한약 및 절식요법을 포함한 한의 종합 치료와 공간척추도인안교를 병행한 청소년 비만 환자의 체성분 변화 증례보고 1예 (A Case Report on Changes in Body Composition in an Adolescent Obese Patient Treated with Complex Korean Medicine Treatment with Modified Fasting, and Space Spinal Conduction Exercise and Manipulation)

  • 본 증례보고는 체중 115.8 kg이고 BMI가 35.5 kg/m2인 청소년 고도 비만 환자 1명을 대상으로 145일간 한약 치료와 감로수 절식요법, 전침, 약침 등의 한의 치료와 함께 공간척추도인안교를 시행한 결과, 최초 체중의 약 19.86%에 해당하는 23.0 kg의 체중감량을 보였기에 이를 보고하는 바이다.

감로수 절식요법을 적용한 고혈압 비만환자 증례보고 (A Case Report for the Effects of the Modified Fasting Therapy (Gamrosu) on Obese Patients with Hypertension)

  • 고혈압으로 진단된 비만환자 6예에 10일간 감로수 절식요법을 시행한 결과 절식기 후 혈압은 평균 148/89 mmHg에서 119/79 mmHg로 수축기 혈압은 28.7 mmHg 감소(-19.39%) 하였고, 이완기 혈압은 10.3 mmHg 감소(-11.57%)하였다. 절식기 후 체중, 체질량지수, 체지방량, 체지방률은 각각 평균 -6.65%, -6.10%, -7.76%, -1.42% 감소하였다. 위의 증례결과로 보았을 때 감로수 절식요법은 비만, 고혈압환자에 적용하였을 때 체중감량과 더불어 혈압강하효과를 가져올 것으로 기대된다. 감로수는 절식요법을 진행할 때 생길 수 있는 문제점의 발생을 줄여 10일간 절식요법이 안정되게 진행할 수 있게 도움으로써 혈압강하효과를 증가시킨다.

감로수(甘露水)를 이용한 절식요법의 임상결과에 관한 후향적 관찰연구 (Clinical Outcomes after Modified Fasting Therapy Supplied with Gamrosu: A Retrospective Observational Study)

  • Objectives: Two historical evidence supported the concept of Gamrosu. The first one was Jeho-tang, a selected thirst quencher in Chosun Dynasty and the second one was Saeng-Maek-san, Dongeuibogam's recommendation as one of the qi-vigorating summer beverages. Gamrosu is a modified fasting therapy beverage which is manufactured from those two prescriptions and the carbohydrates (420.6 kcal/d). A retrospective observational study was conducted to evaluate the clinical outcomes. Methods: Thirty-three cases were reviewed at three local Korean Medical clinics that each site's participant has finished the modified fasting for 10 consecutive days. Clinical outcomes were reviewed at pre- and post-fasting sessions by retrieving the changes of body composition, blood pressure, blood chemistries & urine tests, and subjective symptoms & fatigue scores. Results: Demographics of the observed participants were 17 of male and 16 of female. Post-Gamrosu session, -6.89% of body weight, -8.97% of body fat mass were reduced with the nutrition indices being improved (P<0.001). -8.72% of systolic blood pressure, -39.86% of serum triglyceride, -6.75% of fast blood sugar and -8.12% of waist circumference were improved (P<0.05). The levels of high sensitivity C-reactive protein (-58.34%), CRP (-43.55%) and eosinophil (-21.30%) showed the significant diminished profiles (P<0.05). Liver/kidney functions and the standard of electrolytes were maintained within normal range in stable manners. The fatigue scale scores indicated significant lower scores. Conclusions: Taken together, obesity-related clinical outcomes after a modified fasting therapy with Gamrosu were sufficiently feasible and the observed findings should be considered for further prospective clinical studies.

