Unicortical Bone Necrosis of the Fibula Free Flap Associated to the Fixation with a Nonlocking 2.0-mm Reconstruction Plate and Screws

  • The authors present a unique case of osteonecrosis of a cortical half of a fibula free flap that has not been reported in the literature yet. This complication was associated with the impairment of the vascularization of periosteum in the cortical half of fibula that was fixated with a nonlocking reconstructive 2.0-mm plate and screws but other factors could have been involved. The patient was submitted to excision of a cemento-ossifying fibroma that resulted in a left hemimaxilectomy mesoinfrastructure defect classified as the Cordeiro type 2B. The 42-year-old female patient was submitted to reconstruction with an osteomusculocutaneous fibula free flap plus a segment of fibula graft. The two bone segments of the free flap used to reconstruct the anterior and left alveolar crest were fixated with a reconstructive 2.0-mm plate of matrixMANDIBLE system. The only reported complication was an oronasal fistula that healed with conservative treatment and the referred osteonecrosis of the external cortical half of the fibula free flap with plate exposure at 2.5 years postoperatively. Surgical excision of the osteonecrosed cortical half of the fibula with the plate and screws was performed, while the other cortical underwent bone union as corroborated by computed tomography scans.

Large myxomatous odontogenic tumor in the jaw: a case series

  • Objectives: Myxomatous odontogenic tumors (MOTs) are the third most common odontogenic tumors in the oral and maxillofacial region. Due to its slow-growing, but locally invasive nature, the tumor is usually detected by accident or only when it becomes a large mass, which causes facial deformity. Materials and Methods: Current study reports three unusual cases of MOT including huge myxoma involve the mandible in middle-aged man, MOT with ossifying fibroma pattern in mandible, and MOT in maxilla of young female patient. The diagnosis and treatment strategy of MOTs was also summarized and updated. Results: In reported three cases of patients with large MOTs, surgical treatment was indicated with fibular free flap reconstruction in the mandible and plate reconstruction in the maxilla. The tumors were successfully treated with radical resection and did not show signs of recurrence during the follow-up period. Conclusion: Surgical treatment indication depends on size, the position of the lesion, patient systemic condition and surgeon individual experience. In the case of a large tumor, radical resection and reconstruction is the standard surgical strategy. The conservative surgical treatment including enucleation with wide curettage is still under controversy. The recurrence rate for MOTs is significantly high, up to 30%, therefore long-term follow-up is essential.

Retrospective Analysis of Canine Tumors(non-mammary and non-cutaneous) in Korea(2003-2006) (개 비유선 및 비피부 유래 종양의 발생현황분석(2003-2006))

  • During a designated period(Jan. 2003$\sim$Jun. 2006), a total of 2,051 biopsy and necropsy cases submitted to Veterinary Medical Teaching Hospital of Seoul National University and local practitioners were diagnosed as canine tumors based on microscopic evaluation in the Department of Veterinary Pathology, College of Veterinary Medicine, Seoul National University. Four hundred and twenty of 2,051 tumor specimens excluding mammary(883, 43.1%) and cutaneous(748, 36.5%) tumors were included in this retrospective study. They were composed of genital tumors(189, 45.0%) followed by alimentary(113, 26.9%), hematopoietic(52, 12.4%), urinary(20, 4.8%), bone & joint(15, 3.6%), ocular & otic(9, 2.1%), respiratory(6, 1.4%), endocrine(6, 1.4%), and miscellaneous(10, 2.4%). Particular top ten tumor most frequently diagnosed were seminoma(48, 11.4%) followed by fibrous epulis(38, 9.0%), lymphoma(38, 9.0%), leiomyoma(33, 7.9%), fibroma(26, 6.2%), ovarian cyst(19, 4.5%), melanoma(15, 3.6%), papilloma(14, 3.3%), cystic endometrial hyperplasia(13, 3.1%), granulosa cell tumor(13, 3.1%) in descending order comprising 257(61.2%). The affected age of the animals with ten frequent tumors ranged from 3 months to 17 years old with a mean of 9.2 years old(n=218). There were no significant sex prevalence except female leiomyoma. The top seven anatomical sites of tumors inclusive were gingiva(62, 14.8%), testis(61, 14.5%), ovary(48, 11.4%), vagina(40, 9.5%), uterus(36, 8.6%), lymph node(30, 7.1%), and spleen(21, 5.0%).

