The Analysis of Non-pharmacological Intervention Study for Nonalcoholic Fatty Liver Disease: Intervention Types and Measurement Parameters (비알코올성 지방간질환에 대한 이해와 비약물적 중재 및 측정지표에 대한 문헌분석)

  • Purpose: This study was intended to analyze the types and measurement parameters of non-pharmacological interventional studies for nonalcoholic fatty liver disease (NAFLD). Methods: NAFLD related literatures were systematically reviewed. The existing literatures were searched electronically using the data base of PubMed, a Medline data base of the National Library of Medicine with the key words of nonalcoholic fatty liver disease, NAFLD, nonalcoholic steatohepatitis, and NASH. The criteria for inclusion in this review were 1) non-pharmacological intervention, 2) human, 3) English. Finally, 20 articles were included in the review. Results: The major findings of this study were as follows: 1) the types of non-pharmacological intervention were exercise (35%), caloric restriction (30%), and lifestyle modification with combination both of exercise and caloric restriction (35%), 2) Almost all studies adopted various measurement parameters derived from pathophysiological mechanism-based biomarkers such as anthropometric indices, biochemical parameters, body fat mass, and liver biopsy results. Conclusion: Non-pharmacological interventions have been reported to be effective to improve NAFLD status, and many objective biomarkers confirmed supported these findings. Therefore, the development of nursing interventions for NAFLD subjects is needed and the consideration of using mechanism-based biomarkers is suggested to verify nursing outcomes objectively.

Effects of Gamisaenggan-tang on High Fat Diet-induced Nonalcoholic Fatty Liver Disease (가미생간탕(加味生肝湯)이 고지방식이로 유발된 지방간에 미치는 영향)

  • Objectives : The aim of this study was to investigate the effects of Gamisaenggan-tang on high fat diet induced nonalcoholic fatty liver disease. Methods: Rats were randomly divided into four groups. The Normal group was fed a solid diet containing 10% fat. The Gamisaenggan-tang (GS) group was fed a solid diet containing 10% fat and Gamisaenggan-tang (90mg/100g body weight). The Control group was fed a solid diet containing 60% fat. The HFD-Gamisaenggan-tang (HFD-GS) group was fed a solid diet containing 60% fat and Gamisaenggan-tang (90mg/100g body weight). Six weeks later, rats body weight, liver weight, serum ALT, GGT, ALP levels were measured. Histological findings (Oil red O staining), hepatic triglyceride, TNF-${\alpha}$, and TGF-${\beta}$ levels in the liver tissue were studied. Results: Average body weight of the HFD-GS group was significantly less than that of the Control group. There were no significant liver weight differences among each group. The GGT levels of the HFD-GS group were significantly less than those of the Control group. However, there were no significant differences in the ALT or ALP levels among the groups. TNF-${\alpha}$ protein production assessed by western blot analysis was reduced by Gamisaenggan-tang. Greater fat accumulation was observed in the liver tissue of the Control group than in the HFD-GS group, which means the Gamisaenggan-tang has an inhibitory effect on the accumulation of fat in the liver. Conclusion : The results suggest that Gamisaenggan-tang can be potential candidate for the treatment of nonalcoholic fatty liver disease in clinics.

Influence of Gibangganbang on Ethanol-Induced Fatty Change and Regeneration Ability of Liver (지방간방(脂肪肝方)이 Ethanol로 유도(誘導)된 지방간(脂肪肝) 및 간재생능(肝再生能)에 미치는 영향(影響))

  • This study was to investigate the effects of Gibangganbang on the liver injury induced dimethylnitrosamine, CCl4, ethanol and partial hepatectomy. Hydroxyproline, hepatic fibrosis, hepatic inflammatory cell infiltration, fatty value, lipidoperoxide levels, glutathione levels, mitotic index, contents of protein in the serum and liver tissues were measured and observed. The results obtained were as follows. 1. The increasing level of hydroxyproline volume induced by dimethylnitrosamine in mice was decreased by the oral administration of Gibangganbang. 2. The degree of hitological fibrosis and hepatic inflammatory cell infiltration induced by $CCl_4$ was decreased by the oral administration of Gibangganbang. 3. The degree of fatty value and the increasing level of lipidoperoxide in liver tissues was decreased by the oral administration of Gibangganbang. 4. The level of glutathione in liver tissues was increasing by the oral administration of Gibangganbang. 5. The increasing level of microsomal lipidoperoxide in vitro assay was decreasing by the oral administration of Gibangganbang. 6. The increasing level of the mitotic index, weight of liver, contents of protein, RNA and DNA synthesis of the liver tissues after partial hepatectomy was activated by the oral administration of Gibangganbang. These results suggest that Gibangganbang not only inhibits liver cirrhotic change and ethanol-induced fatty change but also activates antioxidant enzymes and regeneration ability ocf liver.

