IEC 61267 to obtain an edge spread function from which the MTF is calculated. We can compare image in the processing parameters to change between original and processed image data. The angle of the edge with respect to the axes of detector was varied in order to determine the MTF as a function of direction. Each MTF is integrated within the spatial resolution interval of 1.35-11.70 cycles/mm at the 50% MTF point. Each image enhancement parameters consists of edge, frequency, contrast, LUT, noise, sensitometry curve, threshold level, windows. The digital device is also shown to have good uniformity of MTF and image parameters across its modality. The measurements reported here represent a comprehensive evaluation of digital radiography system designed for use in the DRS. The results indicate that the parameter enables very good image quality in the digital radiography. Of course, the quality of image from a parameter is determined by other digital devices in addition to the proper clinical image.

Background : Coding of principal diagnosis is essential component for producing reliable health statistics. We performed this study to evaluate the current practice of principal diagnoses determination and coding, and to give some basic data to improve coding of principal diagnosis. Method : Nineteen medical record administrators (MRAs) of 3 university hospitals participated in coding principal Dx. from August 1, 2001 to August 31, 2001. From each hospital, 10 medical records of patients with high frequency disease were selected randomly. Each 10 medical records were grouped into three (A. B, C). Then, these 30 medical records were given to each MRAs for coding. At the same time questionnaire was given to each of them. Questions were to prove how they decide and code the principal diagnosis among many current diagnoses; how they decide and code the principal diagnosis when they see irrelevant diagnosis recorded as the principal diagnosis in medical record, when only tentative diagnoses were recorded without final diagnosis, and when different diagnoses were recorded in different sheets of same record. Agreement of coding among 3 hospitals were compared and survey results were analysed with SAS 6.12. Results : Agreement of coding was found in medical records 5-6 of each 10 medical records. Causes of disagreement were as follows. Difference of clinician's opinion from each hospital; mixed use of guideline from KCD-3 and guideline from DRG; difference in 4th digit classification according to the absence of pathology report in the medical record; difference of abbreviations among hospitals. 57.9% of MRAs selected the principal diagnosis recorded by physician, 42.1% of MRAs decided principal diagnosis after consulting to KCD-3 guideline. When there were difficulties in determining the principal diagnosis, 42.1% of MRAs decided principal diagnosis after discussion with the physician, 26.3% after discussion with fellow MRAs. Conclusion : There were differences in codings among hospitals. To minimize the difference, we suggest the development of disease-specific guidelines for coding in addition to the current general guideline such as KCD-3. To do this, Coding Clinic which can produce guidelines is needed.
Objectives The purpose of this study was to investigate the progress of weight and waist circumference(WC) through oriental obesity treatment and clarify the correlation between weight loss and WC change. Methods The subjects were treated from January 2007 to September 2009 in J Oriental Medical Center. Herbal medicine, electroacupuncture, oriental obesity physiotherapy were performed for a month. We checked the change of weight and WC at every treatment. We classified the WC into upper, middle, lower parts specifically and measured them individually. Measurements were analyzed by using one-way repeated measures ANOVA and we also investigated correlation between weight loss and WC change. Results (1) During the first and second treatment sessions, the weight loss was the highest, $0.96{\pm}0.86\;kg$($1.46{\pm}1.32\;%$) and during the 5th and 6th treatment sessions, the lowest $0.37{\pm}0.616\;kg$. The final weight loss was $3.71{\pm}1.476\;kg$($5.51{\pm}2.12\;%$). (2) After 8 treatment sessions, the reductions of upper, middle and lower WC were $3.59{\pm}2.26\;%$, $5.93{\pm}2.75\;%$, $5.51{\pm}3.22\;%$, respectively. As the patients received more treatment, there was a progressive decline in the variation of weight and middle WC. However, the decline rate of upper and middle WC fluctuated during the treatments, forming a W-shaped curve. (3) Analysis of the correlation between the weight loss and WC change shows that the reduction of middle WC was most closely associated with the weight loss. Conclusions This study indicated that the rate of weight loss and WC reduction showed stagnation during the 5th and 6th treatment sessions, and the reduction of middle WC was most closely associated with the weight loss. Since the progress of weight and WC change is an important field of research on obesity, further systemic studies would be needed for the foundation of a clinical guideline.

Objectives : This study is designed to clarify whitening, anti-inflammatory effect of fractions extracted from the mixture of Phaseolus radiatus L. and ethanol. Methods : In this experiment, we were intended to reveal whitening, anti-inflammatory effect of fractions extracted from the mixture of Phaseolus radiatus L. and ethanol. The whitening activity was confirmed by UV blocking activity, tyrosinase inhibiting activity, and melanin formation inhibiting activity. Anti-inflammatory activity is confirmed by measurement of cytotoxicity level by MTT assay and measurement of Cytokine expression, which is the main mediator of inflammation reaction. Results : As a results, overall activity was high in the ethyl acetate fraction. Tyrosinase inhibitory activity was less than 20% at all concentrations, but the activity to inhibit melanin self-production was higher than that of ethyl acetate fraction at $32.19{\pm}2.79%$ at $100{\mu}g/m{\ell}$. And ethyl acetate fraction had a relatively high UV blocking activity. In the anti-inflammatory test, the concentration-dependent activity was shown, and the chloroform and ethyl acetate fractions showed significant NO production inhibitory activity. Cytokine expression was superior to that of the final stage of B cell differentiation, and cell viability was over 80% except for the chloroform fraction at the concentration of $200{\mu}g/m{\ell}$. Conclusions : The results of this experiment confirmed the whitening effect and anti-inflammatory effect of Phaseolus radiatus L.'s extracts and fractions and report the possibility of application as external medicine.