Tumors of the Foot and Ankle (족부 및 족관절에 발생한 종양)

  • Purpose : Tumors of the foot and ankle are uncommon compared with those arising in the other sites, and the malignant tumors are rare. We analyzed the data of patients who have been diagnosed as having a tumor of the foot and ankle. Materials and Methods : Between 1989 and 1998, we treated 72 patients and analyzed on their clinical characteristics retrospectively. Results : Fifty-three cases were benign. Included are 30 cases of soft tissue tumor and 23 cases of bone tumor. Malignant tumor was in 19 cases (26%), including 17 cases of soft tissue tumor, one cases of metastatic tumor. The most common benign tumor was fibroma in soft tissue and osteochondroma in bone. Malignant melanoma was the most common malignant tumor of the foot (7 cases). The predilection site for benign tumors was around toes while for the malignant tumor mostly arises in the heel. Among 19 malignant tumors, local recurrence developed in 3 cases and the distant metastasis occurred in 8 cases. Conclusion : The ratio of malignant tumor and metastasis was high. Therefore, when we faced with a tumor of the foot and ankle, the histopathologic confirmation is essential through biopsy before the definite initial treatment.

Femur Fractures Associated with Benign Bone Tumors in Children (양성 골종양을 동반한 소아 대퇴골의 병적 골절)

  • Purpose: We evaluate the results of treatment of pathologic femur fractures secondary to bone tumors in children. Materials and Methods: Between January 1995 and June 2004, 18 patients(20 cases) were evaluated. Their mean age of the first episode of fracture was 10.2 years and mean follow-up period is 42.5 months. Primary bone tumors, the location of fracture, time to union and complications were evauated. Results: Fractures occurred at proximal portion in 14 cases, shaft 3 cases and distal portion 3 cases. The bone tumors causing pathologic fracture were fibrous dysplasia(9 c ases), simple bone cyst(4 cases), aneurysmal bone cyst(4 cases), nonossifying fibroma(2 cases) and eosinophilic granuloma(1 case). In the treatment for fractures, cast was in 11 cases, internal fixation 8 cases and external fixation in 1 case. In the treatment for tumors, observation was in 11 cases, curettage & bone graft in 8 cases and resection in 1 case. In polyostotic fibrous dysplasia, all cases were treated by cast initially but deformity developed in all cases. Fracture prevention and deformity correction were obtained with intramedullary nailing. Conclusion: Adequate choice of treatment of bone tumor and fracture will result in good prognosis.

A CLINICAL ANALYSIS OF BENIGN ODONTOGENIC TUMOR (양성 치성종양의 임상적 분석)

