A Case Report on Enhanced Lipid Metabolism by Soluble Dietary Fiber Supplementation during the Gamrosu Modified Fasting Therapy Period (감로수 절식요법기에 수용성 식이섬유를 공급하여 지질대사가 개선된 증례)

  • A highly obese female patient (body mass index=$30.8kg/m^2$) participated in a 10-day Gamrosu modified fasting therapy with soluble dietary fiber supplementation to enhance lipid metabolism. Gamrosu is a modified fasting therapy beverage which is made from medical herbs and carbohydrates (431 kcal/d). Before and after fasting, we evaluated the efficacy of therapy by measuring the changes of body composition and blood chemistry. After the modified fasting therapy, -6.1% of body weight and -5.6% of body fat mass were decreased. With regard to blood chemistry, all the plasma lipid levels were lowered. -37.4% of total cholesterol, -39.7% of low density lipoprotein cholesterol, -39.0% of triglyceride and -27% of high density lipoprotein (HDL) cholesterol were reduced. Further studies are needed to alleviate the reduction of HDL-cholesterol to apply the Gamrosu modified fasting to hyperlipidemia.

A Comparative Study of the Concentration of Salivary and Blood Glucose in Normal and Diabetic Subjects

  • Advantage of saliva analysis are the ease of sample collection and that samples can be collected more frequently with much less stress on the patient. The objective of the present study was to comparatively evaluate the concentrations of saliva and fasting serum glucose in both normal and diabetic subjects. The mean salivary glucose level in diabetic patients was $15.66{\pm}17.1$ mg/dl and $1.78{\pm}1.72$ mg/dl (P = 0.0006) in the control group. The mean fasting serum glucose level in diabetic patients was $202.12{\pm}66.91$ mg/dl, while that in the control group was $94.21{\pm}14.97$ mg/dl (P < 0.0001). The 0.95 degree of correlation between salivary and fasting serum glucose could be demonstrated. The concentration of salivary and fasting serum glucose was not significant different betweeen the measurements for male and female. In the oral glucose tolerance test (75g), the glucose concentration in saliva progressively increased during the first 30 minutes of the test and then progressively decreased, reaching at minutes 120 ~ 180 lowest point as like fasting serum glucose concentration. We can conclude that salivary glucose concentration was significantly higher in the diabetic subjects and that there was significant correlation between salivary and fasting serum glucose concentration. Measurement of salivary glucose could be a useful test having good correlation between salivary and fasting serum glucose concentration.

Changes in lipid and carnitine concentrations following repeated fasting-refeeding in mice

  • The purpose of this study was to evaluate the effects of repeated fasting and refeeding on lipid metabolism. Thirty male ICR mice, aged 6 weeks, were fed an AIN-93 control diet during the experimental period. The mice were divided into 5 groups: Non fasting group (ad libitum-fed, NF), fasting for 3 days (F), fasting for 3 days and then refeeding for 4 days repeated once (FRF1), fasting for 3 days and then refeeding for 4 days repeated twice (FRF2), and fasting for 3 days and then refeeding for 4 days repeated three times (FRF3). Rates of body weight gain, epididymal fat weight, and serum TG were significantly decreased in the F, FRF1, FRF2, and FRF3 groups, compared to the NF group. LDL-cholesterol was significantly higher in the FRF3 group than the NF and F groups, but HDL-cholesterol and HDL/TC were significantly lower in the FRF3 group than in the NF and F groups. Serum total carnitine was significantly lower in the FRF1, FRF2, FRF3 groups than the NF and F groups. However, rates of serum and hepatic acyl-carnitine concentration were significantly lower in FRF1, FRF2, and FRF3 than in NF and F. Repeated tasting-refeeding resulted in visible reductions of body weight and fat mass, but it caused ill-effects with lipid and carnitine metabolism in the body.

Expression of Tryptophan Hydroxylase in the Hypothalamus and Hippocampus of Fasting and Anorexia Mutant Mice (절식시킨 생쥐와 식욕부진 돌연변이 생쥐의 시상하부와 해마에서의 Tryptophan Hydroxylase의 발현)