쥐의 insulin-like growth tractor리 유전자 발현의 대사조절기전에 관안 연구 (Metabolic Regulation of Insulin-like Growth Factor-1 Expression)

  • Insulin-like growth factor-I(IGF-I)은 성장호르몬의 여러 가지 성정촉진 작용을 매개하는 분열 유발성 폴리펩티드이며, 조직의 수선과 재생, 창상치유 및 골대사와 같은 과정들에서 중요한 역할을 하는 것으로 알려져 있고, 비교적 여러 조직에서 발현되고 있는 IGF-I 유전자의 전사조절에 대한 정확한 분자적 기전과 호르몬 및 대사 상태가 그것을 어떻게 조절하는지 아직 밝혀져 있지 않다. 쥐를 절삭시키므로써 대사 상태를 변조시켰을 때, 간 조직내 IGF-I mRNA의 발현에 미치는 절식의 영향을 살펴보기 위하여 solution hybridizatioon/RNase protection 방법으로 분석 하였다. IGF-I의 exon 1 및 exon 2에 의하여 encode된 tran-scripts 모두가 감소된 결과를 얻었고, 이러한 감소는 전사 수준에서 일어난 것으로 nuclear run-on 분석에 의하여 확인하였다. 또한 절식시킨 쥐에서 IGF-I mRNA의 양을 조절하는 cia-acting elements를 IGF-I 유전자의 5'-flanking 지역과 exon 1과 econ 2에서 밝히고자 절식시킨 쥐의 신선한 간조직에서 핵 추출물을 얻어 IGF-I의 여러 가지 DNA fragments와 반응시켜 DNase I protection 분석을 한 결과, IGF-I 유전자의 주요한 전사 개 시점으로부터 downstream에 있는 sequences가 절식으로 변조시킨 대사상태에서 IGF-I의 발현 조절에 중요하며 이곳에는 전사인자인 C/EBP family의 isoform들을 포함한 간조직에 풍부하게 존재하는 여러 전사인자들이 결합할 것으로 제안하였다.

일부 농촌 성인의 당뇨병 유병률 추정 (The Estimation of Prevalence of Diabetes Mellitus in a Rural Adult Population)

  • The objectives of this study are to estimate the prevalence of diabetes mellitus (DM) and to identify risk factors associated with fasting blood sugar in a rural adult population. We carried out a cross - sectional study on the residents over 30-year old. Among the 1077 eligible subjects, 725 persons responded to the study;the study consisted of interview on family and past history of DM, anthropometry, blood pressure, and blood sugar level in each subjects. Only 707 subjects of 725 participants had fasting blood sugar(FBS) examined and the rest, 18 subjects were had casual blood sugar examined due to failure of fasting over 8 - hour. When subjects had $FBS\geq140mg/dl$, 2 hours postprandial blood sugar level was checked after 75g oral glucose loading. The prevalence of DM was 8.9%, and age standarized prevalence rate adjusted to Korean population of 1995 was 5.8%. Mean and SD of fasting blood sugar in men was $99.8{\pm}22.6$, and in women was $111.5{\pm}29.9$. Mean levels of fasting blood sugar were significantly higher in women than in man and as their ages advanced the prevalences increased in both sexes. PP2 blood sugar levels were significantly higher in elder age and in persons with higher FBS levels than others. The risk factors associated with FBS were past history of diabetes, sex, socioeconomic status and waist - to - hip circumference ratio;the risk of diabetes was increased in female, people with past DM history, central obesity and low socioeconomic state. The low socioeconomic status associated with DM in this study, which is different from other study results, should be pursued in further studies.

Elevated Serum Insulin is an Independent Risk Factor for Hepatocellular Carcinoma: A Case Control Study from Nepal