  • The author studied on the 128 cases of benign odontogenic tumors which had been diagnosed with biopsy during the period of Jan. 1989 to Dec. 1998 at the Kyungpook National University Hospital, Yeungnam University Medical Center, Keimyung University Dongsan Medical Center, and Taegu Catholic Medical Center. This study contained the clinicostatistical analysis of the frequency in relation to sex, age, locations, chief complaints, duration, radiographic findings, recurrence, teeth, and treatment methods. The results were as follow : 1. Of a total of 128 benign odontogenic tumors, ameloblastomas(57 cases; 44.5%) and odontomas (44 cases ; 34.4%) mostly occupied. The other types of lesions were 8 calcifying odontogenic cysts, 7 benign cementoblastomas, 4 myxomas, 3 adenomatoid odontogenic tumors, 2 calcifying epithelial odontogenic tumors, 2 ameloblastic fibro-odontomas, and 1 odontogenic fibroma. 2. In age and sex distribution, benign odontogenic tumors occured slightly more often in males(53.9%) than females(46.1%) and the majority of cases(79.7%) were found during 2nd, 3rd, and 4th decade. 3. There was a predilection for mandibular lesions(mandible-maxilla ratio, 2.6 : 1). 4. The most common chief complaint was swelling(29.7%) and in respect to duration, the cases less than 1 year(50.0%) mainly appeared. 5. There were 7 cases(13.0%) of recurrence on ameloblastoma and there was no recurrence in the others. 6. In Ameloblastoma It commonly occured during 3rd and 4th decade(59.6%) and mean age was 30.2 years. The majority of cases were occurred in mandible(96.5%) , especially mandibular molar and angle area(71.9%). The most common chief complaint was swelling(47.4%) and in respect to duration, the cases less than 1 year(52.6%) mainly appeared. In relation to teeth, there were resorption of root(52.6%), displacement of teeth(31.6%), and in relation to impacted teeth(43.9%). There was higher recurrence rate in the cases by conservative treatment(14.7%) than radical treatment(10.0%). As regards radiographic findings, conservative treatments were prevalent in the cases of unilocular type(85.7%) as compared with multilocular type(48.5%). and there was higherrecurrence rate in the cases of multilocular type(18.2%) than unilocular type(4.8%). As regards the type of treatment in relation to age, conservative treatments were prevalent in patients younger than 20 years of age. 7. In Odontomas It commonly occured during 2nd decade(50.0%) and in maxillary anterior teeth(40.9%). The most common chief complaint was delayed retention and permanent impaction of teeth(72.7%), and most frequently associated with impacted teeth(79.5%).

A Case of Small Cell Lung Cancer Metastasis to the Gingiva (치은으로 전이된 소세포 폐암 1예)

  • The incidence of lung cancer and its mortality rate are increasing in Korea. At the time of diagnosis, 40% patients of lung cancer patients had metastatic lesions. The common metastatic sites are the contralateral lung, bone, liver, adrenal gland and the brain. Metastasis to oral mucosa is rarely encountered in lung cancer and metastasis to the gingiva is more uncommon. Approximately 1% of malignant carcinomas in the oral cavity are the result of metastases, and 10-25% of metastatic cancers originate from lung cancer. Clinically metastatic gingival lesions are benign including hemangioma, pyogenic granuloma, giant-cell granuloma or a peripheral fibroma. Often metastases to the gingiva are diagnosed too late and by the time they are detected, they have metastases to other organs. Here we report a case of small cell lung carcinoma that had metastased to the gingiva with review of relevant literature.

Arthroscopic Excisional Debridement of Cyst-like lesion in juxta-articular Knee Joint (관절경을 이용한 슬관절주위 낭종유사 병변의 절제제거술)

  • Purpose : A cyst-like lesion within and around the knee joint is very rare, with very few articles available in the literature. The aim of this study is to evaluate effectiveness of arthroscopic treatment of cyst-like lesion within and around the knee joint which reported rarely. Materials and Methods : We are reporting 14 cases of cyst-like lesion around the knee joint. Our cases include 3 ganglion cyst in infrapatellar fat pad, 1 fibroma, 1 giant cell tumor and 1 epidermoid cyst which have not yet been reported. The diagnosis of cyst-like lesion in Juxta-articular knee Joint was made only by MRI study and we confirmed pathology. Male was 9, female was 5 cases, average age was 24(11-43)year old. Follow up was average 45(12months-8years)months. Minimum follow up was 12 months. Results : All the patients were treated arthroscopic excisional debridements successfully. All the cases were excellent result in last follow up by functional criteria in Lysholm knee score average 98.9(95-100). All examimation was normal except 3 cases in last follow up. But all patients satisfied in arthroscopic treatment. Conclusion : Arthroscopic excisional debridement of the cyst-1ike lesion in juxta-arthcular knee joint is excellent method, but long term follow up needed in the future.

