한국인 대학생군 대상의 청.장년층 대사이상증후군 위험성 평가 (Evaluation of the Risk of Metabolic Syndrome for the Young Adults in Korean Students of a University)

  • Metabolic syndrome, defined as the clustering of several metabolic disorders including obesity (waist circumference ${\geq}90$ if male or ${\geq}80$ if female, cm), dyslipidemia ($TG{\geq}150$ or HDL-C<40 if male or <50 if female, mg/dl), hypertension ($BP{\geq}130/85mmHg$) and hyperglycemia (fasting plasma $glucose{\geq}110mg/dl$), increases the cardiovascular risk of the general population. Recently, risk of this syndrome arises in young adults world widely. Therefore, we randomly selected and evaluated the risk of metabolic syndrome of total 43 people (group I-22, group II-21) for 2 years. Group I was 22 peoples (15 males, 7 females) with age of 22 thru 35 year old (average 28 year old) and group II was 21 people (19 male, 2 female) with age of 22 thur 32 years old (average 24 year old) in Cheongju area from March 1st thru 30th of 2008 in Cheongju area from September 1st thru 30th of 2007 in order to find out how serious this phenomenon is in young adult of Korea. 13.95% (n=7) of total people has a metabolic syndrome by NCEP/ATPIII definition among this group (group I-6, group II-1). Those of 6 have 3 or over risk factor for metabolic syndrome such as obesity, hypertension, fasting blood glucose and hypetriglyceridemia at the same time (group I-5, group II-1). Group I have more risk factor because of more higher age than group II. Therefore we need aggressively to monitor and provide them for early diagnosis, educational programs and assistance for lifestyle changes in order to prevent metabolic syndrome among young adults.

Effects of corn gluten hydrolyzates, branched chain amino acids, and leucine on body weight reduction in obese rats induced by a high fat diet

  • In this study, we compared corn gluten hydrolyzates, BCAAs, and leucine for their effects on body weight reduction in high fat-induced obese rats in order to determine the major active components in the corn gluten hydrolyzates. After obesity was induced for 13 weeks with high fat diet, the overweight-induced SD rats (n = 64) were stratified according to body weight, randomly blocked into eight treatments, and raised for 8 weeks. Four groups were changed to a normal diet and the other groups remained on the high fat diet. Each of the groups within both diets was fed either casein, corn gluten hydrolyzates, leucine, or branched chain amino acids, respectively. Daily food intake, body weight gain, and food efficiency ratio were significantly lower in the corn gluten hydrolyzate groups compared to the other groups, regardless of the high fat diet or normal fat diet. The rats fed the corn gluten hydrolyzates diet had the lowest perirenal fat pad weights whereas muscle weight was significantly increased in the corn gluten hydrolyzates groups. Plasma triglyceride, hepatic total lipid, and total cholesterol contents were significantly reduced in the corn gluten hydrolyzates groups. Other lipid profile measurements were not significantly changed. Plasma triglyceride and hepatic total lipid were also significantly reduced in the BCAA and leucine groups. Leptin levels were significantly lower and adiponectin was significantly higher in the corn gluten hydrolyzates groups. Fasting blood glucose, insulin, C-peptide, and HOMA-IR levels were also significantly reduced in the corn gluten hydrozylates groups, regardless of fat level.

Effects of a Weight Loss Program on Body Composition and Resting Energy Expenditure according to UCP 2 Genotype in Overweight Subjects

  • The objective of this study was to examine the effects of a weight loss program on the degree of obesity and levels of resting energy expenditure (REE) in overweight subjects according to their mitochondrial uncoupling protein 2 (UCP 2) genotype. Twenty-three subjects with a body mass index (BMI) greater than 27 were recruited from the Obesity Clinic of the Kyung-Hee University Hospital during the period of December 2000 - August 2001. The subjects were genotyped for the exon 8 allele; 15 subjects were found to be of del/del genotype, 8 were del/ins, and none were of ins/ins genotype. No significant association was found between the different UCP 2 genotypes and the initial levels of weight, fat mass (FM), lean body mess (LBM), BMI, REE, and REE/LBM ratio. After 12 weeks of a weight loss program, body weight and FM were significantly decreased, while LBM, total body water (TBW), and REE were not changed, irrespective of UCP 2 genotype. Initial fasting plasma levels of albumin, glucose, triglyceride, lipoprotein cholesterol, insulin, free triiodo-thyronine (T3), free fatty acid (FFA), and leptin were not different according to the UCP 2 genotype; furthermore, these blood parameters were not changed after the 12-week weight loss program. However, plasma levels of leptin decreased in both the del/del and ins/del genotypes, from 18.7 ng/ml to 13.4 ng/ml (p<.05), and from 18.1 ng/ml to 13.9 ng/ml (p<.05), respectively, after the weight loss program. In conclusion, this study found no significant association between the del/del or del/ins UCP 2 genotypes and differing levels of REE or differing degrees of obesity, either before or after a weight loss program. This study provided evidence that a well- managed weight loss program could maintain levels of REE, which plays an important role in the maintenance of energy balance.

