In 2016, we developed serious content for diabetes (Roly Poly 160) for diabetes, and then revised and supplemented them to serious game contents for self-care throughout diabetes, reflecting the needs of diabetes and health educators. We investigated the clinical efficacy. This study was one-group pretest-posttest design, 49 people with diabetes who were admitted to a hospital in the G region were given self-management training through 'Roly Poly 160' for 5 days, from February 27 to April 21, 2017, once a day. As a result of the study, the fasting blood glucose level and the postprandial blood glucose level of the subjects were significantly decreased (p<.000), and the self-efficacy and cognitive function were significantly increased (p<.000).
Objective: To examine the effects of sodium intake on the correlations between the saltsensitive gene ${\alpha}$-adducin 1 (ADD1) and inflammatory cytokines in Korean childhood obesity. Methods: A total of 2,070 students aged 8-9 years old participated in this study. The anthropometrics, serum biochemistry profile, inflammatory cytokines, and three-day dietary assessment were analyzed according to sex, obesity degree, and ADD1 polymorphism. Results: The obesity prevalence was higher in boys (15.6%) than in girls (11.9%). Boys also showed higher values in anthropometrics; lipid, glucose, and insulin profiles; total calorie intakes, as well as those of sodium and calcium compared with those of the girls. The more obese were boys and girls, the higher were the anthropometrics and the blood levels (total cholesterol, triglyceride, low-density lipoprotein cholesterol, fasting blood sugar, and insulin), but the lower was high-density lipoprotein cholesterol. The obese boys had significantly higher sodium and Na/K intakes, while the obese girls had higher visfatin level and Na/K intake. In addition, an increase in the risk factors for blood pressure and obesity in ADD1 variants was identified. Serum tumor necrosis $factor-{\alpha}$($TNF-{\alpha}$) significantly increased with increasing sodium intake in the ADD1 W allele carriers, regardless of sex. The presence of obesity with the ADD1 W allele induced inflammatory accelerators such as $TNF-{\alpha}$ or C-reactive protein(CRP) with higher sodium intake. Conclusion: Obese children with an ADD1w allele can experience a more complex form of obesity than non-obese when exposed to an obesity-inducing environment and need to be controlled sodium intake in the diet.

Purpose : The lipid profile is a major predictive index for cardiovascular disease, but there have only been a few studies conducted on the relationship between lipid ratio and cardiovascular disease risk factors in the Korean population. To address this research gap, we investigated the association between three lipid ratios and cardiovascular disease risk factors among Korean adults. Methods : This study used data from the seventh Korea National Health and Nutrition Examination Survey conducted in 2016. Lipid ratios included triglyceride (TG)/high density lipoprotein cholesterol (HDL), total cholesterol (TC)/HDL, and low density lipoprotein cholesterol (LDL)/HDL. Cardiovascular risk factors included in this research were: systolic blood pressure $(SBP){\geq}140$, diastolic blood pressure $(DBP){\geq}90$, fasting blood sugar $(FBS){\geq}126$, $HbA1c{\geq}6.5$, body mass index $(BMI){\geq}25$, waist circumference (WC) $men{\geq}90$, $women{\geq}85$, and metabolic syndrome (MetS). A complex samples logistic regression test was performed to analyze the association between lipid ratios and cardiovascular disease risk factors. Results : 1) TG/HDL ratio had statistically significant relationships with DBP, FBS, HbA1c, BMI, WC and MetS. 2) TC/HDL ratio was correlated to SBP, DBP, FBS, BMI, WC, and MetS. 3) LDL/HDL ratio had association with BMI, WC, and MetS. Conclusion : We identified significant association between lipid ratios and cardiovascular disease risk factors. The three lipid ratios were particularly strongly associated with BMI, WC, and MetS.
