학령기 입원아동의 병원관련 공포에 관한 탐색연구 (Identification and Measurement of Hospital-Related Fears in Hospitalized School-Aged Children)

  • When children are admitted to hospital, they have to adapt to new and unfamiliar stimuli. Children may respond with fear to stimuli such as pain or unfamiliar experiences. One goal of nursing is to help hospitalized children to adapt effectively to their hospital experience. Accordingly, nurses need to assess childrens' fears of their hospital experience to contribute to the planning of care to alleviate these fears. The problem addressed by this study was to identify and measure hospital-related fears(hereafter called HRF) in hospitalized school-aged children. The study was conceptualized with Roy's model. A descriptive qualitative approach was used first, followed by a quantitative approach. This study was conducted from November 30, 1989 to January 12, 1991. The sample consisted of 395 hospitalized school-aged children selected through an allocated sampling technique in nine general hospitals. The HRF questionnaire (three point likert scale ) was developed by a delphi technique. The data were analyzed by an SAS program. Factor analysis was used for the examination of component factors. Differences in the HRF related to demographic variables were examined by t-test, analysis of variance and the Scheffe test. The crude scores of the HRF scale were transformed into T- scores to calculate the standard scores. The results included the following : 1. Forty-four items were derived from 188 statements identifying the childrens' hospital-re-lated fears. These items clustered into 14 factors, fear of injections, operations, bodily harm others' pain, medical rounds, physical examinations, medical staff, disease process, blood and X-rays, drugs and cockroaches, tests, harsh discipline from parents or staff, being absent from school, and separation from family. The 14 factors was classified into four categories,'pain','the unfamiliar','the un-known' and 'separation'. 2. The reliability of the HRF instruments was .92(Cronbach's alpha). In the factor analysis, Cronbach's alpha coefficients for the 14 factors ranged from .84 to .86 and Cronbach's alpha coefficients for the four categories ranged from .70 to .84. Pearson correlation coefficient scores for relationships among the 14 factors ranged from ,11 to .50, and among the four categories, from ,44 to ,63, indicating their relative independence. 3. The total group HRF score ranged from 45 to 130 in a possible range of H to 132, with a mean of 74.51. The fears identified by the children were, in order, injections, harsh discipline by parents or staff, bodily harm, operations, medical staff, disease process, and medical rounds ; the least feared was others' pain. The fear item with the highest mean score was surgery and the lowest was examination by a doctor. HRF scores were higher for girls than for boys, and for grade 1 students than for grade 6 students. HRF scores were lower for children whose fathers were over 40 than for those whose fathers were in the 30 to 39 age group, and whose mothers were over 35 than for those whose mothers were in the 20 to 34 age group. HRF scores were lower when the mother rather than any other person stayed with the child. The expressed fear of pain, the unfamiliar, the un-known and of separation directs nurses' concern to the threat felt by hospitalized children to their concept of self. This study contributes to the assessment of fears of hospitalized children and of stimuli impinging on those fears. Accordingly, nursing practice will be directed to the alleviation of pain, pre-admission orientation to the hospital setting and routines, initiation of information about procedures and experiences and arrangments for mothers to stay with their children. Recommendations were made for further research in different settings and for development and testing of the instrument.

Children's fears and future prospects

  • This paper outlines the findings of a consumer survey conducted in 1996 and 2001 by the University of Bonn, Germany, across 15 European countries. The survey involved a sample of 3,300 respondents in 1996 and around 11,000 respondents in 2001, throughout all 15 EU countries. Children and adolescents, between the ages of 10 and 17, were surveyed about their consumption habits and their attitudes towards the environment. The paper outlines the key findings on 'Children's fears and future prospects'. Children are exposed to a great mass of information, not only derived from the media but also from what they experience directly in their everyday-life. Some of this information translates into worries that adopt a clearer (more realistic) form when children become adolescents, as they might be easily involved in those events, and the understanding of the processes that may cause or continue them is deeper. Today's children and adolescents are under more pressure, since the social/political/economical spectrum is constantly changing. Change produces anxiety, fear of what will result from these transformations, as it implies that the social order will be reorganized. This paper reports on a long term comparative study among children and adolescents throughout the European Union, analyzed by country, age group and gender. It draws different preoccupations of young generations and consequent future expectations. It concludes by considering the influence of social and political organizations, as well as the family, on children and adolescents, as these might help to sustain a positive attitude towards various future events.

청주시 일부 중학생들의 구강진료와 관련된 공포에 관한 조사 (The Survey of Fear Associated with Dental Treatment in Some Middle School Students in Cheongju)

  • The objective of this study was to examine possible causes of fear of dental treatment in middle school students. The subjects were a total of 347 students who were selected by convenience sampling from among the students of the middle schools located in Cheongju, Korea. They were administered a self report survey. The results showed that overall dental fear was higher among female students than male students. With respect to associative dental fears, they felt fear the most when they waited sitting on the dental chair. In fears of specific dental stimuli, needles were the most fearful stimulus. In the areas of fears of specific dental stimuli and overall dental fear, the subjects with indirect pain experiences felt fear more. There exists a need to develop dental health care programs that will help to manage and decrease fear of dental treatment.