  • The control of food intake is a complex phenomenon caused by interactions between central and peripheral control mechanisms. The hypothalamic and brain stem regions have been identified as centers for food intake and energy expenditure in animals and humans. Of these, the ventromedial and lateral hypothalamic areas are involved in the control of food intake. Also, large amounts of neurotransmitters known to be involved in feeding are present in the hippocampus. Paricularly, tryptophan hydroxylase(TPH), known as a factor in the control of food intake, is present in high levels in the paraventricular nucleus of the hypothalamus and the hippocampus. In this study, TPH expression levels in the hypothalamic and hippocampal regions of fasting, anorexia mutant, and control mice were compared using RT-PCR and immunohistochemical methods. Differences in body weight among the fasting, anorexia mutant, and control groups wire observed. No statistical significance was noted in the number of TPH-immunoactivity in the hypothalamic nuclei, but relatively higher populations of such fibers were observed in the fasting group : the control group yielded samples with an overall value of 170.3${\pm}$3.5 in terms of immunoreactivity-induced optical density, whereas the fasting group yielded a value of 168.3${\pm}$2.6, and the anorexia mutant group 171.3${\pm}$0.8(lower values represent higher immunoreactivity), In fasting mice, stained neuronal bodies were observed in the CA3 and dentate gyrus regions of the hippocampus, which was different from the hippocampal regions of the control and anorexia mutant mice. The RT-PCR procedures were performed using whole brains, precluding any statistically noticeable findings in relation to specific regions, although the fasting and anorexia mutant groups showed 123.3% and 102.9%, respectively, of the TPH mRNA level in the control. The overall results present evidences of the role of TPH in the decrease in food intake during fasting caused by exogenic factors and in genetically acquired anorexia. Influence of Pre-operative Fasting Time on Blood Glucose in Older Patients (수술 전 금식이 노인환자의 혈당에 미치는 영향)

  • Purpose: This study was performed to identify changes in blood glucose at preoperative fasting time in surgical patients over 60 yr. Methods: Data collection was performed from July, 2008 through July, 2009. Participants consisted of 80 nondiabetic surgical patients. Blood glucose was checked from 3 to 5 times. The 5 times were 2-hr fasting on the pre-operative day (T1, n=80), 8 hr (T2, n=80), 10 hr (T3, n=17), 12 hr (T4, n=34) and 14 hr fasting on the day of the operation (T5, n=29). Results: Of the patients, 27.5% had a blood glucose level of less than 79 mg/dL at T2; 17.6% at T3; 32.4% at T4; and 17.2% at T5. Mean blood glucose levels were 93.8 mg/dL at T1; 88.4 mg/dL at T2; 91.7 mg/dL at T3; 87.4 mg/dL at T4: and 94.1 mg/dL at T5. Blood glucose was the lowest at T2 (p<.001). Conclusion: As 17.6-32.4% of the patients showed the blood glucose level of less than 79 mg/dL at 8-14 hr pre-operative fasting, the authors recommend that surgical patients p>60 yr-of-age be observed for hypoglycemia during pre-operative fasting of more than 10 hr and that surgical patients >60 yr-of-age with risks for hypoglycemia be scheduled for operation within 10 hr preoperative fasting.

The Clinical Study on the Effect of Reduction in Body Weight and the Change of Body Composition after Fasting Therapy (절식요법의 체중감량효과와 체성분 변화에 대한 관찰)

  • Objectives : This study was aimed to investigate the change of the reduction in body weight and the change of body composition after fasting therapy, Chulsikyopub, which was put into practice in Dept. of oriental rehabilitation medicine at Kyung Hee oriental hospital. Method : We measured body weight, body composition, and the degree of obesity 2 weeks after fasting therapy and 10 weeks after fasting therapy with body composition analyzer(Inbody 2.0). Statistical comparisons between pre-therapy group and post-therpay group were done by paired t-test and Wilcoxon's signed rank test. Results : 2 weeks after fasting therapy, body weight was decreased 7.33kg(9.18%), body fat was decreased 2.47Kg(7.24%), muscle weight was decreased 4.66kg(11.37%), body fat ratio was increased 1.08%, and body metabolic rate(BMR) was decreased 118.23kcal(8.22%). 10 weeks after fasting therapy, each of body weight and fat was decreased 10.4Kg, muscle weight was increased 0.3Kg(0.7%), body fat ratio was decreased 7.9%, and BMR was increased 6.8Kcal(0.47%). But muscle weight and BMR did not satisfy statistical significance. Conclusions : fasting therapy for 10 weeks reduced body weight and body fat statistically significant without loss of muscle weight and BMR.

