  • The following suggested algorithm is completed care report for the family medical history. Rn=$U\;Pnj+U\;Dn^i$ : (j=1,2,...,j), (i=1,2,...,i), (n=1,2,...,n) The Rn(completed care report) integrates comprehensive patients reports ranging from patient $P^2\;to\;P^j$ including $P^1$ (oneself) with the doctors' care reports up to the care No. no by i number of doctors ($D^1$ =doctor in charge, $D^{2,3...i}$=doctors on corporation program.) This approach, since a participation in a family membership effectuates all of family members, can minimize the membership fees, thus enabling inter-family health care on a home doctor basis.

여성 가족부양자의 건강상태 및 가족부양의 부정적.긍정적인 영향 (Health Status of Women Caregivers and Negative and Positive Impacts of Family Caregiving)

  • The purpose of this study was to examine the health status of the daughter and daughter- in-law caregivers who care for a cognitively and/or functionally impaired elderly, individual to identify factors that were related to reported health outcomes, and to investigate the negative and positive impacts of family caregiving. Data was collected from 120 daughter and daughter-in-law caregivers and care-recipients using face to face interviews. Most caregivers were daughters- in-law (77.5%) and most care-recipients were female (88.3%). Sixty-eight percent (n=81) of caregivers reported depressive symptomatology. General health also deteriorated by caregiving. Caregivers reported several negative impacts (difficulties): care-recipients' problematic behaviors, deterioration of their own health, pressure from social norms related to family caregiving in Korea, intrafamily conflict, and economic problems. Contrary to the popular belief, caregivers reported diverse positive impacts of family caregiving (68.3%): a sense of filial responsibilities, recognition from elderly, family members, relatives, and society, education for the children, and familial harmony. Higher depression score was predicted by lower family income, the presence of cognitive impairment of care-recipients, and higher level of social conflict of caregivers. Poor general health of caregivers was predicted by older caregivers' age, lower competing roles of caregivers, and poor emotional health. While not seeking to deny the negative aspects of family caregiving, it is also necessary to understand positive aspects of family caregiving to see complete picture of caring for an elderly family member.

  • For the solution of population problem, a population education is urgently need since family member size(that is population size) will be on the decrease fro the reason of changes in family consciousness by population education. The population education should be carried out interdisciplinary, but, above all, home economics is more suitable to the population education. In this study, the issues are the contents of population education in relation to family relations of home economics. Those contents are as follows: 1. Structure-function analysis of PRDI DNA polymerase

  • PRDI DNA polymerase is the smallest member of the family B DNA polymerase (Jung et al., 1987). This DNA polyerase is specified by bacteriophage PRDI which infects a wide variety of gram-negative bacteria(Mindich and Bamford, 1988). Because PRDI is highly amenable to genetic and biochemical manipulation, it is a convenient model system with which to study structure-function relationships of DNA polymerase molecules. To determine the functional roles of the highly conserved regions of the family B DNA polymerases, we have initiated site-directed mutagenesis with PRD1 DNA polymerase, and our results show that mutations at the conserved regions within PRD1 DNA polymerase inactivate polymerase complementing activity and catalytic activity.

Small Multidrug Resistance(smr) 플라스미드 pKH4의 염기서열 결정 (Complete Nucleotide Sequence of Small Multidrug Resistance Plasmid pKH4)

  • The complete nucleotide sequence of pKH4, a small multidrug resistance (smr) plasmid isolated from multidrug resistant Staphylococcus aureus SA5, was determined. Sequence analysis has revealed that pKH4 has two open reading frames for Rep and Smr proteins. The comparison of the amino acid sequence of Smr protein of pKH4 with those of other Smr proteins of various Staphylococcus showed that Smr protein of pKH4 is a new member of the SMR family.

뇌졸중 환자의 주간호제공자를 위한 비디오재활교육의 효과 (Effects of Video-guided Education for Primary Family-caregivers of Stroke Patients)

  • Purpose: Family-caregivers of stroke patients usually go through hardship and life style changes during the protracted course of a family member's rehabilitation. There is need for programs to educate family-caregivers to better manage the medical crisis. In this study an evaluation was done of the degrees of burden and well-being experienced by primary family-caregivers following video-guided education (VGE) on rehabilitation and family lifestyle changes. Method: Fifty-eight primary family-caregivers of stroke patients on a neurological ward were divided into VGE (29) and control (29) groups. VGE was started within 7 days of patient admission. Interventions included VGE, counseling, and demonstration - re-demonstration. The control group received standard education but not VGE. Data were analyzed using Chi-square test, t-test, ANCOVA, and Pearson correlation coefficients with the SAS program. Results: The VGE group had a significantly lower score for total burden (F=7.19, p=.010) and for sub-scale of time-dependent burden (F=8.44, p=.005) than the control group. There was a negative correlation between primary family-caregiver burden and well-being (r=-.7151, p<.001). Conclusion: Results suggest that the rehabilitation program using VGE was an effective nursing intervention to reduce the burden of primary family-caregivers of stroke patients.