제2형 당뇨환자의 개인별 맞춤영양교육의 효과 평가 (Individual Diabetes Nutrition Education Can Help Management for Type II Diabetes)

  • This study was designed to evaluate the effect of the individual nutrition education for type 2 diabetes who participate the diabetes buffet. The subjects were 66 patients and divided into education (n=34) and control groups (n=32). The mean age of education and control groups were 59.8 and 56.6 years old, respectively. There were no differences in age and body mass index (BMI) between two groups. Initial glycosylated hemoglobin (HbA1c), post prandial plasma glucose (PP2), total plasma cholesterol, and blood pressure were not different between two groups. But fasting blood sugar (FBS) was higher in education group than in control group. On completion of the study, the education group showed significant decreases in body weight, BMI, FBS and PP2, however, the control group showed no changes in body weight, BMI and PP2, and showed a significant increase in FBS. Initial calorie and protein intakes of the education group did not meet the prescribed amount, however, mineral and vitamin intakes were higher than estimated average requirement (EAR). By the end of study, calorie and protein intakes were significantly increased to meet the prescription. In order to evaluate the effect of diet education, awareness of calorie requirement was used as an index of understanding diet prescription. The degree of awareness of calorie requirement was dependent on age: younger patients showed higher awareness than older subjects. The subject who showed better understanding of diet prescription showed lower levels of HbA1c, FBS, and PP2 at the end of the experiment period. The results of this study clearly show that individual diabetes diet education is effective to make the patient understand their diet prescription, and is effective to control body weight and blood sugar level. Awareness of calorie requirement could be used as an index of understanding of prescribed diet. Since age is an important variable to determine the awareness of calorie requirement, different strategies of nutrition education should be developed for different age groups, especially patients over 70 years old.

우리나라 성인의 인슐린 저항성과 관련된 영양소 및 식품군 섭취: 제 4기 국민건강영양조사 자료를 활용하여 (The relationship between intake of nutrients and food groups and insulin resistance in Korean adults: Using the Fourth Korea National Health and Nutrition Examination Survey (KNHANES IV, 2007-2009))

  • The aim of this study was to examine the relationship between dietary variables and the prevalence of insulin resistance (IR) in middle-aged Korean adults using data from the 2007-2009 Korea National Health and Nutrition Examination Survey. Because IR is closely linked with metabolic syndrome, subjects were divided into three groups according to symptoms of metabolic syndrome: the 'Normal group' without any symptoms, the 'Risk group' with one or two symptoms, and the Metabolic syndrome (MetS) group' with three or more symptoms. Subjects between the ages of 30 and 65 years with no prior diagnosis or treatment for diabetes, hypertension, or dyslipidemia were selected. The number of subjects per group was as follows: 2,085 adults in the Normal group, 3,699 adults in the Risk group, and 1,160 adults in the MetS group. Metabolic syndrome was defined according to Adult Treatment Panel III criteria with modified waist circumference cutoff values (men ${\geq}$ 90 cm, women ${\geq}$ 85 cm). Subjects with HOMA-IR > 2.0 were classified as IR. Homeostasis Model Assessment of Insulin Resistance (HOMA-IR) was calculated using the following formula: (fasting plasma glucose ${\times}$ fasting plasma insulin)/22.5. Nutrients and food groups intake were obtained from a single 24-hour recall. Subjects with IR in the Normal group were more obese and less physically active than non-IR subjects. In the MetS group, subjects with IR were more obese and had a lower prevalence of smoking and drinking, compared with non-IR subjects. Men with IR in the Normal group had a tendency to consume more oils and sugars than non-IR men, while women with IR in the same group had higher intake of carbohydrate, dietary glycemic index, and dietary glycemic load than non-IR women. Women with IR in the Risk group had lower energy intake but higher intake of oils and sugars than non-IR women. In the MetS group, consumption of fruits was higher in subjects with IR than in non-IR subjects. In conclusion, findings of this study suggest that dietary carbohydrate intake, including glycemic index, may be associated with IR in healthy women. Further research in prospective cohort studies in order to examine the effects of dietary carbohydrate on IR incidence will be necessary.

