일부 농촌지역 비닐하우스 재배자들의 농부증 실태와 관련요인 (A Study on Farmer's Syndrome and Its Risk Factors of Vinyl House Worker and Farmer in a Rural Area)

  • 충청남도 청양군의 2개면 14개리에 거주하는 비닐하우스 재배자 177명과 일반 농민 213명을 대상으로 1998년 2월 24일부터 3월 15일까지 피로도, 주관적 건강상태, 농부증과 이의 위험요인을 파악하기 위하여 설문조사를 실시한 결과는 다음과 같다. 1. 피로도는 비닐하우스 재배자와 일반 농민간에 차이가 없었으며, 성별로는 여자가 남자보다(p<0.01), 교육수준이 낮을수록(p<0.01), 8시간 미만의 수면시간 (p<0.05), 비흡연자, 비음주자에서 피로도가 높았고, 연령, 식생활습관, 체질량지수는 관계가 없었다. 작업환경은 비닐하우스 재배자에서 총농사기간(r=0.159, p<0.05), 일반 농민은 총경작면적(r=-0.143, p<0.05)과 농작업동반가족수(r=-0.136, p<05)에서 약한 상관관계를 보였다. 2. 주관적 건강상태는 비닐하우스 재배자와 일반 농민간에 차이가 없었으며, 성별로는 여자가 남자보다(p<0.01), 교육수준이 낮을수록 (p<0.01), 8시간 미만의 수면시간(p<0.05), 비흡연자에서 주관적 건강상태가 좋지 않았으며, 연령, 음주, 식생활습관, 체질량지수, 작업환경은 관련성이 없었다. 3. '농부증 있음'은 비닐하우스 재배자 49.2%, 일반 농민 52.1%로 비슷하였으며, 성별로 비닐하우스 재배자와 일반 농민 모두 여자에서 57.0%, 58.8%로 남자 37.1%, 40.3%보다 높았다(p<0.01). 교육수준은 비닐하우스 재배자의 경우 무학 58.1%, 초등학교 50.0%, 중졸 이상 36.1%로 학력이 낮을수록 높았고(p<0.01), 일반 농민도 각각 56.3%, 51.0%, 33.3%로 같은 경향이었다(p<0.01). 흡연과 음주여부는 비닐하우스 재배자의 경우 비흡연군과 비음주군에서 높았으나, 일반 농민에서는 차이가 없었고, 체질량지수도 비닐하우스 재배자의 경우 세장형 57.1%, 비만형 63.9%로 정상형 42.5%보다 높았으나(p<0.05), 일반 농민은 차이가 없었다. 4. 농부증의 위험요인에 대한 다변량 로지스틱 회귀분석에서 성별, 수면시간이 중요한 요인이었으며, 남자를 기준하였을 때 여자의 교차비는 2.53(p<0.01), 수면시간 8시간 미만을 기준하였을 때 8시간 이상은 0.74로(p<0.01), 여자에서 발생위험이 높았고, 수면시간이 8시간 이상일수록 발생위험은 낮아졌다. 5. 비닐하우스 재배지와 일반 농민에서 많이 호소하는 주관적 신체증상은 '등이 결리거나 허리가 아파서 일하기가 어렵다.' '팔과 다리가 몹시 쑤시고 아픈 일이 있다', '손발이 저리거나 찌릿찌릿 하다'. '팔과 다리에 통증이 있어서 불편하다' '손의 힘이 전에 비해 약해졌다'에서 과반수 이상이 있다고 응답하였으며, 비닐하우스 재배자에서 일반 농민보다 많은 증상은 '피부의 일부분이 검게 변하였다'. '습진이나 무좀 등이 피부질환이 있다', '치질로 고생한 일이 있다'였다. 6. 대응분석 결과 비닐하우스 재배자는 피부계 질환이 높게 관찰되었으며, 남자는 소화기계 장애나 기타 질환, 여성에서는 내분비질환, 근육계통의 질환이 많은 것으로 나타났고, 농부증도 일반 농민과 여자에서 상대적으로 높은 것으로 나타났다. 결론적으로 피로도, 주관적 건강상태, 농부증도 비닐하우스 재배자와 일반 농민간에 차이가 없었으며, 주로 농사와 노동일을 병행하는 여성에서 많고, 연령이 증가함에 따라 유병률도 증가하는 경향을 감안하면 추후 농촌의 노령환된 여성들에 대한 중점적인 관리가 필요하다. 또한 작업환경의 차이에 의하여 비닐하우스 재배자에서만 나타나는 미약하지만 특징적인 증상이 있으므로 이에 대한 지속적인 조사와 관리의 필요성이 있다.

