Studies on the Intradermal Reaction in Bovine Eurytremiasis (우췌질증(牛膵蛭症)의 피내반응(皮內反應)에 관한 연구(硏究))

  • The studies were conducted on the allergic skin reaction of bovine eurytremiasis. In order to confirm the appearance of allergic skin reaction in this disease, a protein fraction of Eurytrema SPP(EPF) was extracted, and it's antigenicity was studied by means of intradermal reaction on the rabbits, goats and bovine. Rabbits used in this studies were divided into three groups: the first group was sensitized with EPF, the second group was sensitized with EPF, and the third group was served as control which is consisted with 5 non-sensitized and 2 saline injected rabbits. Three healthy native goats and bovine also used. Among these, 86 cases were eurytremiasis, 31 cases were fascioliasis and 71 cases non-infected. A protein fraction was also made from adult worm of Fasciola SPP(FPF) and tested the cross reaction between these two different fluke infestations. The results obtained were as follow: 1. Intradermal reaction was appeared in the infected cattle(bovine eurytremiasis) as well as in the EPF sensitized rabbits and goats. 2. In the EPF sensitized rabbits, the wheal of the intradermal reaction reacted the maximum size (increased wheal size of 5.0 to 9.0 mm) in 40 to 60 minutes and erythema in 60 minutes after intradermal injection of EPF antigen. In the EPF sensitized goats, the size of wheal reacted the maximum(increased wheal size of 5.5 to 8.0 mm) in 30 to 40 minutes, and then disappeared in 120 to 180 minutes after injection of EPF antgen. On the contrary, in the control groups of rabbits and goats, the wheal (7.0 mm) produced following antigen injection reached only to 7.5 to 8.5 mm. in 10 minutes without developing erythema, and then perished in 40 to 60 minutes after injection of EPF antigen. In the infected cattle, the wheal of the intradermal reaction reached the maximal sizes(increased wheal size of 4.0 to 11.0 mm) in 15 to 20 minutes and perished in 60 to 90 minutes after injection of EPF antigen. 3. It would be standardized that the positive reaction is above 4.0 mm, the negative below 2.0 mm and the suspective is 3.0 mm of the infected cattle. In the results of intradermal reaction, 87.2%(75 of 86 cases) was positive, 4.7%(4 of 86 cases) negative and 8.1% (7 of 86 cases) was suspective. However, in the non-infected control cattle, the wheal produced following injection disappeared in 30 to 40 minutes without increasing the wheal size in the most subjects or with incresing only to 1.0 to 2.0 mm in a few cattle. 4. In the results of intradermal reaction which were reciprocally tested in EPF sensitized rabbits by FPF antigen and in FPF sensitized. rabbits by EPF antigen, there were cross reacted. In the 31 cases if fascioliasis tested by EPF antigen, 11 cases were not specifically reacted with EPF antigen. Judging from the results of cross reaction in experimental rabbits and natural bovine fascioliasis, it would be expected when both EPF and EPF antigens are given intracutaneously on the same bovine patients of eurytremiasis or fascioliasis, the former antigen will produce the larger wheal in eurytremiasis, while the latter antigen will produce also the larger wheal in fascioliasis.

  • An abattoir survey was carried out on 376 Korean native cattle reared and slaughtered in Soon-chun area to determine the prevalence of Fasciola hepatica infections. The average prevalence of fascioliasis was 44%; the infection rate had a tendency to increase with age. The mean(${\pm}SE$) length and width of the flukes were $24.9{\pm}2.5mm$ and $10.5{\pm}1.7mm$, respectively.

  • An abattoir survey was carried out on 300 Korean native cattles reared and slaughtered in North Kyeongnam area to determine the prevalence of Fasciola hepatica infections. The average prevalence of fascioliasis was 46.4% ; the infection rate had a tendency to increase with age. Some hematological and biochemical indices were assayed in Korean native cattle naturally harbouring Fasciola hepatica infection and compared with uninfection control. Affected animal revealed reduction total erythrocyte count and hemoglobin, which was consistent in total leucocyte count and packed cell volume. Infected cattle had lower level of serum total protein and albumin, was consistent serum calcium and glucose indices, had higher levels of SGOT, SGPT and cholesterol indices.