  • Aim: To investigate associations of fasting insulin and glucose levels in serum with hepatocellular carcinoma risk. Materials and Methods: This hospital based study was carried out using data retrieved from the register maintained in the Department of Biochemistry of the Nepalese Army Institute of Health Sciences, between 1st December, 2011 and 31st June, 2013. The variables collected were age, fasting plasma glucose, fasting plasma insulin and ALT. Quantitative determination of human insulin concentrations was accomplished by chemiluminescence enzyme immunoassay. Results: Of the total 220 subjects enrolled in our present study, 20 cases were of HCC and 200 were healthy controls. The maximum number of cases of hepatocellular carcinoma in category cutpoints of fasting insulin levels fell in the range of >6.10 ${\mu}U/ml$. The highest insulin levels (>6.10 ${\mu}U/ml$) were seen to be associated with an 2.36 fold risk of HCC when compared with fasting insulin levels of (<2.75 ${\mu}U/ml$). Furthermore, the insulin levels (2.75-4.10 ${\mu}U/ml$) of category cutpoints also conferred a 1.57 fold risk for HCC when compared with lowest fasting insulin levels of (<2.75 ${\mu}U/ml$). Conclusions: The effect of an insulin level in increasing HCC risk appeared consistent, influencing incidence, risk of recurrence, overall survival, and treatment-related complications in HCC patients.

규칙적 운동과 제2형 당뇨병 발생에 관한 전향적 코호트 연구 -공복혈당장애군을 대상으로- (A Prospective Cohort Study of Exercise and the Incidence of Type 2 Diabetes in Impaired Fasting Glucose Group)

  • Objectives : To determine the relationship between exercise and the incidence of type 2 diabetes in an impaired fasting glucose group. Methods : This prospective cohort study was conducted in 19,440 men and 4,297 women, aged 30-69 years, with impaired fasting glucose at baseline who had undergone biennial medical evaluation through the National Health Insurance Corporation from 2000 to 2004. Impaired fasting glucose was defined as fasting glucose of 100 to 125 mg/dl and the subjects were divided into 3 groups depending on weekly exercise frequency. Multivariate logistic regression analysis was used to evaluate the association between the baseline exercise status and incidence of type 2 diabetes. Results : During the 4-year follow-up, a total of 3,239 men and 283 women developed type 2 diabetes, a cumulative incidence of 16.6% for men, and 6.5% for women. Also, 1,688 men (21.2%) and 127 women (15.2%) developed type 2 diabetes in the obese group. The adjusted relative risk (RR) of developing type 2 diabetes in non-exercising men was significantly higher than exercising men regularly (RR= 1.375, 95% CI=1.236-1.529)(p<0.0001), and the RR for non-exercising women was higher than exercising women regularly (RR=1.124, 95% CI=0.711-1.778). The RR for non-exercise men/women in the obese group was 1.571 (95% CI=1.351-1.827)(p<0.0001)/1.869 (95% CI=0.846-4.130). Conclusions : Regular exercise is effective in preventing type 2 diabetes in people with impaired fasting glucose, and particularly in obese people. People with risk factors for diabetes should participate in a regular exercise program.

인삼이 칼륨결핍랫트 장점막의 $Na^+$,$K^{+}-ATPase$ 활성에 미치는 영향 (Effect of Ginseng on $Na^+$, $K^{+}-ATPase$ Activities of Potassium Deficient Rat Intestinal Mucosa)

  • We have studied the effect of fasting on $Na^+$, $K^{+}-ATPase$ activities in the rat intestinal mucosa. Rats were fasted for $18{\sim}48hr$. Intestinal microsomal fraction was prepared by the method of Robinson and ATPase activities were determined by the modified method of Fiske and Subbarow. $Na^+$, $K^{+}-ATPase$ activity was not changed after fasting for 18 and 24 hr but significantly decreased after fasting for 48 hr. Fasting over 18 to 48 hr period had no effect on the $Mg^{++}-ATPase$. Thus, it may be concluded that 48 hr fasting has inhibitory effect on rat intestinal absorptive capabilities. In order to study the effect of Ginseng on the $Na^+$, $K^{+}-ATPase$ activities of the small intestine in chronic $K^{+}-depleted$ rats, rats were fed $K^{+}-depleted$ diets for 3 weeks and Ginseng ethanol extracts were administered orally for 3 weeks concomitantly. ATPase activity was measured by the same method as fasting group. $Na^+$, $K^{+}-ATPase$ activity in the $K^{+}-depleted$ diet group was increased and Ginseng ethanol extracts inhibited the increase of enzyme activity induced by $K^{+}-depleted$ diet. Thus, it may be suggested that increase in the intestinal $Na^+$, $K^{+}-ATPase$ activity of chronic $K^{+}-depleted$ group may be due to the compensatory mechanism and administration of Ginseng with $K^{+}-depleted$ diet may be associated with inhibition of increase in the enzyme activity of the $K^{+}-depleted$ group due to the prevention of the $K^+$ loss in the $K^{+}-depletion$.