범죄에 대한 두려움이 민간경비 선택에 미치는 영향에 관한 연구 (Study on the effect of fear of crime on the selection of private security)

  • 인간은 경제적 요인, 물리적 환경 요인, 사회적 환경요인, 미디어에 의한 영향 등 이루 셀 수 없을 만큼 많은 요인에 의해 삶의 활동에 영향을 받는다. 특히, 범죄 발생으로 인한 범죄에 대한 두려움의 발생은 인간의 활동영역을 축소시키는데 영향을 주고 있다. 범죄에 대한 두려움은 직접적인 범죄피해경험, 주변의 풍문 혹은 미디어에 의한 간접적인 범죄피해 사실 인지, 주변 환경의 무질서 목격 등으로 인하여 범죄에 대한 두려움이 발생하게 된다. 따라서, 범죄에 대한 두려움을 감소시키기 위해서는 범죄를 예방하여 안전한 환경을 조성해야 하지만, 사회가 발전함에 따른 범죄발생의 증가에 비해 경찰의 범죄예방 활동은 그에 따라가지 못해 문제가 발생하고 있다. 이러한 문제를 해결하기 위해 민간경비 활동을 통해 범죄예방을 하고 있는데, 실제 민간 경비를 선택함에 있어서 범죄에 대한 두려움이 영향을 주는 요인에 대해서 이 연구를 통해 살펴보기로 한다. 연구의 목적을 달성하기 위해서 이 연구에서는 2011년 8월 한달 동안 서울과 대구의 '담장허물기' 운동을 실시하여 자연적 감시를 확보한 단독주택 100호를 무작위로 선정하여, 단독주택 거주자를 설문을 통해 범죄에 대한 두려움이 민간경비 선택에 미치는 영향에 대해서 살펴보았다. 그 결과, 범죄에 대한 두려움에 영향을 주는 요인인 직접적이 범죄피해경험, 간접적인 범죄피해경험, 무질서 중에서 직 간접적인 요인에 의한 범죄두려움은 민간경비 선택에 영향을 준다고 나타났다.

The COVID-19 Pandemic: Fears and Overprotection in Pediatric Patients with Inflammatory Bowel Disease and Their Families

  • Purpose: The coronavirus disease 2019 (COVID-19) pandemic has influenced the lives of people worldwide. Little is known about the effects of the COVID-19 pandemic on the behavior and fears of pediatric patients with inflammatory bowel disease (IBD) and their families. We conducted a survey to determine the COVID-19 exposure, related perceptions, and information sources; medication compliance; and patients' and parents' behaviors, fears, and physician contact. Methods: An anonymous cross-sectional survey of pediatric patients with IBD and their parents at one pediatric gastroenterology unit of a university medical center was performed. Results: A total of 46 pediatric patients with IBD and 44 parents completed the survey. Parents of pediatric patients with IBD had high fear of their children becoming infected with severe acute respiratory syndrome coronavirus 2. They perceived schools as the most hazardous environment, whereas the children did not. Half the pediatric patients with IBD feared infection. Patients and parents felt sufficiently informed about COVID-19. The primary source of guidance for pediatric patients was their parents (43%), followed by television and social media, whereas the parents mainly consulted internet news websites (52.2%), television, and public health institutes. Pediatric patients with IBD adhered to their prescribed medication. They also showed cautious behavior by enhancing hand hygiene (84%) and leaving the house less frequently than before. However, in-person medical visits remained favored over video consultations. Conclusion: Although parents expressed overprotective concerns, both parents and pediatric patients with IBD are coping well with the COVID-19 pandemic. IBD-relevant information should be actively conveyed.

  • 저자 등은 개원 치과의원에서 하악 매복지치 발치중 유발된 불안장애와 과환기증 및 발치 창상 주위 출혈과 동통을 보였던 57세 여환을 신속한 정신안정법 적용하에 수액 약물요법과 외과적 처치(잔존 치근 남긴채 창상 지혈, 봉합, 배액술 등)로 관리하여 양호한 경과를 치험하면서, 평소 진료시 정신과적 장애문제에 큰 관심을 가져야 하며, 특히 수술시 스트레스 감소법에 더욱 유념하게 되었다.