  • Fascioliasis is a parasitic infection caused by liver flukes. Although several cases have been reported in Korea, phylogenetic analysis of isolates is lacking. In this study, a 66-year-old woman with right upper quadrant (RUQ) abdominal pain was diagnosed as fascioliasis involving abdominal muscle by imaging study. She received praziquantel treatment, but symptoms were not improved. Lateral movement of the abscess lesion was followed. Trematode parasite was surgically removed from the patient's rectus abdominis muscle. The fluke was identified as Fasciola hepatica based on sequence analysis of 18S rDNA. To determine the phylogenetic position of this Fasciola strain (named Korean Fasciola 1; KF1), the cox1 gene (273 bp) was analyzed and compared with the genes of 17 F. hepatica strains isolated from cows, sheep, goats, and humans from various countries. Phylogenetic analysis showed that KF1 was closely related with the isolates from China goat.

  • A 56 year-old Korean housewife/farmerlgoat keeper suffered from right upper quadrant pain and fever with chills. In the abdominal sonogram and computerized tomography, multiple, 2-3 cm, irregular shaped cavities were observed in the right lobe of liver. A liver biopsy revealed extensive central necrosis with Characot-Leyden crystals surrounded by palisading histiocytes, eosinophil-rich inflammatory infiltration. Worm was not observed. However, the serologic test for Fusciola-specific IgG antibody by micro-ELISA was positive. Prior antibody levels did not differ and eosionophilia persisted 6 and 16 months after praziquantel treatment although the cavitaxy lesions in the liver disappeared 6 months after the treatment. Reported herein is a human case of invasive fascioliasis diagnosed clinically by a combination of radiological, histopathological and serological studies.

  • A synthetic peptide was prepared based on the antigenic region of Paragonimus westermani pre-procathepsin L, and its applicability for immunodiagnosis for human paragonimiasis (due to Paragonimus heterotremus) was tested using an ELISA to detect IgG4 antibodies in the sera of patients. Sera from other helminthiases, tuberculosis, and healthy volunteers were used as the references. This peptide-based assay system gave sensitivity, specificity, accuracy, and positive and negative predictive values of 100%, 94.6%, 96.2%, 100%, and 88.9%, respectively. Cross reactivity was frequently seen against the sera of fascioliasis (75%) and hookworm infections (50%). Since differential diagnosis between paragonimiasis and fascioliasis can be easily done by clinical presentation and fascioliasis serology, this cross reaction is not a serious problem. Sera from patients with other parasitoses (0-25%) rarely responded to this synthetic antigen. This synthetic peptide antigen seems to be useful for development of a standardized diagnostic system for paragonimiasis.

  • Fascioliasis is a foodborne zoonotic parasitic disease. We report 4 cases occurring in the same family, in whom diagnosis of acute fascioliasis was established after series of tests. One case was hospitalized with fever, eosinophilia, and hepatic lesions. MRI showed hypodense changes in both liver lobes. The remaining 3 cases presented with the symptom of stomachache only. Stool analysis was positive for Fasciola eggs in 2 adult patients. The immunological test and molecular identification of eggs were confirmed at the National Institute of Parasitic Diseases, Chinese Center for Disease Control and Prevention, Shanghai, China. The results of serological detection were positive in all the 4 patients. DNA sequencing of PCR products of the eggs demonstrated 100% homology with ITS and cox1 of Fasciola hepatica. The conditions of the patients were not improved by broad-spectrum anti-parasitic drugs until administration of triclabendazole.

Prevalence of Fascioliasis of Korean Native Cattle in Kangwon Province in Korea (강원도 사육 한우의 간질 감염실태)

  • A field survey of fascioliasis of Korean native cattle raising and raised in specialized commercial breeding farms and local farms in Kangwon province using both intradermal test and sedimentation technique for feces was carried out from November to December, 1996. Fecal samples were taken from fascioliasis positive cattle by the intradermal test for the fecal examination. Liver tissues were randomly collected from an abattoir for histopathological examination of liver fluke infection in cattle. The results are as follows. 1. By the intradermal test for a total of 211 cattle raising in both Wonju and Wheongsung, Kangwon province, 60 heads(28.4%) showed positive reaction. Among 60 positive cattle, eggs of Fasciola hepatica were found from 51 heads(85.0%) by sedimentation technique. 2. According to the cattle raising areas, the positive rates by the intradermal test were 26.7%(20 out of 75 heads) in Wonju and 29.4%(40 out of 136 heads) in Wheongsung. 3. According to the age of cattle examined, the positive rates by the intradermal test in 1~3, 4~6 and 7~10 years old were 11.7%(7 out of 100 heads), 68.3%(41 out of 93 heads) and 20.0%(12 out of 18 heads), respectively. 4. The overall infection rates of fluke larvae from the slaughtered cattle at an abattoir in Wonju was 24.7%(37 out of 150 heads). In histopathology, liver lesions were observed such as inflammation with infiltration of eosinophils, polymorphonuclear cells, mononuclear cells and multinucleated giant cells, proliferation of connective tissues, calcification and abscess formation.