정상인(正常人) 및 당뇨병환자(糖尿病患者)에서의 경구당부하시(經口糖負荷時) 혈중(血中) Insulin과 C-Peptide의 변동(變動) (A Study of the Insulin and the C-Peptide Responses to Oral Glucose Load in Nondiabetic and Diabetic Subjects)

  • The present study was undertaken to evaluate the significance of the insulin and the C-peptide rseponse to oral glucose loads in normal and diabetic subjects and to establish the effects of the obesity. In this study, the authors have measured plasma insulin and C-peptide by means of radioimmunoassay in 10 nonobese normal, 5 obese normal, 13 nonobese moderate diabetic patients, 9 obese moderate diabetic patients and 9 severe diabetic patients. The results obtained were as follows; 1. In 10 nonobese normal subjects, the plasma insulin level at fasting state and at 30, 60, 90, and 120 min after oral glucose loads were $15.7{\pm}3.4,\;48.3{\pm}9.8,\;40.4{\pm}6.7,\;37.4{\pm}6.5\;and\;26.0{\pm}4.2uU/ml(Mean{\pm}S.E.)$ and C-peptide were $1.9{\pm}0.3,\;3.9{\pm}0.6,\;6.3{\pm}0.6,\;5.7{\pm}0.5\;and\;4.0{\pm}0.5ng/ml$. The change of C-peptide was found to go almost parallel with that of insulin and the insulin value reaches to the highest level at 30 min whereas C-peptide reaches to its peak at 60min. 2. The plasma insulin level in 5 obese normal subjects were $38.9{\pm}12.3,\;59.5{\pm}12.3,\;59.2{\pm}17.1,\;56.1{\pm}20.0\;and\;48.4{\pm}17.2uU/ml$ and the C-peptide were $5.5{\pm}0.4,\;6.8{\pm}0.5,\;7.9{\pm}0.8,\;7.9{\pm}0.8\;and\;7.8{\pm}2.0ng/ml$. The insulin response appeared to be greater than nonobese normal subjects. 3. In 13 nonobese moderate diabetic patients, the plasma insulin levels were $27.1{\pm}4.9,\;44.1{\pm}6.0,\;37.3{\pm}6.6,\;35.5{\pm}8.1\;and\;34.7{\pm}10.7uU/ml$ and the C-peptide levels were $2.7{\pm}0.4,\;4.9{\pm}0.7,\;6.5{\pm}0.5,\;7.0{\pm}0.3\;and\;6.7{\pm}1.0ng/ml$. There was little significance compared to nonobese normal groups but delayed pattern is noted. 4. In 9 obese moderated diabetic patients, the plasma insulin levels were $22.1{\pm}7.9,\;80.0{\pm}19.3,\;108.0{\pm}27.0,\;62.0{\pm}17.6\;and\;55.5{\pm}10.1uU/ml$ and the C-peptide levels were $5.2{\pm}0.4,\;8.0{\pm}1.0,\;10.4{\pm}1.6,\;10.4{\pm}1.7\;and\;10.1{\pm}1.0ng/ml$ and its response was also greater than that of nonobese moderate diabetic patients. 5. The plasma insulin concentrations in 9 severe diabetic subjects were $8.0{\pm}3.8,\;12.1{\pm}3.5,\;16.8{\pm}4.6,\;19.6{\pm}5.2\;and\;15.0{\pm}5.0uU/ml$ and the C-peptide levels were $1.6{\pm}0.3,\;2.4{\pm}0.4,\;4.1{\pm}0.6,\;4.0{\pm}0.8\;and\;4.5{\pm}0.7ng/ml$ and the insulin and C-peptide responses were markedly reduced in severe diabetic groups. 6. There were-significant differences between each groups of patients on the magnitude of total insulin or C-peptide areas, the insulinogenic index and the C-peptide index.