Glycemic control of type 2 diabetic patients after short-term zinc supplementation

  • This study was carried out to determine whether a short-tenn zinc supplementation contributes to beneficial changes in glycemic control among type 2 diabetic patients. Seventy-six diabetic subjects and 72 normal adults participated in this study. Subjects were divided into supplemented and control groups. Forty-four diabetic patients and 34 normal subjects were supplemented with 50 mg zinc daily as zinc gluconate for 4 weeks. Zinc status was assessed from fasting plasma levels and urinary excretion. The effects of zinc supplementation on fasting blood glucose, $HbA_{1c}$, insulin, and C-peptide were measured at the beginning of the study and after 4 weeks of supplementation. The changes in glycemic control indicators were compared between diabetic groups, classified by baseline $HbA_{1c}$ levels, and by diabetic duration. At baseline, the incidence of marginal zinc deficiency in the diabetic group, as determined by plasma zinc level, was approximately twice as high as in the normal adult group. The changes of $HbA_{1c}$ concentration, and fasting blood glucose following supplementation were not statistically significant in diabetic subjects. In normal subjects, a significant decrease of $HbA_{1c}$ occurred only in the zinc supplemented group. No significant changes were observed for serum insulin and C-peptide in diabetic as well as normal subjects. However, when the changes were compared by baseline $HbA_{1c}$ level, we found that diabetic subjects with $HbA_{1c}\;{\geq}\;7.5%$ showed significantly improved levels of $HbA_{1c}$ and fasting glucose after Zn supplementation. While such improvement in fasting blood glucose was significant among diabetics with shorter diabetic duration, significant levels of increase in serum insulin and C-peptide were observed in zinc supplemented subjects with longer diabetic duration. Fasting blood glucose was significantly decreased, whereas serum insulin and C-peptide were increased in diabetics with marginal zinc status. Therefore, we suggest that Zn supplementation for a short-term period may improve glycemic control in diabetic patients with higher $HbA_{1c}$ levels and marginal zinc status.

한국인에서 TLR4 변이가 공복 시 혈당에 미치는 효과 (Effects of TLR4 Variants on Fasting Glucose Levels in a Korean Population)

  • 당뇨병은 대사 증후군 및 심혈관 질환을 비롯한 다양한 건강상의 부작용과 관련이 있다. 또한 당뇨병의 증가로 사회 및 의학적 관심이 증가하고 있다. 제 2 형 당뇨병의 특징 중 하나는 혈당이 증가한다는 것이다. 공복 혈당치는 가장 기본적이고 널리 사용되는 당뇨병의 지표이다. TLR4 발현은 심장에서 비교적 높다. 그러나 공복 시 혈당과 TLR4의 관련성에 대한 연구가 있었다. 한국인에서 TLR4 유전자와 공복 시 혈당의 단일 염기 다형성(SNP)의 관련성을 알아 보았다. 서울시에서 총 994 명을 대상으로 TLR4 SNP를 이용한 연구를 실시했다. 공복 시 혈당과 관련하여 TLR4 유전자 영역은 연쇄 불균형으로 나타 났으며 유전자의 범위가 매우 크기 때문에 여러 개의 LD 블록으로 보였다. TLR4 유전자의 SNP는 평균 공복 시 혈당과 관련이 있었다(p<0.01). 공복 시 혈당과 관련 크기는 p-value를 $-log^{10}$의 값으로 하였다. rs1329067의 minor allele frequency는 16.4%였고 AA 유전자형을 가진 사람들은 GG 유전자형을 가진 사람들 보다 공복 시 혈당이 높았다. 이 연구는 TLR4의 유전 적 변이가 한국 성인의 혈당 수치에 영향을 미친다는 것을 암시한다.