  • Fibromyalgia syndrome is defined as a diffuse, aching musculoskeletal pain associated with multiple and discrete predictable tender points along with stiffness. A primary form associated without any disease is uncommon compared to the secondary diffuse fibrositis. We witnessed a case of a primary fibromyalgia syndrome. A 28-year-old woman was suffering from pain on posterior neck, right shoulder, upper back, left hand and arm for two years. She also complained of morning stiffness, fatigue, and headache. We treated her with stellate ganglion block with 0.25% bupivacaine, medication that included amitriptyline, TENS and superlizer. We also recommended aerobic exercise.

Deep sedation for dental treatment in a Down syndrome patient with Eisenmenger syndrome: A case report

  • Eisenmenger syndrome (ES) is characterized by pulmonary arterial hypertension and right-to-left shunting. The signs and symptoms of ES include cyanosis, shortness of breath, fatigue, hemoptysis, and sudden death. In patients with ES, it is important that the systemic and pulmonary circulations be properly distributed and maintained. General dental treatment is not known to be particularly dangerous. To control pain and anxiety, local anesthetics without epinephrine are usually recommended. However, in cases of difficulty of cooperation, general anesthesia for dental treatment makes the condition worse. In the present case, intravenous deep sedation with propofol and remifentanil was administered for behavioral management during dental treatment successfully.

  • Nociplastic pain by the "International Association for the Study of Pain" is defined as pain that arises from altered nociception despite no clear evidence of nociceptive or neuropathic pain. Augmented central nervous system pain and sensory processing with altered pain modulation are suggested to be the mechanism of nociplastic pain. Clinical criteria for possible nociplastic pain affecting somatic structures include chronic regional pain and evoked pain hypersensitivity including allodynia with after-sensation. In addition to possible nociplastic pain, clinical criteria for probable nociplastic pain are pain hypersensitivity in the region of pain to non-noxious stimuli and presence of comorbidity such as generalized symptoms with sleep disturbance, fatigue, or cognitive problems with hypersensitivity of special senses. Criteria for definitive nociplastic pain is not determined yet. Eight specific disorders related to central sensitization are suggested to be restless leg syndrome, chronic fatigue syndrome, fibromyalgia, temporomandibular disorder, migraine or tension headache, irritable bowel syndrome, multiple chemical sensitivities, and whiplash injury; non-specific emotional disorders related to central sensitization include anxiety or panic attack and depression. These central sensitization pain syndromes are overlapped to previous functional pain syndromes which are unlike organic pain syndromes and have emotional components. Therefore, nociplastic pain can be understood as chronic altered nociception related to central sensitization including both sensory components with nociceptive and/or neuropathic pain and emotional components. Nociplastic pain may be developed to explain unexplained chronic pain beyond tissue damage or pathology regardless of its origin from nociceptive, neuropathic, emotional, or mixed pain components.

임상간호사의 주요 건강문제와 직무 스트레스가 프리젠티즘에 미치는 영향 (The Effects of Major Health Issues and Job Stress on Presenteeism among Clinical Nurses)

  • Purpose: The purpose of this study was to investigate the effects of major health issues and job stress on presenteeism among clinical nurses. Methods: The investigator conducted a survey on 226 clinical nurses at a general hospital in Seoul from March 3 to April 15, 2017, and analyzed their responses. Results: The findings showed that job stress did not have a significant effect on the nurses' presenteeism. Fatigue (t=3.55,p<.001) impacted job loss, one of the subcategories of presenteeism, with an explanatory power of 12.1%. Premenstrual syndrome (t=-2.67,p=.008) and fatigue (t=-2.46,p=.015) affected perceived productivity with an explanatory power of 23.6%. Conclusion: Based on these findings, the study highlighted the need for effective management programs to tackle fatigue and premenstrual syndrome among clinical nurses' major health issues in order to reduce their productivity loss.

Correlation between Muscular Fatigue and EMG Activity during the Prolonged Casual Computer Work

  • An experimental study was conducted to investigate the effects of turtle neck syndrome, so called, on muscular fatigue and muscle activity. Six subjects (males) participated and performed the prolonged casual computer work in the study. EMG signals from six muscles of the dominant neck-back region were acquired and recorded for 10 seconds at the beginning and the end of three hours computer work. EMG was recorded from six muscles by using a computerized data recording and analysis system. Power spectrum function of EMG was calculated off-line by means of a signal processing software package. Power spectrum functions were smoothed with a moving average filter of 21 points and normalized with respect to the maximal value achieved during the trials. Muscle activity and median frequencies of Sternocleidomastoid(SCM) in turtle neck posture was approximately 51%, which were less than those in normal neck posture. SCM also showed the biggest decrease in median frequency. Results will provide the insight into the neck-back injury mechanism of turtle neck patients. Furthermore, they will be helpful in developing rehabilitation programs for restoring patients' neck-back functions.

