We describe the successful insertion of a nasotracheal tube following repeated cuff rupture. The patient was a 55-year-old woman with a history of nasal trauma and multiple rhinoplasties, who underwent elective Lefort I osteotomy and bilateral sagittal split osteotomy for correction of skeletal facial deformity. During fiberoptic bronchoscope-guided nasal intubation after the induction of general anesthesia, the tracheal tube repeatedly ruptured in both nares, despite extensive preparation of the nasal airways. We covered the cuff with a one-inch tape, intubated to the level of the oropharynx, pulled the tracheal tube out through the mouth, and removed the tape. The tracheal tube was then backed out to the level of the uvula, and was successfully advanced.
Purpose: The nasal bone fracture is known as the most common facial fracture. Several authors reported the classification of nasal bone fracture. Stranc classified the type of nasal fractures based on a pattern of impaction and level of injuries. Stranc classification proposed here is based on careful clinical observation and relates to both treatment and prognosis. The aim of this study is to determine any predictive value to the preoperative classification of nasal fracture, using the description by Stranc and Robertson. Methods: We reviewed 310 patients with nasal bone fracture treated at our hospital for last two years. Results: Lateral impact type of nasal bone fracture predominated more than frontal impact in the ratio of 2.3:1. The most common type of Stranc classification was lateral impact plane I(48.4%). Male predominated more than female in the ratio of 3:1. The most frequent age group was first decade(27.1%), second decade in frontal impact(30.5%), first decade in lateral impact (30.7%). The most common etiology was violence (31.3%) followed by slip down(21.3%), and traffic accident(18.1 %).The most common associated fracture was orbital(22.9%) followed by zygoma(10%), and maxilla (6.1%). The most common complication was septal deviation(20.0%) in frontal impact, and nasal deformity (26.0%) in lateral impact. The incidence of nasal deformity in lateral impact(26.0%) was more higher than frontal impact(15.8%) Conclusion: By assessing the pathomechanics and resultant degree of injury to the nasal skeleton, a better understanding of the treatment plan and prognosis was obtained. Using this information, satisfactory informed patient consent can be obtained.
The need to assess preoperative psychological characteristics of orthognathic surgery patients has recently been recognized. Fifty six patients were involved in this study, they were investigated in the preoperative psychosocial aspects. Korean Version Eysenk Personality Questionaire and Korean Version Tennessee Self-Concept Scale were used for psychological test. The results were as follows : 1. There was no significant difference of sex and age in the personality dimension, the selfconcept scale, and the preoperative tests(P>0.05). 2. 37% of subjects showed phonetic problem, 60% masticatory problem and 56% social problem for facial deformity. The first choice was the face in the order of correction for mastication, phonation and face. And 83% of the subjects forecasted the change in their face after surgical correction, The answer was generally vague. 64% of the subjects expected their personality to become more lively as a result of their operation. 3. Total positive self score and personal self score were less improved in the groups with high scores of toughmindness and neuroticism(P<0.05), degree of suffering was increased in the group with high scores of neuroticism(P<0.05). 4. Degree of subjective deformity, self-consciousness, and degree of dissimilarity were more improved in the group of intraversion, there was a significant correlation with degree of dissimilarity in the group with high score of toughmindness(r=-.64, P<0.05). 5. There was a significant correlation with the forecast of change in the groups with high score in extraversion(r=.59, P<0.01) and with low score in neuroticism(r=-.39, P<0.05).
We report a case involving a young female patient with severe mandibular retrognathism accompanied by mandibular condylar deformity that was effectively treated with Le Fort I osteotomy and two genioplasty procedures. At 9 years and 9 months of age, she was diagnosed with Angle Class III malocclusion, a skeletal Class II jaw relationship, an anterior crossbite, congenital absence of some teeth, and a left-sided cleft lip and palate. Although the anterior crossbite and narrow maxillary arch were corrected by interceptive orthodontic treatment, severe mandibular hypogrowth resulted in unexpectedly severe mandibular retrognathism after growth completion. Moreover, bilateral condylar deformities were observed, and we suspected progressive condylar resorption (PCR). There was a high risk of further condylar resorption with mandibular advancement surgery; therefore, Le Fort I osteotomy with two genioplasty procedures was performed to achieve counterclockwise rotation of the mandible and avoid ingravescence of the condylar deformities. The total duration of active treatment was 42 months. The maxilla was impacted by 7.0 mm and 5.0 mm in the incisor and molar regions, respectively, while the pogonion was advanced by 18.0 mm. This significantly resolved both skeletal disharmony and malocclusion. Furthermore, the hyoid bone was advanced, the pharyngeal airway space was increased, and the morphology of the mandibular condyle was maintained. At the 30-month follow-up examination, the patient exhibited a satisfactory facial profile. The findings from our case suggest that severe mandibular retrognathism with condylar deformities can be effectively treated without surgical mandibular advancement, thus decreasing the risk of PCR.

The midline nasal dermoid cysts are rare congenital neoplasms, which are diagnosed frequently in childhood. Masses are often noticeable at birth gaining size over time with recurrent infections and usually arise from the nasal cavity or lower 1/3 of the nasal dorsum. CT scan as the primary investigation is helpful to determine accurately the size and extent of the lesion as well as the integrity of adjacent bony structures. MRI scan is recommended to rule out an intracranial extension or sinus tracts. Treatment of choice is the complete surgical excision preserving the cyst wall. Here in, we present an unusual case of nasofrontal dermoid cyst in a 19-year-old boy without radiographic evidence of transcranial extension. In this case, we surgically removed nasofrontal dermoid cyst via transcolumellar approach. We also corrected saddle nose deformity after mass removal. Therefore, in this case, we experienced a successful case in which the nasofrontal dermoid cyst was totally removed without facial scar and deformity.