Streptozotocin-Nicotinamide로 유도된 제2형 당뇨모델 쥐에서 조각자(Gleditschiae Spina) 추출물의 항당뇨효과 (Antihyperglycemic of Gleditschiae Spina Extracts in Streptozotocin-Nicotinamide Induced Type 2 Diabetic Rats)

  • 본 연구는 제2형 당뇨모델 쥐에서 조각자 추출물의 항당뇨 효과를 알아보기 위해 SD계 수컷 흰쥐에 streptozotocin (STZ)을 복강주사 15분 후 nicotineamide(NA)로 복강에 재주사하여 제2형 당뇨를 유발시켰다. 그리고 당뇨대조군(DC), 당뇨 유발군+acarbose 4 mg/kg(AC), 당뇨유발군+조각자 추출물(Gleditschiae Spina ethanol extract) 50 mg/kg (GSE), 그리고 정상군(NC)으로 나누어 10주간 식이와 함께 물을 자유 급여하였다. 10주 동안 매일 식이 섭취량과 음료섭취량 및 매주 체중과 공복혈당을 측정하고, 마지막 주에 경구포도당 부하검사(OGTT)를 실시하였으며, 부검 시 혈액을 채취하여 혈장 TRAP 수준을 측정하였다. 정상군에 비해 당뇨그룹(DC, AC, GSE)의 체중 증가는 유의적으로 감소하였고, 당뇨 유발군들 간에는 유의적 차이가 없었다. 식이섭취량, 음료섭취량 및 식이이용효율은 정상군에 비해 당뇨그룹 DC, AC, GSE에서 유의적으로 높게 나타났으나, 음료섭취량은 DC에 비해 AC, GSE군에서 유의적으로 감소하였다. 정상군은 10주 동안 정상 수준 공복혈당을 나타내었고, acarbose는 8주부터 공복혈당이 유의적으로 감소하였으며, 9주까지 GSE군은 DC군보다 공복혈당은 낮았지만 유의적 차이는 보이지 않다가 마지막 10주에서는 DC군보다 공복혈당이 유의적으로 감소하였다. 경구 당부하 검사 결과에서는 포도당 투여 30분 후부터 혈당이 상승되기 시작하여 정상군보다 당뇨군들에서 유의적으로 상승하였고, 당뇨군들에서는 DC군에 비해 AC와 GSE군에서 유의적으로 혈당상승이 억제되었다. 10주간의 GSE 보충은 DC군에 비해 혈장 내총항산화(TRAP) 수준을 유의적으로 증가시키는 것으로 나타났다. 따라서 제2형 당뇨병에서 조각자 추출물을 장기간 투여한다면 혈당증가 억제에 있어 확연한 효과를 보일 것으로 생각된다. 이는 조각자 추출물의 항산화 활성과도 연관이 있을 것으로 추측되며, 향후 조각자 추출물의 혈당강하작용 효과를 가지는 물질의 분리 및 정제에 대한 연구가 이루어져야 할 것으로 사료된다.

What is the meaning of hs-CRP and HbA1c in patients with dry eye syndrome in diabetes?

  • 본 연구는 2016년 6월 16일부터 2016년 12월 31일까지 건강검진센터를 방문한 726명을 대상으로 Schirmer's 검사와 대사증후군 검사를 통해 전신적 만성 염증성 상태가 국소적 만성 염증성 상태로서의 안구건조증에 미치는 영향을 알아보고자 시행하였다. 대상자중 hs-CRP, HbA1c를 동시 측정한 214명을 대상으로 연구를 진행하였다. 연구대상자 중 고 연령일수록 (p<0.001), 성별로는 여성에서 유의하게 안구건조증이 증가하였으며(p=0.020), 공복혈당과 hs-CRP와는 연관이 없었다. 또한 HbA1c가 증가할수록 안구건조증에 대한 위험도가 증가하였다(β=1.960, p=0.025). 따라서 안구건조증은 만성 염증성 상태로 인한 눈물샘의 미세혈관 변화에 의한 것이 아니며 당뇨환자의 경우 단기간의 혈당 조절보다 장기간의 혈당 조절상태가 더욱 중요하게 작용할 수 있음을 알 수 있으며 당뇨병 환자에서 안구 건조증의 높은 유병률은 만성 염증으로 인한 미세 혈관 변화보다는 자율 기능 장애로 인한 것으로 생각된다.