Herbal formula MJY2018 protects against Alcohol-induced liver injury mice model (알코올 유발 간 손상 마우스 모델에서 복합 추출물 MJY2018의 간 보호 및 항산화 효과)

  • Objectives : This study investigated the liver-protective effects of MJY2018, a Herbal formula, against alcoholic fatty liver disease and anti-oxidative effects. Methods : Its effects were investigated in an alcoholic fatty liver disease model in male C57BL/6 mice, which were fed Lieber-DeCarli liquid diet containing ethanol. MJY2018 (100 and 200 mg/kg bw/day) or silymarin (50 mg/kg bw/day) were orally administered daily in the alcoholic fatty liver disease mice for 16 days. Results : The results indicate that MJY2018 promotes hepatoprotection by significantly reducing aspartate transaminase (AST) and alanine transaminase (ALT) levels as indicators of liver damage in the serum. Furthermore, MJY2018 reduced accumulation of triglyceride and total cholesterol, increased levels of superoxide dismutase (SOD) and glutathione (GSH) in the livers of the alcoholic fatty liver disease mice model. Additionally, it improved the serum alcohol dehydrogenase (ADH) activity. Conclusions : These results indicate that MJY2018 were effective in improving and protecting oxidative stress and alcoholic liver disease.

Fatty Liver Analysis through Quantitative Measurement Study of Ultrasonography Images (초음파 검사 영상의 정량적인 측정 연구를 통한 지방간 분석)

  • This study attempted to find out the degree of agreement between ultrasound image findings along with analysis of attenuation index and scatter distribution index values within tissues through quantitative measurement analysis using liver ultrasound images. From August 2022 to October 2022, liver ultrasound was performed on 45 patients who were suspected of having fatty liver and who received a prescription for liver ultrasound. As a result of the study, as a result of analyzing the agreement between the ultrasound image findings and the tissue attenuation index, the Kappa value was 0.82 (p<0.05), showing a very high agreement between the two examination methods. In addition, as a result of the agreement analysis between the ultrasound image findings and the scatter distribution index in the tissue, the Kappa value was 0.642 (p<0.05), showing high agreement between the two examination methods. At the time of fat liver prediction, the use of liver ultrasound findings and quantitative ultrasonography techniques, such as intra-tissue attenuation index and intra-tissue scatter distribution index, may be helpful in determining the degree of progression of fatty liver patients.

The Correlation of Sonographic Finding of Fatty Liver with Hematologic Examination and Body Fat Percentage (초음파상 지방간과 혈액학적 검사 및 체지방률과의 상관관계)

  • Ultrasonography has been used as a basic examination of a medical check up for prevention and diagnostics of diseases. Even the person who has no particular subjective symptoms can have a variety of diseases. Especially fatty liver is found in many cases. In this study, we tested 3582 persons who are in between the ages of 15 to 81 and observed that 1390 persons had fatty liver while 2192 persons are normal. We classified the grade of fatty liver and compared their life styles with the results of liver function test and BMI. The results are as follows. Ratio of the subjects who had a fatty liver is 38.8%. Male and female ratio was 46.2% and 24.2%. On the correlation among the fatty liver, the body mass index and the body fat%, the average value of body mass index and body fat% were significantly higher in the group of the fatty liver than in those of the normal liver. The influence of the related factor and the correlation on the fatty liver was shown that it was more related with the order of age, body mass index, triglyceride, ALT, body fat%, sex, HDL-Cholesterol, LDL-Cholesterol, and GGT. The result of the ultrasonography carried out for the purpose of regular health check up indicates that even the 38.8% of those who was diagnosed as normal condition could have the fatty liver and have possibility of other diseases. Therefore, if there are any troubles related to liver function and lipid through hematologic examination or when practicing follow-up study with ultrasonography concerning the correlation relation between the body fat% and dietary preference, alcohol consumption and exercise, the ultrasonography is definitely useful for prevention and treatment of diseases.