Effects of Fasting and High-fat Diet Feeding on Uncoupling Protein 3 mRNA Levels of Skeletal Muscle in Rats (절식과 고지방식 섭취가 골격근 UCP3 mRNA 발현에 미치는 영향)

  • PURPOSE. The purpose of this study was to investigate the effects of fasting and high-fat diet feeding on uncoupling protein 3 (UCP3) mRNA levels, uncoupling the respiratory chain and producing heat, of skeletal muscle in rat. METHODS. Fasting experiment: Forty Male Sprague-Dawley rats (5 wk) were divided into non-fasting groups (CON) and fasting groups (FG) for 0 day, 0.5 day (12 hr), 1 day, 2 day and 3 day. The rats of CON were sacrificed at 0 and 3 day. High-fat diet experiment: Forty Male Sprague-Dawley rats (5 wk) were divided into low-fat diet groups (LF) and high-fat diet group feeding for 0 day, 0.5 day (12 hr), 1 day, 2 day and 3 day. The rats of LF were sacrificed at 0 and 3 day. Analysis: Analysis of UCP3 mRNA expression was used by Real-time PCR. RESULTS. UCP3 mRNA levels of FG group were increased according to time course for 2 days- fasting but decreased at 3 day-fasting. UCP3 mRNA of HF were increased during HF diet feeding for 2 day, and peaked at 1 day-HF feeding, but decreased 2 day and 3 day-HF feeding CONCLUSION. Therefore, it may be rational that UCP3 is up-regulation when a large amount of fatty acids influx occurs in skeletal muscles as well as might have a role for fine adjustments of energy expenditure.

Intermittent Fasting: a Promising Approach for Preventing Vascular Dementia

  • Vascular dementia is the most common neuropsychiatric syndrome and is characterized by synaptic dysfunction, neuroinflammation, and cognitive dysfunction. Vascular dementia is associated with various environmental, genetic, and lifestyle risk factors. Recent research has focused on the association between vascular dementia and dietary patterns, suggesting that dietary regulation leads to better control of energy metabolism, improvements in brain insulin resistance, and the suppression of neuroinflammation. Intermittent fasting is a calorie-restriction method known to be more effective in promoting fat loss and regulating the impairment of glucose metabolism as compared with other dietary restriction regimens. Herein, the authors review the effects of intermittent fasting with regard to vascular dementia based on recent evidence and propose that intermittent fasting could be a therapeutic approach for ameliorating vascular dementia pathology and preventing its onset.

A Clinical Case Study on the Changes of Body Composition and Blood Chemistry after Modified Fasting Therapy in a Patient with Obesity (절식요법을 시행한 비만 환자의 체성분 및 혈액학적 변화 증례보고 1례)

  • This study was performed to report the effectiveness of modified fasting therapy using very-low-calorie-diet on a patient with obesity. The patient was diagnosed as severe obesity and participated in a 14-day fast with the Signature, a liquor which is made from fermented medical herbs and carbohydrates (intake <600 kcal/day) accompanying 7 prefasting relief days, and 16 days with stepwise reintroduction of food. The fasting patient also received intensive oriental medical treatments such as acupuncture, moxibustion, cupping therapy and was encouraged to take more than two hours' of walking exercise a day. We have evaluated the efficacy of treatments by measuring the changes of body composition and blood chemistry according to each period of fasting therapy. After treatment, Body Fat Mass of the patient was decreased more than Muscle Mass and the level of AST, ALT was decreased to normal range. These results suggest that the modified fasting therapy was effective on the patient with obesity.

Differential Expression of Neuropetide Y in the Hypothalamic Areas of Fasting and Anorexia Mutant Mice (Neuropeptide Y에 의한 식욕조절 관찰연구)

  • The present study was conducted to identify the mechanism about the regulation of appetite by examining the expression patterns of neuropeptide Y in the hypothalamus of mice either fasting mouse for 24 hours or with anorexia mutant mouse. In order to investigate the patterns of expression of neurpeptide Y, immunohistochemistry was employed for measurements at the tissue level, along with the molecular biological techniques of reverse transcription polymerase chain reaction(RT-PCR) and dot blotting. The results of this study are as follows. The level of expression of neruopeptide Y, a neuropeptide known to enhance appetite, was shown to be lowered in the arcuate nucleus(ARC), paraventricular nucleus(PVN), lateral hypothalamic area(LHA), and dorsomedial hypothalamic nucleus(DMN) in both the fasting and anorexia mutant groups when measured via immunohistochemistry, a tissue-level method. RT-PCR and dot blotting, the molecular biological methods employed in this study, revealed that the level of neuropeptide Y mRNA in the entire hypothalamus was similar in the control and fasting groups and lower in the anorexia mutant group. The results of the present study showed that while the levels of expression of the neuropeptide Y in the various hypothalamic regions studied did not exhibit regular increases or decreases when measured immunohistochemically. But the entire hypothalamus via molecular biological methods showed that the changes in these levels were more definite in the anorexia mutant group than in the fasting group.