Purpose: This study aims to provide preliminary data for weight management, and prevention and management of cardiovascular and cerebrovascular diseases. We examined the effect of changes in the weight of workers at a manufacturing company over three years on their metabolic syndrome and metabolic syndrome diagnosis components. Methods: Necessary data were collected from the questionnaire and the results of the Korean National Health Screening of 2015 and 2017, which included 228 workers at a manufacturing company in G region. The collected data were analyzed using the SPSS/WIN 23.0 program. ANCOVA was used to examine the differences in the metabolic syndrome diagnosis components according to weight change. In addition, multiple logistic regression analysis was used to obtain the odds ratios of metabolic syndrome and metabolic syndrome analysis component, based on the weight changes in the normal weight group and the obesity group. Results: Waist measure, systolic blood pressure, and blood pressure were found to have significant effects based on participants' weight change over three years. These factors increased with a larger increase in weight at a statistically significant level. This study analyzed the weight changes of the normal weight group and the obesity group considering the data from the National Health Screening of 2015, and found that the risk of metabolic syndrome increased at a statistically significant level as body weight increased; thus, the obesity group showed a higher risk in this regard. It was also found that waist measure, fasting blood sugar, and high-density low cholesterol increased at a statistically significant level as body weight increased. Conclusion: Health administrators need to recognize the importance of workers' weight management, select an intensive management group based on a time series analysis of weight changes, and develop and implement programs to manage the metabolic syndrome diagnosis components.
We performed a study to examine the association between diet quality and nonalcoholic fatty liver disease (NAFLD). Our study included 3,586 women aged 40-64 years who participated in the sixth Korea National Health and Nutrition Examination Survey. The study subjects were classified into the NAFLD group (n=816) and the normal group (n=2,770) using the hepatic steatosis index. The anthropometric indices, blood profiles, and dietary intake data of the subjects were obtained. The waist circumference, body mass index, and the serum levels of triglycerides, fasting blood sugar, HbA1c, and systolic and diastolic blood pressures were higher in the NAFLD compared to the normal groups (p<0.001, respectively). The intakes of protein (g/kg body weight, p<0.001), potassium (p<0.001), and vitamin A (p=0.006) were significantly lower in the NAFLD group. It was observed that the higher the total Korean Healthy Eating Index score, the lower the risk of NAFLD. A reverse relationship was shown between the NAFLD risk and the intakes of total fruits, total vegetables, vegetables excluding Kimchi and pickled vegetables, meat, fish, eggs and beans. Therefore, it is recommended that middle-aged women in Korea increase their intakes of fruits, vegetables, and foods high in protein for the proper management of NAFLD.

In medical selection of Daehan Education Insurance, We doubted that 140 cases(male 117 cases, female 23 cases) might have the fatty infiltration of the liver by ultrasonography. We examined the weight and the hypertension of 140 patients by biochemically analyzing the blood extracted under fasting according to the age-distribution. The results are as follows: 1. 90% out of them had body weight than normal subject. 34.29% out of them were hypertension patients and most of them were $30{\sim}39's$ and $40{\sim}49's$. 2. As compared with the normal subjects in serum lipid value, HDL cholesterol value was low, but total cholesterol, Triglyceride, phospholipid and total lipid value were significantly high. Especially the statistical value of Triglyceride was very significantly high. 3. Total abnormal rate of sGOT value in Liver function test is 34.43% and the mean value of it is 66 unit and the abnormal rate of sGPT value is all 3.14% and the value of it is 70 unit. 4. The abnormal rate of Alkaline phosphatase value is 9.29% and the abnormal rate of total Bilirubine value is 14.29%. As HBsAg positive rate is 2.14% and HBs Ab positive rate is 31.43%, the positive rate of HBsAg represents Lower positive rate than Korean mean value. 5. The abnormal rate of the fasting blood glucose is 15.00% and the abnormal rate of Blood sugar pc 2hrs is 30.71%. 6. The abnormal case of protein, Albumin, Globulin, BUN, craetinine and Hemoglobin value was not found to any of them and in the above liver function test except serum lipid value, the rate not showing the abnormal value represents 45.3%(male) and 52.17%(female).