Effects of the Feeding Platycodon grandiflorum and Codonopsis Ianceolata on the Fatty Acid Composition of Serum and Liver in Rats (도라지 및 더덕 첨가식이가 흰쥐의 혈청 및 간장의 지방산 조성에 미치는 영향)

  • This study was designed to observe the effects of the feeding Platycodon grandiflorum, Codonopsis Ianceolata, perilla oil and safflower oil on the fatty acid composition in serum and liver of dietary hypercholesterolemic rats. Experimental groups mixed with 5% cellulose +10% lard (group 1, control group), 2% cholestyraemine +10% lard (group 2), 5% C. ianceolata+ 10% perilla oil (group 3), 5% P. grandiflorum+10% perilla oil (group 4), 5% C. ianceolata+10% safflower oil (group 5) and 5% P. grandiflorum+10% safflower oil (group 6) were administered to the male rats of the Sprague Dawley for 3 weeks. In the fatty arid compositions of serum total fatty arid, phospholipid, triglyceride and cholesteryl ester, the linoleic acid content was most in the PUFA and it was major fatty acid. Particularly, the other components except the phospholipid fraction in serum lipids were influenced by the fatty acid composition of the test lipids from the fact that linoleic acid content was remarkably more in the groups 5 and 6. In the fatty arid compositions of liver total fatty acid, phospholipid, triglyceride and cholesteryl ester, the linoleic acid content was more in the PUFA and so it was major fatty acid. Particularly, the arachidonic and eicosapentaenoic acid contents of phospholipid fraction in liver lipids were rather more. from the above research, fatty arid composition of serum and liver lipid components were influenced by the fatty acid composition of the test lipids from the fact that linoleic acid takes up the highest percentage.

Overview for Prevalence and Pathology of Non-Alcoholic Fatty Liver Disease (비알콜성지방간의 유병율과 병리기전에 대한 문헌적 고찰)

  • Objectives : Non-alcoholic fatty liver disease (NAFLD) is known to be increasing and becoming a health-related issue worldwide. This study aimed to analyze its prevalence and characterize NAFLD. Methods : NAFLD-related papers were surveyed via PubMed and in Korean medical journals, and then the prevalence and pathology were reviewed. Results : The prevalence of NAFLD in the general population is around 10~30% worldwide. The prevalence of NAFLD in Korea is estimated as 15~30%, which is higher than in China and Japan. The most important etiological-factors of NAFLD include central obesity resulting from excessive calorie intake and less physical activity, which lead to adiponectin hypoactivity and insulin resistance. The Oriental medicine view point of NAFLD pathology is phlegm-dampness by dysfunction of free flow in liver. Conclusions : This study provided an overview of the prevalence and pathology of NAFLD, and can support the development of a strategy for traditional Korean medicine-based prevention or treatment of NAFLD.

New Perspectives in Pediatric Nonalcoholic Fatty Liver Disease: Epidemiology, Genetics, Diagnosis, and Natural History

  • Nonalcoholic fatty liver disease (NAFLD) is the most common cause of chronic liver disease in children. The global prevalence of pediatric NAFLD from general populations is 7.6%. In obese children, the prevalence is higher in Asia. NAFLD has a strong heritable component based on ethnic difference in the prevalence and clustering within families. Genetic polymorphisms of patatin-like phospholipase domain-containing protein 3 (PNPLA3), transmembrane 6 superfamily member 2, and glucokinase regulatory protein (GCKR) are associated with the risk of NAFLD in children. Variants of PNPLA3 and GCKR are more common in Asians. Alterations of the gut microbiome might contribute to the pathogenesis of NAFLD. High fructose intake increases the risk of NAFLD. Liver fibrosis is a poor prognostic factor for disease progression to cirrhosis. Magnetic resonance spectroscopy and magnetic resonance proton density fat fraction are more accurate for steatosis quantification than ultrasound. Noninvasive imaging methods to assess liver fibrosis, such as transient elastography, shear-wave elastography, and magnetic resonance elastography are useful in predicting advanced fibrosis, but they need further validation. Longitudinal follow-up studies into adulthood are needed to better understand the natural history of pediatric NAFLD.

Improvement Effect of Fermented Silkworm (Bombyx mori L.) Powder against Orotic Acid-induced Fatty Liver in Rats (발효누에분말 투여가 orotic acid 유발 흰쥐에 미치는 지방간 개선 효과)

  • Current study was to investigate the potential effects of silkworm, Bombyx mori L. fermented with Bacillus subtilis (BFSP) and Aspergillus kawachii (AFSP) at the 5% (w/w) levels in Sprague-Dawley rats, which was intoxicated with 1% (w/w) orotic acid (OA) for 10 days. The rat group administered silkworm powder showed improvements in fatty liver condition. Consumption of fermented silkworm powder reduced triglyceride concentrations in the liver tissues and serum and increased the serum lipid concentrations to normal levels, thereby aiding in improving fatty liver conditions. These effects were more pronounced in the BFSP than that in SP or AFSP in orotic acid-induced hepatotoxicity and oxidative stress. Based on these results, fermented silkworms are considered to be a material with significant potential for development into a functional health food that can improve fatty liver conditions.