  • Tremors are trembling movements and are seen in association with alcoholic intoxication, certain drugs, thyrotoxicosis, multiple sclerosis, hysteria, and nervous tension. Dental fears, such as, pain, drill, unknown, dependency, helplessness, mutilation and oral change, induce the neuroendocrine response (release of epinephrine and norepinephrine, etc). The clinical manifestations of epinephrine or other vasopressor overdose include : anxiety, tenseness, restlessness, throbbing headache, tremor, perspiration, weakness, dizziness, pallor, palpitation and respiratory difficulty. Signs of local anesthetic overdose appear clinically whenever the anesthetic level in the blood rises to an appropriate level in an individual. The clinical signs of moderate overdose levels include : talkativeness, apprehension, excitability, slurred speech, tremor and muscular twitching. This is a case report about the severe tremor care after the endodontic treatment under right mandibular block anesthesia in a 56-years old female patient.

무용전공 여고생에서 섭식장애 인지와 비만도에 따른 체중조절 태도 및 식이 자기효능감, 섭식행동 (The Weight Control-Related Attitudes, Dietary Self-Efficacy and Eating Behaviors according to the Perception of Eating Disorder and Obesity Index in Dancing Major High School Girls)

  • This study was performed in order to compare the differences in dietary self-efficacy and eating behaviors as well as the relationship of weight control attitudes according to eating disorder perception and obesity index for high school girls majoring in dance. Almost half (45.5%) of the subjects perceived eating disorders, and the rate of underweight group was 45.7%. Forty-four percent of the group that perceived eating disorders were in modern dance, while 55.0% of the group that did not perceived eating disorders were in Korean dance (p < 0.05). For the obesity index, a similar rate was shown between the different types of dance for the underweight group, but in the normal group, 54.5% were in Korean dance (p < 0.05). Most (82.0%) of the group that perceived eating disorders had fears on obesity, while only 53.3% of the non-perceiving group had fears on obesity (p < 0.01). Over half (58.3%) of the underweight group had fears on obesity, while 77.2% of the normal group showed fear in that (p < 0.05). For necessity of weight loss to do dance performances, 25.0% of the underweight group and 57.9% of the normal group need that (p < 0.05). Most of the group that perceived eating disorders (92.0%) answered that they had experience in very low calorie diets, while only 40.0% of the non-perceiving group had experience (p < 0.001). For necessity of calorie education, 42.0% of the group that perceived eating disorders and only 20.0% of the non-perceiving group answered is needed (p < 0.05). For eating behavior, the group that perceived eating disorders had lower total scores compared to the non-perceiving group (p < 0.001), The group that perceived eating disorders showed lower scores for restraint eating (p < 0.01) and emotional factors (p < 0.001) except by external eating. This study showed that high school girls majoring in dance have high interest in weight control and problems with wrongful body images and eating behaviors, as well as need for calorie education were suggested. Thus, it is necessary to develop and execute a proper weight control education program for the subjects. (Korean J Community Nutrition 14(4) : 383${\sim}$391, 2009)

성인의 예방처치경험에 따른 스케일링 공포도 (Level of fear on scaling according to preventive treatment experiences in the adults)

  • Objectives: The purpose of this study is to investigate the effect of preventive treatment experience on scaling fear level. Methods: A total of 259 adults who had visited the dental clinic were analyzed. The results were summarized as follows. Statistical analysis of the collected data was performed using the SPSS WIN 20.0 statistical program. The general characteristics, scaling experience, and the characteristics of the subjects were analyzed. Frequency of scaling according to general characteristics was analyzed by independent sample t-test, Scaling fears according to treatment experience were tested by t-test. Correlation analysis was performed for scaling fears according to the reliability of dental hygienist. Regression analysis was carried out to investigate factors affecting scaling fear. Results: Level of fear during scaling was higher in females (3.03) than in males (2.54) and that after scaling was scored higher in females (2.68) than in males (2.34) by general characteristics (p<0.001). The adults who were not healthy in oral health showed the highest levels of fear during (3.29) and after (3.00) scaling by oral health status (p<0.001). Adults who had brushing education experience showed lower fear level than those who did not after scaling (p<0.01) according to the experiences of preventive treatments. With respect to the correlation of trust level to the dental hygienists with the scaling fears, it showed higher in the trust level (-0.688) as lower level of scaling fear (-0.642) in the scaling (p<0.01). Confidence level of dental hygienist (-0.661), brushing education experience (-0.121), and oral health status (-0.121) were influenced upon the regression analysis. Conclusions: Oral health education and dental hygiene education are increasing. It is thought that active efforts are needed to promote and maintain oral health.

기어가공 CELL 개발 및 평가 (The Development and Evaluation of Gear Machining Cells)

  • This paper presents FMC prototypes developed to propagate manufacuting automation for small and medium sized enterprises in Korea. The system consists of two cells used to machine oil pump drive gears, turing cell and hobbing cell. Characteristics and processes of the fears are anaylzed for the cell design. The system configuration and functions of individual component machines are described. Furthermore, the developed system is compared with the current manual system in terms of manpower and cost reduction and productivity increase.