Patients with cleft lip and palate usually present midfacial depression and anterior cross-bite. This dentofacial deformity has been believed due to the undergrowth of maxilla and/or the collapse of premaxilla. But, in the case that the collapsed premaxilla exists only, the reconstruction of the premaxilla has to be required for the correction of that deformity. These cases show the surgical treatment of midfacial depression and anterior cross-bite in the cleft lip and palate. After the careful diagnosis for the collapse of premaxilla, the reconstruction using distraction osteogenesis was done successfully. As a result, the anterior overbite / overjet, and facial esthetics were improved remarkably, and the occlusion was also recovered to normal state. In conclusion, the premaxillary reconstruction by distraction osteogenesis in cleft lip and palate patients is a good treatment method based on the pathophysiology.

We observed 469 cases (male 217 cases, female 252 cases) with dento-facial deformity for 10 years from Jan 1994 to Dec 2003. The ratio of male to female was 1:1.2 and the mean age was 23.3 years (male 24.0 years, female 22.6 years) ranged from 11 to 43 years. The most dominant group was related to mandibular prognathism (80.4%). A rate of 83.8% demonstrated mandibular protrusion according to Slavicek's classification (n=160), and 85.4% were classified as skeletal Class III type according to Sugawara's classification (n=151). Surgical method were divided into 355 cases of one jaw surgery (single method), 26 cases of one jaw surgery (combined method), and 77 cases of two jaw surgery. Sagittal split osteotomy were performed on 316 cases (69.0%). The average operation time and blood loss in SSRO were $4.1{\pm}2.2$ hrs. and $138.8{\pm}222.6$ ml (n=152).
The dentigerous cyst originates through alteration of the reduced enamel epithelium after the crown of the tooth has been completely formed, with accumulation of fluid between the reduced enamel epithelium and the tooth crown. The dentigerous or follicular cyst comprises about 20% of all dental cysts. It usually occurs between 20 and 30 years of age but is occasionally seen in children or in old people. The most common sites of this cyst are the mandibular third molar and maxillary cuspid areas, since these are the most commonly impacted teeth. Many of these cysts give no clinical symptoms until noticeable asymmetry of the face develops. Rarely it develops to an ameloblastoma, epidermoid carcinoma and mucoepidermoid carcinoma, so early removal leads to better result. Roentgenographic examination of the jaw involved by a dentigerous cyst will reveal a radiolucent area. Recently the authors have experienced a giant dentigerous cyst which produced the bony deformity of the maxillary and hard palate bone. The cyst was completely removed under the local anesthesia by Caldwell-Luc approach.

Purpose: Reconstruction of a full thickness defect of the nose is a difficult task for plastic surgeons because the anatomical characteristic, shape, and function of the nose all need to be taken into consideration. Most often, a local flap or a composite graft is used, but for a large defect, reconstruction using free flaps is the most ideal method. In free flap reconstruction, the chondrocutaneous preauricular area can be a suitable donor site. We performed a chondrocutaneous preauricular free flap with an interpositional vascular graft for reconstruction of a nasal ala. Methods: A 46 year-old male presented to the hospital with a right alar deformity induced by a dog bite. During the surgery, the existing scar tissue was removed and thereby a newly formed full thickness defect was reconstructed using the chondrocutaneous preauricular free flap with an interpositional vascular graft harvested from the descending branch of the lateral femoral circumflex vessel between the facial and superficial temporal vessels of the free flap. Results: The flap survived without flap loss and showed symmetry in its overall shape, contour, texture, and color. The patient was satisfied with the results and the surgery yielded no additional scars at the nasolabial fold area. Conclusion: The chondrocutaneous preauricular free flap is a valuable method in reconstruction of full thickness defects of the nose, and using the descending branch of the lateral femoral circumflex vessel as the interpositional vascular graft at the anastomotic site produces reliable results.
For the total treatment of skeletal malocclusions, 3-dimensional evaluation and diagnosis are essential. Although anteroposterior discrepancies can be evaluated through various methods, the satisfactory methods for evaluations of facial asymmetry and transverse discrepancies are yet to be found. The adequate diagnosis and treatment of transverse discrepancies may be more important in the maintenance of functional occlusion as well as for the stability of results obtained from orthognathic surgery than the anteroposterior or vertical discrepancies. Since the soft tissue effects from the transverse discrepancies may not be pronounced, especially when combined with anteroposterior or vertical discrepancies which have prominent characteristics, the differentiation of their effects may be difficult from visual inspection alone. Therefore it is essential that the normal facial proportions would be established from the posteroanterior cephalometry as a reference for the accurate diagnosis and treatment. The present study evaluates 76 subjects from Yonsei University freshmen with normal facial symmetry and occlusion. Posteroanterior cephalograms were taken from the subjects and the normal values and facial proportions are obtained. The results are as follows. 1. The transverse and vortical values from posteroanterior cephalometry and their ratio, with means and standard deviations are calculated. 2. The ratio of vertical values to transverse values is 0.837 (male 0.836, female 0.841). 3. The Proportion of maxillary and mandibular widths is 0.747 (male 0.745, female 0.752), with statistically significant correlation. 4. Various degree of significant correlations are observed in the following craniofacial widths; (Cranial width, Bizygomaticofrontal suture width, Facial width, Maxillary width, Upper & Lower Intermolar width, Mandibular width). 5. Although the facial height as well as other line measurements increase as the facial widths increase, angle measurement ($Bj\ddot{o}rk$ Sum, Mandibular Plane Angle, Gonial Angle), decreases and posterior to anterior facial height ratio increases, therefore indicating the tendency for a brachycephalic facial type. These results may be used as references for the treatment planning in orthognathic and orthodontic treatments for the dentofacial deformity patients.