치매노인 가족의 치매특별등급서비스 이용 경험 (Experience of a Special Rating Dementia Service for Family Caregivers of Elderly People with Dementia)

  • Purpose: This study tries to explore experiences with a special rating dementia service among family caregivers of elderly people with dementia. Methods: The participants were 11 family caregivers of elderly people with dementia and had used a day-care service from the special rating dementia service for more than 3 months. Data were collected through individual in-depth interviews and analyzed using content analysis. Results: Three categories emerged: "reminiscence of an entangled life", "feeling relieved from a stifled life", and "hope for a better life through help and support" with 8 sub-categories and 23 codes. By using the special rating dementia service, the participants experienced relief from the burden of caring for their elderly family member with dementia and restored their social relations or family relationships. They experienced a positive transition in which their caring for elderly family members with dementia was integrated into their daily lives, and they expressed their desires for better policies. Conclusion: These findings imply the importance and necessity of the special rating dementia service. Practical and systematic family support programs are necessary and proactive publicity activities by the government and public agencies are suggested to ensure that family caregivers can easily use the special rating dementia system.

  • This study was conducted to identify the adaptation process of the families with chronically ill patients. A descriptive comparative research design was used to the subjects of forty families that have stroke patients at an oriental medical hospital. Research tools were Demands of Illness Inventory(Woods, Haberman & Packard, 1987), Relative and Friend Support Index(McCubbin, Patterson & Glynn, 1982), Family Coping-Coherence Index (McCubbin, Larsen & Olson, 1982), Family Hardiness Index(McCubbin, McCubbin & Thompson, 1986), and Family Member Wellbeing Scale(McCubbin, McCubbin & Thompson, 1986). Data were collected from October 7, 1998 to November 7, 1998 at an oriental medical hospital located in IkSan city. The number of cases was forty and the data were analyzed by SPSS $PC^+$. Descriptive statistics of frequency, number, mean and standard deviation were used to report the results. The results were as follows ; 1. There was no significant differences between the two groups on the family stress. 2. There was significant differences between the two groups on the family strength (t value = - 3.09, p value=$.004^{**}$). 3. There was significant differences between the two groups on the family adaptation(t value= -2.08, pvalue=$.05^*$).

뇌졸중 환자를 돌보는 가족간호자의 부담감에 대한 종적연구 (A Longitudinal Study on the Burdens of Caregivers in Families with Stroke Patients)

  • This longitudinal study examines the burdens and related factors for stroke patient's primary family caregivers. Sixty-one patients treated for stroke at a general hospital in Chung-nam province and family member caregivers participated in this study from July 28, 1998 to August 1, 1999. Family caregivers' burdens were assessed by the burden scale originally developed by Zarit(1980) and Novak & Geust(1989) and modified by Oh's(1993) for use in Korea. The burden scale instrument consists of five subscales.: financial burden, social burden, physical burden, dependency burden, and emotional burden. Repeated ANOVA and Stepwise multiple regression were used in the data analyses. The results were as follows: The burden mean score was 3.23 in the hospital, 3.26 after hospital release, and 3.27 in the home environment. In the hospital, a patient's degree of cognition, and social support for family caregivers were significant factors affecting the sense of burden felt by family caregivers. After hospital release, the significant factors affecting the sense of burden were the degree to which stroke patients could participate in daily living activities, social support for family caregivers, and changing to a second caregiver. In the home environment, the most significant factor affecting the sense of burden was social support for family caregivers.

MiT Family Transcriptional Factors in Immune Cell Functions

  • The microphthalmia-associated transcription factor family (MiT family) proteins are evolutionarily conserved transcription factors that perform many essential biological functions. In mammals, the MiT family consists of MITF (microphthalmia-associated transcription factor or melanocyte-inducing transcription factor), TFEB (transcription factor EB), TFE3 (transcription factor E3), and TFEC (transcription factor EC). These transcriptional factors belong to the basic helix-loop-helix-leucine zipper (bHLH-LZ) transcription factor family and bind the E-box DNA motifs in the promoter regions of target genes to enhance transcription. The best studied functions of MiT proteins include lysosome biogenesis and autophagy induction. In addition, they modulate cellular metabolism, mitochondria dynamics, and various stress responses. The control of nuclear localization via phosphorylation and dephosphorylation serves as the primary regulatory mechanism for MiT family proteins, and several kinases and phosphatases have been identified to directly determine the transcriptional activities of MiT proteins. In different immune cell types, each MiT family member is shown to play distinct or redundant roles and we expect that there is far more to learn about their functions and regulatory mechanisms in host defense and inflammatory responses.