Expression of vascular endothelial growth factor receptors in tumor and stromal cells of tongue squamous cell carcinoma

  • This study was to evaluate the expression of vascular endothelial growth factor receptors (VEGFRs) in tumor and stromal cells of tougue squamous cell carcinoma (SCC). We also wanted to characterize the differences, from the angiogenic aspect, between cancer-associated stromal cells and non-malignant stromal cells. Paraffin-embedded tumor specimens from eleven patients with tongue SCCs were studied. Immunohistochemical staining for VEGFR-1,-2, and -3 was performed on the tumor cells, stromal fibroblasts and tumor-associated macrophages of the specimens. The expression of all 3 receptors was detected in the tumor cells themselves of the biopsy specimens. All 3 receptors were also expressed on stromal cells, except that VEGFR-2 was not expressed in stromal fibroblasts. In radical excision specimens, the staining intensity for VEGFR-1, -2 in the tumor cells and VEGFR-1,-3 in the tumor-associated macrophages was significantly lower than that in the biopsy specimens (P < 0.05). By using the general marker of fibroblast and macrophage, 5B5 and CD68, respectively, we performed double immunofluorescence staining for 5B5 and each VEGFR in the stromal fibroblasts and for CD68 and each VEGFR in the tumor-associated macrophages of the radical excision specimens. We used 4 cases of fibroma and 4 cases of chronic inflammation tissue as the controls. It was found that only each marker was expressed in the control group, however, 5B5/VEGFR-1 and 5B5/VEGFR-3 in the stromal fibroblasts, and CD68/VEGFR-1 and CD68/VEGFR-3 in the tumor-associated macrophages were double stained in the radical excision specimens. Although our study used small number of specimens, the results of our study showed that in tongue SCC, in association with the angiogenesis, the stromal cells showed the activated phenotype and this was different from the nonmalignant stromal cells.

Clinical Experiences for Primary Cardiac Tumors (원발성 심장 종양의 임상적 고찰)

  • Primary cardiac tumors are known to be rare. We studied the surgical results for primary cardiac tumors. Material and Method: Between August 1980 and December 2003, we classified 86 patients who had operation for primary cardiac tumors in our center into 3 groups; myxoma, nonmyxoma benign tumors, and malignant tumors. The mean age was $44.3\pm20.8$ years and 59 patients $(66.3\%)$ were female. In postoperative pathologic diagnosis, there were 81 cases $(94.2\%)$ of benign tumors in which myxoma was the most common tumor $(70\;cases,\;78.7\%);$ 5 fibroma $(5.6\%)$, 3 rhabdomyoma $(3.4\%)$, and 5 malignant tumors $(5.8\%)$. Result: $86.4\%$ of benign tumor was myxoma and the mean age was $50.4\pm15.4\;(range\;7\~80)$ years. Tumor was more common in females (49 cases) and most common preoperative symptom was dyspnea $(62.9\%)$. 57 cases were located at left atrial septum and only one case, which was located at right ventricular septum, was resected incompletely. There were no hospital deaths and one patient had mitral valve replacement on the first operative day due to newly developed postoperative mitral regurgitation. The mean follow up period was $109.3\pm71.8$ months and there was no evidence of recurrence in this period. 11 cases $(12.8\%)$ were non myxoma benign tumors; 5 fibromas, 3 rhabdo-myomas, etc. There were two hospital deaths and the causes of death were fungal endocarditis and hypoxia. There were no reoperations in nonmyxoma benign tumors. Malignant tumors were in 5 cases $(5.8\%);$ undifferentiated sarcoma in 2, rhabdomyosarcoma in 1, etc. Although there were no hospital mortalities, 3 patients who were followed up died from complications of tumors. Conclusion: Myxomas showed very excellent prognosis after complete resection and nonmyxoma benign tumors showed relatively good results for relief of symptoms. Surgery helped to relieve symptoms for malignant tumors, but the prognosis was poor.