  • The sick sinus syndrome refers to a combination of symptoms (dizziness, confusion, fatigue, syncope, and congestive heart failure) caused by sinus node dysfunction and marked sinus bradycardia, sinoarterial block, or sinus arrest. 'Abdominal pulsation' means a disagreeable symptom that is defined as sensation of beating at the abdomen. We experienced a 84 year-old female patient who had dizziness, fatigue, abdominal pulsation and frequent voiding. These symptoms were related to previous episodes and presumed sick sinus syndrome by symptoms, ECGs and 24 hrs of holter monitoring. In the point of Differentiation of Syndrome (辨證), this patient was diagnosed as 'Water retention(水氣)' and was administered with Yeonggyechulgam-tang(岺桂朮甘湯). After the treatment, abdominal pulsation (frequency) and dizziness (VAS) were improved. Futhermore, the interval of voiding was longer and each volume of urination increased.

A Systematic Review of Acupuncture for Chronic Fatigue Syndrome

  • Objectives : To evaluate and summarize the efficacy and safety of acupuncture treatment (AT) in chronic fatigue syndrome (CFS). Methods : Fifteen databases (Pubmed, Cochrane, EMBASE, AMED, CINAHL, CNKI, Wanfang, and eight Korean databases) were searched up to September 2016. Only trials in which acupuncture was the sole treatment were included. Fatigue was used as the primary outcome measure, while the quality of life, pain, mood disorders, and adverse events were used as secondary outcome measures. We adopted three classifications: AT vs Sham AT, AT vs Wait-list, AT vs Western medication. The Cochrane risk of bias tool was used to assess the methodological quality. Results : A total of 11 randomized controlled trials involving 869 participants were identified. In comparison with Sham AT, AT significantly alleviated fatigue and pain, but no conclusions about the quality of life and mood disorders could be drawn. In the Wait-list group and Western medication groups, patients with CFS might feel less fatigued following acupuncture treatment, but the evidence was insufficient due to lack of study. Nine of 11 RCTs (81.8%) reported adverse events and there were two cases of mild subcutaneous hemorrhage, but no serious adverse cases. Conclusion : This review found evidence that patients with CFS may generally benefit from alleviation of symptoms by acupuncture treatment, and there is no evidence of worsening symptoms or causing of serious adverse events. A positive effect on fatigue and pain was observed, but no conclusion for improving quality of life and mood disorders.

이압요법이 안구건조증에 미치는 효과: 대학생을 대상으로 (The Effects of Auriculotherapy on Relieving Symptoms of Dry Eye Syndrome)

  • Purpose: This study aims to examine the effects of auriculotherapy on reducing signs of dry eye syndrome. Methods: This study is based on a pretest-posttest nonequivalent control group design. The participants who scored higher than 13 on the Ocular Surface Disease Index (OSDI) were divided randomly into two groups, the experimental group (n=45) and the control group (n=44). Auriculotherapy was performed by researchers for 2 weeks. For the experimental group, seed stickers were applied to the eye, liver, shenmen, and tubercle point of their ears. The pretest and the posttest included the same questionnaires of the OSDI as well as the Visual Function Questionnaire (NEI-VFQ25) and the eye fatigue. Data were analyzed by descriptive statistics, $x^2$ test, t-test and paired t-test, using SAS 20.0 program. Results: The difference in the OSDI's scores showed a significant decrease between the pretest and the posttest (t=-4.12, p<.001) in the experimental group. No significant difference was found in the NEI-VFQ25 (t=-0.10, p=.921, t=-0.91 p=.374). However, the experimental group showed a significant decrease in eye fatigue (t=5.78, p<.001), whereas the control group showed no difference (t=1.38, p=.18). Conclusion: Auriculotherapy can be used effectively on relieving the symptoms of dry eye syndrome and eye fatigue. However, the quality of life related to eyes was not improved by auriculotherapy.

POEMS syndrome에서의 $^{18}F-FDG$ PET/CT 소견 ([ $^{18}F-FDG$ ] PET/CT in POEMS Syndrome)

  • POEMS syndrome is a rare disorder, also known as Crow-Fukase, PEP or Takatsuki syndrome. The acronym, POEMS, represents polyneuropathy, organomegaly, endocrinopathy, M protein and skin change. However, there are associated features not included in the acronym such as sclerotic bone lesions, Castleman disease, papilledema, thromobocytosis, peripheral edema, ascites, effusion, polycythemia, fatigue and clubbing. In most cases, osseous lesions in POEMS syndrome present as an isolated sclerotic deposit and that reveal as osteosclerotic myeloma. Several cases of $^{18}F-FDG$ PET in multiple myeloma involvements were reported, but there was no previous literature that reported FDG PET findings in POEMS syndrome. We describe here a 66-year-old patient with POEMS syndrome who underwent $^{18}F-FDG$ PET/CT image.