Hemodialysis(HD)-associated hypotension is a frequent complication, but it is difficult to manage. Until now, several maneuvers have been tried to prevent the HD-associated hypotension. Of these, the sodium content of dialysate was regarded as an important factor for maintaining blood pressure during HD. In this study, we evaluated the effect of hypertonic dialysate on blood pressure, interdialytic weight gain and the incidence of thirst. The study was done for 6 weeks successively with 3 different groups. Each patient was dialysed with 3 different dialysates for 2 weeks: Group I(Conventional HD: sodium concentration: 137 mEq/L), Group II(Hypertonic HD: 147 mEq/L) and Group III (Sequential HD: from 147 to 140 mEq/L). Hemodynamic parameters(blood pressure, pulse rate and ultrafiltration rate), biochemical parameters(hematocrits, blood urea nitrogen, creatinine, osmolality, sodium, potassium, chloride, fasting blood sugar) and complications (interdialytic weight gain & thirsty sensation) were compared among 3 groups. The results were as follows: 1. Decline of systolic blood pressure and diastolic blood pressure at the time of a 3 hour check during hemodialysis was lower in the Group II than Group I and III (p=0.002; p=0.012). and decline of diastolic blood pressure at the time of a 4 hour check during hemodialysis was lower in the Group II and III than Group I (p=0.04). 2. Incidence of hypotensive episodes during dialysis was significantly lower in Group II than group I (p=0.0287). 3. The ultrafiltration in Group III at the time of 1 hour, 2 hour and 3 hour check during hemodialysis was higher than that in Group I and II at the time of 1 hour, 2 hour and 3 hour check during hemodialysis respectively (p=0.0001; p=0.0001; p=0.0004). 4. Interdialytic weight gain was higher in Group I($3.1{\pm}0.8$) than Group I($2.8{\pm}0.8$) and III ($2.9{\pm}0.9$) (p=0.0422). 5. Hematologic and biochemical results were not significantly different among 3 Groups. 6. Frequency of thirst was different in Group I, II and III, $0.05{\pm}0.12$, $0.41{\pm}0.24$and $0.22{\pm}0.29$ respectively (p=0.0259). The results suggest that hypertonic HD was effective in preventing HD-associated hypotension but interdialytic weight gain and thirst sensations were increased as compared with a conventional method. In this situation, sequential HD seems to be an alternative method to minimizes the side effect of hypertonic HD.
Objectives: This study analyzed the effects of diabetes interventions published in journals of nursing, medical and public health, to suggest directions for future research. Methods: A comprehensive search of three databases was undertaken (Korean studies, Riss4U, RICHIS) using research terms such as "diabetes", "nursing", "public health", "intervention" and "program". One hundred fifty four original articles were retrieved and 21 articles were finally analyzed. The analyzed articles satisfied the inclusion criteria and their effect size was calculated. Results: Most studies about diabetes intervention were published form 2001. 2005 (57.1%). The psycho-social variables, 'self-efficacy' and 'self-care' presented good effects on exercise or exercise prescription intervention. In addition, the physiological variables, 'fasting blood sugar' and 'glycosylated glucose' presented good effects on the interventions using four more elements or the interventions held in a public health center. Conclusion: Further research should consider the essential elements that benefit diabetes patients' behaviors or physiological indicators.
Purpose: The purpose of this study was to examine the effects of the laughter therapy program on perceived stress and psycho-neuro-endocrine-immune responses in obese women. Methods: A nonequivalent control group with a pretest-posttest design was used. The participants (n=60), whose age ranged from 30 to 50 years (pre-menopausal and body mass index of over $25kg/m^2$), were assigned to the experimental group (n=24) or control group (n=26). The experimental group was provided with the laughter therapy program (12 sessions) for 6 weeks. Results: There were significant differences in perceived stress, psychological stress response, fasting blood sugar, interleukin-6, and tumor necrosis factor alpha between the two groups after the program. However, there were no significant differences in normalized low frequency (norm LF), normalized high frequency (norm HF), LF/HF ratio, and cortisol between the two groups after the program. Conclusion: It was found that the laughter therapy program had positive effects on some variables in terms of perceived stress and psycho-neuro-endocrine-immuno responses. It is suggested that the laughter therapy in this study can provide the direction for developing a program for obese women.

This study was conducted to assess the biochemical nutritional status of 310 elderly(males: 130, females : 180) residing in Puchon City. Subjects were apparently healthy, home staying and aged over 60. Average height of males and females were 163.2cm and 148.5cm, respectively. Female aged over 80 showed 148.5cm, which was significantly lower than that of group aged 60~69. The rate of moderate obesity for males assessed with BMI($\geq$27) was 7.7%, and 23.4% for females. The hypertension rate assessed by SBP($\geq$160mmHg) was 8.0% and 10.5% for males and females, respectively. The proportion of hypertension with indice of DBP( 95mmHg) was 9.4% and 23.4%. The lower concentration of Hb(males: <13g/dl, females: <12g/dl) occurred in 10.7% for males and 11.7% for females. No one except a few was found with protein deficiency assessed with serum albumin, serum protein and the ratio of urinary nitrogen to creatinin. Twenty five percent of males and 35.2% of females belonged to hypercholesterolemia($\geq$ 240mg/dl). The proportion of elderly whose fasting blood sugar was higher than 140mg/dl was 23.7% and 23.2%.