한국 20-30대 성인의 Atherogenic Index of Plasma에 따른 건강지표 및 생활습관비교 (Comparison of health indicators and lifestyle according to atherogenic index of plasma in Korean adults in their 20s and 30s)

  • 본 연구는 20-30대 성인을 대상으로 심혈관계 질환 예측인자인 AIP가 어떤 요인들과 상관성이 있는지를 밝혀 중년 이후 심혈관계질환의 발병을 조기에 예방할 수 있도록 하는 근거를 제공하고자 수행이 되었다. 연구대상자는 2013-2015년 국민건강영양조사에 참여한 20-39세 성인 3,040명으로 AIP에 따라 5분위수로 나누어서 일반사항, 건강지표, 식습관, 영양섭취상태를 비교하였다. 남성과 여성 대상자 모두 나이가 많을수록, 비만할수록, 흡연할수록 AIP 평균이 높았고, 교육수준의 경우 남성은 교육수준이 높을수록 여성은 교육 수준이 낮을수록 AIP 평균이 높았다. 남성은 음주 정도에 따라 AIP 평균에 차이가 있었고, 운동을 하지 않는 남성이 운동을 하는 남성보다 AIP 평균이 높았다. 건강지표는 남녀 모두 AIP가 높은 분위로 갈수록 수축기 혈압, 이완기 혈압, 공복혈당, 인슐린, 총 콜레스테롤, 중성지방, LDL 콜레스테롤이 유의하게 증가하는 경향이 나타났다. 남성의 경우 당화혈색소 수치도 AIP가 높아질수록 유의하게 증가하는 경향이 나타났지만, 여성에서는 유의적인 차이는 없었고, HDL 콜레스테롤의 경우 남녀 모두 AIP가 높아질수록 수치가 유의하게 낮아지는 경향이 있었다. 식습관은 여성에서 AIP가 높은 분위수로 갈수록 아침식사를 먹지 않는 비율이 높았고, 음료의 섭취량이 높았으며 커피의 섭취량은 낮았다. AIP에 따른 영양소 섭취량은 남녀 모두 차이가 없었다. 본 연구 결과를 통해 20-30대 성인에서도 나이가 많을수록 비만할수록 흡연을 할수록 운동을 안 할수록 동맥경화의 위험도가 높아짐을 확인하였고, AIP는 젊은 성인에서도 수축기 혈압, 이완기 혈압, 공복혈당, 인슐린, 총 콜레스테롤, 중성지방, LDL 콜레스테롤, HDL 콜레스테롤 등의 건강지표와 상관성이 있었다. 또한 여성에서 음료의 섭취가 많고 커피의 섭취가 적을수록 AIP가 증가하여서 탄산음료와 가당 음료의 섭취를 줄이는 것이 바람직함을 확인하였다. 따라서 20-30대 젊은 성인도 체중조절과 바람직한 생활습관을 통해 심혈관질환 예방을 위한 노력이 필요함을 확인하였다.

Hypotriglyceridemic effects of brown seaweed consumption via regulation of bile acid excretion and hepatic lipogenesis in high fat diet-induced obese mice

  • BACKGROUND/OBJECTIVES: The present study aimed to further investigate the potential health beneficial effects of long-term seaweed supplementation on lipid metabolism and hepatic functions in DIO mice. MATERIALS/METHODS: Four brown seaweeds (Undaria pinnatifida [UP], Laminaria japonica [LJ], Sargassum fulvellum [SF], or Hizikia fusiforme [HF]) were added to a high fat diet (HFD) at a 5% ratio and supplemented to C57BL/6N mice for 16 weeks. Triglycerides (TGs) and total cholesterol (TC) in the liver, feces, and plasma were measured. Fecal bile acid (BA) levels in feces were monitored. Hepatic insulin signaling- and lipogenesis-related proteins were evaluated by Western blot analysis. RESULTS: Fasting blood glucose levels were significantly reduced in the LJ, SF, and HF groups compared to the HFD group by the end of 16-week feeding period. Plasma TG levels and hepatic lipid accumulation were significantly reduced in all 4 seaweed supplemented groups, whereas plasma TC levels were only suppressed in the UP and HF groups compared to the HFD group. Fecal BA levels were significantly elevated by UP, LJ, and SF supplementation compared to HFD feeding only. Lastly, regarding hepatic insulin signaling-related proteins, phosphorylation of 5'-AMP-activated protein kinase was significantly up-regulated by all 4 types of seaweed, whereas phosphorylation of protein kinase B was up-regulated only in the SF and HF groups. Lipogenesis-related proteins in the liver were effectively down-regulated by HF supplementation in DIO mice. CONCLUSIONS: Brown seaweed consumption showed hypotriglyceridemic effects in the prolonged DIO mouse model. Specifically, combinatory regulation of BA excretion and lipogenesis-related proteins in the liver by seaweed supplementation contributed to the reduction of plasma and hepatic TG levels, which inhibited hyperglycemia in DIO mice. Thus, the discrepant and species-specific functions of brown seaweeds provide novel insights for the selection of future targets for therapeutic agents.
